    Recurrent central giant cell granuloma in the mandible : surgical treatment and dental implant restoration

    El granuloma central de células gigantes es una rara lesión intraósea benigna de los maxilares. El tratamiento tradicional ha sido el curetaje local, aunque los sub-tipos agresivos tienen una alta tendencia a la recurrencia. Este caso clínico describe un granuloma central de células gigantes recurrente en el cuerpo mandibular de una mujer de 48 años. El tratamiento inicial de la lesión consistió en un curetaje con ostectomía periférica. Cuando se detectó la recurrencia un año más tarde, se realizó una resección ósea en bloque y la regeneración del defecto con un injerto óseo compuesto de hueso autógeno, xenoinjerto y plasma rico en plaquetas autólogo. A los 24 meses de seguimiento se observó una adecuada formación de hueso nuevo. Se insertaron dos implantes dentales y se construyó una prótesis implanto-soportada, proporcionando un restauración dentaria satisfactoria

    Myxofibroma of the maxilla. Reconstruction with iliac crest graft and dental implants after tumor resection

    Odontogenic fibromyxomas are benign odontogenic tumors of mesenchymal origin of rare presentation in the oral cavity, which exhibit locally aggressive behavior and are prone to local recurrence. The controversy has mainly been on therapeutic management with recommendations varying, depending on the clinical cases, from simple curettage of lesion to segmental bone resection. We present a case report describing the reconstruction of an osseous defect in the maxilla and the restoration with dental implants in a 32 year old female patient after radical surgical excision due to an odontogenic fibromyxoma with locally aggressive behavior. The primary reconstruction of maxillary discontinuity defect was carried out by an immediate non-vascularized cortico-cancellous iliac crest graft. Using a computer-guided system for the implant treatment-planning, three dental implants were secondary placed in the bone graft by means of flapless implant surgery. The patient was subsequently restored with an implant-supported fixed prosthesis that has remained in continuous function for a period of three years. The surgical, reconstructive and restorative treatment sequence and techniques are discussed. © Medicina Oral S. L

    La relación céntrica ha sido objeto de disparidad de criterios en la odontología durante más de un siglo. A lo largo de este trabajo, vamos a exponer diferentes métodos usados tanto para obtener el registro, como para comprobar su certeza y corroborar la posición condilar. Dado que no existe evidencia científica en el tema, en este trabajo, revisaremos los estudios experimentales que aparecen en la literatura. Los hemos divididos en dos partes, la primera relacionada con la definición de relación céntrica y la segunda parte relacionada con técnicas de registro, posición condilar y la discrepancia entre relación céntrica y oclusión céntrica.The centric relation has been controversial on dentistry during more of one century. Along of this work, we are going to expose different methods to recording as checking the accuracy and condilar position. Given that there aren’t scientific evidence, in this work, we review the researches ublicated.     Psychopathology in mothers of a pediatric sample of children and adolescents affected by obesity or excess weight

    Objetivo: Analizar la presencia de sintomatología psicopatológica en una muestra de madres de niños, niñas y adolescentes atendidos por obesidad o sobrepeso en la Unidad de Endocrinología Pediátrica del Hospital Infantil “Virgen del Rocío” de Sevilla. Método: Se realizó una primera entrevista en la que se contactaba con las familias que acudían a dicha unidad para el seguimiento nutricional de su hijo o hija y, tras la firma del consentimiento informado, se les hacía entrega del “Cuestionario de Salud General de Goldberg” (GHQ-28). Una vez que las madres cumplimentaban dicho cuestionario, lo remitían posteriormente por correo. Se presentan los resultados de 41 participantes. Resultados: La mayoría de las madres de niños y adolescentes con problemas de sobrepeso u obesidad tratados en la unidad hospitalaria estudiada no muestran síntomas de problemas psicopatológicos, existiendo el mayor número de casos con significación clínica en la escala de ansiedad-insomnio (12,7%). Conclusiones: No se encuentran indicadores de una especial psicopatología entre las madres de niños, niñas y adolescentes con obesidad/sobrepeso, sólo destacando un mayor número de síntomas de ansiedad.Objective: To analyze the presence of psychopathological symptoms in a sample of mothers whose children and adolescents were treated because of obesity or overweight in the Pediatric Endocrinology Unit at Children's Hospital "Virgen del Rocío" of Seville. Method: The participants were a group of families that went to the pediatric endocrinology unit. After the mothers signed the informed consent, we gave them the "General Health Questionnaire Goldberg" (GHQ-28), that once filled had to be returned by post mail. We present the results of 41 participants. Results: Most mothers did not show symptoms of psychopathological problems. Nonetheless, there were a 12.7% of cases with clinical significance in the anxiety-insomnia scale. Conclusions: There are no indicators of special psychopathology in the mothers of children with obesity or overweight, being anxiety symptoms the main problem they showed

    Impact of the coronavirus pandemic on maxillofacial trauma:a retrospective study in southern Spain

    The coronavirus pandemic has impacted health systems worldwide, with Spain being one of the most affected countries. However, little is known about the extent to which the effects of staying home, social distancing, and quarantine measures have influenced the epidemiology of patients with maxillofacial trauma. The aim of this study was to analyze the impact of the coronavirus pandemic on the incidence, demographic patterns, and characteristics of maxillofacial fractures in the largest hospital in southern Spain. Data from patients who underwent surgery for maxillofacial fractures during the first year of the pandemic between 16 March 2020 and 14 March 2021 (pandemic group) were retrospectively compared with a control group during the equivalent period of the previous year (pre-pandemic group). The incidence was compared by weeks and by lockdown periods of the population. Demographic information, aetioloy, fracture characteristics, treatment performed, and days of preoperative stay were evaluated. Descriptive and bivariate statistics were calculated (p<0.05). During the first year of the pandemic, there was a 35.2% reduction in maxillofacial fractures (n=59) compared to the pre-pandemic year (n=91, p=0.040). A significant drop was detected during the total home lockdown period of the population (p=0.028). In the pandemic group, there was a reduction in fractures due to interpersonal aggressions, an increase in panfacial fractures, a significant increase in other non-facial injuries associated with polytrauma (p=0.037), a higher number of open reduction procedures with internal fixation, and a significantly longer mean preoperative stay (p=0.016). The first pandemic year was associated with a decline in the frequency of maxillofacial trauma and a change in the pattern and characteristics of fractures. Inter-annual epidemiological knowledge of maxillofacial fractures may be useful for more efficient planning of resource allocation and surgical practice strategy during future coronavirus outbreaks and population lockdowns

    Mesoporous Silica Nanoparticles Decorated with Carbosilane Dendrons as New Non-viral Oligonucleotide Delivery Carriers

    A novel nanosystem based on mesoporous silica nanoparticles covered with carbosilane dendrons grafted on the external surface of the nanoparticles is reported. This system is able to transport single-stranded oligonucleotide into cells, avoiding an electrostatic repulsion between the cell membrane and the negatively charged nucleic acids thanks to the cationic charge provided by the dendron coating under physiological conditions. Moreover, the presence of the highly ordered pore network inside the silica matrix would make possible to allocate other therapeutic agents within the mesopores with the aim of achieving a double delivery. First, carbosilane dendrons of second and third generation possessing ammonium or tertiary amine groups as peripheral functional groups were prepared. Hence, different strategies were tested in order to obtain their suitable grafting on the outer surface of the nanoparticles. As nucleic acid model, a single-stranded DNA oligonucleotide tagged with a fluorescent Cy3 moiety was used to evaluate the DNA adsorption capacity. The hybrid material functionalised with the third generation of a neutral dendron showed excellent DNA binding properties. Finally, the cytotoxicity as well as the capability to deliver DNA into cells, was tested in vitro by using a human osteoblast-like cell line, achieving good levels of internalisation of the vector DNA/carbosilane dendron-functionalised material without affecting the cellular viability

    Chemical and mechanical stability of air annealed cathodic arc evaporated CrAlON coatings

    This study reports the synthesis and characterization of ternary Cr-Al-O and quaternary Cr-Al-O-N coatings deposited by cathodic arc physical vapour deposition, for various nitrogen and oxygen mass flow ratios during the growth process. The composition, microstructure, indentation hardness and modulus of the films have been characterized by scanning electron microscopy, electron probe micro-analysis, X-ray diffraction, and nanoindentation techniques. The evolution of the microstructure and mechanical properties of the coatings after ambient air annealing from 800 °C up to 1100 °C have been investigated. As the oxygen to nitrogen mass flow increases, the as-deposited coatings exhibit lower hardness, higher roughness, lower crystallinity and a more marked columnar structure. At oxygen to nitrogen mass flow ratios bigger than 10/90, the coatings exhibit a stoichiometry of the type (CrAl)2+εO3−ε. Only the coatings with an oxygen to nitrogen mass flow ratio smaller than 10/90 retained nitrogen in their compositions. In all cases, the coatings developed a cubic fcc lattice structure. After annealing at 1100 °C the resulting microstructure showed a clear dependency upon the initial composition of the films. The evolution of the microstructure during the high temperature tests, as well as the analysis of the nanoindentation hardness, composition and thickness also provided valuable information about the combined effects of the thermal stability and the oxidation of the deposited coatings
