Thresholds for Abdominal Aortic Aneurysm Repair in England and the United States.
Background Thresholds for repair of abdominal aortic aneurysms vary considerably among countries. Methods We examined differences between England and the United States in the frequency of aneurysm repair, the mean aneurysm diameter at the time of the procedure, and rates of aneurysm rupture and aneurysm-related death. Data on the frequency of repair of intact (nonruptured) abdominal aortic aneurysms, in-hospital mortality among patients who had undergone aneurysm repair, and rates of aneurysm rupture during the period from 2005 through 2012 were extracted from the Hospital Episode Statistics database in England and the U.S. Nationwide Inpatient Sample. Data on the aneurysm diameter at the time of repair were extracted from the U.K. National Vascular Registry (2014 data) and from the U.S. National Surgical Quality Improvement Program (2013 data). Aneurysm-related mortality during the period from 2005 through 2012 was determined from data obtained from the Centers for Disease Control and Prevention and the U.K. Office of National Statistics. Data were adjusted with the use of direct standardization or conditional logistic regression for differences between England and the United States with respect to population age and sex. Results During the period from 2005 through 2012, a total of 29,300 patients in England and 278,921 patients in the United States underwent repair of intact abdominal aortic aneurysms. Aneurysm repair was less common in England than in the United States (odds ratio, 0.49; 95% confidence interval [CI], 0.48 to 0.49; P<0.001), and aneurysm-related death was more common in England than in the United States (odds ratio, 3.60; 95% CI, 3.55 to 3.64; P<0.001). Hospitalization due to an aneurysm rupture occurred more frequently in England than in the United States (odds ratio, 2.23; 95% CI, 2.19 to 2.27; P<0.001), and the mean aneurysm diameter at the time of repair was larger in England (63.7 mm vs. 58.3 mm, P<0.001). Conclusions We found a lower rate of repair of abdominal aortic aneurysms and a larger mean aneurysm diameter at the time of repair in England than in the United States and lower rates of aneurysm rupture and aneurysm-related death in the United States than in England. Comparison and Mapping Facilitate Relation Discovery and Predication
Relational concepts play a central role in human perception and cognition, but little is known about how they are acquired. For example, how do we come to understand that physical force is a higher-order multiplicative relation between mass and acceleration, or that two circles are the same-shape in the same way that two squares are? A recent model of relational learning, DORA (Discovery of Relations by Analogy; Doumas, Hummel & Sandhofer, 2008), predicts that comparison and analogical mapping play a central role in the discovery and predication of novel higher-order relations. A picogram and nanometer scale photonic crystal opto-mechanical cavity
We describe the design, fabrication, and measurement of a cavity
opto-mechanical system consisting of two nanobeams of silicon nitride in the
near-field of each other, forming a so-called "zipper" cavity. A photonic
crystal patterning is applied to the nanobeams to localize optical and
mechanical energy to the same cubic-micron-scale volume. The picrogram-scale
mass of the structure, along with the strong per-photon optical gradient force,
results in a giant optical spring effect. In addition, a novel damping regime
is explored in which the small heat capacity of the zipper cavity results in
blue-detuned opto-mechanical damping.Comment: 15 pages, 4 figure
Fast cavity-enhanced atom detection with low noise and high fidelity
Cavity quantum electrodynamics describes the fundamental interactions between
light and matter, and how they can be controlled by shaping the local
environment. For example, optical microcavities allow high-efficiency detection
and manipulation of single atoms. In this regime fluctuations of atom number
are on the order of the mean number, which can lead to signal fluctuations in
excess of the noise on the incident probe field. Conversely, we demonstrate
that nonlinearities and multi-atom statistics can together serve to suppress
the effects of atomic fluctuations when making local density measurements on
clouds of cold atoms. We measure atom densities below 1 per cavity mode volume
near the photon shot-noise limit. This is in direct contrast to previous
experiments where fluctuations in atom number contribute significantly to the
noise. Atom detection is shown to be fast and efficient, reaching fidelities in

Subregional hippocampal morphology and psychiatric outcome in adolescents who were born very preterm and at term
Background: The hippocampus has been reported to be structurally and functionally altered as a sequel of very preterm birth ( < 33 weeks gestation), possibly due its vulnerability to hypoxic-ischemic damage in the neonatal period. We examined hippocampal volumes and subregional morphology in very preterm born individuals in mid- and late adolescence and their association with psychiatric outcome. Methods: Structural brain magnetic resonance images were acquired at two time points (baseline and follow-up) from 65 ex-preterm adolescents (mean age = 15.5 and 19.6 years) and 36 termborn controls (mean age=15.0 and 19.0 years). Hippocampal volumes and subregional morphometric differences were measured from manual tracings and with three-dimensional shape analysis. Psychiatric outcome was assessed with the Rutter Parents' Scale at baseline, the General Health Questionnaire at follow-up and the Peters Delusional Inventory at both time points. Results: In contrast to previous studies we did not find significant difference in the cross-sectional or longitudinal hippocampal volumes between individuals born preterm and controls, despite preterm individual having significantly smaller whole brain volumes. Shape analysis at baseline revealed subregional deformations in 28% of total bilateral hippocampal surface, reflecting atrophy, in ex-preterm individuals compared to controls, and in 22% at follow-up. In ex-preterm individuals, longitudinal changes in hippocampal shape accounted for 11% of the total surface, while in controls they reached 20%. In the whole sample (both groups) larger right hippocampal volume and bilateral anterior surface deformations at baseline were associated with delusional ideation scores at follow-up. Conclusions: This study suggests a dynamic association between cross-sectional hippocampal volumes, longitudinal changes and surface deformations and psychosis proneness. Copyright
A Semi-Lagrangian scheme for a modified version of the Hughes model for pedestrian flow
In this paper we present a Semi-Lagrangian scheme for a regularized version
of the Hughes model for pedestrian flow. Hughes originally proposed a coupled
nonlinear PDE system describing the evolution of a large pedestrian group
trying to exit a domain as fast as possible. The original model corresponds to
a system of a conservation law for the pedestrian density and an Eikonal
equation to determine the weighted distance to the exit. We consider this model
in presence of small diffusion and discuss the numerical analysis of the
proposed Semi-Lagrangian scheme. Furthermore we illustrate the effect of small
diffusion on the exit time with various numerical experiments
Cognitive reserve in granulin-related frontotemporal dementia: from preclinical to clinical stages
OBJECTIVE
Consistent with the cognitive reserve hypothesis, higher education and occupation attainments may help persons with neurodegenerative dementias to better withstand neuropathology before developing cognitive impairment. We tested here the cognitive reserve hypothesis in patients with frontotemporal dementia (FTD), with or without pathogenetic granulin mutations (GRN+ and GRN-), and in presymptomatic GRN mutation carriers (aGRN+).
METHODS
Education and occupation attainments were assessed and combined to define Reserve Index (RI) in 32 FTD patients, i.e. 12 GRN+ and 20 GRN-, and in 17 aGRN+. Changes in functional connectivity were estimated by resting state fMRI, focusing on the salience network (SN), executive network (EN) and bilateral frontoparietal networks (FPNs). Cognitive status was measured by FTD-modified Clinical Dementia Rating Scale.
RESULTS
In FTD patients higher level of premorbid cognitive reserve was associated with reduced connectivity within the SN and the EN. EN was more involved in FTD patients without GRN mutations, while SN was more affected in GRN pathology. In aGRN+, cognitive reserve was associated with reduced SN.
CONCLUSIONS
This study suggests that cognitive reserve modulates functional connectivity in patients with FTD, even in monogenic disease. In GRN inherited FTD, cognitive reserve mechanisms operate even in presymptomatic to clinical stages
A cross-national study on the antecedents of work–life balance from the fit and balance perspective
Drawing on the perceived work–family fit and balance perspective, this study investigates demands and resources as antecedents of work–life balance (WLB) across four countries (New Zealand, France, Italy and Spain), so as to provide empirical cross-national evidence. Using structural equation modelling analysis on a sample of 870 full time employees, we found that work demands, hours worked and family demands were negatively related to WLB, while job autonomy and supervisor support were positively related to WLB. We also found evidence that resources (job autonomy and supervisor support) moderated the relationships between demands and work–life balance, with high resources consistently buffering any detrimental influence of demands on WLB. Furthermore, our study identified additional predictors of WLB that were unique to some national contexts. For example, in France and Italy, overtime hours worked were negatively associated with WLB, while parental status was positively associated with WLB. Cooperative coupling of ultracold atoms and surface plasmons
Cooperative coupling between optical emitters and light fields is one of the
outstanding goals in quantum technology. It is both fundamentally interesting
for the extraordinary radiation properties of the participating emitters and
has many potential applications in photonics. While this goal has been achieved
using high-finesse optical cavities, cavity-free approaches that are broadband
and easy to build have attracted much attention recently. Here we demonstrate
cooperative coupling of ultracold atoms with surface plasmons propagating on a
plane gold surface. While the atoms are moving towards the surface they are
excited by an external laser pulse. Excited surface plasmons are detected via
leakage radiation into the substrate of the gold layer. A maximum Purcell
factor of  is reached at an optimum distance of
 from the surface. The coupling leads to the observation of
