    Entesopathy of the lateral epicondyle of the humeral bone – current treatment options

    Chronic pain located on the lateral surface of the elbow in the area of the lateral epicondyle of the humerus is characteristic for a disease called tennis elbow, which belongs to the group of enthesopathies, i. e. diseases related to bone tendon attachment. The cause of the disease are micro-injuries caused by repetitive movements and chronic overloads. Degenerative changes occur in the tendons of the finger and wrist extensor muscles which attach to the lateral epicondyle of the humerus. The problem usually affects the extensor carpi radialis brevis muscle. A typical location for pain is the lateral side of the elbow joint but it can often radiate to the forearm and wrist. In our paper we presented the most commonly used and most effective methods of treatment of the disease. The least invasive methods of physiotherapy and rehabilitation include home exercise, ultrasound and phonophoresis, cryotherapy, light therapy, laser therapy and radial shock wave terapy. We compared the efficacy of pharmacological methods such as non-steroidal anti-inflammatory drugs, steroid injections with lidocaine, platelet-rich plasma, hyaluronic acid, botulinum toxin, collagen. The available literature provides varied reports on the efficacy of drugs being administered directly into the joint cavity. This results from inconsistent research methods and assessment criteria. There is considerable controversy as to the best therapeutic approach in everyday clinical practice. The aim of this paper was to present synthetically information and research results concerning the use of selected therapeutic agents in intra-articular injection treatment.     The use of ultrasound in the assessment of the glenoid labrum of the glenohumeral joint. Part II: Examples of labral pathologies

    Labral pathologies of the glenohumeral joint are most commonly caused by trauma. The majority of lesions affect the anterior part of labrum, resulting from much higher frequency of anterior shoulder dislocations over posterior ones. Another subgroup of labral lesions, not directly related to joint instability, are SLAP tears. Other findings include degenerative changes of labrum and paralabral cysts. Diagnostic imaging is crucial for making a decision regarding operative treatment. Apart from a standard X-ray examination, the imaging mainly relies on magnetic resonance or computed tomography arthrography. Based on their own experience, the authors propose the use of ultrasound in the assessment of labral tears of the glenohumeral joint. Different signs indicating labral pathology may be discovered and assessed during ultrasound examination. They include permanent displacement of the labrum onto the glenoid, labral instability during dynamic examination, lack of the labrum in the anatomical position, hypoechoic zone at the base of the labrum >2 mm in width, residual or swollen labrum as well as paralabral cyst(s). The most frequent appearance of labral pathology is displacement of the anteroinferior labrum onto the external aspect of the glenoid typically seen after anterior shoulder dislocation. The another most important US feature is labral instability while dynamically examined. The swelling or reduced size of the labrum usually indicates degeneration. Key words: tendinopathy, jumper’s knee, therapy, growth factors, PRP.     Streszczenie Cel pracy: Celem pracy była analiza wyników leczenia tendinopatii więzadła rzepki autogennym osoczem bogatopłytkowym bogatoleukocytarnym (L-PRP). Materiał i metody: Badaniem objęto 43 mężczyzn leczonych autogennym L-PRP z powodu tendinopatii więzadła rzepki, powszechnie nazywanej „kolanem skoczka”. L-PRP przygotowano metodą Curasan. Do oceny stanu czynnościowego badanych kolan wykorzystano skale: IKDC-2000 i KSS. Stopień satysfakcji oceniano za pomocą skali VAS. Badania przeprowadzono przed rozpoczęciem leczenia oraz 3, 6, 9, 12 i 24 miesiące po ostrzyknięciach. Wyniki i wnioski: Na podstawie uzyskanych wyników badań stwierdzono dużą skuteczność i trwałość leczenia tendinopatii więzadła rzepki L-PRP. W 2-letnim okresie obserwacji liczba punktów uzyskanych przez pacjentów w zastosowanych do oceny skalach świadczyła o normalizacji funkcji kolan i znacznej poprawie ich sprawności.     Abstract The aim: The aim of the study was to analyze the results of treatment of patellar tendinopathy with autologous leukocyte platelet-rich plasma (L-PRP). Material and methods: The prospective study included 43 men treated with autologous L-PRP because of patellar tendinopathy of the, commonly known as ‘jumper's knee’. L-PRP was prepared using the Curasan method. To assess the functional state of the examined knees such scales were used:  IKDC-2000 and KSS. Satisfaction level was evaluated with VAS scale. The follow-up examinations was carried out before treatment and 3, 6, 9, 12 and 24 months after the injections. Results and conclusions: On the basis of obtained results high efficiency and durability of results of the patellar tendinopathy L-PRP injections was found.
