    A meta-analysis investigating the correlation between treatment integrity and youth client outcomes

    Objective: The relation between treatment integrity and youth client outcomes in psychosocial treatment research has implications for therapist training, study design, and study interpretation. Despite work done in this area, this relation remains unclear. The aim of the current study was to meta-analyze and describe characteristics of investigations of integrity and outcome in youth. Method and Analytic Plan: A total of N = 30 studies were identified. Conceptual and methodological factors were coded. Pearsonā€™s r represented the magnitude of the integrity-outcome correlation. Robust variance estimation was used to account for dependency of within-study effect sizes. Two models were run with identical methods, Model 1 did not impute/estimate effect sizes and Model 2 imputed/estimated missing effect sizes. Each model was built iteratively, including an unconditional model and moderator model. Tests of heterogeneity, publication bias, and sensitivity analyses were conducted. Results: The mean integrity-outcome effect size in Model 1 and Model 2 were negative and statistically significant (r = -.11, p \u3c .0001; r = -.09, p \u3c .0001, respectively). Treatment integrity component and quality of treatment integrity procedures did not moderate the integrity-outcome correlation. Publication bias revealed some trim-and-fill analyses, indicating the possibility of publication bias. Outliers did not play a role in study findings. Discussion: There appears to be a small correlation between treatment integrity and outcome. A small sample and inconsistent reporting practices made additional moderation analyses inadvisable. Potential explanations for findings were explored. Recommendations are provided, including consistent reporting standards and sophisticated research paradigms for therapy process research

    Sticking to the recipe: How do adherence and differentiation to a CBT protocol affect client outcomes in youths with anxiety?

    Objective: Understanding the pathways through which treatments work to change symptom and diagnostic outcomes is important to the development and delivery of evidence-based treatments. This study assessed the extent to which adherence (therapistā€™s delivery of prescribed therapeutic interventions) and differentiation (therapistā€™s delivery of non-prescribed therapeutic interventions) to Coping Cat, a CBT program, affected client symptom and diagnostic outcomes. Method: The Therapy Process Observational Coding System for Child Psychotherapy ā€“ Revised Strategies Scale (McLeod et al., 2015) was used to characterize therapeutic interventions delivered within and outside of the Coping Cat program with youths aged 7-15 receiving treatment in one efficacy (n = 51; 41% female; 84% Caucasian, M age = 10.37) and one effectiveness (n = 17; 56% female, 39% Caucasian, M age = 10.90) trial. Youth- and parent-report symptom checklists and diagnostic interviews were used to assess symptom and diagnostic remission. Multiple hierarchical regression analyses and hierarchical binomial logistic regression were used to investigate the relation between adherence and differentiation and symptom change and remission of principal diagnosis. Results: Neither adherence nor differentiation were significantly related to symptom or diagnostic outcomes. No clear trend emerged, and results were inconsistent across parent and youth report, outcome type, and setting. Conclusion: These results are consistent with past literature. Two interpretations exist: (1) that there is no relation between treatment delivery and outcomes, and (2) that methodological and analytic flaws undercut the ability of the analyses to identify a relation

    Closed-loop feedback control for microfluidic systems through automated capacitive fluid height sensing

    Precise fluid height sensing in open-channel microfluidics has long been a desirable feature for a wide range of applications. However, performing accurate measurements of the fluid level in small-scale reservoirs (<1 mL) has proven to be an elusive goal, especially if direct fluid-sensor contact needs to be avoided. In particular, gravity-driven systems used in several microfluidic applications to establish pressure gradients and impose flow remain open-loop and largely unmonitored due to these sensing limitations. Here we present an optimized self-shielded coplanar capacitive sensor design and automated control system to provide submillimeter fluid-height resolution (āˆ¼250 Ī¼m) and control of small-scale open reservoirs without the need for direct fluid contact. Results from testing and validation of our optimized sensor and system also suggest that accurate fluid height information can be used to robustly characterize, calibrate and dynamically control a range of microfluidic systems with complex pumping mechanisms, even in cell culture conditions. Capacitive sensing technology provides a scalable and cost-effective way to enable continuous monitoring and closed-loop feedback control of fluid volumes in small-scale gravity-dominated wells in a variety of microfluidic applications.United States.     Instrumentation issues in implementation science

    Abstract Background Like many new fields, implementation science has become vulnerable to instrumentation issues that potentially threaten the strength of the developing knowledge base. For instance, many implementation studies report findings based on instruments that do not have established psychometric properties. This article aims to review six pressing instrumentation issues, discuss the impact of these issues on the field, and provide practical recommendations. Discussion This debate centers on the impact of the following instrumentation issues: use of frameworks, theories, and models; role of psychometric properties; use of ā€˜home-grownā€™ and adapted instruments; choosing the most appropriate evaluation method and approach; practicality; and need for decision-making tools. Practical recommendations include: use of consensus definitions for key implementation constructs; reporting standards (e.g., regarding psychometrics, instrument adaptation); when to use multiple forms of observation and mixed methods; and accessing instrument repositories and decision aid tools. Summary This debate provides an overview of six key instrumentation issues and offers several courses of action to limit the impact of these issues on the field. With careful attention to these issues, the field of implementation science can potentially move forward at the rapid pace that is respectfully demanded by community stakeholders

    The Society for Implementation Research Collaboration Instrument Review Project: A methodology to promote rigorous evaluation

    Abstract Background Identification of psychometrically strong instruments for the field of implementation science is a high priority underscored in a recent National Institutes of Health working meeting (October 2013). Existing instrument reviews are limited in scope, methods, and findings. The Society for Implementation Research Collaboration Instrument Review Projectā€™s objectives address these limitations by identifying and applying a unique methodology to conduct a systematic and comprehensive review of quantitative instruments assessing constructs delineated in two of the fieldā€™s most widely used frameworks, adopt a systematic search process (using standard search strings), and engage an international team of experts to assess the full range of psychometric criteria (reliability, construct and criterion validity). Although this work focuses on implementation of psychosocial interventions in mental health and health-care settings, the methodology and results will likely be useful across a broad spectrum of settings. This effort has culminated in a centralized online open-access repository of instruments depicting graphical head-to-head comparisons of their psychometric properties. This article describes the methodology and preliminary outcomes. Methods The seven stages of the review, synthesis, and evaluation methodology include (1) setting the scope for the review, (2) identifying frameworks to organize and complete the review, (3) generating a search protocol for the literature review of constructs, (4) literature review of specific instruments, (5) development of an evidence-based assessment rating criteria, (6) data extraction and rating instrument quality by a task force of implementation experts to inform knowledge synthesis, and (7) the creation of a website repository. Results To date, this multi-faceted and collaborative search and synthesis methodology has identified over 420 instruments related to 34 constructs (total 48 including subconstructs) that are relevant to implementation science. Despite numerous constructs having greater than 20 available instruments, which implies saturation, preliminary results suggest that few instruments stem from gold standard development procedures. We anticipate identifying few high-quality, psychometrically sound instruments once our evidence-based assessment rating criteria have been applied. Conclusions The results of this methodology may enhance the rigor of implementation science evaluations by systematically facilitating access to psychometrically validated instruments and identifying where further instrument development is needed

The Society for Implementation Research Collaboration (SIRC) Instrument Review Project employed an enhanced systematic review methodology (Implement Sci 2: 2015) to identify quantitative instruments of implementation outcomes relevant to mental or behavioral health settings. Methods Full details of the enhanced systematic review methodology are available (Implement Sci 2: 2015). To increase the feasibility of the review, and consistent with the scope of SIRC, only instruments that were applicable to mental or behavioral health were included. The review, synthesis, and evaluation included the following: (1) a search protocol for the literature review of constructs; (2) the literature review of instruments using Web of Science and PsycINFO; and (3) data extraction and instrument quality ratings to inform knowledge synthesis. Our evidence-based assessment rating criteria quantified fundamental psychometric properties as well as a crude measure of usability. Two independent raters applied the evidence-based assessment rating criteria to each instrument to generate a quality profile. Results We identified 104 instruments across eight constructs, with nearly half (nā€‰=ā€‰50) assessing acceptability and 19 identified for adoption, with all other implementation outcomes revealing fewer than 10 instruments. Only one instrument demonstrated at least minimal evidence for psychometric strength on all six of the evidence-based assessment criteria. The majority of instruments had no information regarding responsiveness or predictive validity. Conclusions Implementation outcomes instrumentation is underdeveloped with respect to both the sheer number of available instruments and the psychometric quality of existing instruments. Until psychometric strength is established, the field will struggle to identify which implementation strategies work best, for which organizations, and under what conditions

    Optical polarization observations of NGC 6231: evidence for a past SN fingerprint

    We present the first linear multicolor polarization observations for a sample of 35 stars in the direction of the Galactic cluster NGC 6231. We have found a complex pattern in the angles of the polarimetric vectors. Near the core of this cluster the structure shows a semi-circular pattern that we have interpreted as a re-orientation of the dust particles, showing the morphology of the magnetic field. We propose that a supernova event has occurred some time ago and produced a shock on the local ISM. We discuss in this paper independent confirmations othis event, both from the studies on the diffuse interstellar absorptions Crawford (2001) and the results of the pre-main sequence stars (PMS) given by Sung et al.     SummaryObesity and type 2 diabetes have a heritable component that is not attributable to genetic factors. Instead, epigenetic mechanisms may play a role. We have developed a mouse model of intrauterine growth restriction (IUGR) by in utero malnutrition. IUGR mice developed obesity and glucose intolerance with aging. Strikingly, offspring of IUGR male mice also developed glucose intolerance. Here, we show that in utero malnutrition of F1 males influenced the expression of lipogenic genes in livers of F2 mice, partly due to altered expression of Lxra. In turn, Lxra expression is attributed to altered DNA methylation of its 5ā€² UTR region. We found the same epigenetic signature in the sperm of their progenitors, F1 males. Our data indicate that in utero malnutrition results in epigenetic modifications in germ cells (F1) that are subsequently transmitted and maintained in somatic cells of the F2, thereby influencing health and disease risk of the offspring
