    Intermittent Preventive Therapy and Treatment of Malaria during Pregnancy: A Study of Knowledge among Pregnant Women in Rufiji District, Southern Tanzania

    Purpose: To assess the knowledge and awareness of pregnant women regarding the use of sulfadoxinepyrimethamine (SP) for intermittent preventive therapy (IPT) and artemether-lumefantrine (ALu) for treatmentof malaria during pregnancy.Methods: The study was conducted in Rufiji district, southern Tanzania from March 2011 to September 2011. Four hundred and seventy (470) pregnant women in their second and third trimesters were interviewed whenattending antenatal clinics at the selected hospitals, health centers and dispensaries. Focus group discussions (FGDs) were also conducted with 46 pregnant women at the health facilities in the district.Results: More than half (54.3 %) of pregnant women did not know if SP it was used for IPT. Most women (76.6 %) did not know the use of SP for IPT in relationship with gestation age. Overall, the results show that most women had very low knowledge about the use of SP for IPT. Forty three (9.1 %) pregnant women reported to have had malaria during their current pregnancies. The antimalarials reported to be used by pregnant women were quinine 18(42.9 %), SP (23.8 %), ALu (21.4%) and sulphamethoxyprazinepyrimethamine (2.4%). Irrespective of the gestation age of pregnancy, almost all (98.3 %) pregnant women perceived ALu as unsafe drug to be used during pregnancy.Conclusion: Most pregnant women had minimum knowledge about the use and benefits of SP for IPT and ALu for treatment of malaria during  pregnancy. Some erroneous beliefs about the safety of ALu during pregnancy were also identified among pregnant women. For effective implementation of IPT policy and treatment of malaria during pregnancy, pregnant women should be sensitized and educated on the use andbenefits of antimalarial drugs.Keywords: Artemether-lumefantrine, Antenatal clinic, Knowledge, Pregnant women, Sulfadoxinepyrimethamin

    Health workers\u27 counselling practices on and women\u27s awareness of pregnancy danger signs in selected rural health facilities in Burkina Faso, Ghana and Tanzania

    Background: This study assessed health workers’ counselling practices on danger signs during antenatal consultation and pregnant women’s awareness of these signs and identified factors affecting counselling practices and women’s awareness. The study is part of QUALMAT, an intervention research project funded by the European Commission aiming to improve maternal and newborn health. QUALMAT is conducted in Burkina Faso, Ghana and Tanzania. Methods: A cross-sectional study was performed in 12 selected primary healthcare facilities in each country. WHOguidelines were used as standard for good counselling. We assessed providers’ counselling practice on seven danger signs through direct observation study (35 observations/facility). Exit interviews (63 interviews/facility) were used to assess women’s awareness of the same seven signs. We used negative binomial regression to assess associations with health services’ and sociodemographic characteristics. Results: About one in three women were not informed on any danger sign. For most single signs, less than half of the women were counselled. Vaginal bleeding and severe abdominal pain were the signs most counselled on (between 52 and 66%). At study facilities in Burkina Faso 58% of women were not able mentioning a danger sign, in Ghana this was 22% and in Tanzania 30%. Fever, vaginal bleeding and severe abdominal pain were signs most frequently mentioned. Kind of health worker (depending on training) was significantly associated with counselling practices. Depending on the study site, characteristics significantly associated with awareness of signs were women’s age, gestational age, gravidity and women’s educational level. Conclusion: Counselling practice is poor and not very efficient. A new approach of informing pregnant women ondanger signs is needed. Adopting a more client-centred approach might be an option. However as effects of ANC education remain largely unknown it is very well possible that improved counselling will not have effect on maternal and newborn mortality and morbidity

    Breeding in bambara groundnut (Vigna subterranea (L.) Verdc.): strategic considerations

    Bambara groundnut (Vigna subterranea (L.) Verdc.) has a large number of landraces throughout Africa where small-scale farmers have preserved its genetic diversity on-farm. To date, the full genetic diversity of the crop remains largely unexploited. Until recently bambara groundnut never received any appreciable research effort, especially for its genetic improvement. Until then, only selection breeding was practised in which existing landraces were evaluated and their seeds multiplied. However, no new combinations resulting from hybridisation had ever been produced. Recently, collaborative research efforts involving partners from Africa and Europe have produced the first crosses of bambara groundnut. The creation of these crosses is a significant scientific and practical achievement and opens up the possibility of breeding true varieties of this crop. This paper shows how different strategies have been combined to establish the basis of a strategic breeding programme in bambara groundnut. The paper also illustrates the use of landraces in the bambara groundnut breeding programme, as an example of the contribution that landraces can make to increasing productivity in marginal environments and the conservation of a crop's genetic resources on-farm.African Journal of Biotechnology Vol. 4 (6), pp. 463-471, 200

    Analysis of Perinatal Mortality at a Teaching Hospital in Dar es Salaam, Tanzania, 1999-2003

    We conducted a retrospective analysis of perinatal mortality at Muhimbili National Hospital (MNH), Dar es Salaam, Tanzania 1999-2003 in order to categorise/classify perinatal deaths as well as to identify key factors in perinatal care that could be improved. Data were retrieved from the MNH obstetric database and causes of early neonatal deaths were traced from the neonatal ward register. The study includes all foetuses weighing ≥500g. A modified Nordic-Baltic classification was used for classification of perinatal deaths. Over a 5-year period there were 77,815 babies born with a perinatal mortality rate of 124 per 1000 births, 78% of which was labour related stillbirth. The PMR was 913/1000 for singleton births and 723/1000 for multiple births for babies weighing less than 1500 grams and 65/1000 for singleton births and 116/1000 for multiple births for babies weighing 2500 grams or more. Babies weighing less than 1500 grams contributed 26% of PMR, whereas 41% occurred in babies weighing 2500 grams or more. The majority (79%) of neonatal deaths had Apgar scor

    Staffing needs for quality perinatal care in Tanzania

    In Tanzania maternal and perinatal mortalities and morbidities are problems of public health importance, and have been linked to the shortage of skilled staff. We quantified the available workforceand the required nursing staff for perinatal care in 16 health institutions in Dar es Salaam. WHO safe motherhood needs assessment instruments were used to assess the availability of human resources,WHO designed Workload Indicators for Staffing Need (WISN) and Tanzanian standard activities and components of the workload for labour ward nursing were used to calculate nurse staffing requirementsand WISN ratios. There was a severe shortage of essential categories of health staff for perinatal care in all institutions. The ranges of WISN ratios for nursing staff working in the municipal hospitals’ labourwards were; nurse officers 0.5 – 1, trained nurses/midwives 0.2 - 0.4 and nurse assistants 0.1. These findings reflect extremely huge perinatal care workload pressure and suggest the urgent need for morestaff in order to achieve the global millennium development goals set for maternal and infant survival (Afr J Reprod Health 2008; 12[3]:113-124)

    Genomic and pedigree‐based predictive ability for quality traits in tea (Camellia sinensis (L.) O. Kuntze)

    © 2021, The Author(s). Genetic improvement of quality traits in tea (Camellia sinensis (L.) O. Kuntze) through conventional breeding methods has been limited, because tea quality is a difficult and expensive trait to measure. Genomic selection (GS) is suitable for predicting such complex traits, as it uses genome wide markers to estimate the genetic values of individuals. We compared the prediction accuracies of six genomic prediction models including Bayesian ridge regression (BRR), genomic best linear unbiased prediction (GBLUP), BayesA, BayesB, BayesC and reproducing kernel Hilbert spaces models incorporating the pedigree relationship namely; RKHS-pedigree, RKHS-markers and RKHS markers and pedigree (RKHS-MP) to determine the breeding values for 12 tea quality traits. One hundred and three tea genotypes were genotyped using genotyping-by-sequencing and phenotyped using nuclear magnetic resonance spectroscopy in replicated trials. We also compared the effect of trait heritability and training population size on prediction accuracies. The traits with the highest prediction accuracies were; theogallin (0.59), epicatechin gallate (ECG) (0.56) and theobromine (0.61), while the traits with the lowest prediction accuracies were theanine (0.32) and caffeine (0.39). The performance of all the GS models were almost the same, with BRR (0.53), BayesA (0.52), GBLUP (0.50) and RKHS-MP (0.50) performing slightly better than the others. Heritability estimates were moderate to high (0.35–0.92). Prediction accuracies increased with increasing training population size and trait heritability. We conclude that the moderate to high prediction accuracies observed suggests GS is a promising approach in tea improvement and could be implemented in breeding programmes

    Expression of Insoluble Influenza Neuraminidase Type 1 (NA1) Protein in Tobacco

    The avian influenza virus, particularly H5N1 strain, is highly virulent to poultry and mankind. Several expression systems, like yeast, baculovirus and mammalian cells, have been adopted to produce vaccine candidate for this lethal disease. The present research aimed at developing a recombinant vaccine candidate, neuraminidase type 1 (NA1), for the Malaysia isolate of H5N1 in Nicotiana benthamiana. The NA1 gene was fused directly in-frame in cowpea mosaic virus (CPMV)-based pEAQ-HT vector with C-terminal polyhistidine-tag incorporated to ease the subsequent purification step. The expression of the NA1 gene in tobacco was confirmed at RNA and protein levels at 6 days post-infiltration (Dpi). From the insoluble fraction of the protein, a recombinant glycosylated NA1 protein with a molecular weight of ~56 kDa was immunogenically detected by a specific anti-NA polyclonal antibody. We report for the first time the insolubility of the plant-made NA1 protein where a native sequence was used for its expression. This study signifies the necessity of the use of optimised sequences for expression work and provides great opportunity for the exploration of plant-manufactured NA1 protein as vaccine candidate
