    Monitoring Fetal Heart Rate during Pregnancy: Contributions from Advanced Signal Processing and Wearable Technology

    Monitoring procedures are the basis to evaluate the clinical state of patients and to assess changes in their conditions, thus providing necessary interventions in time. Both these two objectives can be achieved by integrating technological development with methodological tools, thus allowing accurate classification and extraction of useful diagnostic information. The paper is focused on monitoring procedures applied to fetal heart rate variability (FHRV) signals, collected during pregnancy, in order to assess fetal well-being. The use of linear time and frequency techniques as well as the computation of non linear indices can contribute to enhancing the diagnostic power and reliability of fetal monitoring. The paper shows how advanced signal processing approaches can contribute to developing new diagnostic and classification indices. Their usefulness is evaluated by comparing two selected populations: normal fetuses and intra uterine growth restricted (IUGR) fetuses. Results show that the computation of different indices on FHRV signals, either linear and nonlinear, gives helpful indications to describe pathophysiological mechanisms involved in the cardiovascular and neural system controlling the fetal heart. As a further contribution, the paper briefly describes how the introduction of wearable systems for fetal ECG recording could provide new technological solutions improving the quality and usability of prenatal monitoring. © 2014 Maria G.     CT diagnosis of small bowel obstruction caused by internal hernia from persistent attachment of a Meckel's diverticulum to the umbilicus by the obliterated omphalomesenteric duct

    We report a case of small bowel obstruction (SBO) caused by internal hernia from persistent attachment of a Meckel's diverticulum (MD) to the umbilicus by the obliterated omphalomesenteric duct that was diagnosed by multidetector CT and confirmed during laparoscopic surgery. Although clinical, pathological and radiological features of MD and its complications are well known, the diagnosis of MD is difficult to establish preoperatively. CT findings that allow the diagnosis of this very unusual cause of SBO are presented here with laparoscopic surgery correlation

    Transcriptional activation of the miR-17-92 cluster is involved in the growth-promoting effects of MYB in human Ph-positive leukemia cells.

    MicroRNAs, non-coding regulators of gene expression, are likely to function as important downstream effectors of many transcription factors including MYB. Optimal levels of MYB are required for transformation/maintenance of BCR-ABL-expressing cells. We investigated whether MYB silencing modulates microRNA expression in Philadelphia-positive (Ph+) leukemia cells and if MYB-regulated microRNAs are important for the MYB addiction of these cells. Thirty-five microRNAs were modulated by MYB silencing in lymphoid and erythromyeloid chronic myeloid leukemia-blast crisis BV173 and K562 cells; 15 of these were concordantly modulated in both lines. We focused on the miR-17-92 cluster because of its oncogenic role in tumors and found that: i) it is a direct MYB target; ii) it partially rescued the impaired proliferation and enhanced apoptosis of MYB-silenced BV173 cells. Moreover, we identified FRZB, a Wnt/β-catenin pathway inhibitor, as a novel target of the miR-17-92 cluster. High expression of MYB in blast cells from 2 Ph+leukemia patients correlated positively with the miR-17-92 cluster and inversely with FRZB. This expression pattern was also observed in a microarray dataset of 122 Ph+acute lymphoblastic leukemias. In vivo experiments in NOD scid gamma mice injected with BV173 cells confirmed that FRZB functions as a Wnt/β-catenin inhibitor even as they failed to demonstrate that this pathway is important for BV173-dependent leukemogenesis. These studies illustrate the global effects of MYB expression on the microRNAs profile of Ph+cells and supports the concept that the MYB addiction of these cells is, in part, caused by modulation of microRNA-regulated pathways affecting cell proliferation and survival.     Antepartum Fetal Monitoring through a Wearable System and a Mobile Application

    Prenatal monitoring of Fetal Heart Rate (FHR) is crucial for the prevention of fetal pathologies and unfavorable deliveries. However, the most commonly used Cardiotocographic exam can be performed only in hospital-like structures and requires the supervision of expert personnel. For this reason, a wearable system able to continuously monitor FHR would be a noticeable step towards a personalized and remote pregnancy care. Thanks to textile electrodes, miniaturized electronics, and smart devices like smartphones and tablets, we developed a wearable integrated system for everyday fetal monitoring during the last weeks of pregnancy. Pregnant women at home can use it without the need for any external support by clinicians. The transmission of FHR to a specialized medical center allows its remote analysis, exploiting advanced algorithms running on high-performance hardware able to obtain the best classification of the fetal condition. The system has been tested on a limited set of pregnant women whose fetal electrocardiogram recordings were acquired and classified, yielding an overall score for both accuracy and sensitivity over 90%. This novel approach can open a new perspective on the continuous monitoring of fetus development by enhancing the performance of regular examinations, making treatments really personalized, and reducing hospitalization or ambulatory visits.     Boosting the Performance of PC-based Software Routers with FPGA-enhanced Network Interface Cards

    The research community is devoting increasing attention to software routers based on off-the-shelf hardware and open-source operating systems running on the personalcomputer (PC) architecture. Today's high-end PCs are equipped with peripheral component interconnect (PCI) shared buses enabling them to easily fit into the multi-gigabit-per-second routing segment, for a price much lower than that of commercial routers. However, commercially-available PC network interface cards (NICs) lack programmability, and require not only packets to cross the PCI bus twice, but also to be processed in software by the operating system, strongly reducing the achievable forwarding rate. It is therefore interesting to explore the performance of customizable NICs based on field-programmable gate array (FPGA) logic devices we developed and assess how well they can overcome the limitations of today's commercially-available NIC

    Modeling The Habitat Distribution Of Spiny Dogfish (Squalus Acanthias), By Sex, In Coastal Waters Of The Northeastern United States

    A hierarchical Bayesian approach was used to model the spatiotemporal habitat distribution of spiny dogfish (Squalus acanthias) of both sexes (adults) caught during trawl surveys conducted by the Northeast Area Monitoring and Assessment Program in inshore coastal waters between New England and North Carolina during 2007-2013. The best model for predicting catch per unit of effort (CPUE) for this species includes the following relevant variables: bathymetry, sea surface temperature, salinity, chlorophyll- a (chl-a) concentration, season and time of survey, and a random spatial effect for both sexes. Predicted CPUE was related to depth for both sexes; females occurred in shallower waters than those in which males occurred. Also, more females than males were predicted to occur in warmer, less saline and more productive (higher chl-a concentration) waters. Seasonality and time of predicted CPUE indicated that the abundance of females was higher in inshore coastal waters in the spring and in the morning, and the abundance of males was greater in the afternoon and in the fall in the same area. Collectively, these results provide information that enhances our understanding of differences in habitat selection and spatiotemporal distribution of the 2 sexes of this species-information that can help to modify present management strategies for the U.S. Atlantic fishery
