    Risk, uncertainty and medical practice:Changes in the medical professions following disaster

    Risk and uncertainty can destabilise and reconstruct the relationships between medicine, policy and publics. Through semi-structured interviews with medical staff following the Fukushima 3.11 Disaster, this paper demonstrates the way in which disruption (caused by disaster), coupled with uncertainty (in this case, around radiation risk) can serve to transform medical practices. After Fukushima, a deficit in publicly-trusted approaches to disaster management meant that the role and status of key medical professionals was transformed. This reorganisation of medical work included the development of new forms of expertise, the stretching of expertise beyond previously well-defined professional boundaries, and shifts in the way in which medical professionals understand and interact with publics. These changes signified the rise of new relationships between the medical workers and their community, as well as adjustments in what were regarded as the boundaries of medical work. Given both the ubiquitous threat of disasters and calls for increased engagement between the medicine and the public, this case study provides insight into the forms which such engagements can take, especially when bound by conditions of uncertainty. The paper draws upon the theoretical literature around the impact of uncertainty on policy, and combines this with medical sociological literature on the nature of medical expertise. The paper examines the shifting of medical expertise towards mode 2 forms, and evidences the impact of a democratised science of risk on the roles and functions of medical practice

    Combinatorial motif analysis of regulatory gene expression in Mafb deficient macrophages

    <p>Abstract</p> <p>Background</p> <p>Deficiency of the transcription factor MafB, which is normally expressed in macrophages, can underlie cellular dysfunction associated with a range of autoimmune diseases and arteriosclerosis. MafB has important roles in cell differentiation and regulation of target gene expression; however, the mechanisms of this regulation and the identities of other transcription factors with which MafB interacts remain uncertain. Bioinformatics methods provide a valuable approach for elucidating the nature of these interactions with transcriptional regulatory elements from a large number of DNA sequences. In particular, identification of patterns of co-occurrence of regulatory <it>cis</it>-elements (motifs) offers a robust approach.</p> <p>Results</p> <p>Here, the directional relationships among several functional motifs were evaluated using the Log-linear Graphical Model (LGM) after extraction and search for evolutionarily conserved motifs. This analysis highlighted GATA-1 motifs and 5’AT-rich half Maf recognition elements (MAREs) in promoter regions of 18 genes that were down-regulated in <it>Mafb</it> deficient macrophages. GATA-1 motifs and MafB motifs could regulate expression of these genes in both a negative and positive manner, respectively. The validity of this conclusion was tested with data from a luciferase assay that used a <it>C1qa</it> promoter construct carrying both the GATA-1 motifs and MAREs. GATA-1 was found to inhibit the activity of the <it>C1qa</it> promoter with the GATA-1 motifs and MafB motifs.</p> <p>Conclusions</p> <p>These observations suggest that both the GATA-1 motifs and MafB motifs are important for lineage specific expression of <it>C1qa</it>. In addition, these findings show that analysis of combinations of evolutionarily conserved motifs can be successfully used to identify patterns of gene regulation.</p

    Disappearing everyday materials:The displacement of medical resources following disaster in Fukushima, Japan

    This study draws upon interviews of medical staff working in the city of Minamisoma, Japan, following the 2011 Triple Disaster. It investigates staff responses to the disruption of material resources as a consequence of the disaster and its management. The disruption of spaces, and the loss of oxygen supplies, food, and medications impacted upon staff experience and the ability of institutions to care for patients. This resulted in a restructuring of spaces and materials as workers made efforts to reconfigure and reestablish healthcare functions. This is one of the few qualitative studies which draws upon the experience and perspectives of health workers in understanding material disruption following disaster. This is particularly important since this case did not involve the breakdown of lifeline infrastructure, but rather, brought to attention the way everyday material objects shape social experience. In highlighting these effects, the paper makes the case for the social scientific investigation of the impact of disasters on healthcare, shedding light on an area of research currently dominated by disaster medicine

    Probable association of T Tauri stars with the L1014 dense core

    Using the Wide Field Grism Spectrograph 2 (WFGS2), we have carried out slit-less spectroscopy, g'r'i' photometry, and slit spectroscopy on the L1014 dense core. We detected three Halpha emission line stars. We interpret one as weak-line T Tauri star (WTTS) and the others as classical T Tauri stars (CTTS). Since their g'-i' colors and/or classified spectral types are consistent with those of T Tauri stars and two of them show less extinction than the cloud, these three stars are likely to be T Tauri stars associated with L1014. Adopting an age range for T Tauri stars, 1-10 Myr, the color-magnitude diagram suggests a distance of ~400-900 pc, rather than the previously assumed distance, 200 pc. This could strongly affect on the mass estimate of L1014-IRS, which is thought to be either a very young protostar or proto-brown dwarf.Comment: 5 pages, 5 figures, to be published in Vol.58, No.5, October 25, 200

    Association between masticatory ability and oral functions

    Mastication is the process of breaking ingested food with the teeth and mixing it with saliva to form a mass that is easy to swallow. However, few studies have reported on oral functions, such as occlusal force, tongue pressure, and mastication. The purpose of this study was to evaluate the association between masticatory function and oral functions, such as occlusal force and tongue pressure. In this study, there were 113 patients (41 men and 72 women; mean age, 68.4 ± 11.3 years) who visited dentists at the Hiroshima University Hospital, Hiroshima, Japan between April 2015 and November 2018. Masticatory function of the patients was evaluated using a masticatory ability test system. In addition, occlusal force was measured using a pressure-sensitive film and the maximum tongue pressure was measured with a tongue pressure measuring device according to a conventional method. The relationship of masticatory ability with occlusal force and tongue pressure was examined using multivariate analysis while considering patients? age, gender, and the number of remaining teeth. Masticatory ability was significantly related to occlusal force, maximum tongue pressure, age, body mass index, the number of remaining teeth, and occlusal contact area (p< 0.05). Multiple regression analysis identified that masticatory ability was significantly associated (p< 0.05) with occlusal force and maximum tongue pressure. Masticatory ability was significantly associated with occlusal force and maximum tongue pressure, indicating that the large muscle mass in the oral cavity is indispensable for improving masticatory function

    In Vitro Difference between an Attenuated ML-17 and its Parental Virulent JaOH0566 Strain of Japanese Encephalitis Virus

    Two Japanese encephalitis (JE) virus strains, namely a virulent JaOH0556 and an attenuated ML-17 derived from it were studied to investigate their in vitro phenotypic differences. When both strains were concentrated by polyethylene glycol (PEG) 6000 precipitation, infectivity of the ML-17 was recovered only 4.94% compared to 42.21% for the parental JaOH0566 strain, and the virion peak was very low for the ML-17 in contrast to a sharp peak for the JaOH0566 in sucrose density gradient centrifugation. When both strains were incubated with sucrose solutions, the ML-17 was slightly more labile than the JaOH0566 at high concentration of sucrose. These findings may indicate difference in the virion structure of both strains
