LEARNING THE ROPES: A SOFTWARE TOOL FOR CLIL COURSES IN MARITIME SCHOOLS AND INSTITUTIONS
This paper outlines a proposal for maritime English language teaching in public and private Nautical Schools and other maritime educational institutions and establishments in Italy, using a content and language integrated learning (CLIL) approach. The courses are addressed in particular to those students who would like to take up a marine career as officers, engineers or other crew members of the Merchant Navy, and thus require an adequate knowledge of seafaring terminology, but can also be interesting for those wishing to explore the origins and development of maritime language. In order to provide a more challenging environment and better opportunity for the learning of seafaring terms and expressions in English, students are supported by Mariterm, a lexical database, organized in semantic relations, available at the Institute for Computational Linguistics (ILC) of the National Research Council (CNR) in Pisa.
Adrenal pheochromocytoma incidentally discovered in a patient with parkinsonism
To evaluate the diagnostic route of pheochromocytoma (PHEO) in a patient under dopaminergic treatment. A 70-year-old man with Parkinsonism and under treatment with levodopa and carbidopa came to our observation for evaluation of arterial hypertension and right adrenal mass discovered incidentally. To evaluate adrenal hormone levels we performed a dexamethasone suppression test, plasma aldosterone levels and 24-hr urinary metanephrine, which revealed elevated levels of catecholamines metabolities. 123-I-metaiodobenzylguanidine SPECT scintiscan revealed raised activity within the right adrenal gland concordant with the mass. The diagnosis of PHEO was posed and an elective laparoscopic adrenalectomy was performed; histopathological examination confirmed the PHEO diagnosis. Pseudo-outbreak of Mycobacterium gordonae in a teaching hospital: importance of strictly following decontamination procedures and emerging issues concerning sterilization
Aim of this study was to investigate a pseudo-outbreak of Mycobacterium gordonae analyzing isolates detected from clinical and environmental samples. Mycobacterium gordonae was detected in 7 out of 497 broncho-alveolar lavage (BAL) samples after bronchoscopy procedure in patients admitted to a teaching hospital between January and April 2013. During this pseudo-outbreak clinical, epidemiological, environmental and molecular investigations were performed. None of the patients met the criteria for non-tuberculous mycobacterial (NTM) lung disease and were treated for M. gordonae lung disease. Environmental investigation revealed M. gordonae in 3 samples: in tap water and in the water supply channel of the washer disinfector. All the isolates were subjected to genotyping by pulsed-field gel electrophoresis (PFGE). The PFGE revealed that only patients' isolates presented the same band pattern but no correlation with the environmental strain was detected. Surveillance of the outbreak and the strict adherence to the reprocessing procedure and its supplies resulted afterwards in no detection of M. gordonae in clinical respiratory samples. Clinical surveillance of patients was crucial to establish the start of NTM treatment. Regular screening of tap water and endoscopic equipment should be adopted to compare the clinical strains with the environmental ones when an outbreak occurs
Characterization of the Role of the Rpf Motif in Mycobacteriophage Tape Measure Proteins
In order to inject their DNA into the bacterial cytoplasm and establish infection, bacteriophages must ensure their genetic material successfully traverses both the bacterial membrane(s) and the layer of peptidoglycan surrounding the host cell. Phages accomplish this in a variety of ways, and some have virion-associated murein hydrolase enzymes that facilitate this process, particularly in conditions where the peptidoglycan is highly cross-linked. Phages that infect the mycobacteria must also contend with these barriers to infection, as well an impermeable layer of mycolic acids that decorates the cell surface; however, the mechanisms by which they do this are mostly unknown. In this regard, three small sequence motifs have been identified within mycobacteriophage tape measure proteins (TMPs) − extended molecules that span the tail lumen and determine its length − at least two of which have similarity to host proteins with muralytic activity. This suggests that phages may utilize regions of the TMP, which because of its location within the tail might be uniquely primed for host interaction, to facilitate localized peptidoglycan hydrolysis and DNA injection. The focus of this study is the motif found in the TMPs of mycobacteriophages Barnyard and Giles that has identity to a group of bacterial proteins known as resuscitation promoting factors (Rpfs). These Rpf proteins stimulate growth of non-growing bacteria and seem to exert their activity by cleaving inert peptidoglycan in the cell wall. Notably, the Barnyard Rpf Motif is contained within a 70 kDa C-terminal cleavage product of TMP, which appears to be cell wall- and/or membrane-associated during infection. Further, mycobacteria expressing TMP fragments containing this motif show aberrant behavior in culture and on solid media, and hybrid proteins in which the Rpf domain of the Micrococcus luteus Rpf protein is replaced with either of the phage motifs have muralytic activity in vivo. A recombineering-based method for generating mutations on lytically replicating mycobacteriophages has been developed and utilized to make multiple mutations in the Giles TMP motifs. Mutant phages infect host cells in late-stationary phase with a reduced efficiency, an observation that further supports a role for these motifs in cell wall hydrolysis during infection
Relationship between prolactin plasma levels and white matter volume in women with multiple sclerosis
BACKGROUND:
The role of prolactin (PRL) on tissue injury and repair mechanisms in multiple sclerosis (MS) remains unclear. The aim of this work was to investigate the relationship between PRL plasma levels and brain damage as measured by magnetic resonance imaging (MRI).
METHODS:
We employed a chemiluminescence immunoassay for measuring plasma levels of PRL. We used a 1.5 T scanner to acquire images and Jim 4.0 and SIENAX software to analyse them.
RESULTS:
We included 106 women with relapsing remitting (RR) MS and stable disease in the last two months. There was no difference in PRL plasma levels between patients with and without gadolinium enhancement on MRI. PRL plasma levels correlated with white matter volume (WMV) (rho = 0.284, p = 0.014) but not with grey matter volume (GMV). Moreover, PRL levels predicted changes in WMV (Beta: 984, p = 0.034).
CONCLUSIONS:
Our data of a positive association between PRL serum levels and WMV support the role of PRL in promoting myelin repair as documented in animal models of demyelination. The lack of an increase of PRL in the presence of gadolinium enhancement, contrasts with the view considering this hormone as an immune-stimulating and detrimental factor in the inflammatory process associated with MS
Oral contraceptives combined with interferon β in multiple sclerosis
Objective: To test the effect of oral contraceptives (OCs) in combination with interferon b (IFN-b)
on disease activity in patients with relapsing-remitting multiple sclerosis (RRMS).
Methods: One hundred fifty women with RRMS were randomized in a 1:1:1 ratio to receive IFNb-1a
subcutaneously (SC) only (group 1), IFN-b-1a SC plus ethinylstradiol 20 mg and desogestrel
150 mg (group 2), or IFN-b-1a SC plus ethinylestradiol 40 mg and desogestrel 125 mg (group 3).
The primary endpoint was the cumulative number of combined unique active (CUA) lesions on
brain MRI at week 96. Secondary endpoints included MRI and clinical and safety measures.
Results: The estimated number of cumulative CUA lesions at week 96 was 0.98 (95% confidence
interval [CI] 0.81–1.14) in group 1, 0.84 (95% CI 0.66–1.02) in group 2, and 0.72 (95% CI
0.53–0.91) in group 3, with a decrease of 14.1% (p 5 0.24) and 26.5% (p 5 0.04) when comparing
group 1 with groups 2 and 3, respectively. The number of patients with no gadoliniumenhancing
lesions was greater in group 3 than in group 1 (p 5 0.03). No significant differences
were detected in other secondary endpoints. IFN-b or OC discontinuations were equally distributed
across groups.
Conclusions: Our results translate the observations derived from experimental models to patients,
supporting the anti-inflammatory effects of OCs with high-dose estrogens, and suggest possible
directions for future research

G2A Attenuates Propionibacterium acnes Induction of Inflammatory Cytokines in Human Monocytes.
Background:Acne vulgaris is a disease of the pilosebaceous unit characterized by increased sebum production, hyperkeratinization, and immune responses to Propionibacterium acnes (PA). Here, we explore a possible mechanism by which a lipid receptor, G2A, regulates immune responses to a commensal bacterium. Objective:To elucidate the inflammatory properties of G2A in monocytes in response to PA stimulation. Furthermore, our study sought to investigate pathways by which lipids modulate immune responses in response to PA. Methods:Our studies focused on monocytes collected from human peripheral blood mononuclear cells, the monocytic cell line THP-1, and a lab strain of PA. Our studies involved the use of enzyme-linked immunosorbent, Western blot, reverse transcription polymerase chain reaction, small interfering RNA (siRNA), and microarray analysis of human acne lesions in the measurements of inflammatory markers. Results:G2A gene expression is higher in acne lesions compared to normal skin and is inducible by the acne therapeutic, 13-cis-retinoic acid. In vitro, PA induces both the Toll-like receptor 2-dependent expression of G2A as well as the production of the G2A ligand, 9-hydroxyoctadecadienoic acid, from human monocytes. G2A gene knockdown through siRNA enhances PA stimulation of interleukin (IL)-6, IL-8, and IL-1β possibly through increased activation of the ERK1/2 MAP kinase and nuclear factor kappa B p65 pathways. Conclusion:G2A may play a role in quelling inflammatory cytokine response to PA, revealing G2A as a potential attenuator of inflammatory response in a disease associated with a commensal bacterium
The role of physical activity against chemotherapy-induced peripheral neuropathy: a narrative review
Several studies investigated the side effect of adjuvant cancer treatments, and different types of preventive techniques or treatments have been assessed. Chemotherapy-induced peripheral neuropathy (CIPN) is the most common neurological side effect. Exercise training has been widely studied as an adjuvant therapy to prevent CIPN and improve post-chemotherapy functional outcome and quality of life (QoL). This narrative review aims to summarize the data obtained from the latest studies about physical activity (PA) for the prevention and treatment of CIPN and associated QoL measures. Literature research was conducted to obtain studies including PA interventions for patients with CIPN. Ten studies met inclusion criteria and were therefore summarized and discussed, focusing on exercise type and functional outcome. It seems clear that, regardless of the type of exercise, PA plays a positive role in the treatment of CIPN, providing a significant symptom improvement. There has been no standardization of type, quantity, and intensity of PA administered to the subjects in the various studies probably due to a physiological difference between samples, grade of neuropathy, and difference among therapies
Estado mental de apego não resolvido, cuidado maternal e segurança do apego infantil em duas dÃades mãe-filho
This study aims to describe the attachment representations of two mothers classified as unresolved in their state of mind with respect to attachment, as well as the specific characteristics of their interactions with their children: the quality of their care and the security of the child’s attachment. The Adult Attachment Interview was used to evaluate attachment representations, the Maternal Behavior Q-Set 2.1 was used for the quality of maternal sensitivity, and the security of the child’s attachment was evaluated with the Attachment Q-Set 3.0. The results show that while attachment is unresolved in both mothers due to their own experiences of childhood abuse, their sub-classification (secure attachment in one case and insecure in the other) is associated with differences in how they perceive and respond to the child’s needs, and in the security of the child’s attachment.El presente estudio busca describir las caracterÃsticas de las representaciones de apego de dos madres clasificadas con un estado mental de apego no resuelto, asà como las caracterÃsticas particulares de la interacción con su hijo/a: la calidad de su cuidado y la seguridad del apego del niño/a. Para las representaciones se empleó la Entrevista de Apego Adulto, para evaluar el cuidado de las madres el Maternal Behavior Q-Set 2.1, y la seguridad en el apego del niño/a se evaluó a través del Attachment Q-Set 3.0. Los resultados muestran que a pesar de que ambas madres tienen un apego no resuelto en función a experiencias de abuso vividas en la infancia, la sub-clasificación (apego Seguro en un caso e Inseguro en otro) está asociada a diferencias en la manera de percibir y responder a las necesidades de su hijo/a y en la seguridad del apego de éstos últimos.O presente estudo busca descrever as caracterÃsticas das representações de apego de duas mães classificadas com estado mental de apego não resolvido, bem como as caracterÃsticas especÃficas de suas interações com seus filhos: a qualidade do cuidado e a segurança do apego da criança. Acaricidal activity of bufadienolides Isolated from Drimia pancration against Tetranychus urticae, and structural elucidation of arenobufagin-3-O-α-L-rhamnopyranoside
Chemical characterization of the bulbs of Drimia pancration was conducted to isolate four steroidal saponins (1–4). Earlier, we focused on the structural elucidation of compounds 1–3. Herein, by means of 1H-NMR, 13C-NMR, Nuclear Overhauser Effects (NOE), and 2D-NMR spectra, the full stereochemical structure of 4 is reported, and all the 1H and 13C signals are assigned. Com- pounds 1–4 were tested for their acaricidal properties against the two-spotted spider mite Tetranychus urticae. Our results showed excellent activity of compound 1, with an LD50 (μg/cm2) of 0.29 and a LD90 (μg/cm2) of 0.96, whereas compounds 2, 3, and 4 showed moderate activity. Fur- thermore, the acaricidal and cytotoxic properties of the crude extract were also investigated. Of note, after 96 h of exposure, the acaricidal activity of compound 1 was higher than that of the positive control, hexythiazox. Indeed, for compound 1, LD50 and LD90 were 0.29 and 0.96 μg/cm2, respec- tively, while hexythiazox LD50(90) was 18.7 (132.5) μg/cm2. Additionally, D. pancration extract, after 72 h, induced a high cytotoxic effect in HaCaT and THP-1 cell lines, with an IC50 of 7.37 ± 0.5 μg/mL and 3.50 ± 0.15 μg/mL, respectively. Overall, D. pancration can be considered as a green source of novel acaricides effective against mites of agricultural importance, such as T. urticae, pending proper field validation and the assessment of non-target effects on other invertebrate species
