Efficient optical quantum state engineering
We discuss a novel method of efficiently producing multi-photon states using
repeated spontaneous parametric downconversion. Specifically, by attempting
downconversion several times, we can pseudo-deterministically add photons to a
mode, producing various several-photon states. We discuss both expected

Security of high-dimensional quantum key distribution protocols using Franson interferometers
Franson interferometers are increasingly being proposed as a means of
securing high-dimensional energy-time entanglement-based quantum key
distribution (QKD) systems. Heuristic arguments have been proposed that purport
to demonstrate the security of these schemes. We show, however, that such
systems are vulnerable to attacks that localize the photons to several
temporally separate locations. This demonstrates that a single pair of Franson
interferometers is not a practical approach to securing high-dimensional
energy-time entanglement based QKD. This observations leads us to investigate
the security of modified Franson-based-protocols, where Alice and Bob have two
or more Franson interferometers. We show that such setups can improve the
sensitivity against attacks that localize the photons to multiple temporal
locations. While our results do not constituting a full security proof, they do
show that a single pair of Franson interferometers is not secure and that
multiple such interferometers could be a promising candidate for experimentally
realizable high-dimensional QKD.Comment: 14 pages (single column format
Detection-Loophole-Free Test of Quantum Nonlocality, and Applications
We present a source of entangled photons that violates a Bell inequality free
of the "fair-sampling" assumption, by over 7 standard deviations. This
violation is the first experiment with photons to close the detection loophole,
and we demonstrate enough "efficiency" overhead to eventually perform a fully
loophole-free test of local realism. The entanglement quality is verified by
maximally violating additional Bell tests, testing the upper limit of quantum
correlations. Finally, we use the source to generate secure private quantum

Efficient generation of single and entangled photons on a silicon photonic integrated chip
We present a protocol for generating on-demand, indistinguishable single
photons on a silicon photonic integrated chip. The source is a time-multiplexed
spontaneous parametric down-conversion element that allows optimization of
single-photon versus multiphoton emission while realizing high output rate and
indistinguishability. We minimize both the scaling of active elements and the
scaling of active element loss with multiplexing. We then discuss detection
strategies and data processing to further optimize the procedure. We simulate
an improvement in single-photon-generation efficiency over previous
time-multiplexing protocols, assuming existing fabrication capabilities. We
then apply this system to generate heralded Bell states. The generation
efficiency of both nonclassical states could be increased substantially with

Estimation of the length of interactions in arena game semantics
We estimate the maximal length of interactions between strategies in HO/N
game semantics, in the spirit of the work by Schwichtenberg and Beckmann for
the length of reduction in simply typed lambdacalculus. Because of the
operational content of game semantics, the bounds presented here also apply to
head linear reduction on lambda-terms and to the execution of programs by
abstract machines (PAM/KAM), including in presence of computational effects
such as non-determinism or ground type references. The proof proceeds by
extracting from the games model a combinatorial rewriting rule on trees of
natural numbers, which can then be analyzed independently of game semantics or

Socio-demographic and clinical characteristics of re-presentation to an Australian inner-city emergency department: implications for service delivery
BACKGROUND: People who have complex health care needs frequently access emergency departments for treatment of acute illness and injury. In particular, evidence suggests that those who are homeless, or suffer mental illness, or have a history of substance misuse, are often repeat users of emergency departments. The aim of this study was to describe the socio-demographic and clinical characteristics of emergency department re-presentations. Re-presentation was defined as a return visit to the same emergency department within 28 days of discharge from hospital. METHODS: A retrospective cohort study was conducted of emergency department presentations occurring over a 24-month period to an Australian inner-city hospital. Characteristics were examined for their influence on the binary outcome of re-presentation within 28 days of discharge using logistic regression with the variable patient fitted as a random effect. RESULTS: From 64,147 presentations to the emergency department the re-presentation rate was 18.0% (n = 11,559) of visits and 14.4% (5,894/40,942) of all patients. Median time to re-presentation was 6 days, with more than half occurring within one week of discharge (60.8%; n = 6,873), and more than three-quarters within two weeks (80.9%; n = 9,151). The odds of re-presentation increased three-fold for people who were homeless compared to those living in stable accommodation (adjusted OR 3.09; 95% CI, 2.83 to 3.36). Similarly, the odds of re-presentation were significantly higher for patients receiving a government pension compared to those who did not (adjusted OR 1.73; 95% CI, 1.63 to 1.84), patients who left part-way through treatment compared to those who completed treatment and were discharged home (adjusted OR 1.64; 95% CI, 1.36 to 1.99), and those discharged to a residential-care facility compared to those who were discharged home (adjusted OR 1.46: 95% CI, 1.03 to 2.06). CONCLUSION: Emergency department re-presentation rates cluster around one week after discharge and rapidly decrease thereafter. Housing status and being a recipient of a government pension are the most significant risk factors. Early identification and appropriate referrals for those patients who are at risk of emergency department re-presentation will assist in the development of targeted strategies to improve health service delivery to this vulnerable group
Functional assessment of older patients in the emergency department: comparison between standard instruments, medical records and physicians' perceptions
BACKGROUND: We evaluated the accuracy of physician recognition of functional status impairment in older emergency departments (ED) patients. In particular, we evaluated the accuracy of medical records (a comparison of the information in the medical record with the functional status based on proxy interviews), and the accuracy of physician knowledge (a comparison of the information obtained from the responsible physician with the functional status based on proxy interviews). METHODS: Cross-sectional study on 101 frail older patients selected at random from among those attending ED, their ED physicians, and respondents. The study was conducted at ED in four general university teaching hospitals in a city, from July through November 2003. Functional data shown on patients' medical records were compared against functional data obtained from respondents (family members), using Kendall's Tau-b statistic. In addition patients' Katz Indices (which assesses six basic activities of daily living – basic ADL) based on interviews with ED physicians were compared against those obtained from respondents, using the coefficient of concordance weighted kappa (κ). Each patient and his respondent were paired with a single physician. RESULTS: The correlation between information on dependence for basic ADL obtained from medical records and that furnished by respondents, was 0.41 (95% CI 0.27–0.55). Concordance between the respective Katz Indices obtained from physicians and respondents was 0.47 (95% CI 0.38–0.57). CONCLUSION: Older subjects' functional status is not properly assessed by emergency department physicians
