    Breast cancer management pathways during the COVID-19 pandemic: outcomes from the UK ‘Alert Level 4’ phase of the B-MaP-C study

    Abstract: Background: The B-MaP-C study aimed to determine alterations to breast cancer (BC) management during the peak transmission period of the UK COVID-19 pandemic and the potential impact of these treatment decisions. Methods: This was a national cohort study of patients with early BC undergoing multidisciplinary team (MDT)-guided treatment recommendations during the pandemic, designated ‘standard’ or ‘COVID-altered’, in the preoperative, operative and post-operative setting. Findings: Of 3776 patients (from 64 UK units) in the study, 2246 (59%) had ‘COVID-altered’ management. ‘Bridging’ endocrine therapy was used (n = 951) where theatre capacity was reduced. There was increasing access to COVID-19 low-risk theatres during the study period (59%). In line with national guidance, immediate breast reconstruction was avoided (n = 299). Where adjuvant chemotherapy was omitted (n = 81), the median benefit was only 3% (IQR 2–9%) using ‘NHS Predict’. There was the rapid adoption of new evidence-based hypofractionated radiotherapy (n = 781, from 46 units). Only 14 patients (1%) tested positive for SARS-CoV-2 during their treatment journey. Conclusions: The majority of ‘COVID-altered’ management decisions were largely in line with pre-COVID evidence-based guidelines, implying that breast cancer survival outcomes are unlikely to be negatively impacted by the pandemic. However, in this study, the potential impact of delays to BC presentation or diagnosis remains unknown

    The impact of subdivisions designed for conservation on prime farmland conversion

    In the last two decades, a novel approach to development, which has been commonly referred to as subdivisions designed for conservation, has offered an alternative development approach that is relatively less destructive to natural features, such as farmland. The purpose of this dissertation is to investigate if housing subdivisions designed for conservation can be used as a development strategy that stems the ongoing loss of prime farmland near sprawling metropolitan areas. This study is set in the Village of Grayslake, Illinois, where one of the early pioneers of such subdivisions, Prairie Crossing, is located. Three research questions guide this study and are as follows: what are the motivations of developers and homebuyers who embrace the Prairie Crossing conservation community that was designed with sustainability in mind, is there an observable difference between the amount of prime farmland converted for the Prairie Crossing conservation community and for conventional housing subdivisions located nearby in the Village of Grayslake, Illinois, and how much value is captured in the selling prices of houses constructed in the Prairie Crossing conservation community. The findings from the first research question suggest that people, on average, gravitate to and embrace Prairie Crossing because of its eco-friendly nature and/or its aesthetically pleasing layout and natural amenities. The index values calculated for the second research question pertaining to density indicate that Prairie Crossing consumes more prime farmland than conventional housing subdivisions. Caveats to this specific finding are discussed in the concluding chapter of this dissertation. The hedonic price model created to determine whether or not a premium is associated with the selling prices of houses in Prairie Crossing suggests one is present and has estimated it to be 36.31 percent. In conclusion, subdivisions designed for conservation appear to be an alternative approach to residential development that not only has the potential of abating the conversion rate of prime farmland but also has been embraced by a niche market that is willing to pay a premium to live in them

    There’s no place like home: place attachment among the elderly in Greensburg, Kansas

    Master of ArtsDepartment of GeographyJeffrey S. SmithIn a matter of minutes, a small, western Kansas community by the name of Greensburg was over 95% destroyed by a tornado. After the storm, the community’s civic leaders decided to rebuild Green. As a result, the modified cultural landscape no longer resembled the once familiar town that was viewed as a place of attachment by its predominantly elderly population. The purpose of this thesis is to better understand how the May 4th, 2007, tornado affected the elderly’s emotional connection to Greensburg. To identify how the town’s landscape changed I used before and after photographs. In order to more fully comprehend how their attachment to the community has changed, interviews were conducted with several elderly residents who rebuilt in Greensburg, as well as those who moved away. The results suggest that the elderly experienced a significant change in their bond to the town. Typically the elderly did not embrace going Green and focused more on retaining their memories of how the town used to be. Most significant to their development and change in place attachment were the relationships they developed and maintained with fellow community members. Overall, it was the people that made Greensburg home and a place of attachment. When many of them left for good after the tornado, the elderly’s place attachment to Greensburg was forever changed

    On May 4, 2007, an EF-5 tornado passed through Greensburg leveling 95 percent of the town. Because city leaders encouraged everyone to use “green” building techniques as they rebuilt their homes and businesses, not only has the return to normalcy been exceedingly slow, but some of the town’s older residents feel that officials overlooked their needs. These minor episodes of discord enabled us to learn what features are most important to people in their retirement-age. They include: identifiable landmarks, a space to socialize, and age-specific businesses. We assert that the lessons learned in Greensburg are applicable to other communities with a sizeable older-aged population. As baby boomers rapidly enter retirement they will seek places to live that are elderly-friendly and enable them to effectively bond with place. As previous research attests, people who have a strong attachment to place commonly lead a good quality-of-life

    Assessment, endoscopy, and treatment in patients with acute severe ulcerative colitis during the COVID-19 pandemic (PROTECT-ASUC): a multicentre, observational, case-control study

    BackgroundThere is a paucity of evidence to support safe and effective management of patients with acute severe ulcerative colitis during the COVID-19 pandemic. We sought to identify alterations to established conventional evidence-based management of acute severe ulcerative colitis during the early COVID-19 pandemic, the effect on outcomes, and any associations with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and severe COVID-19 outcomes. MethodsThe PROTECT-ASUC study was a multicentre, observational, case-control study in 60 acute secondary care hospitals throughout the UK. We included adults (≥18 years) with either ulcerative colitis or inflammatory bowel disease unclassified, who presented with acute severe ulcerative colitis and fulfilled the Truelove and Witts criteria. Cases and controls were identified as either admitted or managed in emergency ambulatory care settings between March 1, 2020, and June 30, 2020 (COVID-19 pandemic period cohort), or between Jan 1, 2019, and June 30, 2019 (historical control cohort), respectively. The primary outcome was the proportion of patients with acute severe ulcerative colitis receiving rescue therapy (including primary induction) or colectomy. The study is registered with ClinicalTrials.gov, NCT04411784. FindingsWe included 782 patients (398 in the pandemic period cohort and 384 in the historical control cohort) who met the Truelove and Witts criteria for acute severe ulcerative colitis. The proportion of patients receiving rescue therapy (including primary induction) or surgery was higher during the pandemic period than in the historical period (217 [55%] of 393 patients vs 159 [42%] of 380 patients; p=0·00024) and the time to rescue therapy was shorter in the pandemic cohort than in the historical cohort (p=0·0026). This difference was driven by a greater use of rescue and primary induction therapies with biologicals, ciclosporin, or tofacitinib in the COVID-19 pandemic period cohort than in the historical control period cohort (177 [46%] of 387 patients in the COVID-19 cohort vs 134 [36%] of 373 patients in the historical cohort; p=0·0064). During the pandemic, more patients received ambulatory (outpatient) intravenous steroids (51 [13%] of 385 patients vs 19 [5%] of 360 patients; p=0·00023). Fewer patients received thiopurines (29 [7%] of 398 patients vs 46 [12%] of 384; p=0·029) and 5-aminosalicylic acids (67 [17%] of 398 patients vs 98 [26%] of 384; p=0·0037) during the pandemic than in the historical control period. Colectomy rates were similar between the pandemic and historical control groups (64 [16%] of 389 vs 50 [13%] of 375; p=0·26); however, laparoscopic surgery was less frequently performed during the pandemic period (34 [53%] of 64] vs 38 [76%] of 50; p=0·018). Five (2%) of 253 patients tested positive for SARS-CoV-2 during hospital treatment. Two (2%) of 103 patients re-tested for SARS-CoV-2 during the 3-month follow-up were positive 5 days and 12 days, respectively, after discharge from index admission. Both recovered without serious outcomes. InterpretationThe COVID-19 pandemic altered practice patterns of gastroenterologists and colorectal surgeons in the management of acute severe ulcerative colitis but was associated with similar outcomes to a historical cohort. Despite continued use of high-dose corticosteroids and biologicals, the incidence of COVID-19 within 3 months was low and not associated with adverse COVID-19 outcomes
