    Myocarditis related to Campylobacter jejuni infection: A case report

    BACKGROUND: Myocarditis can develop as a complication of various infections and is most commonly linked to enterovirus infections. Myocarditis is rarely associated with bacterial infections; salmonellosis and shigellosis have been the most frequently reported bacterial cause. We report a case of myocarditis related to Campylobacter jejuni enteritis. CASE PRESENTATION: A 30-year-old previously healthy man presented with a history of prolonged chest pain radiating to the jaw and the left arm. Five days prior to the onset of chest pain, he developed bloody diarrhea, fever and chills. Creatine kinase (CK) and CK-MB were elevated to 289 U/L and 28.7 μg/L. Troponin I was 30.2 μg/L. The electrocardiogram (ECG) showed T wave inversion in the lateral and inferior leads. The chest pain resolved within 24 hours of admission. The patient had a completely normal ECG stress test. The patient was initiated on ciprofloxacin 500 mg po bid when Campylobacter jejuni was isolated from the stool. Diarrhea resolved within 48 hours of initiation of ciprofloxacin. The diagnosis of Campylobacter enteritis and related myocarditis was made based on the clinical and laboratory results and the patient was discharged from the hospital in stable condition. CONCLUSION: Myocarditis can be a rare but severe complication of infectious disease and should be considered as a diagnosis in patients presenting with chest pain and elevated cardiac enzymes in the absence of underlying coronary disease. It can lead to cardiomyopathy and congestive heart failure. There are only a few reported cases of myocarditis associated with Campylobacter infection

    Monitoring ground movement at Volcán de Colima, Mexico, using Sentinel-1 data and SqueeSAR®

    Volcán de Colima is a highly active stratovolcano in western Mexico which presents a significant hazard to over 300,000 people who live within ca 40 km of the volcano. Due to its persistent activity, the volcano is actively monitored and researched, and understanding the patterns of behaviour is vital to accurate hazard assessment. Sentinel-1 SAR images from ascending and descending orbits allow 1D and 2D ground motions to be retrieved using multi-interferogram techniques. SqueeSAR®’s unique processing allows a better characterisation of subtle ground movements in remote, rural mountainous areas compared to many other multi-interferogram techniques. A dataset of 147 SAR scenes (2017-2019) has been processed to show patterns of lava subsidence (<150 mm of downward vertical deformation over 2 years), as well as volcano deflation and apparent westward lateral movement. These data indicate that viscous andesitic lava flows may remain mobile for years following eruption and emplacement, and that the entire volcanic edifice is subsiding. Despite the apparent quiescence, volcanic edifices can remain highly dynamic after the termination of explosive or effusive activity. We interpret that the western flank of Volcán de Colima may become steeper with time and may be of long-term concern for hazard assessment activities.     BKM Lie superalgebras from counting twisted CHL dyons

    Following Sen[arXiv:0911.1563], we study the counting of (`twisted') BPS states that contribute to twisted helicity trace indices in four-dimensional CHL models with N=4 supersymmetry. The generating functions of half-BPS states, twisted as well as untwisted, are given in terms of multiplicative eta products with the Mathieu group, M_{24}, playing an important role. These multiplicative eta products enable us to construct Siegel modular forms that count twisted quarter-BPS states. The square-roots of these Siegel modular forms turn out be precisely a special class of Siegel modular forms, the dd-modular forms, that have been classified by Clery and Gritsenko[arXiv:0812.3962]. We show that each one of these dd-modular forms arise as the Weyl-Kac-Borcherds denominator formula of a rank-three Borcherds-Kac-Moody Lie superalgebra. The walls of the Weyl chamber are in one-to-one correspondence with the walls of marginal stability in the corresponding CHL model for twisted dyons as well as untwisted ones.     Approaching Zero-Temperature Metallic States in Mesoscopic Superconductor-Normal-Superconductor Arrays

    Systems of superconducting islands placed on normal metal films offer tunable realizations of two-dimensional (2D) superconductivity; they can thus elucidate open questions regarding the nature of 2D superconductors and competing states. In particular, island systems have been predicted to exhibit zero-temperature metallic states. Although evidence exists for such metallic states in some 2D systems, their character is not well understood: the conventional theory of metals cannot explain them, and their properties are difficult to tune. Here, we characterize the superconducting transitions in mesoscopic island-array systems as a function of island thickness and spacing. We observe two transitions in the progression to superconductivity; both transition temperatures exhibit unexpectedly strong depression for widely spaced islands. These depressions are consistent with the system approaching zero-temperature metallic states. The nature of the transitions and the state between them is explained using a phenomenological model involving the stabilization of superconductivity on each island via a weak coupling to and feedback from its neighbors.Comment: 15 pages, 5 figure

    Translating research into practice in Leeds and Bradford (TRiPLaB): a protocol for a programme of research

    Background The National Institute for Health Research (NIHR) has funded nine Collaborations for Leadership in Applied Health Research and Care (CLAHRCs). Each CLAHRC is a partnership between higher education institutions (HEIs) and the NHS in nine UK regional health economies. The CLAHRC for Leeds, York, and Bradford comprises two 'research themes' and three 'implementation themes.' One of these implementation themes is Translating Research into Practice in Leeds and Bradford (TRiPLaB). TRiPLaB aims to develop, implement, and evaluate methods for inducing and sustaining the uptake of research knowledge into practice in order to improve the quality of health services for the people of Leeds and Bradford. Methods TRiPLaB is built around a three-stage, sequential, approach using separate, longitudinal case studies conducted with collaborating NHS organisations, TRiPLaB will select robust innovations to implement, conduct a theory-informed exploration of the local context using a variety of data collection and analytic methods, and synthesise the information collected to identify the key factors influencing the uptake and adoption of targeted innovations. This synthesis will inform the development of tailored, multifaceted, interventions designed to increase the translation of research findings into practice. Mixed research methods, including time series analysis, quasi-experimental comparison, and qualitative process evaluation, will be used to evaluate the impact of the implementation strategies deployed. Conclusion TRiPLaB is a theory-informed, systematic, mixed methods approach to developing and evaluating tailored implementation strategies aimed at increasing the translation of research-based findings into practice in one UK health economy. Through active collaboration with its local NHS, TRiPLaB aims to improve the quality of health services for the people of Leeds and Bradford and to contribute to research knowledge regarding the interaction between context and adoption behaviour in health services

    Comparison of Influenza and SIV Specific CD8 T Cell Responses in Macaques

    Macaques are a potentially useful non-human primate model to compare memory T-cell immunity to acute virus pathogens such as influenza virus and effector T-cell responses to chronic viral pathogens such as SIV. However, immunological reagents to study influenza CD8+ T-cell responses in the macaque model are limited. We recently developed an influenza-SIV vaccination model of pigtail macaques (Macaca nemestrina) and used this to study both influenza-specific and SIV-specific CD8+ T-cells in 39 pigtail macaques expressing the common Mane-A*10+ (Mane-A01*084) MHC-I allele. To perform comparative studies between influenza and SIV responses a common influenza nucleoprotein-specific CD8+ T-cell response was mapped to a minimal epitope (termed RA9), MHC-restricted to Mane-A*10 and an MHC tetramer developed to study this response. Influenza-specific memory CD8+ T-cell response maintained a highly functional profile in terms of multitude of effector molecule expression (CD107a, IFN-γ, TNF-α, MIP-1β and IL-2) and showed high avidity even in the setting of SIV infection. In contrast, within weeks following active SIV infection, SIV-specific CD8+ effector T-cells expressed fewer cytokines/degranulation markers and had a lower avidity compared to influenza specific CD8+ T-cells. Further, the influenza specific memory CD8 T-cell response retained stable expression of the exhaustion marker programmed death-marker-1 (PD-1) and co-stimulatory molecule CD28 following infection with SIV. This contrasted with the effector SIV-specific CD8+ T-cells following SIV infection which expressed significantly higher amounts of PD-1 and lower amounts of CD28. Our results suggest that strategies to maintain a more functional CD8+ T-cell response, profile may assist in controlling HIV disease

    Antihypertensive medication prescription patterns and time trends for newly-diagnosed uncomplicated hypertension patients in Taiwan

    <p>Abstract</p> <p>Background</p> <p>Knowledge of existing prescription patterns in the treatment of newly-diagnosed hypertension can provide useful information for improving clinical practice in this field. The aims of this study are to determine the prescription patterns and time trends for antihypertensive medication in newly-diagnosed cases of uncomplicated hypertension in Taiwan and to compare these with current clinical guidelines.</p> <p>Methods</p> <p>A total of 6,536 newly-diagnosed patients with uncomplicated hypertension, aged ≥30 years, were identified from the representative 200,000-person sample in the computerized reimbursement database of the National Health Insurance in Taiwan. These patients were followed from 1998 to 2004 with all diagnoses, prescription data and medication charges being retrieved for subsequent analysis.</p> <p>Results</p> <p>Prescription patterns varied by age, gender and clinical facilities, with mono-therapies being found to be dominant in the first year, albeit declining over time. Calcium channel blockers and beta-blockers were the most frequently prescribed antihypertensive drugs, either alone or in combinations. Although least expensive, the prescription rates of diuretics were low, at 8.3% for mono-therapies and 19.9% overall. The prescription rate for angiotensin receptor blockers (ARBs) was elevated considerably over time. After controlling for other related factors by multiple logistic regression analysis, ARBs were found to be prescribed mainly by medical centers or regional hospitals.</p> <p>Conclusion</p> <p>These findings indicate the existence of a gap between current clinical practice and the desired goal of cost-effectiveness in antihypertensive treatment in Taiwan, which should be corrected.</p

    Different distribution of cardiovascular risk factors according to ethnicity: A study in a high risk population

    This study compares the distribution of cardiovascular risk factors in different ethnic groups at high risk of developing cardiovascular diseases within general practices. A total of 430 patients (179 Dutch, 126 Turks, 50 Surinamese, 23 Moroccans, 23 Antilleans and 29 from other ethnic groups) were included in the study. Data collection consisted of questionnaires and physical and clinical examinations. 54% was female. The mean age was 53.1 (sd 9.9) years. There were important ethnic differences in the distribution of cardiovascular risk factors. Compared to the Dutch, ethnic minorities had significantly greater odds of being diabetic (OR = 3.2-19.4); but were less likely to smoke (OR = 0.10-0.53). Turkish individuals had a lower prevalence of hypercholesterolemia but were 2.4 times more likely to be obese than the Dutch. Hypertension was very common in all ethnic groups and no significant ethnic differences were found. These findings provide additional evidence of the need for tailored interventions for different ethnic groups in general practices
