Clinical Results of a Brindley Procedure: Sacral Anterior Root Stimulation in Combination with a Rhizotomy of the Dorsal Roots
The Brindley procedure consists of a stimulator for sacral anterior-root stimulation and a rhizotomy of the dorsal sacral roots to abolish neurogenic detrusor overactivity. Stimulation of the sacral anterior roots enables micturition, defecation, and erections. This overview discusses the technique, selection of patients and clinical results of the Brindley procedure. The Brindley procedure is suitable for a selected group of patients with complete spinal cord injury and detrusor overactivity. Overall, the Brindley procedure shows good clinical results and improves quality of life. However, to remain a valuable treatment option for the future, the technique needs some adequate changes to enable analysis of the implanted parts, to improve revision techniques of the implanted parts, and to abolish the sacral dorsal rhizotomy
Development and operation of a pixel segmented liquid-filled linear array for radiotherapy quality assurance
A liquid isooctane (CH) filled ionization linear array for
radiotherapy quality assurance has been designed, built and tested. The
detector consists of 128 pixels, each of them with an area of 1.7 mm
1.7 mm and a gap of 0.5 mm. The small pixel size makes the detector ideal for
high gradient beam profiles like those present in Intensity Modulated Radiation
Therapy (IMRT) and radiosurgery. As read-out electronics we use the X-Ray Data
Acquisition System (XDAS) with the Xchip developed by the CCLRC.
Studies concerning the collection efficiency dependence on the polarization
voltage and on the dose rate have been made in order to optimize the device
operation.
In the first tests we have studied dose rate and energy dependences, and
signal reproducibility. Dose rate dependence was found lower than 2.5 % up to 5
Gy min, and energy dependence lower than 2.1 % up to 20 cm depth in
solid water. Output factors and penumbras for several rectangular fields have
been measured with the linear array and were compared with the results obtained
with a 0.125 cm air ionization chamber and radiographic film,
respectively. Finally, we have acquired profiles for an IMRT field and for a
virtual wedge. These profiles have also been compared with radiographic film
measurements. All the comparisons show a good correspondence. Signal
reproducibility was within a 2% during the test period (around three months).
The device has proved its capability to verify on-line therapy beams with

No elliptic islands for the universal area-preserving map
A renormalization approach has been used in \cite{EKW1} and \cite{EKW2} to
prove the existence of a \textit{universal area-preserving map}, a map with
hyperbolic orbits of all binary periods. The existence of a horseshoe, with
positive Hausdorff dimension, in its domain was demonstrated in \cite{GJ1}. In
this paper the coexistence problem is studied, and a computer-aided proof is
given that no elliptic islands with period less than 20 exist in the domain. It
is also shown that less than 1.5% of the measure of the domain consists of
elliptic islands. This is proven by showing that the measure of initial
conditions that escape to infinity is at least 98.5% of the measure of the
domain, and we conjecture that the escaping set has full measure. This is
highly unexpected, since generically it is believed that for conservative
systems hyperbolicity and ellipticity coexist

171558.pdf (publisher's version ) (Open Access)OBJECTIVES: To evaluate the effect of subject-controlled, on-demand, dorsal genital nerve (DGN) stimulation on non-neurogenic urgency urinary incontinence (UUI) in a domestic setting. MATERIALS AND METHODS: Non-neurogenic patients >18 years with overactive bladder symptoms and UUI were included. Exclusion criteria were mainly stress urinary incontinence. Patients underwent 1 week of subject-controlled, on-demand, DGN stimulation, delivered by a percutaneously placed electrode near the DGN connected to an external stimulator (pulse-rate 20 Hz, pulse-width 300 mus). Patients activated the stimulator when feeling the urge to void and stimulated for 30 s. The amplitude was set at the highest tolerable level. A bladder diary including a severity score of the UUI episodes/void (scores: 0 = none, 1 = drops, 2 = dashes, 3 = soaks) and a padtest was kept 3 days prior to, during, and 3 days after the test period. The subjective improvement was also scored. RESULTS: Seven patients (4 males/3 females) were enrolled, the mean age was 55 years (range 23-73). Six completed the test week. In the remaining patient the electrode migrated and was removed. 5/6 finalized the complete bladder diary, 1/6 recorded only the heavy incontinence episodes (score = 3). 4/6 completed the padtest. In all patients who finalized the bladder diary the number of UUI episodes decreased, in 3/5 with >/=60%. The heavy incontinence episodes (score = 3) were resolved in 2/6 patients, and improved >/=80% in the other 4. The severity score of the UUI episodes/void was improved with >/= 60% in 3/5 patients. The mean subjective improvement was 73%. CONCLUSION: This feasibility study indicates that subject-controlled, on-demand DGN stimulation using a percutaneously placed electrode is possible over a longer time period, in a home setting, with a positive effect on non-neurogenic overactive bladder symptoms with UUI. Although the placement is an easy procedure, it is difficult to fixate the electrode to keep it in the correct position. Improvements in hardware, like a better fixated electrode and an easy to control stimulator, are necessary to make SODGNS a treatment possibility in the future
Spatial-domain interferometer for measuring plasma mirror expansion
International audienceWe present a practical spatial-domain interferometer for characterizing the electronic density gradient of laser- induced plasma mirrors with sub-30-femtosecond temporal resolution. Time-resolved spatial imaging of an intensity- shaped pulse reflecting off an expanding plasma mirror in- duced by a time-delayed pre-pulse allows us to measure characteristic plasma gradients of 10â100 nm with an ex- pansion velocity of 10.8 nm/ps. Spatial-domain interferom- etry (SDI) can be generalized to the ultrafast imaging of nm to ÎŒm size laser-induced phenomena at surfaces
Driven Brownian transport through arrays of symmetric obstacles
We numerically investigate the transport of a suspended overdamped Brownian
particle which is driven through a two-dimensional rectangular array of
circular obstacles with finite radius. Two limiting cases are considered in
detail, namely, when the constant drive is parallel to the principal or the
diagonal array axes. This corresponds to studying the Brownian transport in
periodic channels with reflecting walls of different topologies. The mobility
and diffusivity of the transported particles in such channels are determined as
functions of the drive and the array geometric parameters. Prominent transport
features, like negative differential mobilities, excess diffusion peaks, and
unconventional asymptotic behaviors, are explained in terms of two distinct
lengths, the size of single obstacles (trapping length) and the lattice
constant of the array (local correlation length). Local correlation effects are
further analyzed by continuously rotating the drive between the two limiting

On the Lebesgue measure of Li-Yorke pairs for interval maps
We investigate the prevalence of Li-Yorke pairs for and
multimodal maps with non-flat critical points. We show that every
measurable scrambled set has zero Lebesgue measure and that all strongly
wandering sets have zero Lebesgue measure, as does the set of pairs of
asymptotic (but not asymptotically periodic) points.
If is topologically mixing and has no Cantor attractor, then typical
(w.r.t. two-dimensional Lebesgue measure) pairs are Li-Yorke; if additionally
admits an absolutely continuous invariant probability measure (acip), then
typical pairs have a dense orbit for . These results make use of
so-called nice neighborhoods of the critical set of general multimodal maps,
and hence uniformly expanding Markov induced maps, the existence of either is
proved in this paper as well.
For the setting where has a Cantor attractor, we present a trichotomy
explaining when the set of Li-Yorke pairs and distal pairs have positive

On Non-Abelian Symplectic Cutting
We discuss symplectic cutting for Hamiltonian actions of non-Abelian compact
groups. By using a degeneration based on the Vinberg monoid we give, in good
cases, a global quotient description of a surgery construction introduced by
Woodward and Meinrenken, and show it can be interpreted in algebro-geometric
terms. A key ingredient is the `universal cut' of the cotangent bundle of the
group itself, which is identified with a moduli space of framed bundles on
