Clinical evolution of asthma from early childhood to development age
The aim of study was to conduct evaluation over twenty years on patients with relapsing wheezing breathing since their early years of life. The research included a period of observation started in 1978 up to 2002. The follow-up included 381 patients. The enrolment was carried out during the first 15 years, after being evaluated during outpatient or/ and hospitalization the children with a clinical picture resembling asthma (at least three episodes of dyspnea for a year, at least 2 years prior to our first observation). During follow-up the rhole of some risk factors in childhood able to affect the persistency of asthma in adulthood was considered. An early onset of symptoms resulted to be a favourable prognostic sign. At the end of study 294 (77.16%) patients at 18 year of age already had a remission of the disease. Patients with normal IgE levels and negative Rast and Prick skin tests presented a higher clinical remission: the rate were statistically significant. A symptomatic and preventive therapy practised regularly resulted to be a favourable factor for remission. The severity of symptoms result to be an unfavourable prognostic factor. In conclusion, if asthma is recognized precocity can mitigate and remove symptoms and so improved adulthood lifestyle
Newborn hearing-screening project using transient evoked otoacoustic emissions: western sicily experience
Abstract
OBJECTIVE:
To study the incidence of congenital sensorineural hearing loss in all newborns introducing a screen test with a protocol no expensive, with a good "screen sensitivity" that could let an earlier identification of hearing impairment beginning early intervention by 2 months of age and increasing the probability of having language development within the normal range of development.
METHODS:
The study was conducted in Sciacca hospital from the beginning of 2003 to our days and was carried out with transient evoked otoacoustic emission using the criteria for PASS or RETEST and considering eventual prenatal and perinatal risk factors. All the newborns were divided into four groups each one with its personal secondary step program.
RESULTS:
In the years 2003-2004 the number of the newborns in Sciacca hospital was: 538 for 2003, 653 for 2004 with a total of 1191; all these infants were divided in three groups: resident in Sciacca, resident in the Sciacca borderlands and resident out of the district of Sciacca. The coverage (percentage of the target population who undergo the screen) was of 90% in the 2003 (483 newborns) and of 90% in the 2004 (585 newborns) with two cases of congenital sensorineural hearing loss identified. The incidence of sensorineural hearing loss, in the District of Sciacca, was estimated to be 2.07/1000 in 2003 and 1.70/1000 in 2004.
CONCLUSIONS:
The higher incidence of sensorineural hearing loss in our study is due to a high prevalence of consanguineous marriage in Sicily that was shown to be linked with hearing impairment. The "sensitivity value" was 95% at the first step but became 99% after the second step with a few number of false positive (0.74%). All the infants with a diagnosis of sensorineural hearing loss began a rehabilitation program before the age of 5 months and they have a good speech development and speech intelligibility
Sudden Sensorineural Hearing Loss as Prodromal Symptom of Anterior Inferior Cerebellar Artery Infarction
Sudden sensorineural hearing loss is a clinical condition characterized by a sudden onset of unilateral or bilateral hearing loss. In recent years sudden deafness has been frequently described in association with anterior inferior cerebellar artery (AICA) infarction generally presenting along
with other brainstem and cerebellar signs such as ataxia, dysmetria and peripheral facial palsy. The authors report a rare clinical case of a 53-year-old man who suddenly developed hearing loss and tinnitus without any brainstem or cerebellar signs. Computed tomography of his brain was normal, and the audiological results localized the lesion causing deafness to the inner ear. Surprisingly, magnetic resonance imaging
showed an ischemic infarct in the right AICA territory. This case represents the fifth in the literature to date but it confirms
that AICA occlusion can cause sudden deafness even without brainstem or cerebellar signs. Therefore, we recommend
submitting the patient for neuroimaging, as an emergency, in order to exclude infarction of the AICA territory. By doing this, it may be possible to limit the extent of the lesion
by commencing early therapy
“SINDROME DI BECHET, CELIACHIA E DIABETE MELLITO DI I TIPO”: ....RARO CASO A TIPIZZAZIONE ANOMALA
La malattia di Behcet è una malattia rara, a genesi autoimmune, multifattoriale ad esordio tra la seconda e la terza decade di vita, a più elevata incidenza nei cosiddetti territori della via della seta (Medio Oriente) oltre che, nel Bacino del Mediterraneo. In Europa la prevalenza è 1:500.000 con maggiore incidenza nel sesso femminile.
Clinicamente è caratterizzata da lesioni ulcerose ricorrenti del cavo orale che si accompagnano a ulcere genitali o lesioni cutanee, oculari o Pathergy Test positivo . Il "Behcet's Disease International Study Group" ha identificato nella presenza di ulcere del cavo orale associate a due delle lesioni sovracitate la diagnosi clinica di malattia di Behcet.
Nel 60-70% dei casi è descritta un’associazione con il fenotipo HLA-B51.
Nonostante si tratti di patologia su base autoimmunitaria, difficilmente si associa ad altre manifestazioni cliniche quali Morbo celiaco, Diabete, Crohn et al.
Gli autori descrivono un caso clinico di una ragazza di 24 anni già affetta da diabete mellito e celiachia, giunta alla nostra osservazione per lesioni ulcero-necrotiche a carico della tonsilla palatina di sinistra, da circa 20 giorni. L’ipotesi diagnostica iniziale di malattia di Behcet era stata scartata per l'assenza di sintomi o segni associati, nonchè per la tipizzazione del sistema HLA risultata positiva per l’MHC-II B-63.
La consulenza reumatologica confermava una diagnosi di morbo celiaco poiché le manifestazioni orali risultavano compatibili con una fase evolutiva dello stesso (positività nel 25% dei pazienti con morbo celiaco).
La progressione dei fenomeni ulcerotici a carico della tonsilla omolaterale e la comparsa di manifestazioni analoghe a carico della tonsilla controlaterale, in una fase di apparente quiescenza di celiachia, ha indotto gli autori ad ipotizzare la presenza di una Malattia di Behcet, sebbene questa si associ ad altre malattie autoimmuni solamente nel 2-4% dei casi. Esclusivamente la biopsia escissionale e la comparsa a distanza di mesi, di lesioni genitali, ha permesso di porre diagnosi di certezza di una rara associazione, la quarta ad oggi descritta in letteratura, di Diabete Mellito di I tipo, Malattia Celiaca e Morbo di Behcet a tipizzazione atipica
Tinnitus e Sordità
OBIETTIVI
Con il termine di "Acufene" o "Tinnitus" definiamo la percezione di un rumore in assenza di qualunque
sorgente sonora esterna al nostro organismo. La presenza di acufeni aumenta progressivamente con l'età
(12% dopo i 60 anni), e questo, secondo gli Autori, non è tanto correlato con la senescenza quanto con la
concomitante sordità, la cui prevalenza aumenta con l'età. L'obiettivo di questo studio è quello di mettere
in evidenza la correlazione fra il tipo di sordità e il tipo di acufene.
MATERIALI
Lo studio è stato condotto dalla Sezione di Audiologia del Dipartimento di Bio‐technologie dell'Università di
Palermo su 197 soggetti affetti da acufene e sordità. I pazienti sono stati sottoposti ad indagini audiologiche
che comprendevano: visita audiologica, esame audiometrico,impedenzometria ed acufenometria. In base
al tipo di sordità è stata individuata una sordità di trasmissione, una sordità neurosensoriale (limitata alle
alte frequenze:4‐8KHz; limitata alle medie frequenze:1,2,3 KHz; limitata alle basse frequenze:0,25‐0,5 KHz;
pantonale) ed una sordità di tipo misto. L'acufenometria ha rilevato la frequenza del tinnitus.
RIASSUNTO
Sono stati esaminati 197 pazienti con sordità; nel 10,66% la sordità era indentificata come trasmissiva, nel
74,62% come neurosensoriale e nel 14,72% di tipo misto. La perdita uditiva era limitata alle alte frequenze
nel 58,50%, alle basse frequenze nel 11.56% e pantonale nel 29.93% dei casi. È stata rilevata una sordità
lieve nel 58,88% dei casi, moderata nel 22,34%, severa nel 15,74 e profonda nel 3,05%. La frequenza
dell’acufene, misurata con l'acufenometria, era calibrata alle alte frequenze nel 61,42% dei casi, alle medie
frequenze nel 22,84%, alle basse frequenze nel 12,69% mentre è risultata variabile e non identificabile nel
3,04%. Dei 147 pazienti con sordità neurosensoriale il 72,10% avevano un acufene tarato alle alte
frequenze mentre dei pazienti con sordità neurosensoriale limitata alle alte frequenze il 88,37% avevano un
acufene tarato alle alte frequenze.
CONCLUSIONI
In questo studio, condotto su un totale di 197 pazienti, la perdita uditiva si è rilevata con una netta
prevalenza di tipo neurosensoriale (74,62%), sostenendo le attuali teorie per cui una riduzione della
funzionalità cocleare rappresenta la causa più comune di tinnitus. La sordità neurosensoriale, nella
maggioranza dei casi, era limitata alle alte frequenze (58,50%), questi risultati dimostrano che esiste una
correlazione tra tinnitus a tonalità acuta e sordità neurosensoriale limitata alle alte frequenze, suggerendo
che la riorganizzazione delle vie uditive, indotto dalla perdita uditiva, potrebbe essere una delle principali
cause del sintomo tinnitus
Infarto acuto dell'AICA e sordità improvvisa
OBIETTIVI
Gli autori descrivono un caso clinico, uomo adulto di 57 anni, di sordità improvvisa come sola
manifestazione di infarto della arteria cerebellare antero‐inferiore (AICA) di destra in presenza di un quadro
radiologico da interessamento della regione pontina di destra la cui valutazione radiologica, eseguita in
urgenza ha permesso l’identificazione del quadro.
MATERIALI
Il colpo ischemico acuto nella distribuzione dell’AICA conosciuto come “Sindrome AICA” è un quadro clinico
patologico che generalmente è accompagnato da una sintomatologia mista di tipo otologica e neurologica;
si caratterizza infatti per: sordità improvvisa, vertigini di tipo periferiche e/o centrali da irritazione acuta
con nistagmo ed atassia omolaterale con lateropulsione. Il coinvolgimento pontino inoltre si traduce con un
deficit a carico del nervo facciale (debolezza facciale) e del trigemino. Anatomicamente l’AICA nel 75% dei
casi nasce dal terzo caudale della arteria basilare e vascolarizza la regione anteromediale del cervelletto, la
porzione laterale del ponte, l’emergenza del V°, del VII°, dell’VIII° nervo cranico ed il tratto spinotalamico.
La sofferenza ischemica delle strutture del condotto uditivo interno e dell’orecchio interno nella ‘Sindrome
AICA’ sono legate al deficit dell’arteria uditiva interna che, trattandosi nel 90% dei casi di un ramo diretto
dell’AICA, è la diretta responsabile della sintomatologia labirintica. L’ipotesi diagnostica di Sindrome AICA
scaturisce dalla presenza contemporanea di sintomi ot‐neurologici che inducono lo specialista ad avvalersi
di TC e RMN (SE T2W; DWI; FLAIR) in urgenza.
RIASSUNTO
La presenza in corso di infarto dell’AICA della sola sordità improvvisa con caratteristiche cocleari è rara oltre
che di difficile reperimento per la mancata effettuazione della RMN ma comunque deve essere sempre
tenuta in considerazione.
CONCLUSIONI
La diagnosi eziologia di sordità improvvisa da ‘Sindrome AICA’ è eseguita solamente tramite l’ausilio di RMN
eseguita in regime di urgenza per cui gli autori suggeriscono fortemente l’inserimento di RMN in urgenza
tra le indagini diagnostiche di sordità improvvisa
Speech perception outcomes after cochlear implantation in prelingually deaf infants: The Western Sicily experience
Objectives: To describe audiometric characteristics and speech perception performances of prelingually deaf Sicilian children after cochlear implantation; to identify the influence of cochlear implant (CI) user and family's characteristics on speech recognition and intelligibility outcomes. Methods: Twenty-eight infants with a congenital or acquired hearing impairment and implanted before the 3rd year of life were studied; all children suffered from bilateral sensorineural hearing loss (SNHL) with evidence of lack of hearing aids benefit and no evidence of intellectual disability. The study of the main characteristics associated with CI user and family's profile was performed with a clinical assessment including pre-implant and post-implant (1, 3, 6, 12 and 18 months) behavioural audiometry (evaluating average threshold for the frequencies 0.5, 1, 2 and 4 KHz) and speech recognition tests (IT-MAIS, MUSS, CAP and SIR). Results: Our cohort was characterized by an early diagnosis of SNHL (5.77 and 12.17 months for congenital and acquired HL respectively), a short length of deafness (average = 6.78 months) and an implantation before the 3rd year of life (mean = 24.25 months; range from 10 to 36). Analysis of audiometric threshold revealed a significantly improved capacity to detect sounds within the conversational speech spectrum after 12 months from implantation (r = 0.99; p < 0.001). The main speech recognition test evidenced speech perception and speech intelligibility performances (CAP median value of 3; SIR category = 3 in 46.42%) equal to those children with same characteristics reported by literature. With the exception of 'daily CI use' (p < 0.001), none of the variables associated with CI user and family's profile resulted significant predictor of speech perception improvement. Conclusions: This work demonstrates that all children of our cohort, with an early diagnosis of SNHL and a CI surgery performed before the 3rd year of life, presented a progressive audiometric and speech improvement through the first 12-18 months after cochlear implantation. The study also highlights that, differently from the others variables studied, a continuous CI use influences significantly speech perception and recognition outcomes
Audiological Investigation of Otitis Media in Children with Atopy
Otitis media (OM) describes an inflammatory process within the middle ear space that is generally associated with accumulation of fluid and which may lead to hearing loss, learning difficulties and delay of language development. The pathogenesis of OM is multifactorial, involving the adaptive and native immune system, Eustachian tube dysfunction, viral and bacterial load, genetic and environmental factors. Surgical Management of Retraction Pockets: Does Mastoidectomy have a Role?
Abstract
Introduction Retraction pocket is a condition in which the eardrum lies deeper within the middle ear. Its management has no consensus in literature.
Objective To assess the role of mastoidectomy in the management of retraction pockets added to a tympanoplasty.
Methods Prospective study of patients with retraction pocket and referred to surgery. The patients were randomly assigned to two groups: one managed with tympanoplasty and mastoidectomy and the other group with tympanoplasty only. The minimum follow-up considered was 12 months. The outcomes were: integrity of eardrum, recurrence, and hearing status.
Results This study included 43 patients. In 24 cases retraction occurred in the posterior half of the eardrum, and in 19 patients there was clinical evidence of ossicular interruption. The two groups of treatment were composed by: 21 patients that underwent tympanoplasty with mastoidectomy and 22 patients had only tympanoplasty. One case of the first group had a recurrence. In 32 cases patients follow up was longer than 48 months. The average air-bone gap changed from 22.1 dB to 5 dB. The percentage of air-bone gap improvement was assessed at 60% in those patients treated with mastoidectomy, and 64.3% in those without it (p > 0.5).
Conclusion Tympanoplasty and ossiculoplasty should be considered to treat atelectatic middle ear and ossicular chain interruption. Mastoidectomy as a way to increase air volume in the ear seems to be a paradox; it does not add favorable prognostic factor to management of retraction pockets
