    A Novel Data Engineering Process Which Integrates Alert Information, Security Logs, And SOC Analysts

    We build up a user centric ML system for the cyber security operation center in endeavor environment. We examine the regular data sources in SOC, their work process, and how to leverage and procedure these data sets to construct an effective ML system. The work is besieged towards two groups of readers. The primary group is data scientists or ML researchers who do not have cyber security domain awareness but want to build ML systems for safety operations center. The second group of people is those cyber security practitioners who have deep information and expertise in cyber security, but do not have ML knowledge and wish to construct one by them. All through the work, we use the system we built in the Symantec SOC construction setting as an example to display the full steps from data collection, label creation, feature engineering, ML algorithm selection, and model show evaluations, to risk score making

    Study of pregnancy outcome in elderly gravida

    Background: Elderly gravida which is defined as age of mother as 35 years or more at the time of delivery has become increasingly common in last two to three decades. This accelerating demographic shift is of major clinical and public health concern, because advanced maternal age has consistently been associated with adverse pregnancy outcomes.Methods: An observational prospective study consisting of 120 pregnant women. All participants were divided into Study group and Control group.Results: Women of advanced maternal age have higher frequency of antenatal complications leading to early pregnancy termination and operative interference. There is significant increase in the incidence of adverse perinatal outcome due to preterm deliveries and NICU admission and neonatal death.Conclusions: With improved health services and better prospects of women who are >35 years of becoming pregnant, these pregnancies are fraught with complications. Women should realistically appraise the risks of pregnancy in later life. Hence these women should be counselled, and their complications managed with utmost care

    Development and Validation of Clinical Whole-Exome and Whole-Genome Sequencing for Detection of Germline Variants in Inherited Disease

    Context.-With the decrease in the cost of sequencing, the clinical testing paradigm has shifted from single gene to gene panel and now whole-exome and whole-genome sequencing. Clinical laboratories are rapidly implementing next-generation sequencing-based whole-exome and whole-genome sequencing. Because a large number of targets are covered by whole-exome and whole-genome sequencing, it is critical that a laboratory perform appropriate validation studies, develop a quality assurance and quality control program, and participate in proficiency testing. Objective.-To provide recommendations for wholeexome and whole-genome sequencing assay design, validation, and implementation for the detection of germline variants associated in inherited disorders. Data Sources.-An example of trio sequencing, filtration and annotation of variants, and phenotypic consideration to arrive at clinical diagnosis is discussed. Conclusions.-It is critical that clinical laboratories planning to implement whole-exome and whole-genome sequencing design and validate the assay to specifications and ensure adequate performance prior to implementation. Test design specifications, including variant filtering and annotation, phenotypic consideration, guidance on consenting options, and reporting of incidental findings, are provided. These are important steps a laboratory must take to validate and implement whole-exome and whole-genome sequencing in a clinical setting for germline variants in inherited disorders

    Ayurvedic management of Diabetic Foot Ulcer - A Case Study

    There are 424.9 million adults estimated to currently have diabetes globally.[1] Diabetic patients have an inherent risk of foot problems. Globally every 30 seconds, a major amputation occurs; 85% of which are preceded by trivial foot lesions.[2] In Ayurvedic literature, it is explained that Pidakas are formed in Adhoshaka in patients with Prameha due to the weakness of Rasayanis of lower limbs.[3] Such Pidakas if neglected over a period of time, without treatment will undergo Paaka and form a Vrana. Acharya Sushruta has described Shashti Upakramas (60 modalities) for the successful management of Vranas. Among them this article highlights the use of Shodhana, Kshara Varma, Chedana, Ropana and Bandhana Upakramas in the management of Diabetic Foot Ulcer/ Madhumehajanya Dushtavrana and also the need of Shastrakarma (surgical intervention) at the right time. Using various modeling techniques it is convenient to recognize various facial expressions. In the field of image processing it is very interesting to recognize the human gesture by observing the different movement of eyes, mouth, and nose. Recognition of face emotion can be carried out, any neural network for recognizing the facial expression. My seminar includes multilayer perceptron techniques of facial emotion recognition systems. An even simpler multi-layer perceptron model is presented with three outputs. Multilayer perceptrons (MLP) has been proven to be very successful in many applications including classification.     A Rare Site of Metachronous Metastases from Renal Cell Carcinoma

    Secondary metastatic involvement of the testis is a rare occurrence, particularly in cases of metastasis from renal cell carcinoma (RCC). We present a case of metachronous contralateral testicular metastasis from RCC in a 55-year-old man, occurring 2 years after radical nephrectomy. Following a thorough evaluation that ruled out systemic disease, the patient underwent a Chevassu procedure and right inguinal orchidectomy. Histopathological analysis confirmed metastatic involvement of the right testis by RCC. Metastasis to the testis from RCC is uncommon, with only a few cases reported in the literature. Isolated metachronous metastasis without systemic involvement is even rarer. This case highlights the importance of considering testicular metastasis in patients with a history of RCC, emphasizing the need for comprehensive evaluation and surgical resection when feasible, as it has been associated with prolonged survival

    Study of coronary artery calcium by multi-slice spiral CT as a tool for high risk cardiovascular screening

    Background: Strong relationship has been demonstrated between the presence of occlusive CAD and coronary artery calcification (CAC) detected at autopsy, fluoroscopy and computed tomography (CT scan). CT scan quantifies the relative burden of CAC deposits as a marker of atherosclerosis. We explored utility of multi-slice spiral CT scan for detection of CAC as a tool for screening in asymptomatic high-risk cases above 40 years of age.Methods: Fifty-eight asymptomatic cardiovascular high-risk cases were included as per selection criteria. Detailed clinical history was recorded in every case regarding age, sex, history of risk factors for CAD like systemic hypertension, diabetes mellitus, smoking and family history of CAD. Every case was evaluated for fasting and postprandial blood sugar and fasting lipid profile. Body mass index (BMI) was calculated. An ECG was also recorded. Coronary artery calcium was estimated in each patient by multi -slice spiral CT scan. Fifty age and gender matched consenting participants were enrolled as controls and comparisons drawn.Results: Out of 58 cases scanned, 41 had no detectable CAC by multi-slice CT scan whereas in 17 cases (29.2%), calcium was detected. Mean CAC score was observed to increase as number of risk factors increased.Conclusions: Multi-slice CT scan is a useful tool for detection of coronary artery calcium (CAC) when utilised for high risk screening in older adults having one or more known cardiovascular risk factors

    Mucoadhesive carbamazepine gel for in situ olfactory delivery

    Purpose: To formulate mucoadhesive carbamazepine gel for delivery to the brain via the olfactory mucosa. Methods: Carbamazepine transfersomes were formulated using Lipoid S 100 and sodium cholate. The transfersomes were evaluated for entrapment efficiency, in vitro release transmission electron microscopy, zeta potential, polydispersity index. The transfersomes were then incorporated into gellan gum gel, and the in situ gel formulation was evaluated for drug content, gel strength, in vitro release and mucoadhesive force. Transfersomes were also evaluated for bioanalytical study in rats. Result: TEM analysis showed good regular spheres. The negative zeta potential ensures resistance to aggregation. The gel strength of the formulations was in the range of 0.6 to 7.4 g. In vitro diffusion study of transfersomal gel showed Fickian diffusion mechanism. Formulation F6 was optimized depending for gel strength (6.4 g) , drug content (99.47 ± 0.25 %), and good mucoadhesive force (50.24 ± 0.76 dyne/cm2). Bioanalytical study of F6 showed increased drug concentration in brain. Conclusion: Mucoadhesive carbanmazepine gel can be used effectively to achieve increased concentration of drug in the brain via olfactory mucosal rout
