    Situação trabalhista do portador de doença de Chagas crônica, em um grande centro urbano

    This study evaluated the working conditions of Chagas' disease patients in the city of Campinas, São Paulo, focusing on two-hundred-fifty patients with steady employment and treated at the University Hospital (HC-FCM/Unicamp): 98% were working-age and 77.6% were men. The origin of the patients reflected the migratory process occurring among this population. Most of the patients had limited professional skills, while 63.6% had not finished primary school and 21.6% were illiterate. However, 63.6% were regularly employed under duly processed work contracts. Their jobs were mainly in general services (21.6%) and heavy industry (21.2%). Some 55% of the patients reported a monthly income less than or equal to U$100.00, and 40.4% reported having been fired at least once during the last ten years, in 8.9% of the cases because of a diagnosis of Chagas' disease. Of the patients undergoing pre-hiring physical examinations (57.2%), 9.1% were refused, 92.3% of whom due to positive serology for T. cruzi. Finally, 78.4% reported not belonging to a labor union. The study demonstrated the precarious working conditions and discrimination experienced by workers with Chagas' disease.Este estudo avaliou a situação trabalhista dos chagásicos em Campinas (SP) e região. Entrevistaram-se 250 pacientes com vínculo trabalhista, acompanhados no HC-FCM/Unicamp: 98% integravam a faixa da população economicamente ativa e 76% eram homens. Sua origem confirmou o processo migratório vivido pelos mesmos. A maioria tinha pouca qualificação profissional, com 69,2% dos indivíduos com primeiro grau incompleto e 21,6% analfabetos. Porém, 63,6% estavam empregados com vínculo empregatício regulamentado, concentrando-se principalmente na prestação de serviços (21,6%) e indústria de transformação (21,2%). Dos entrevistados, 55,2% referiram receber até dois salários mínimos, e 40,4% afirmaram ter sido demitidos ao menos uma vez em dez anos, demissão associada ao diagnóstico da doença por 8,9% desses. Também dos 57,2% submetidos a exames admissionais, 9,1% foram recusados, 92,3% dos quais pela soropositividade. Quanto à participação em sindicatos, 78,4% negaram.     Cluster Analysis To Identify Elderly People's Profiles: A Healthcare Strategy Based On Frailty Characteristics.

    The new social panorama resulting from aging of the Brazilian population is leading to significant transformations within healthcare. Through the cluster analysis strategy, it was sought to describe the specific care demands of the elderly population, using frailty components. Cross-sectional study based on reviewing medical records, conducted in the geriatric outpatient clinic, Hospital de Clínicas, Universidade Estadual de Campinas (Unicamp). Ninety-eight elderly users of this clinic were evaluated using cluster analysis and instruments for assessing their overall geriatric status and frailty characteristics. The variables that most strongly influenced the formation of clusters were age, functional capacities, cognitive capacity, presence of comorbidities and number of medications used. Three main groups of elderly people could be identified: one with good cognitive and functional performance but with high prevalence of comorbidities (mean age 77.9 years, cognitive impairment in 28.6% and mean of 7.4 comorbidities); a second with more advanced age, greater cognitive impairment and greater dependence (mean age 88.5 years old, cognitive impairment in 84.6% and mean of 7.1 comorbidities); and a third younger group with poor cognitive performance and greater number of comorbidities but functionally independent (mean age 78.5 years old, cognitive impairment in 89.6% and mean of 7.4 comorbidities). These data characterize the profile of this population and can be used as the basis for developing efficient strategies aimed at diminishing functional dependence, poor self-rated health and impaired quality of life.132224-3

    Medicine adeshion in eldery people in an ambulatorial attendance

    The objectives of this study were to evaluate the elderly drug adhesion in an outpatient geriatric service linked to the State University Hospital of Campinas (São Paulo, Brazil) as well as to identify the related events to this adhesion. One hundred sixty five elders were submitted to an interview. It was used an instrument to collect information about the patients' identification, besides the self-reported state of health and drug utilization. The data were evaluated through the descriptive and comparative analysis. They showed that most of the elderly (88.5%) have confirmed the drug adhesion and among them 91.1% were living with company. The elderly who were living alone presented three-time more chance of non-adhesion to the drug therapy (OR: 3.655). Those that have referred drug side effects presented seven-time more chance of non-adhesion (OR: 7.092). The associated events which better predict the drug non-adhesion were: living alone (OR: 4.346) and side effects (OR: 8.219).Este estudo objetivou avaliar a adesão ao tratamento medicamentoso em idosos em seguimento ambulatorial e identificar os fatores relacionados a esta adesão. Foram entrevistados 165 idosos em seguimento ambulatorial no Hospital das Clínicas da Universidade Estadual de Campinas (Unicamp), São Paulo. Utilizou-se instrumento próprio, com informações relativas à identificação dos sujeitos, dados de saúde autorreferidos e relativos à terapêutica medicamentosa. Os dados foram submetidos às análises descritiva e de comparação. A maioria dos idosos revelou adesão ao tratamento (88,5%), dentre os quais 91,1% moravam acompanhados. Os idosos que moravam sozinhos apresentaram três vezes mais chances de não aderência ao tratamento (OR: 3,655). Os efeitos colaterais dos medicamentos mostraram sete vezes mais chances de não aderência (OR: 7,092). Os fatores combinados que apresentaram maior chance de predizer a não adesão à terapêutica foram mora sozinho (OR: 4,346) e efeitos colaterais (OR: 8,219).3507351

    Sarcopenia is the main factor involved in the development of frailty syndrome. The aims here were to investigate relationships between lower-limb muscle strength and the variables of sex, age and frailty criteria; compare lower-limb muscle strength with each frailty criterion; and assess the power of each criterion for estimating the risk of frailty among elderly outpatients. DESIGN AND SETTING: Cross-sectional study at the Geriatrics Outpatient Clinic of a university hospital in Campinas. METHOD: A non-probabilistic convenience sample of 150 elderly people of both sexes who were followed up as outpatients was assessed. Sociodemographic data (sex and age) and physical health data (frailty criteria and the five-times sit-to-stand test) were gathered. Descriptive, comparative and multivariate logistic regression analyses were performed. RESULTS: The majority of the elderly people (77.3%) were 70 years of age or over, with predominance of females (64.2%) and had a low score for the five-times sit-to-stand test (81.4% scored 0 or 1); 55.3% of the elderly people presented three or more frailty criteria. A significant association was found between lower-limb muscle strength and the variables of age and number of frailty criteria. CONCLUSIONS: Lower levels of lower-limb muscle strength were associated with advanced age and greater presence of signs of frailty. O objetivo foi investigar a relação entre força muscular de membros inferiores e as variáveis sexo, idade e critérios de fragilidade; comparar a força muscular de membros inferiores com cada critério de fragilidade e verificar seu poder de estimativa do risco para fragilidade em idosos ambulatoriais. TIPO DE ESTUDO E LOCAL: Estudo transversal no Ambulatório de Geriatria de um hospital universitário de Campinas. MÉTODO: Foi avaliada uma amostra de conveniência não-probabilística de 150 idosos de ambos os sexos em acompanhamento ambulatorial, com coleta de dados sócio-demográficos (sexo e idade) e de saúde física (critérios de fragilidade e teste de levantar e sentar da cadeira cinco vezes consecutivamente). Foram realizadas análises descritivas, de comparação e de regressão logística multivariada. RESULTADOS: A maioria dos idosos (77,3%) apresentou idade igual ou superior a 70 anos, com predomínio do sexo feminino (64,0%) e baixo escore no teste de levantar e sentar da cadeira cinco vezes consecutivas (81,4% escore 0 ou 1), 55,3% dos idosos apresentaram três ou mais critérios de fragilidade. Verificou-se associação significativa entre a força muscular de membros inferiores e as variáveis idade e número de critérios de fragilidade. CONCLUSÕES: Menores níveis de força muscular de membros inferiores estão associados a idade avançada e maior presença de sinais de fragilidade. Além disso, a força muscular de membros inferiores também está associada com os critérios redução da velocidade de marcha e da força de preensão palmar

    Clinical relationship between Chagas' disease and primary arterial hypertension in an outpatient reference service

    PURPOSE: To evaluate the clinical relationship between Chagas' disease and primary arterial hypertension. METHODS: The study involved 878 chronic chagasic outpatients followed by the Chagas' Disease Study Group of the University Hospital (Unicamp), over a 15-year period. Initially, the age, gender, race, clinical form of Chagas' disease and the presence of hypertension were noted for each patient. Subsequently, the data for matched hypertensive and normotensive chagasic patients were compared. RESULTS: Of the chagasic patients 37% were hypertensive, of those, 65% had some form of heart disease compared to 49% in the normotensive group. Of those patients with Chagas' disease and arterial hypertension, 41% were blacks and 35% were caucasians. Fifty percent of the hypertensive chagasic patients were over 45 years old compared to only 29% of the non-hypertensive patients. CONCLUSION: Most of the hypertensive chagasic patients were 45 or more years old and showed some degree of heart failure compared to the normotensive group.OBJETIVO: Caracterizar a associação entre doença de Chagas e hipertensão arterial primária. MÉTODOS: Estudaram-se os chagásicos crônicos acompanhados pelo Grupo de Estudos em Doença de Chagas (Unicamp), no período de 15 anos, considerando-se: hipertensão arterial, forma clínica da doença, sexo, idade e raça. Confrontaram-se dados dos chagásicos hipertensos com os dos não hipertensos. RESULTADOS: Constatou-se que entre 878 chagásicos, 37% eram também hipertensos, dos quais 65% tinham cardiopatia, contra 49% do grupo normotenso; 31% dos chagásicos hipertensos apresentavam descompensação hemodinâmica, contra 24% dos não hipertensos. Entre os chagásicos hipertensos 41% eram negróides e 35% caucasóides; 50% dos chagásicos hipertensos tinham mais de 45 anos contra 29% dos não hipertensos. CONCLUSÃO: A associação entre moléstia de Chagas e hipertensão arterial primária cursou com um freqüente e mais grave comprometimento cardíaco, sendo esta mais prevalente nas faixas etárias mais elevadas.43143

    Clinical and laboratory characterization of hypertensive Chagas' disease patients without evident heart failure

    This study evaluated the characteristics of 125 Chagas disease patients aged > 25 years or over who were attended at the Clinical Hospital of the State University of Campinas, State of São Paulo. Arterial pressure, age, gender, skin color, heart disease, body mass index, lipid profile, blood glucose level, alcohol and tobacco dependence, dyslipidemia, diabetes, anxiety disorders and obesity were investigated. It was found that the hypertensive Chagas disease patients were older than the non-hypertensive ones (p = 0.028). Among the hypertensive patients, there were more women (p = 0.015); higher blood glucose, LDL cholesterol and total cholesterol levels (p = 0.005, p = 0.024 and p = 0.017); more diabetics (p = 0.006); and more cardiac damage (p = 0.04) and left ventricular hypertrophy (p = 0.003). Only the age of patients with cardiac damage was shown to be higher (p = 0.003). The hypertensive Chagas disease patients presented clinical and laboratory characteristics that were similar to those of the general hypertensive population. This association may compound the harmful effects on the cardiovascular system.Este estudo avaliou as características de 125 chagásicos, > 25 anos, atendidos no Hospital de Clínicas da Universidade Estadual de Campinas, SP, considerando-se: pressão arterial, idade, gênero, cor, cardiopatia, índice de massa corporal, perfil lipídico, glicemia, etilismo, tabagismo, dislipidemia, diabetes, distúrbio de ansiedade e obesidade. Apresentavam hipertensão arterial 69 (55,2%) pacientes. Verificou-se que os chagásicos hipertensos eram mais idosos que os não hipertensos (p = 0,028). Entre os hipertensos havia: mais mulheres (p = 0,015); níveis mais elevados de glicemia, LDL-colesterol e colesterol total (p = 0,005; p = 0,024; p = 0,017); mais diabéticos (p = 0,006), dano cardíaco (p = 0,04) e sobrecarga ventricular esquerda (p = 0,003). Apenas a idade mostrou-se mais elevada nos pacientes com dano cardíaco (p = 0,003). Os chagásicos hipertensos apresentaram características clínico-laboratoriais semelhantes à população hipertensa, em geral. Dessa associação pode haver somatória de efeitos deletérios para o aparelho cardiovascular.16316

    Trombose venosa profunda de membro superior após implante de marcapasso endocavitário

    A trombose venosa profunda (TVP) associada à presença do cabo-eletrodo é uma complicaçao própria da estimulaçao endocavitária (MPEC). Os autores apresentam um relato do caso de um homem de 31 anos, com miocardiopatia chagásica e com esta complicaçao. Após implante do MPEC, o paciente permaneceu assintomático por 1 ano e 2 meses, quando apresentou quadro de TVP do membro superior direito. Tratado com heparina endovenosa e warfarina sódica, houve regressao do quadro. A presença de fatores intervenientes no desenvolvimento da trombose foi excluída. A flebografia evidenciou trombose de veia subclávia direita com desenvolvimento de circulaçao colateral. Concluem que a ocorrência TVP relacionada ao cabo de MPEC é situaçao relativamente freqüente, embora manifestaçoes clínicas sejam incomuns. Como o exame f1ebográfico confirma e localiza a trombose, é indicado nas manipulaçoes e troca do cabo-eletrodo, com o intuito de evitar a mobilizaçao de possíveis trombos, prevenindo a embolia pulmonar. A utilizaçao de anti-agregantes plaquetários na profilaxia da TVP relacionada ao cabo do MPEC merece ser melhor estudada

    Trypanosoma Cruzi: Parasite Persistence In Tissues In Chronic Chagasic Brazilian Patients.

    Chagas disease in the chronic phase may develop into cardiac and/or digestive forms. The pathogenesis of the disease is not yet clear and studies have been carried out to elucidate the role of parasite persistence in affected organs. The aim of this study was to detect and quantify Trypanosoma cruzi in paraffin-embedded tissue samples from chronic patients using NPCR (nested polymerase chain reaction) and QPCR (quantitative polymerase chain reaction) methods. These results were correlated to anatomopathological alterations in the heart and gastrointestinal tract (GIT). Of the 23 patients studied, 18 presented the cardiac form and five presented the cardiodigestive form of Chagas disease. DNA samples were randomly isolated from formalin-fixed paraffin-embedded sections of heart and GIT tissue of 23 necropsies and were analyzed through NPCR amplification. T. cruzi DNA was detected by NPCR in 48/56 (85.7%) heart and 35/42 (83.3%) GIT samples from patients with the cardiac form. For patients with the cardiodigestive form, NPCR was positive in 12/14 (85.7%) heart and in 14/14 (100%) GIT samples. QPCR, with an efficiency of 97.6%, was performed in 13 samples (11 from cardiac and 2 from cardiodigestive form) identified previously as positive by NPCR. The number of T. cruzi copies was compared to heart weight and no statistical significance was observed. Additionally, we compared the number of copies in different tissues (both heart and GIT) in six samples from the cardiac form and two samples from the cardiodigestive form. The parasite load observed was proportionally higher in heart tissues from patients with the cardiac form. These results show that the presence of the parasite in tissues is essential to Chagas disease pathogenesis.10685-9

    Relações entre bem-estar subjetivo e mobilidade e independência funcional por função de grupo de faixas etárias e de gêneros em idosos

    Este estudo teve como objetivo investigar as associações de bemestar subjetivo, a independência nas atividades cotidianas e as medidas de mobilidade e flexibilidade de membros inferiores em idosos em acompanhamento ambulatorial, em relação a grupos etários e de gêneros. Foram avaliados 125 idosos de ambos os gêneros com idade igual ou superior a 60 anos. Os instrumentos utilizados foram: Medida da Independência Funcional (MIF) para avaliar as atividades cotidianas, Short Physical Performance Battery (SPPB) para o desempenho físico, Bem-estar Subjetivo (BES) para satisfação com a vida. Os resultados revelaram que mulheres têm pior desempenho físico e menor independência funcional do que os homens. Os idosos mais velhos tiveram pior pontuação no desempenho físico e são mais dependentes nas atividades de vida diária que os idosos mais jovens. Entretanto eles são mais satisfeitos com vida do que os demais. Os resultados sugerem que indivíduos mais velhos e mulheres têm maior limitação funcional. Entretanto os idosos mais velhos apresentam maior satisfação com a vida.The aim of this study was to investigate the associations of subjective well-being, independence in daily activities, and the measures of mobility and flexibility in the lower limbs of elderly outpatients, according to age groups and gender. 125 people over 60 of both genders were chosen and evaluated. The instruments used were: Functional Independence Measure (FIM) to evaluate their daily activities, Short Physical Performance Battery (SPPB) for their physical performance, and Subjective Well-Being (SWB) for satisfaction with life. The results revealed that women have poorer physical performance and less functional independence than men. The older subjects had lower scores in physical performance and were more dependent in daily life activities than the younger ones, however they were more satisfied with life.     Association between race, obesity and diabetes in elderly community dwellers: data from the FIBRA study

    O objetivo deste trabalho foi investigar o efeito da cor/raça em medidas indicadoras de adiposidade corporal (índice de massa corporal - IMC, circunferência de cintura - CC e relação cintura-quadril - RCQ), bem como sua relação com o diabetes, em idosos residentes na área urbana de sete localidades brasileiras, conforme o gênero. O estudo transversal foi realizado com uma amostra probabilística composta por 2.566 idosos de 65 anos ou mais, participantes do Estudo FIBRA (Fragilidade em Idosos Brasileiros). Foram utilizadas variáveis sociodemográficas autorrelatadas (gênero, idade, cor/raça, escolaridade e renda familiar), medidas antropométricas indicadoras de obesidade geral (IMC) e abdominal (CC e RCQ) e diabetes autorreferida. Ajustando-se para escolaridade e renda, a cor/raça branca associou-se a maiores valores de CC (p = 0,001) e RCQ (p > 0,001), no gênero masculino, independentemente do diabetes. We used several self-reported sociodemographic variables (gender, age, race, schooling and family income), anthropometric measures of general (BMI) and abdominal obesity (WC and WHR) and self-reported diabetes. Adjusting for schooling and income, white race was associated with higher WC values (p = 0.001) and WHR (p > 0.001) for male gender, regardless of diabetes status.
