    Surgical and postsurgical wound care in hidradenitis suppurativa

    Hidradenitis suppurativa (HS) is a chronic inflammatory disorder. Several medical treatments, with varying degree of efficacy, have been developed. However, in most cases of advanced (HS), the definitive treatment option is often represented by surgical excisions. Surgical techniques, reconstructive approach, and local wound care should be accurately designed in order to obtain the best result. In this letter, we analyzed the possible surgical treatments and local wound care. A literature review was performed on the various surgical treatments, reconstructive techniques, and local wound care. Surgical treatment is a common therapeutic modality for HS. Different surgical reconstructive techniques and postsurgical wound care approaches are described for the management of HS patients. There were few high-quality evidence-based studies evaluating the surgical management of HS. Many disparate HS severity scores were used in these studies making comparison between them difficult. Nonetheless, research into different surgical approaches and wound care management has increased substantially in the past decade and has given patients more surgical therapeutic strategies. The description of the best combinations and timing of surgery, wound care and medical therapies, will be a matter of future research for the definition of the optimal management of HS patient

    Centroid moment tensor catalog with 3D lithospheric wavespeed model. The 2016–2017 Central Apennines sequence

    Moment tensor inversions of broadband velocity data are usually managed by adopting Green's functions for 1D layered seismic wave speed models. This assumption can impact on source parameter estimates in regions with complex 3D heterogeneous structures and discontinuities in rock properties. In this work, we present a new centroid moment tensor (CMT) catalog for the Amatrice-Visso-Norcia (AVN) seismic sequence based on a recently generated 3D wave speed model for the Italian lithosphere. Forward synthetic seismograms and FrĂ©chet derivatives for CMT-3D inversions of 159 earthquakes with Mw ≄ 3.0 are simulated using a spectral-element method (SEM) code. By comparing the retrieved solutions with those from time domain moment tensor (TDMT) catalog, obtained with a 1D wave speed model calibrated for Central Apennines (Italy), we observe a remarkable degree of consistency in terms of source geometry, kinematics, and magnitude. Significant differences are found in centroid depths, which are more accurately estimated using the 3D model. Finally, we present a newly designed parameter, τ, to better quantify and compare a-posteriori the reliability of the obtained MT solutions. τ measures the goodness of fit between observed and synthetic seismograms accounting for differences in amplitude, arrival time, percentage of fitted seconds, and the usual L2-norm estimate. The CMT-3D solutions represent the first Italian CMT catalog based on a full-waveform 3D wave speed model. They provide reliable source parameters with potential implications for the structures activated during the sequence. The developed approach can be readily applied to more complex Italian regions where 1D models are underperforming and not representative of the area

    High Magnification Digital Dermoscopy of Basal Cell Carcinoma: A Single-centre Study on 400 cases.

    The aim of this study was to assess the frequency of classic dermoscopic basal cell carcinoma (BCC) features and the sensitivity of new descriptors, such as light brown nests (homogeneous and structured) only visible employing a high magnification digital videomicroscope. A retrospective analysis of 2,024 highly magnified digital images referring to 400 BCCs was performed by 3 independent observers, who assessed 11 classic BCC descriptors and the new ones. Light brown nests were detected in 40.5% of BCCs. Homogeneous ones were observable in 17.8%, and structured nests in 32.8%. Light brown nests were visible in 14.3% of non-pigmented lesions, whereas in the pigmented groups these were observed in 42-54% of the cases. We suggest that brown nests described in this study may improve early recognition of superficial BCCs and of non-pigmented or slightly pigmented ones that may lack classic dermoscopic patterns

    Osteopontin as Candidate Biomarker of Coronary Disease despite Low Cardiovascular Risk: Insights from CAPIRE Study

    Stratification according high cardiovascular (CV) risk categories, still represents a clinical challenge. In this analysis of the CAPIRE study (NCT02157662), we investigate whether inflammation could fit between CV risk factors (RFs) and the presence of coronary artery disease (CAD). In total, 544 patients were included and categorized according with the presence of CAD and CV risk factor burden (low/multiple). The primary endpoint was to verify any independent association of neutrophil-related biomarkers with CAD across CV risk categories. The highest values of osteo-pontin (OPN) were detected in the low RF group and associated with CAD (23.2 vs. 19.4 ng/mL; p = 0.001), although no correlation with plaque extent and/or composition were observed. Con-versely, myeloperoxidase (MPO) and resistin did not differ by CAD presence. Again, OPN was identified as independent variable associated with CAD but only in the low RF group (adjOR 8.42 [95% CI 8.42\u201346.83]; p-value = 0.015). As an ancillary finding, a correlation linked OPN with the neutrophil degranulation biomarker MPO (r = 0.085; p = 0.048) and resistin (r = 0.177; p = 3.4 7 10 125 ). In the present study, OPN further strengthens its role as biomarker of CAD, potentially bridging subclinical CV risk with development of atherosclerosis

    Biomimetic Electrospun Self-Assembling Peptide Scaffolds for Neural Stem Cell Transplantation in Neural Tissue Engineering

    Spinal cord regeneration using stem cell transplantation is a promising strategy for regenerative therapy. Stem cells transplanted onto scaffolds that can mimic natural extracellular matrix (ECM) have the potential to significantly improve outcomes. In this study, we strived to develop a cell carrier by culturing neural stem cells (NSCs) onto electrospun 2D and 3D constructs made up of specific crosslinked functionalized self-assembling peptides (SAPs) featuring enhanced biomimetic and biomechanical properties. Morphology, architecture, and secondary structures of electrospun scaffolds in the solid-state and electrospinning solution were studied step by step. Morphological studies showed the benefit of mixed peptides and surfactants as additives to form thinner, uniform, and defect-free fibers. It has been observed that ÎČ-sheet conformation as evidence of self-assembling has been predominant throughout the process except for the electrospinning solution. In vitro NSCs seeded on electrospun SAP scaffolds in 2D and 3D conditions displayed desirable proliferation, viability, and differentiation in comparison to the gold standard. In vivo biocompatibility assay confirmed the permissibility of implanted fibrous channels by foreign body reaction. The results of this study demonstrated that fibrous 2D/3D electrospun SAP scaffolds, when shaped as micro-channels, can be suitable to support NSC transplantation for regeneration following spinal cord injury

    Structural control on the directional amplification of seismic noise (Campo Imperatore, central Italy)

    Abstract Seismic signals propagating across a fault may yield information on the internal structure of the fault zone. Here we have assessed the amplification of seismic noise (i.e., ambient vibrations generated by natural or anthropogenic disturbances) across the Vado di Corno Fault (Campo Imperatore, central Italy). The fault zone is considered as an exhumed analogue of the normal faults activated during the L'Aquila 2009 earthquake sequence. Detailed structural geological survey of the footwall block revealed that the fault zone is highly anisotropic and is affected by a complex network of faults and fractures with dominant WNW–ESE strike. We measured seismic noise with portable seismometers along a ∌500 m long transect perpendicular to the average fault strike. Seismic signals were processed calculating the horizontal-to-vertical spectral ratios and performing wavefield polarization analyses. We found a predominant NE–SW to NNE–SSW (i.e., ca. perpendicular to the average strike of the fault-fracture network) amplification of the horizontal component of the seismic waves. Numerical simulations of earthquake-induced ground motions ruled out the role of topography in controlling the polarization and the amplitude of the waves. Therefore, the higher seismic noise amplitude observed in the fault-perpendicular direction was related to the measured fracture network and the resulting stiffness anisotropy of the rock mass. These observations open new perspectives in using measures of ambient seismic noise, which are fast and inexpensive, to estimate the dominant orientation of fracture networks within fault zones

    Field cancerization therapy with ingenol mebutate contributes to restoring skin-metabolism to normal-state in patients with actinic keratosis: a metabolomic analysis

    Actinic keratosis (AK) is a skin premalignant lesion, which progresses into squamous cell carcinoma (SCC) if left untreated. Ingenol mebutate gel is approved for local treatment of non-hyperkeratotic, non-hypertrophic AK; it also has the potential to act as a field cancerization therapy to prevent the progression of AK to SCC. To gain better insights into the mechanisms of ingenol mebutate beyond the mere clinical assessment, we investigated, for the first time, the metabolome of skin tissues from patients with AK, before and after ingenol mebutate treatment, with high-resolution magic angle spinning nuclear magnetic resonance spectroscopy. The metabolomic profiles were compared with those of tissues from healthy volunteers. Overall, we identified a number of metabolites, the homeostasis of which became altered during the process of tumorigenesis from healthy skin to AK, and was restored, at least partially, by ingenol mebutate therapy. These metabolites may help to attain a better understanding of keratinocyte metabolism and to unmask the metabolic pathways related to cell proliferation. These results provide helpful information to identify biomarkers with prognostic and therapeutic significance in AK, and suggest that field cancerization therapy with ingenol mebutate may contribute to restore skin metabolism to a normal state in patients with AK
