    Success factors and barriers for continuous quality improvement in acute hospital care

    The aim of this study was to examine success factors and barriers for continuous quality improvement in acute hospital care. Background In spite the improvements and developments in healthcare the last fifty years, adverse events and lack of quality remain a problem. In 2012, the Swedish National Board of Health and Welfare increased the requirement about continuous quality improvement in the revised regulations. Several reports still indicate insufficiencies in continuous quality improvement. Method Interviews with six operational managers from three different hospitals for acute care, in the central of Sweden, were conducted. The interviews were analyzed with conventional qualitative content analysis. Results The results show that success factors and barriers mainly involves three themes 1) Need of integrated support 2) Need of value based leadership and 3) The need of courage Develop integrated support and standardized methods for continuous quality improvement, processes, result follow-up, visualization and operational support. Practice value based leadership that aim for patient related outcomes. Courage at each organizational level is needed to improve possibilities for learning and improvement.Syftet var att undersöka vilka framgÄngsfaktorer respektive svÄrigheter som pÄverkar ett systematiskt kvalitetsarbete inom akutsjukvÄrden Bakgrund Trots hÀlso- och sjukvÄrdens framsteg och utveckling de senaste femtio Ären, Àr vÄrdskador och kvalitetsbrister fortfarande ett problem. I samband med Socialstyrelsens reviderade föreskrift om ledningssystem för systematiskt kvalitetsarbete (2012) skÀrptes dessa krav pÄ hÀlso- och sjukvÄrden. Flera myndighetsrapporter pekar dock pÄ att det fortfarande finns brister i det systematiska kvalitetsarbetet. Metod Studien bygger pÄ intervjuer med sex verksamhetschefer pÄ tre olika akutsjukhus i Mellansverige. En kvalitativ intervjumetod med en induktiv ansats har anvÀnts och dÀrefter har en konventionell innehÄllsanalys genomförts. Resultat Studien visar att framgÄngsfaktorer och svÄrigheter frÀmst berörde de tre teman 1) Behov av integrerat stöd 2) Behov av vÀrderingsstyrt ledarskap och 3) Behov av mod. Utveckla enkla stödstrukturer med enhetliga/standardiserade metoder för kvalitetsarbete, processorientering, resultatuppföljning, visualisering och operativt stöd för systematiskt kvalitetsarbete. TillÀmpa vÀrderingsstyrt ledarskap med resultatstyrd vÄrd som utgÄr frÄn patientens perspektiv. Det behövs mod pÄ alla nivÄer i organisationen för att förbÀttra möjligheterna för lÀrande och utveckling

    "Der vorliegende Beitrag stellt die Probleme der Vorbereitung und DurchfĂŒhrung einer internationalen Vergleichsstudie im Bereich der beruflichen Bildung als Large-Scale-Assessment dar. Dabei werden die Unterschiede der Anlage einer solchen Studie zu den Prinzipien herausgestellt, die zum Beispiel fĂŒr PISA-Studien gelten. Im Zentrum steht die Frage der Strukturierung und Operationalisierung beruflicher Handlungskompetenzen unter den Rahmenbedingungen von Berufsfeldern und Ausbildungskonstellationen. Im Hinblick auf die Erfassung der LeistungsfĂ€higkeit unterschiedlicher Berufsbildungssysteme geht es vor allem um Fragen der Messung beruflicher Handlungskompetenz, um die Erfassung der relevanten Rahmenbedingungen sowie um die Identifikation von international vergleichbaren Berufsfeldern und ErwerbstĂ€tigkeiten.     Normal surface singularities with an integral homology sphere link related to space monomial curves with a plane semigroup

    In this article, we consider an infinite family of normal surface singularities with an integral homology sphere link which is related to the family of space monomial curves with a plane semigroup. These monomial curves appear as the special fibers of equisingular families of curves whose generic fibers are a complex plane branch, and the related surface singularities appear in a proof of the monodromy conjecture for these curves. To investigate whether the link of a normal surface singularity is an integral homology sphere, one can use a characterization that depends on the determinant of the intersection matrix of a (partial) resolution. To study our family, we apply this characterization with a partial toric resolution of our singularities constructed as a sequence of weighted blow-ups

    Designing Activity-Based and Context-Sensitive Ambient Sound Environments in Open-Plan Offices

    This paper addresses the problem of sound disturbance in open office environments. We have in a design-based research study explored how digital, real time generated sound can be added to a work environment and how these sound environments are perceived by respondents when performing work tasks. In this first explorative study we have chosen to focus on designing a digital sound system for activity-based offices, where the physical environment is already designed for particular activities. Our approach is to explore if adding appropriate acoustic designs to the ambient environment can enhance workplaces. Our results show that test subjects perceived that acoustic design could enhance the ambient environments if the acoustic design is pertinent with the environment as a whole

    Moving Usability Testing onto the Web

    Abstract: In order to remotely obtain detailed usability data by tracking user behaviors within a given web site, a server-based usability testing environment has been created. Web pages are annotated in such a way that arbitrary user actions (such as "mouse over link" or "click back button") can be selected for logging. In addition, the system allows the experiment designer to interleave interactive questions into the usability evaluation, which for instance could be triggered by a particular sequence of actions. The system works in conjunction with clustering and visualization algorithms that can be applied to the resulting log file data. A first version of the system has been used successfully to carry out a web usability evaluation
