    The Internationalization of the Student Body at Oceans College, Canada – An Appreciative Inquiry and Shared Leadership Approach to Change

    This Organizational Improvement Plan (OIP) addresses a problem of practice (PoP) at Oceans College (a pseudonym), a multilocation, panprovincial community college. Oceans College currently focuses on recruiting, admitting, and graduating domestic, in-province students. The number of international students, however, is growing. Currently there are no formal processes at the College for international students, unlike at the other postsecondary institutions in the province which actively recruit and enroll international students. The PoP is the lack of readiness at Oceans College to cope with the significant enrollment of international students. Different leadership theories were examined to develop this OIP, and appreciative inquiry (AI) and shared leadership were chosen as frameworks. They both take a positive approach, looking for the best in people. They both fit the matrix organization of Oceans College. Three key tenets used in this OIP are that Oceans College is doing many things well, that understanding the culture of the College is key, and that communication is core to the success of this OIP. The conceptual model I developed is the 4 C’s of Change—context, culture, clarity, and communication. Using this conceptual model, AI, and shared leadership, along with cultural theory, I developed an implementation plan that will allow Oceans College to internationalize the student body over a period of several years. Assessment and evaluation plans were also developed, as was a detailed plan to communicate the change to all stakeholders. The conjunction of theory, concepts, and experience has created an OIP that not only solves the PoP, but also keeps in mind what is already being done well and what would complement the college culture.     Factors Affecting Trypanosome Maturation in Tsetse Flies

    Trypanosoma brucei brucei infections which establish successfully in the tsetse fly midgut may subsequently mature into mammalian infective trypanosomes in the salivary glands. This maturation is not automatic and the control of these events is complex. Utilising direct in vivo feeding experiments, we report maturation of T. b. brucei infections in tsetse is regulated by antioxidants as well as environmental stimuli. Dissection of the maturation process provides opportunities to develop transmission blocking vaccines for trypanosomiasis. The present work suggests L-cysteine and/or nitric oxide are necessary for the differentiation of trypanosome midgut infections in tsetse

    The Impact and Cost of Scaling up GeneXpert MTB/RIF in South Africa

    We estimated the incremental cost and impact on diagnosis and treatment uptake of national rollout of Xpert MTB/RIF technology (Xpert) for the diagnosis of pulmonary TB above the cost of current guidelines for the years 2011 to 2016 in South Africa.We parameterised a population-level decision model with data from national-level TB databases (n = 199,511) and implementation studies. The model follows cohorts of TB suspects from diagnosis to treatment under current diagnostic guidelines or an algorithm that includes Xpert. Assumptions include the number of TB suspects, symptom prevalence of 5.5%, annual suspect growth rate of 10%, and 2010 public-sector salaries and drug and service delivery costs. Xpert test costs are based on data from an in-country pilot evaluation and assumptions about when global volumes allowing cartridge discounts will be reached.At full scale, Xpert will increase the number of TB cases diagnosed per year by 30%-37% and the number of MDR-TB cases diagnosed by 69%-71%. It will diagnose 81% of patients after the first visit, compared to 46% currently. The cost of TB diagnosis per suspect will increase by 55% to USD 60-61 and the cost of diagnosis and treatment per TB case treated by 8% to USD 797-873. The incremental capital cost of the Xpert scale-up will be USD 22 million and the incremental recurrent cost USD 287-316 million over six years.Xpert will increase both the number of TB cases diagnosed and treated and the cost of TB diagnosis. These results do not include savings due to reduced transmission of TB as a result of earlier diagnosis and treatment initiation

    Molecular reductions in glucokinase activity increase counter-regulatory responses to hypoglycemia in mice and humans with diabetes.

    OBJECTIVE: Appropriate glucose levels are essential for survival; thus, the detection and correction of low blood glucose is of paramount importance. Hypoglycemia prompts an integrated response involving reduction in insulin release and secretion of key counter-regulatory hormones glucagon and epinephrine that together promote endogenous glucose production to restore normoglycemia. However, specifically how this response is orchestrated remains to be fully clarified. The low affinity hexokinase glucokinase is found in glucose-sensing cells involved in glucose homeostasis including pancreatic β-cells and in certain brain areas. Here, we aimed to examine the role of glucokinase in triggering counter-regulatory hormonal responses to hypoglycemia, hypothesizing that reduced glucokinase activity would lead to increased and/or earlier triggering of responses. METHODS: Hyperinsulinemic glucose clamps were performed to examine counter-regulatory responses to controlled hypoglycemic challenges created in humans with monogenic diabetes resulting from heterozygous glucokinase mutations (GCK-MODY). To examine the relative importance of glucokinase in different sensing areas, we then examined responses to clamped hypoglycemia in mice with molecularly defined disruption of whole body and/or brain glucokinase. RESULTS: GCK-MODY patients displayed increased and earlier glucagon responses during hypoglycemia compared with a group of glycemia-matched patients with type 2 diabetes. Consistent with this, glucagon responses to hypoglycemia were also increased in I366F mice with mutated glucokinase and in streptozotocin-treated β-cell ablated diabetic I366F mice. Glucagon responses were normal in conditional brain glucokinase-knockout mice, suggesting that glucagon release during hypoglycemia is controlled by glucokinase-mediated glucose sensing outside the brain but not in β-cells. For epinephrine, we found increased responses in GCK-MODY patients, in β-cell ablated diabetic I366F mice and in conditional (nestin lineage) brain glucokinase-knockout mice, supporting a role for brain glucokinase in triggering epinephrine release. CONCLUSIONS: Our data suggest that glucokinase in brain and other non β-cell peripheral hypoglycemia sensors is important in glucose homeostasis, allowing the body to detect and respond to a falling blood glucose.Yousef Jameel Fund Sir Jukes Thorn Trust Elmore Fund Chang Gung University College of Medicin

    Geomagnetically induced currents during the 07-08 September 2017 disturbed period: a global perspective

    Measurements from six longitudinally separated magnetic observatories, all located close to the 53⁰ mid-latitude contour, are analysed. We focus on the large geomagnetic 16 disturbance that occurred during 7 and 8 September 2017. Combined with available geomagnetically induced current (GIC) data from two substations, each located near to a 18 magnetic observatory, we investigate the magnetospheric drivers of the largest events. We analyse solar wind parameters combined with auroral electrojet indices to investigate the driving mechanisms. Six magnetic field disturbance events were observed at mid-latitudes with dH/dt >60 nT/min. Co-located GIC measurements identified transformer currents >15 A during three of the events. The initial event was caused by a solar wind pressure pulse causing largest effects on the dayside, consistent with the rapid compression of the dayside geomagnetic field. Four of the events were caused by substorms. Variations in the Magnetic Local Time of the maximum effect of each substorm-driven event were apparent with magnetic midnight, morning-side, and dusk-side events all occurring. The six events 27 occurred over a period of almost 24 hours, during which the solar wind remained elevated at >700 km s -1, indicating an extended time scale for potential GIC problems in electrical power networks following a sudden storm commencement. This work demonstrates the challenge of understanding the causes of ground-level magnetic field changes (and hence GIC magnitudes) for the global power industry. It also demonstrates the importance of 32 magnetic local time and differing inner magnetospheric processes when considering the global hazard posed by GIC to power grids
