Long-term effects of bariatric surgery on meal disposal and beta-cell function in diabetic and nondiabetic patients.
Gastric bypass surgery leads to marked improvements in glucose tolerance and insulin sensitivity in obese type 2 diabetes; the impact on glucose fluxes in response to a physiological stimulus - such as a mixed meal (MTT) - has not been determined. We administered an MTT to 12 obese type 2 diabetic patients (T2D) and 15 obese nondiabetic subjects (ND) before and one year after surgery (10 T2D and 11 ND) using the double-tracer technique and modeling of ß-cell function. In both groups postsurgery, tracer-derived appearance of oral glucose was biphasic, a rapid increase followed by a sharp drop, a pattern that was mirrored by postprandial glucose levels and insulin secretion. In diabetic patients, surgery lowered fasting and postprandial glucose levels; peripheral insulin sensitivity increased in proportion to weight loss (∼30%), ß-cell glucose sensitivity doubled but did not normalize (viz. 21 nonsurgical obese and lean controls). Endogenous glucose production, however, was less suppressed during the MMT as the combined result of a relative hyperglucagonemia and the rapid fall in plasma glucose and insulin levels.We conclude that, in type 2 diabetes bypass surgery changes the postprandial response to a dumping-like pattern, improves glucose tolerance, ß-cell function, and peripheral insulin sensitivity but worsens endogenous glucose output in response to a physiological stimulus
Globaltest and GOEAST: two different approaches for Gene Ontology analysis
Background Gene set analysis is a commonly used method for analysing microarray data by considering groups of functionally related genes instead of individual genes. Here we present the use of two gene set analysis approaches: Globaltest and GOEAST. Globaltest is a method for testing whether sets of genes are significantly associated with a variable of interest. GOEAST is a freely accessible web-based tool to test GO term enrichment within given gene sets. The two approaches were applied in the analysis of gene lists obtained from three different contrasts in a microarray experiment conducted to study the host reactions in broilers following Eimeria infection. Results The Globaltest identified significantly associated gene sets in one of the three contrasts made in the microarray experiment whereas the functional analysis of the differentially expressed genes using GOEAST revealed enriched GO terms in all three contrasts. Conclusion Globaltest and GOEAST gave different results, probably due to the different algorithms and the different criteria used for evaluating the significance of GO terms
Statistical modeling of ground motion relations for seismic hazard analysis
We introduce a new approach for ground motion relations (GMR) in the
probabilistic seismic hazard analysis (PSHA), being influenced by the extreme
value theory of mathematical statistics. Therein, we understand a GMR as a
random function. We derive mathematically the principle of area-equivalence;
wherein two alternative GMRs have an equivalent influence on the hazard if
these GMRs have equivalent area functions. This includes local biases. An
interpretation of the difference between these GMRs (an actual and a modeled
one) as a random component leads to a general overestimation of residual
variance and hazard. Beside this, we discuss important aspects of classical
approaches and discover discrepancies with the state of the art of stochastics
and statistics (model selection and significance, test of distribution
assumptions, extreme value statistics). We criticize especially the assumption
of logarithmic normally distributed residuals of maxima like the peak ground
acceleration (PGA). The natural distribution of its individual random component
(equivalent to exp(epsilon_0) of Joyner and Boore 1993) is the generalized
extreme value. We show by numerical researches that the actual distribution can
be hidden and a wrong distribution assumption can influence the PSHA negatively
as the negligence of area equivalence does. Finally, we suggest an estimation
concept for GMRs of PSHA with a regression-free variance estimation of the
individual random component. We demonstrate the advantages of event-specific
GMRs by analyzing data sets from the PEER strong motion database and estimate
event-specific GMRs. Therein, the majority of the best models base on an
anisotropic point source approach. The residual variance of logarithmized PGA
is significantly smaller than in previous models. We validate the estimations
for the event with the largest sample by empirical area functions. etc
Minor psychiatric disorders among Brazilian ragpickers: a cross-sectional study
BACKGROUND: Ragpickers are informal workers who collect recyclable materials to earn a small wage. Their life and working conditions are extremely difficult. We examined minor psychiatric disorders (MPD) among a cohort of ragpickers in Pelotas, a city in southern Brazil. METHODS: Ragpickers were matched by sex, age, and years of schooling with a sample of non-ragpickers from the same poor neighborhoods. The cross-sectional study gathered data by interview on 990 individuals in 2004. MPD were assessed using a standard self-reporting questionnaire, the SRQ-20. RESULTS: The prevalence of MPD among ragpickers was 44.7%, higher than reported by neighborhood controls (33.6%; p < 0.001). MPD were more common among females, those of lower economic level, smokers and alcoholics. Among occupational characteristics, MPD prevalence was associated with frequent static postures, low job satisfaction and recent work accidents. CONCLUSION: Ragpickers more frequently report MPD than other poor workers living in the same neighborhoods, with many of the same life conditions. Improving the work lives of these precarious workers should address not only the physical hazards of their jobs but their mental and emotional health as well
Adjuvant and neoadjuvant therapy for gastric cancer using epirubicin/cisplatin/5-fluorouracil (ECF) and alternative regimens before and after chemoradiation
Chemoradiation is now used more commonly for gastric cancer following publication of the US Intergroup trial results that demonstrate an advantage to adjuvant postoperative chemoradiotherapy. However, there remain concerns regarding the toxicity of this treatment, the optimal chemotherapy regimen and the optimal method of radiotherapy delivery. In this prospective study, we evaluated the toxicity and feasibility of an alternative chemoradiation regimen to that used in the Intergroup trial. A total of 26 patients with adenocarcinoma of the stomach were treated with 3D-conformal radiation therapy to a dose of 45 Gy in 25 fractions with concurrent continuous infusional 5-fluorouracil (5-FU). The majority of patients received epirubicin, cisplatin and 5-FU (ECF) as the systemic component given before and after concurrent chemoradiation. The overall rates of observed grade 3 and 4 toxicities were 38 and 15%, respectively. GIT grade 3 toxicity was observed in 19% of patients, while haematologic grade 3 and 4 toxicities were observed in 23%. Our results suggest that this adjuvant regimen can be delivered safely and with acceptable toxicity. This regimen forms the basis of several new studies being developed for postoperative adjuvant therapy of gastric cancer
