    Reliability of measuring thoracic kyphosis angle, lumbar lordosis angle and straight leg raise with an inclinometer

However, attempts to quantify the reliability of the inclinometer in these measurements are scarce. In addition, existing reliability studies within the literature were found to use small sample sizes. The aim of this investigation was to examine the intra-rater reliability of the chief investigator (SM), in order to provide clinicians with data that will allow them to better measure sagittal spinal posture and SLR. A blinded test-retest design was performed to determine the intra-rater reliability of thoracic kyphosis, lumbar lordosis and SLR when assessed using an Isomed inclinometer in normals. Methods. Thirty asymptomatic subjects were assessed on two occasions separated by a time interval of 1 hour to reduce investigator memory bias. Thoracic and lumbar measurements were recorded in a relaxed standing position using an inclinometer; SLR of the dominant leg was assessed with subjects in the supine position. Intraclass correlation coefficients (ICC), 95% confidence intervals (CI), and standard errors of measurement (SEM) were analysed to determine measurement reliability. Results. The chief investigator demonstrated excellent intra-rater reliability in the measurements of thoracic kyphosis, lumbar lordosis and SLR. ICC (2,3) values for all three variables exceeded the 0.90 threshold suggesting that the reliability of these measures are acceptable for clinical application. Conclusions. The inclinometer technique employed in this study to record thoracic kyphosis, lumbar lordosis and SLR is a reliable measurement method.Peer reviewe

    Analysis of Exertion-Related Injuries and Fatalities in Laborers in the United States

    Laborers are particularly vulnerable to exertional injuries and illnesses, as they often engage in heavy physical work for prolonged hours, yet no studies have examined the top causes of catastrophic exertional injuries and fatalities among this population. The purpose of the investigation was to characterize the top causes of exertional injury and fatality within open access, Occupational Safety and Health Administration (OSHA) reportable data. A secondary analysis of OSHA reported injury and fatality data was performed through open access records from OSHA Severe Injury Reports (2015–2022) and OSHA fatality inspection data (2017–2020), respectively. The research team characterized each reported injury and fatality as “exertion-related” or “non-exertion-related. Injury and fatality rates were reported per 100,000 equivalent full-time worker years and included 95% confidence intervals (95% CI). Of 58,648 cases in the OSHA Severe Injury Report database from 2015–2020, 1682 cases (2.9%) were characterized as exertional (0.20 injuries per 100,000 full-time worker years, 95% CI: 0.19, 0.22). Heat-related injuries encompassed 91.9% of the exertional injuries (n = 1546). From the 2017–2022 OSHA fatality inspection database, 89 (1.9%) of 4598 fatalities were characterized as exertion-related (fatality rate: 0.0160 per 100,000 full-time equivalent workers, 95% CI: 0.009, 0.0134). The exertion-related fatalities primarily consisted of heat-related cases (87.6%). Exertion-related injuries and fatalities were most reported in Southeast states, in the construction and excavation industry, and among nonunionized workers. As heat stress continues to be recognized as an occupational health and safety hazard, this analysis further highlights the need for targeted interventions or further evaluation of the impact of heat stress on construction and excavation workers, nonunionized workers, and workers in Southeastern states

    Steroid-sparing agents in giant cell arteritis

    Background: Giant cell arteritis is the commonest form of medium-to-large vessel vasculitis, requiring long-term corticosteroid therapy. The short- and long-term side effects of corticosteroids are many, including weight gain, psychological effects, osteoporosis, cardiometabolic complications, and infections. Materials and Methods: Various agents used in place of or in combination with corticosteroids to reduce corticosteroid-related side effects were reviewed. However, considerable variation in practice was identified giving unclear guidance. This review included the most recent evidence on methotrexate, mycophenolate mofetil, azathioprine, cyclophosphamide, abatacept, and tocilizumab Results and Discussion: Also discussed are encouraging results with tocilizumab in GCA patients. Amongst the agents available for steroid-sparing effects, tocilizumab demonstrated the most robust data and is consequently recommended as the agent of choice for steroid-sparing, for remission induction, remission maintenance, and treating relapsing and refractory cases of GCA.Published versio

    Novel Regulation of Integrin Trafficking by Rab11-FIP5 in Aggressive Prostate Cancer

    The laminin-binding integrins, alpha 3 beta 1 and alpha 6 beta 1, are needed for tumor metastasis and their surface expression is regulated by endocytic recycling. beta 1 integrins share the Rab11 recycling machinery, but the trafficking of alpha 3 beta 1 and alpha 6 beta 1 are distinct by an unknown mechanism. Using a mouse PDX tumor model containing human metastatic prostate cancer, Rab11 family interacting protein 5 (Rab11-FIP5) was identified as a lead candidate for a6b1 trafficking. Rab11-FIP5 and its membrane-binding domain were required for alpha 6 beta 1 recycling, without affecting the other laminin-binding integrin (i.e., alpha 6 beta 1) or unrelated membrane receptors like CD44, transferrin receptor, or E-cad-herin. Depletion of Rab11-FIP5 resulted in the intracellular accumulation of alpha 6 beta 1 in the Rab11 recycling compartment, loss of cell migration on laminin, and an unexpected loss of alpha 6 beta 1 recycling in cell-cell locations. Taken together, these data demonstrate that alpha 6 beta 1 is distinct from alpha 3 beta 1 via Rab11-FIP5 recycling and recycles in an unexpected cell-cell location.Pacific Northwest Prostate Cancer SPORE [P50CA97186]; PO1 NIH grant [PO1CA085859]; Richard M.
