Technique of endovascular rheolytic thrombectomy using AngioJet for treatment of acute pulmonary embolism

    Get PDF
We present a technique of endovascular rheolytic thrombectomy using the AngioJet system for the treatment of acute pulmonary embolism. We describe the procedure in detail, suggesting the optimal catheter, wires and potential complications during the procedure. Recent advances in endovascular techniques, such as aspiration thrombectomy, endovascular thrombus fragmentation devices and rheolytic or rotational thrombectomy, suggest that the endovascular approach can represent an alternative for pharmacological thrombolysis and surgical embolectomy

On the existence of optimal solutions to the Lagrange problem governed by a nonlinear Goursat-Darboux problem of fractional order

    Get PDF
In the paper, the Lagrange problem given by a fractional boundary problem with partial derivatives is considered. The main result is the existence of optimal solutions based on the convexity assumption of a certain set. The proof is based on the lower closure theorem and the appropriate implicit measurable function theorem

Takayasu arteritis as a cause of arterial hypertension. Case report and literature review

    Get PDF
We report a 16-year-old girl in whom Takayasu arteritis (TA) was manifested mainly by severe arterial hypertension on her right arm, which was detected during a routine examination at school. Her systolic blood pressure on the right arm was significantly higher than that on the left one. There was also a pressure difference between the right arm and legs. The pulse of the left external carotid artery and that of the left radial artery was absent. Vascular bruits over interscapular and right supra- and subclavian areas were heard on auscultation. The diagnosis of TA was confirmed by a spiral computed tomography angiography, which showed a thickened thoracic aortic wall and narrowing of its lumen. In addition, complete occlusion of the left common carotid artery and the left subclavian artery was observed. Conclusion: The rarity of the disorder and the heterogeneous nature of its clinical manifestation predispose to a late diagnosis and delayed treatment. Our report highlights the fact that the condition can and does occur in a pediatric population in Europe and hence must be considered in patients presenting with suggestive symptoms and signs, especially in young patients with unexplained hypertension. Clinical suspicion and proper imaging are crucial for the correct diagnosis and management of patients with TA. A brief review of literature completes this report

The utility of Prostar XL percutaneous vascular closure device after stent-graft implantation for infrarenal abdominal aortic aneurysms

Introduction. Over time, endovascular techniques of abdominal aortic aneurysm repair became the principal methods of treatment of this potentially fatal disease. Currently, endovascular aneurysm repair (EVAR) constitutes an effective alternative to open surgery, also in cases of aneurysm rupture. Low degree of invasiveness is the main advantage of this method. Introduction of a system enabling percutaneous stent-graft implantation appears to be the next step in the development of endovascular AAA surgery. Aim of the study is to evaluate the Prostar XLÂŽ closure device with regard to clinical effectiveness, safety, cost-effectiveness. Material and methods. The study included 100 patients (from January 2013 to December 2015) subject to endovascular abdominal aortic aneurysm repair in the infrarenal region of the aorta with the application of the Prostar XlÂŽ closure device. Most patients were operated under epidural anaesthesia. Local anaesthesia (1% Lidocaine) was used in 36 patients due to comorbidities. A possible application of the closure system depended on a preoperative assessment of common femoral arteries using ultrasonography. Presence of atherosclerotic plaque on the anterior wall of the vessel disqualified the patient from percutaneous endovascular aneurysm repair (PEVAR). The main part of the procedure consisted of stent-graft implantation into the abdominal aortic aneurysm. In the final stage, puncture site on the anterior wall of common femoral arteries was closed using previously placed sutures of the Prostar XL system. The wound in the inguinal region was closed with a skin suture. Results. The analysis of obtained results demonstrated significantly shorter mean hospitalization times in patients treated with PEVAR compared to EVAR. Low rates and types of observed complications in the study group (PEVAR) compared to the control group (EVAR) are strongly in favour the percutaneous technique (PEVAR) of endovascular aortic abdominal aneurysm repair in the infrarenal region of the aorta, confirming its minimally invasive character

The origin and evolution of small dry valleys in the last-glacial area on the example of the Pomeranian Lake District (Poland)

Short dry valleys forming a part of the relief characteristic of the temperate zone occur particularly frequently in areas related to the young-glacial zone. Two areas were selected for analysis – Piaski Pomorskie and Jasień, located in northern Poland, at the back of the Pomeranian phase of the Vistulian glaciation. The aim of the research was to determine the role of denudation processes in the landform transformation of the young-glacial areas, manifested in their cutting and creation of, among others, erosional-denudational valleys, and to compare their geomorphological features. The research included field work (recognition of the geological structure, levelling measurements) and laboratory work (grain size analysis, organic matter content, age determination of the deposits). The valleys from both areas vary in size, but have a similar structure of deposits on their bottoms and at the outlets. Within the bottoms of the valleys, the erosively dissected fossil surfaces of the fluvioglacial substrate are visible or there is an erosional pavement. Slope deposits were found in all the valleys, mainly in the form of massive sand or sand and silt series. Their youngest, topmost part lies on organic sediments dated to 81 (95.4%) 253 cal. AD and 1024 (95.4%) 1224 cal. AD. The most distinct difference in the structure of the valleys of both research fields is the presence of boulder levels only in the Jasień area. This may suggest that the genesis of boulder covers depends not only on climatic and plant conditions, which were similar in both areas, but also on the geological structure and morphometry of the valleys. The obtained results indicate that the valley development was dominated by linear erosion in the Bølling and Allerød, mass movements mainly in the cold stage of the late Vistulian and slopewash, which were especially strong in prehistoric and historic times. The differentiation in the size of valleys only to some extent affects their geological structure and processes that accompanied their evolution

Abstract: The infl uence of maleic hydrazide preharvest treatment on quality and storage ability of early onion cultivars at cold store conditions. The aim of the experiment was to determine the infl uence of maleic hydrazide (Fazor) preharvest treatment on some quality parameters and storage ability of nine popular onion hybrid cultivars from the group of early or semi-early ones: 'Bonus', 'Fireball', 'Spirit', 'Sterling', 'Takmark', 'Takstar', 'Teton', 'Paraat', 'Utopia'. As the reference cultivar, late standard Polish cultivar 'Sochaczewska' was used. Onions were stored for 7 months in a cold store, at the temperature of 1-2°C and RH of 80-85%, an then for one week at simulated retail conditions. Weight losses, percentage of sprouted and rooted onions after storage, darkening level of inner buds, as well dry matter, vitamin C, and essential oils content in freshly harvested and stored onions were determined. Results of the experiment showed that the treatment of onions with MH positively infl uenced their storage ability. The best storage ability showed cvs. 'Sochaczewska' and 'Spirit', and the worst ability -'Bonus' and 'Sterling'. MH treatment decreased tendency to sprouting and rooting of onions during storage, and decreased natural weight losses. Vitamin C content in stored onions was higher after MH treatment, compared with untreated onions. Treatment onions with MH signifi cantly affected essential oils content in bulbs, which was lower in the case of the untreated onions. MH treated onions showed the signs of inner buds darkening, but the level of darkening related to the cultivar, and was generally low. The results obtained showed signifi cant infl uence of MH treatment on decreasing storage losses of early-maturing onion cultivars

This article is published with open access at Springerlink.comIn the paper the optimization problem described by some nonlinear hyperbolic equation being continuous counterpart of the Fornasini-Marchesini model is considered. A theorem on the existence of at least one solution to this hyperbolic PDE is proved and some properties of the set of all solutions are established. The existence of a solution to an optimization problem under appropriate assumptions is themain result of this paper. Some application of the obtained results to the process of gas filtration is also presented
