Complete Caudate Lobectomy: Its Definition, Indications, and Surgical Approaches
There are three ways to approach and resect the
caudate lobe of the liver, that is; and isolated caudate
lobectomy, a combined resection of the liver overlying
the caudate lobe, and a transhepatic anterior
approach by splitting parenchyma of the liver
Pseudogap formation in organic superconductors
The condensation of paired fermions into superfluid states changes
progressively depending on the coupling strength. At the midpoint of the
crossover between Bardeen--Cooper--Schrieffer (BCS) weak-coupling and
Bose--Einstein condensate (BEC) strong-coupling limits, paired fermions
condensate most robustly, thereby leading to the emergence of a pseudogap due
to enhanced pairing fluctuations. In the case of electrons in solids,
excessively strong interactions often induce competing electronic orders
instead of strong-coupling superconductivity, and experimental comprehension of
the pseudogap remains incomplete. In this study, we provide experimental
evidence demonstrating the opening of a pseudogap, marking the incipient stage
of the BCS-BEC crossover in the organic system -(BEDT-TTF). By
controlling electron correlations, we investigate the thermodynamic properties
of the BCS-BEC crossover and pseudogap phase. Since the superconductivity of
-(BEDT-TTF) arises from a simple Fermi liquid that does not
exhibit any other electronic orders, our study shed light on the inherent
nature of the BCS-BEC crossover
Black hole evaporation in a heat bath as a nonequilibrium process and its final fate
When a black hole evaporates, there arises a net energy flow from black hole
into its outside environment (heat bath). The existence of energy flow means
that the thermodynamic state of the whole system, which consists of the black
hole and the heat bath, is in a nonequilibrium state. Therefore, in order to
study the detail of evaporation process, the nonequilibrium effects of the
energy flow should be taken into account. Using the nonequilibrium
thermodynamics which has been formulated recently, this paper shows the
following: (1) Time scale of black hole evaporation in a heat bath becomes
shorter than that of the evaporation in an empty space (a situation without
heat bath), because a nonequilibrium effect of temperature difference between
the black hole and heat bath appears as a strong energy extraction from the
black hole by the heat bath. (2) Consequently a huge energy burst (stronger
than that of the evaporation in an empty space) arises at the end of
semi-classical stage of evaporation. (3) It is suggested that a remnant of
Planck size remains after the quantum stage of evaporation in order to
guarantee the increase of total entropy of the whole system
The reliability and validity of the Japanese version of the Levels of Emotional Awareness Scale (LEAS-J)

Abstract

Background

The Levels of Emotional Awareness Scale (LEAS) was developed to assess five levels of emotional awareness: bodily sensations, action tendencies, single emotions, blends of emotion, and combinations of blends. It is a paper and pencil performance questionnaire that presents 20 emotion-evoking scenes. We developed a Japanese version of the LEAS (LEAS-J), and its reliability and validity were examined.

Methods

The LEAS-J level was independently assessed by two researchers who scored each response according to the LEAS scoring manual. High inter-rater reliability and internal consistency were obtained for the LEAS-J. Measures were socioeconomic status, LEAS-J, Toronto Alexithymia Scale-20 (TAS-20), Interpersonal Reactivity Index (IRI), and NEO Five-Factor Inventory (NEO-FFI). TAS-20, IRI and NEO-FFI were the measures used to explore the construct validity of LEAS-J, as it was predicted that higher scores on the LEAS-J would be related to fewer alexithymic features, greater empathetic ability, and a greater sense of cooperation with others. Questionnaires were completed by 344 university students.

Results

The criterion-referenced validity was determined: a significant negative relationship was found with the externally-oriented thinking scores of TAS-20, and positive relationships were found with fantasy, perspective taking, and empathic concern on IRI and with extraversion, openness to experience, and agreeableness on NEO-FFI.

Conclusions

Consistent with our expectations, the findings provide evidence that the LEAS-J has good reliability and validity. In addition, women had significantly higher scores than men on LEAS-J, showing that the gender difference identified in the original LEAS was cross-culturally consistent.</p
Ultrasound for Perioperative Lung Monitoring of Patients Undergoing Thoracic Surgery with One-Lung Ventilation
Background: The diagnostic efficacy of lung ultrasonography (LUS) has been widely investigated. However, the clinical value of LUS for perioperative monitoring has rarely been reported. The aim of this study was to evaluate the ability of LUS to assess lung aeration status after one-lung ventilation (OLV) using a validated scoring system.
Methods: In this prospective observational study, patients undergoing elective video-assisted thoracic surgery (VATS) with OLV underwent a lung ultrasound examination just after induction of anesthesia and at the end of the surgery. After each lung ultrasound examination, a semiquantitative score, the LUS score, was calculated to assess lung aeration on the ventilated dependent side and the non-dependent side separately. The relationship between the LUS scores and various patient-related factors was also investigated.
Results: Twenty-five patients were studied. All lung ultrasound examinations were successfully completed. LUS scores after OLV on the dependent side (median [IQR]: 2 [1–4]) increased significantly from baseline (1 [0–1.5], P < 0.001). Further, LUS scores on the non-dependent side (2 [1.5–3.5]) increased significantly from baseline (1 [0–1.5], P < 0.001). None of the factors analyzed was significantly correlated with LUS scores after OLV.
Conclusion: LUS examination is possible after VATS with OLV on both sides of the thorax. Ultrasonography-measured lung aeration scores increased from baseline on both sides
Isolated gestational proteinuria preceding the diagnosis of preeclampsia : an observational study
Introduction. Some pregnant women develop significant proteinuria in the absence of hypertension. However, clinical significance of isolated gestational proteinuria (IGP) is not well understood. This study aimed to determine the prevalence of IGP in singleton pregnancies and the proportion of women with IGP who subsequently developed preeclampsia (IGP-PE) among all PE cases. Material and methods. This was an observational study of 6819 women with singleton pregnancies at 12 centers, including 938 women with at least once determination of protein-to-creatinine ratio (P/Cr). Significant proteinuria in pregnancy (SPIP) was defined as P/Cr (mg/mg) level >0.27. IGP was defined as SPIP in the absence of hypertension. Gestational hypertension (GH) preceding preeclampsia (GH-PE) was defined as preeclampsia (PE) in which GH preceded SPIP. Simultaneous PE (S-PE) was defined as PE in which both SPIP and hypertension occurred simultaneously. Results. IGP and PE were diagnosed in 130 (1.9%) and 158 (2.3%) of 6819 women, respectively. Of 130 women with IGP, 32 (25%) progressed to PE and accounted for 20% of all women with PE. Hence, women with IGP had a relative risk of 13.1 (95% CI; 9.2-18.5) for developing PE compared with those without IGP [25% (32/130) vs. 1.9% (126/6689)]. At diagnosis of SPIP, P/Cr levels already exceeded 1.0 more often in women with S-PE than in those with IGP-PE [67% (33/49) vs. 44% (14/32), respectively, p = 0.031]. Conclusions. IGP is a risk factor for PE, and IGP-PE accounts for a considerable proportion (20%) of all PE
