    Comparative study between serum level of hepatocyte growth factor and CA-125 in patients with suspicious malignant adnexal masses

    Background: Hepatocyte growth factor has been described to be increased in different cancers. The aim of the present study is to evaluate as a screening marker the‏ serum level of Hepatocyte growth factor among suspicious adnexal masses as compared to serum levels of CA125.Methods: The present study included 80 female patients who are admitted to the Gynecology unit in Elshatby Maternity University Hospital divided into two groups. Forty patients with benign gynecological conditions (control group) and 40 patients with suspicious malignant adnexal masses (cases group). Preoperative blood samples were withdrawn from all patients of both cases and control group to assess the level of serum hepatocyte growth factor (HGF) and serum cancer antigen 125 (CA 125). Both were quantified using ELISA technique.Results: Out of the 40 cases with suspicious malignant adnexal masses, 35 had ovarian cancer while five only were borderline. Patients with ovarian carcinomas had significantly higher preoperative HGF and CA 125 serum levels than patients with borderline pathology. Patients with borderline tumors had a significantly higher serum HGF and CA 125 levels than patients with benign gynecological conditions in control group.Conclusions: HGF in serum was elevated in 71% of patients with suspicious malignant adnexal masses proved to be ovarian cancer by histopathology using a quantitative ELISA. HGF can be used as a screening tool for ovarian cancer

    Management of glucose 6-phosphate dehydrogenase (G6PD) deficient patients undergoing open-heart surgery

    Background: There are scarce studies on the management of glucose 6-phosphate dehydrogenase (G6PD) deficient patients during cardiac surgery. The purposes of this retrospective study were to present and evaluate our experience with G6PD deficient patients who underwent cardiac surgery with cardiopulmonary bypass (CPB). Methods: We included 20 patients with G6PD deficiency who had cardiac surgeries from 2015 to 2019. We used free radical scavenging strategy and careful perioperative management. The patients were compared to a control group of 20 patients with normal G6PD enzyme activity who underwent the same type of operations in the same period. Results: Males represented 80% of G6PD deficient patients. There were significant elevations in preoperative total bilirubin (1.03±0.33 vs. 0.57±0.11 mg/dl, p< 0.001) and reticulocytes (1.87±0.62 vs. 0.54±0.18%) in G6PD deficient patients. Valve surgery was done for 60% of G6PD deficient patients. There were no significant differences between both groups regarding the type of surgery, aortic cross-clamp, CPB, and total operative time. G6PD deficient patients had significantly lower postoperative hemoglobin levels (9.44±0.94 vs. 10.0±0.59 g/dl, p= 0.04) and significantly higher postoperative total bilirubin (1.51±0.51 vs. 0.98±0.45 mg/dl; p=0.002) and reticulocytes (1.85±0.51 vs. 0.57±0.13%; p< 0.001). There was no significant difference regarding postoperative urea and creatinine levels. Ventilation time (10.3±2.7 vs. 8.2±1.9 hours; p=0.01), ICU stay (3.1±0.87 vs. 2.3±0.71 days; p=0.004), and hospital stay (3.1±0.87 vs. 6.0±1.02 days; p<0.001) significantly increased in G6PD deficient patients. The mortality rate was 5% (one patient) in G6PD deficient patients. Conclusion: Despite the management strategy, G6PD deficient patients undergoing cardiac surgery are more liable to hemolysis and hypoxia with more need for blood transfusion and longer ventilation time, ICU, and hospital stays when compared to patients with normal G6PD enzyme activity. Further research to improve the outcomes in G6PD deficient patients is required

    Estimation of post-harvest losses of Manfalouty pomegranate fruits

    Weight loss considered one of the main causes of quality loss in pomegranate fruits during chain marketing. Therefore, this study was conducted on Manfalouty pomegranate (Punica granatum L.) in a private orchard in El Badary, Assiut Governorate, Egypt in 2017 and 2018 to define the various causes of losses during chain handing and estimate it. The fruits harvested at three periods early (September) mid (October) and late season (November). The total losses at harvest were 5.94%, 9.30% and 23.50% for early, mid and late season, respectively. The main cause of losses is due to cracked and infected pests. The total loss of fruits during chain marketing was highest in retail market in comparison with wholesale during early, mid and late season. The main causes of losses due to weight loss and shrinkage fruits. According to data dealing with storage pomegranate fruits at 5±1°C and relative humidity 85-90%, the highest fruit losses found in the third month and this losses due to fruit weight loss and internal chilling injury (brown discoloration) so the storage life of fruit should be two months.     Multiple inputs all-optical logic gates based on nanoring insulator-metal-insulator plasmonic waveguides

    In this paper, we report new nanoscale plasmonic multiple inputs logic gates based on insulator-metal-insulator (IMI) nanoring waveguides. The proposed all-optical gates are numerically analyzed by the finite element method. NOT, AND, NAND, NOR, and EX-NOR all-optical logic gates were suitably designed and investigated based on the linear interface between the propagated waves through the waveguides. The operation wavelength was 1550 nm. The simulation results show that the optical transmission threshold of (0.26) which performs the operation of planned logic gates is accomplished. Moreover, simulation results show that our compact structure of all-optical logic gates may have potential applications in all-optical integrated networks

    Evaluation of Corneal High Order Aberrations in Post Laser-Assisted in Situ Keratomileusis versus Photorefractive Keratectomy in Myope

    Background Aberrations of the eye are the difference between two surfaces: the ideal and the actual wave front. Higher order aberrations are a relatively small component, comprising about 10% of the eye’s total aberrations. High order aberrations increase with age and minor symmetry exists between the right and the left eyes. Aim and objectives To Compare corneal high order aberrations in post laser-assisted in situ keratomileusis with corneal high order aberrations in post Photorefractive keratectomy in myope. Subjects and methods this study was designed as a prospective randomized intervention study that conducted on 40 myopic eyes. The eyes divided into two randomized groups. Group A: 20 myopic eyes corrected with laser-assisted in situ keratomileusis and Group B: 20 myopic eyes corrected with Photorefractive keratectomy. Cases were collected for LASIK and PRK at operative theater in Beni-Suef University Hospital and clear vision laser center in Cairo, from May 2019 till January 2022. Result There was clinically significant difference between pre- and post-operative visual acuity, vertical trefoil and vertical coma of LASIK group and there was clinically significant between pre- and post-operative visual acuity, vertical trefoil and vertical coma of PRK group. But there was no significant difference between LASIK and PRK in visual acuity, spherical aberration, higher order 5 mm and 6 mm, vertical coma and Vertical trefoil. Conclusion There is non-significant increase in higher order aberration and spherical aberration post LASIK and post PRK with lesser amount in PRK

    Electrical Properties of Partial Carbonized Nanoporous Resin Based on Resorcinol-Formaldehyde

    Abstract: Organic xerogel compounds were prepared by sol-gel method from resorcinol-formaldehyde mixtures in acetone using picric acid as catalyst. The electrical properties of the obtained nanoporous carbon structures were explored by changing the pyrolysis temperature. In this study the electrical conductivity σ can be expressed as σ =/σ 0 exp(-Eσ/kT), where Eσ depends on the carbonized temperature. The dc and ac conductivities of the obtained amorphous carbon have been investigated from 80 to 300 °C and in the frequency range between 40 and 10 6 Hz for samples pyrolysed at different temperatures in the insulator-metal transition range. The temperature dependence of samples pyrolysed at low temperatures (T p =600-675 °C) follows a Mott law, whereas samples pyrolysed at high temperature (T p =1000 °C) show an Arrhenius dependence

    Evaluation of knowledge, experiences, and fear toward prescribing and dispensing corticosteroids among Egyptian healthcare professionals: A cross-sectional study

    Background: Corticosteroids (CS) are essential drugs in the treatment of several medical conditions. Assuming different roles, physicians and pharmacists play a primary role in prescribing and dispensing these medications to optimize patients' clinical management. The data on assessing knowledge and experience of healthcare professionals toward CS is scarce. Therefore, this study aimed to assess and compare knowledge, experience, and fears towards CS among Egyptian physicians and pharmacists. Methods: A cross-sectional, self-administrated, validated online questionnaire was used to collect the data from Egyptian healthcare professionals. The questionnaire consisted of four sections with multiple choice questions: sociodemographic (7 questions), knowledge about CS (13 questions), experience with CS prescription/dispensing (5 questions), and fears and preferences toward CS prescription/dispensing (13 questions). Descriptive and inferential statistics were used to analyze the data. Results: A total of 600 responses were analyzed in this study. The study sample was almost two-half of healthcare providers: 303 (50.5%) pharmacists and 297 (49.5%) physicians. Pharmacists had marginally higher knowledge scores as compared to those recorded for physicians (11.29 versus 10.16, respectively; P = 0.047). Physicians had more experience choosing corticosteroids in treatment plans based on their experience (51.8% vs 38.5%) and guideline recommendations (72.8% vs 50.9%) than pharmacists. However, pharmacists had more experience dealing with corticosteroid use based on patients' preferences (19.5% vs 4.9%) and showed a broader scope of experiencing side effects of corticosteroids with their patients. The two professions demonstrated high levels of fear, with pharmacists acknowledging significantly lower concerns about CS than physicians (3.72 versus 4.0, respectively; P = 0.003). Conclusion: Discrepancies exist among healthcare professionals in knowledge and experience, favoring better scientific knowledge of pharmacists related to corticosteroids. Based on these findings, the interprofessional collaborative efforts would provide comprehensive, patient-centered care that maximizes the benefits of CS while minimizing their risks

    Comparing the success rate of external dacryocystorhinostomy with anterior flap versus flap excision in managing chronic dacryocystitis

    Background: Nasolacrimal duct obstruction (NLDO) is characterized by epiphora and recurrent episodes of acute dacryocystitis. Despite the temporary effect of antibiotics in the acute phase, it is primarily managed by dacryocystorhinostomy (DCR). There is a new modification of external DCR that is performed without either anterior or posterior flaps. This study aimed to compare the outcomes of flapless and single-flap external DCR in adult patients with chronic symptomatic dacryocystitis secondary to NLDO. Methods: In this retrospective, non-randomized, interventional, comparative study of patients with chronic dacryocystitis secondary to primary acquired NLDO, we compared the surgical outcomes and complication rates of flapless external DCR to those of external DCR with only anterior flap suturing. We excluded patients who declined participation and those with soft stops, nasal problems, lid margin abnormalities, lid malposition or laxity, previous lacrimal surgery, lacrimal fistula, trauma involving the lacrimal drainage system, lack of adequate follow-up, or severe septal deviation or turbinate hypertrophy. Anatomical and functional success rates were determined at the last follow-up visit and were compared. Postoperative complications were recorded and compared between groups. Results: We included 53 patients with a male-to-female ratio of 16 (30.2%) to 37 (69.8%); 25 eyes underwent flapless DCR (group 1) and 28 eyes underwent anterior flap suturing DCR (group 2). The two groups had comparable demographic characteristics (all P > 0.05). Furthermore, anatomical (92.0% in group 1 and 92.9% in group 2) and functional (84.0% in group 1 and 92.9% in group 2) success rates at final follow-up were comparable between groups (both P > 0.05). At the one-month postoperative examination, premature tube extrusion was more often reported in group 1 (12.0%) compared to group 2 (7.1%). At the two-month follow-up examination, tube extrusion was noted in 4.0% in group 1 and 0.0% in group 2, yet the difference failed to attain statistical significance (P > 0.05). Conclusions: We found that neither surgical method was superior in terms of anatomical or functional success rate at a maximum of one year after external DCR. Flapless DCR is a simple, effective, and reproducible alternative to the single anterior flap suturing technique for managing NLDO in adults with chronic dacryocystitis. However, further randomized clinical trials with larger sample sizes and longer follow-up periods are recommended before generalization can be justified

    Genome-wide identification of NAC transcription factors and regulation of monoterpenoid indole alkaloid biosynthesis in Catharanthus roseus

    NAC transcription factors (TFs) are crucial to growth and defense responses in plants. Though NACs have been characterized for their role in several plants, comprehensive information regarding their role in Catharanthus roseus, a perennial ornamental plant, is lacking. Homology modelling was employed to identify and characterize NACs in C. roseus. In-vitro propagation of C. roseus plants was carried out using cell suspension and nodal culture and were elicited with two auxin-antagonists, 5-fluoro Indole Acetic Acid (5-F-IAA) and α-(phenyl ethyl-2-oxo)-Indole-Acetic-Acid (PEO-IAA) for the enhanced production of monoterpenoid indole alkaloids (MIAs) namely catharanthine, vindoline, and vinblastine. Analyses revealed the presence of 47 putative CrNAC genes in the C. roseus genome, primarily localized in the nucleus. Phylogenetic analysis categorized these CrNACs into eight clusters, demonstrating the highest synteny with corresponding genes in Camptotheca acuminata. Additionally, at least one defense or hormone-responsive cis-acting element was identified in the promoter region of all the putative CrNACs. Of the two elicitors, 5-F-IAA was effective at 200 µM to elicit a 3.07-fold increase in catharanthine, 2.76-fold in vindoline, and 2.4-fold in vinblastine production in nodal culture. While a relatively lower increase in MIAs was recorded in suspension culture. Validation of RNA-Seq by qRT-PCR showed upregulated expression of stress-related genes (CrNAC-07 and CrNAC-24), and downregulated expression of growth-related gene (CrNAC-25) in elicited nodal culture of C. roseus. Additionally, the expression of genes involved in the biosynthesis of MIAs was significantly upregulated upon elicitation. The current study provides the first report on the role of CrNACs in regulating the biosynthesis of MIAs

    Safety and efficacy of implantable phakic contact lens versus implantable collamer lens in myopia correction

    Background: Phakic intraocular lenses (pIOLs) have proven to be excellent substitutes for excimer laser keratorefractive surgery in certain situations. We aimed to assess the efficacy and safety of two pIOLs, the implantable collamer lens (ICL V4c) versus the implantable phakic contact lens (IPCL V2), for myopic correction. Methods: In this prospective randomized clinical trial, we allocated eligible eyes with myopia > - 6 diopters into IPCL or ICL implantation groups, each including 100 eyes of 100 individuals. Preoperative and postoperative assessments at 3, 6, and 12 months included measurements of the spherical equivalent (SE), uncorrected distance visual acuity (UCDVA), best-corrected distance visual acuity (BCDVA), intraocular pressure (IOP), maximum keratometry (K1), minimum keratometry (K2), mean keratometry (Kmean), anterior chamber depth (ACD), anterior chamber angle (ACA), and endothelial cell density (ECD). Results: The groups had comparable demographic characteristics and baseline visual and anatomical values (all P > 0.05). The UCDVA, BCDVA, and SE of the two groups were comparable at baseline and at all postoperative follow-up examinations (all P > 0.05). Both groups experienced significant improvements in UCDVA, BCDVA, and SE at three months postoperatively (all P = 0.001), and measurements remained stable for up to 12 months. Keratometry readings were comparable between the groups over the follow-up period and remained unchanged at all visits (all P > 0.05). The ACA in the ICL group was significantly decreased at three months postoperatively (P = 0.001) and then widened significantly at 6 and 12 months (both P = 0.001). In the IPCL group, the postoperative ACA was significantly decreased at three months (P = 0.001) and was comparable to that in the ICL group (P > 0.01). However, at the 6- and 12-month postoperative visits, the ACA was significantly narrower in the IPCL group than in the ICL group (both P = 0.001). The ACD in both groups was decreased at three months postoperatively (both P = 0.001) and remained stable until the end of the study. The ECD remained comparable between the groups at all postoperative visits (all P > 0.05). We did not observe a significant ECD reduction in either group at any postoperative follow-up visit (all P > 0.05). We encountered no serious complications in either group. Conclusions: ICL and IPCL had comparable safety and efficacy outcomes in terms of anterior chamber morphometrics, visual and refractive results, and corneal parameters. Further multicenter randomized clinical trials with longer follow-up periods, larger sample sizes, and measurement of additional anterior chamber and corneal morphometrics, vault, and other vision parameters are needed to verify these findings
