    Unusual presentation of synovial sarcoma as meniscal cyst: A Case Report

    Periarticular cyst and cystic soft tissue lesion around the knee are common. Synovial sarcoma is a rare and malignant soft tissue tumor accounting for approximately 5 of soft tissue sarcoma. A case is presented where a lesion adjacent to the joint line of the knee was diagnosed clinically and on imaging as a meniscal cyst. MRI signal was homogenous and no concomitant meniscal tears were seen. The tissue diagnosis was monophasic synovial sarcoma. © 2015 BY THE ARCHIVES OF BONE AND JOINT SURGERY

    Longitudinal evaluation, acceptability and long-term retention of knowledge on a horizontally integrated organic and functional systems course

    Undergraduate medical education is moving from traditional disciplinary basic science courses into more integrated curricula. Integration models based on organ systems originated in the 1950s, but few longitudinal studies have evaluated their effectiveness. This article outlines the development and implementation of the Organic and Functional Systems (OFS) courses at the University of Minho in Portugal, using evidence collected over 10 years. It describes the organization of content, student academic performance and acceptability of the courses, the evaluation of preparedness for future courses and the retention of knowledge on basic sciences. Students consistently rated the OFS courses highly. Physician tutors in subsequent clinical attachments considered that students were appropriately prepared. Performance in the International Foundations of Medicine examination of a self-selected sample of students revealed similar performances in basic science items after the last OFS course and 4 years later, at the moment of graduation. In conclusion, the organizational and pedagogical approaches of the OFS courses achieve high acceptability by students and result in positive outcomes in terms of preparedness for subsequent training and long-term retention of basic science knowledge

    Acute Retropharyngeal Calcific Tendinitis in an Unusual Location: a Case Report in a Patient with Rheumatoid Arthritis and Atlantoaxial Subluxation

    Retropharyngeal calcific tendinitis is defined as inflammation of the longus colli muscle and is caused by the deposition of calcium hydroxyapatite crystals, which usually involves the superior oblique fibers of the longus colli muscle from C1-3. Diagnosis is usually made by detecting amorphous calcification and prevertebral soft tissue swelling on radiograph, CT or MRI. In this report, we introduce a case of this disease which was misdiagnosed as a retropharyngeal tuberculous abscess, or a muscle strain of the ongus colli muscle. No calcifications were visible along the vertical fibers of the longus colli muscle. The lesion was located anterior to the C4-5 disc, in a rheumatoid arthritis patient with atlantoaxial subluxation. Calcific tendinitis of the longus colli muscle at this location in a rheumatoid arthritis patient has not been reported in the English literature

    A two-grid nite-element/reduced basis scheme for the approximation of the solution of parameter dependent P.D.E

    In the frame of optimization process in industrial framework, where numerical simulation is used at some stage, the same problem, modeled with partial differential equations depending on a parameter has to be solved many times for different sets of parameters. The reduced basis method may be successful in this frame and recent progress have permitted to make the computations reliable thanks to a posteriori estimators and to extend the method to nonlinear problems thanks to the "magic points" interpolation. However, it may not always be possible to use the code (for example of finite element type that allows for evaluating the elements of the reduced basis) to perform all the "off-line" computations required for an efficient performance of the reduced basis method. We propose here an alternating approach based on a coarse grid finite element the convergence of which is accelerated through the reduced basis and an improved post processing

    Phase I Study of the Prolactin Receptor Antagonist LFA102 in Metastatic Breast and Castration-Resistant Prostate Cancer

    Lessons Learned Despite evidence for a role for prolactin signaling in breast and prostate tumorigenesis, a prolactin receptor-binding monoclonal antibody has not produced clinical efficacy. Increased serum prolactin levels may be a biomarker for prolactin receptor inhibition. Results from the pharmacokinetic and pharmacodynamics (PD) studies suggest that inappropriately long dosing intervals and insufficient exposure to LFA102 may have resulted in lack of antitumor efficacy. Based on preclinical data, combination therapy of LFA102 with those novel agents targeting hormonal pathways in metastatic castration-resistant prostate cancer and metastatic breast cancer is promising. Given the PD evidence of prolactin receptor blockade by LFA102, this drug has the potential to be used in conditions such as hyperprolactinemia that are associated with high prolactin levels. Background. Prolactin receptor (PRLR) signaling is implicated in breast and prostate cancer. LFA102, a humanized monoclonal antibody (mAb) that binds to and inhibits the PRLR, has exhibited promising preclinical antitumor activity. Methods. Patients with PRLR-positive metastatic breast cancer (MBC) or metastatic castration-resistant prostate cancer (mCRPC) received doses of LFA102 at 3–60 mg/kg intravenously once every 4 weeks. Objectives were to determine the maximum tolerated dose (MTD) and/or recommended dose for expansion (RDE) to investigate the safety/tolerability of LFA102 and to assess pharmacokinetics (PK), pharmacodynamics (PD), and antitumor activity. Results. A total of 73 patients were enrolled at 5 dose levels. The MTD was not reached because of lack of dose-limiting toxicities. The RDE was established at 60 mg/kg based on PK and PD analysis and safety data. The most common all-cause adverse events (AEs) were fatigue (44%) and nausea (33%) regardless of relationship. Grade 3/4 AEs reported to be related to LFA102 occurred in 4% of patients. LFA102 exposure increased approximately dose proportionally across the doses tested. Serum prolactin levels increased in response to LFA102 administration, suggesting its potential as a biomarker for PRLR inhibition. No antitumor activity was detected. Conclusion. Treatment with LFA102 was safe and well tolerated, but did not show antitumor activity as monotherapy at the doses tested

    Daily Mobility Patterns: Publicness as a Lived Experience in a Hidden Pedestrian Hub

    Publicness can be considered as a way of experiencing the contemporary city. In creating publicness, we often focus just on places. There is ambiguity about publicness in transport infrastructure point as place or non-place. In that space, groups of individuals are formed, all set on the goal of reaching a destination. That groups can be named momentary communities. This paper sets out to examine how momentary communities have formed, their needs and aspects of usage, and how they have brought publicness as a lived experience. The study area is Mostar Interchange - an important transport point of Belgrade that connects the city and highway system. It is an intersection point of civic and transport infrastructure. At the same time, interchange connects and separates the city. Historically, by its forming, it erased this part of the city and made the radical cut in the urban tissue. Today, the important parts of the development of the city are urban tissue toward the center, as well as the riverfront, while the transport part remains neglected and witnesses radical urban changes in its environment. Therefore, this space can be considered as a liminal space that has potential. The purpose of this paper is to open the questions and reconsider possibilities of inquiries values of liminal spaces treatment, especially in the context of planning practice. In that sense, the focus of methodology is on literature review, historic research, and walking interview as input data and observations as mapping (urban reading) through different frameworks, and also testing that combined approach in the pedestrian hub of Mostar Interchange as a case study of liminal space. The results should show that although interchange is a liminal space, it has its hidden values in the daily mobility patterns of users that reveal the life of a pedestrian hub with its rules and problems. It is necessary to work on new pedestrian mobility in the development of this space in the city which will include neglected needs of momentary communities
