Pyroclastic density currents (PDC) of the 16-17 August 2006 eruptions of Tungurahua volcano, Ecuador: Geophysical registry and characteristics
Tungurahua, located in the Eastern Cordillera of the Ecuadorian Andes, is a 5023 m-high active volcano, notable for its extreme relief (3200 m), steep sides, and frequent eruptive cycles. From 1999 until 2006 Tungurahua experienced short periods of low to moderate strombolian activity, characterized by fire fountaining, explosions, frequent ash falls and debris flows, and no PDC events. Without warning, Tungurahua initiated PDC activity on 15ā16 July 2006, which became more intense on the night of 16ā17 August 2006, which is the focus of this study.
Continuous monitoring of Tungurahua has employed seismic (both short period and broadband (BB) instruments), SO2 gas emission (COSPEC and DOAS), and geodetic methods (EDM, tilt meters, and GPS), in addition to thermal imagery (airborne and ground-based). Acoustic flow monitors (AFM) installed to monitor lahar activity were important for detecting PDC events. Acoustic signals were monitored at Riobamba, 40 km to the SW, as well as by infrasound sensors at Tungurahua's BB seismic stations.
Based on geophysical parameters, visual observations, and PDC deposit characteristics, four phases of distinct eruptive activity are recognized during the 16ā17 August episode. Phase I (08H37 to 21H13 of 16 Aug.) (local time) experienced low to moderate strombolian activity with occasional high energy impulsive bursts and small PDC. Phase II (21H13-16 Aug. to 00H12-17 Aug.) was characterized by a number of discrete events with high amplitude seismo-acoustic signals, followed by the generation of larger PDC that overran monitoring stations and had velocities of 30ā33 m/s. After midnight, Phase III (00H12 to 01H14) saw an intense period of unrelenting eruptive activity corresponding to the episode's greatest energy release. It was characterized by subplinian activity accompanied by a series of high energy outbursts and constant low frequency jetting that together formed a continuous plume. It was during this phase that the largest PDC were produced, reaching the surrounding river valleys. Phase IV (after 01H14) followed the cessation of the paroxysmal eruption, but witnessed many granular PDC generated by degassed lava spill outs from the crater that developed lobe and channel morphology on the cone's lower flanks. Hours later a blocky lava flow issued from the crater.
During these episodes, more than 30 PDC events were detected, the majority being small flows that remained high on the cone. The two largest PDC occurred after midnight, probably generated by fountain collapse. Their descent down the cone's upper steep flanks (~ 28Ā°) and 2.4 km in length favored air entrainment, resulting in PDC with greater fluidity. These flows had volumes of 9 to 17 Ć 106 m3 and produced widespread, but relatively thin (1ā2 m thick) normally-graded deposits at their distal ends. The character and evolution of the PDC activity apparently reflect decreasing volatile contents of the magma and a diminishing magma supply
Ultra-processed food consumption and obesity in the Australian adult population
Background: Rapid simultaneous increases in ultra-processed food sales and obesity prevalence have been observed worldwide, including in Australia. Consumption of ultra-processed foods by the Australian population was previously shown to be systematically associated with increased risk of intakes of nutrients outside levels recommended for the prevention of obesity. This study aims to explore the association between ultra-processed food consumption and obesity among the Australian adult population and stratifying by age group, sex and physical activity level. Methods: A cross-sectional analysis of anthropometric and dietary data from 7411 Australians aged ≥20 years from the National Nutrition and Physical Activity Survey 2011–2012 was performed. Food consumption was evaluated through 24-h recall. The NOVA system was used to identify ultra-processed foods, i.e. industrial formulations manufactured from substances derived from foods and typically added of flavours, colours and other cosmetic additives, such as soft drinks, confectionery, sweet or savoury packaged snacks, microwaveable frozen meals and fast food dishes. Measured weight, height and waist circumference (WC) data were used to calculate the body mass index (BMI) and diagnosis of obesity and abdominal obesity. Regression models were used to evaluate the association of dietary share of ultra-processed foods (quintiles) and obesity indicators, adjusting for socio-demographic variables, physical activity and smoking. Results: Significant (P-trend ≤ 0.001) direct dose–response associations between the dietary share of ultra-processed foods and indicators of obesity were found after adjustment. In the multivariable regression analysis, those in the highest quintile of ultra-processed food consumption had significantly higher BMI (0.97 kg/m2; 95% CI 0.42, 1.51) and WC (1.92 cm; 95% CI 0.57, 3.27) and higher odds of having obesity (OR = 1.61; 95% CI 1.27, 2.04) and abdominal obesity (OR = 1.38; 95% CI 1.10, 1.72) compared with those in the lowest quintile of consumption. Subgroup analyses showed that the trend towards positive associations for all obesity indicators remained in all age groups, sex and physical activity level. Conclusion: The findings add to the growing evidence that ultra-processed food consumption is associated with obesity and support the potential role of ultra-processed foods in contributing to obesity in Australia
Inhibition of the voltage-gated potassium channel Kv1.5 by hydrogen sulfide attenuates remodeling through S-nitrosylation-mediated signaling
The voltage-gated Kāŗ channel plays a key role in atrial excitability, conducting the ultra-rapid rectifier Kāŗ current (IKur) and contributing to the repolarization of the atrial action potential. In this study, we examine its regulation by hydrogen sulfide (HāS) in HL-1 cardiomyocytes and in HEK293 cells expressing human Kv1.5. Pacing induced remodeling resulted in shorting action potential duration, enhanced both Kv1.5 channel and HāS producing enzymes protein expression in HL-1 cardiomyocytes. HāS supplementation reduced these remodeling changes and restored action potential duration through inhibition of Kv1.5 channel. HāS also inhibited recombinant hKv1.5, lead to nitric oxide (NO) mediated S-nitrosylation and activated endothelial nitric oxide synthase (eNOS) by increased phosphorylation of Ser1177, prevention of NO formation precluded these effects. Regulation of Ikur by HāS has important cardiovascular implications and represents a novel and potential therapeutic target
Expression of Regulatory Platelet MicroRNAs in Patients with Sickle Cell Disease
Background: Increased platelet activation in sickle cell disease (SCD) contributes to a state of hypercoagulability and confers a risk of thromboembolic complications. The role for post-transcriptional regulation of the platelet transcriptome by microRNAs (miRNAs) in SCD has not been previously explored. This is the first study to determine whether platelets from SCD exhibit an altered miRNA expression profile. Methods and Findings: We analyzed the expression of miRNAs isolated from platelets from a primary cohort (SCD = 19, controls = 10) and a validation cohort (SCD = 7, controls = 7) by hybridizing to the Agilent miRNA microarrays. A dramatic difference in miRNA expression profiles between patients and controls was noted in both cohorts separately. A total of 40 differentially expressed platelet miRNAs were identified as common in both cohorts (p-value 0.05, fold change>2) with 24 miRNAs downregulated. Interestingly, 14 of the 24 downregulated miRNAs were members of three families - miR-329, miR-376 and miR-154 - which localized to the epigenetically regulated, maternally imprinted chromosome 14q32 region. We validated the downregulated miRNAs, miR-376a and miR-409-3p, and an upregulated miR-1225-3p using qRT-PCR. Over-expression of the miR-1225-3p in the Meg01 cells was followed by mRNA expression profiling to identify mRNA targets. This resulted in significant transcriptional repression of 1605 transcripts. A combinatorial approach using Meg01 mRNA expression profiles following miR-1225-3p overexpression, a computational prediction analysis of miRNA target sequences and a previously published set of differentially expressed platelet transcripts from SCD patients, identified three novel platelet mRNA targets: PBXIP1, PLAGL2 and PHF20L1. Conclusions: We have identified significant differences in functionally active platelet miRNAs in patients with SCD as compared to controls. These data provide an important inventory of differentially expressed miRNAs in SCD patients and an experimental framework for future studies of miRNAs as regulators of biological pathways in platelets. The efficacy of hypotonic and near-isotonic saline for parenteral fluid therapy given at low maintenance rate in preventing significant change in plasma sodium in post-operative pediatric patients: protocol for a prospective randomized non-blinded study
<p>Abstract</p> <p>Background</p> <p>Hyponatremia is the most frequent electrolyte abnormality observed in post-operative pediatric patients receiving intravenous maintenance fluid therapy. If plasma sodium concentration (p-Na<sup>+</sup>) declines to levels below 125 mmol/L in < 48 h, transient or permanent brain damage may occur. There is an intense debate as to whether the administered volume (full rate <it>vs. </it>restricted rate of infusion) and the composition of solutions used for parenteral maintenance fluid therapy (hypotonic <it>vs. </it>isotonic solutions) contribute to the development of hyponatremia. So far, there is no definitive pediatric data to support a particular choice of parenteral fluid for maintenance therapy in post-surgical patients.</p> <p>Methods/Design</p> <p>Our prospective randomized non-blinded study will be conducted in healthy children and adolescents aged 1 to 14 years who have been operated for acute appendicitis. Patients will be randomized either to intravenous hypotonic (0.23% or 0.40% sodium chloride in glucose, respectively) or near-isotonic (0.81% sodium chloride in glucose) solution given at approximately three-fourths of the average maintenance rate. The main outcome of interest from this study is to evaluate 24 h post-operatively whether differences in p-Na<sup>+ </sup>between treatment groups are large enough to be of clinical relevance. In addition, water and electrolyte balance as well as regulatory hormones will be measured.</p> <p>Discussion</p> <p>This study will provide valuable information on the efficacy of hypotonic and near-isotonic fluid therapy in preventing a significant decrease in p-Na<sup>+</sup>. Finally, by means of careful electrolyte and water balance and by measuring regulatory hormones our results will also contribute to a better understanding of the physiopathology of post-operative changes in p-Na<sup>+ </sup>in a population at risk for hyponatremia.</p> <p>Trial registration</p> <p>The protocol for this study is registered with the current controlled trials registry; registry number: <a href="http://www.controlled-trials.com/ISRCTN43896775">ISRCTN43896775</a>.</p
What contributes to patient and parent satisfaction with medication in the treatment of children with ADHD? A report on the development of a new rating scale
Satisfaction with medication is important in the evaluation of overall treatment outcome. There is a lack of consistent and validated rating scales for satisfaction with medication in ADHD, therefore comparison across studies is difficult. Here, we analyse the psychometric properties of the satisfaction with medication scale (SAMS), a new item-based questionnaire that assesses satisfaction with ADHD medication. Furthermore, we evaluate the predictive effect of ADHD symptoms and quality of life (QoL) on satisfaction. Data on satisfaction with Equasym XLĀ® (methylphenidate) were collected in the OBSEER study using the parent (SAMS-P, nĀ =Ā 589) and patient (SAMS-S, nĀ =Ā 552) versions of the SAMS questionnaire. Internal consistency, item-total and cross-informant correlations, and the stability of satisfaction ratings over time were assessed. Satisfaction with medication scores were then correlated with ratings of ADHD symptoms and QoL. Rates of overall satisfaction with Equasym XLĀ® among parents and children were high (>70%), as was internal consistency for both SAMS-P and SAMS-S (Cronbachās alphaĀ >Ā 0.9). Similarly, item-total correlations were high (rĀ =Ā 0.71ā0.90) for SAMS-P and mediumāhigh (rĀ =Ā 0.57ā0.77) for SAMS-S. Cross-informant correlations and the stability of satisfaction ratings were moderate (rĀ =Ā 0.54ā0.59 and 0.48ā0.60, respectively). ADHD symptom and QoL ratings were significantly negative and positive predictors of satisfaction, explaining 36ā52% of satisfaction variance at the final visit. The results show that parent and patient satisfaction was high and could be assessed reliably with the new SAMS questionnaire. Parent and patient ratings were moderately correlated, and symptom severity, functional impairment and QoL were the most significant predictors of satisfaction
Involvement in teaching improves learning in medical students: a randomized cross-over study
<p>Abstract</p> <p>Background</p> <p>Peer-assisted learning has many purported benefits including preparing students as educators, improving communication skills and reducing faculty teaching burden. But comparatively little is known about the effects of teaching on learning outcomes of peer educators in medical education.</p> <p>Methods</p> <p>One hundred and thirty-five first year medical students were randomly allocated to 11 small groups for the Gastroenterology/Hematology Course at the University of Calgary. For each of 22 sessions, two students were randomly selected from each group to be peer educators. Students were surveyed to estimate time spent preparing as peer educator versus group member. Students completed an end-of-course 94 question multiple choice exam. A paired t-test was used to compare performance on clinical presentations for which students were peer educators to those for which they were not.</p> <p>Results</p> <p>Preparation time increased from a mean (SD) of 36 (33) minutes baseline to 99 (60) minutes when peer educators (Cohen's <it>d </it>= 1.3; p < 0.001). The mean score (SD) for clinical presentations in which students were peer educators was 80.7% (11.8) compared to77.6% (6.9) for those which they were not (<it>d </it>= 0.33; <it>p </it>< 0.01).</p> <p>Conclusion</p> <p>Our results suggest that involvement in teaching small group sessions improves medical students' knowledge acquisition and retention.</p
Impact of uniaxial strain and doping on oxygen diffusion in CeO2
Doped ceria is an important electrolyte for solid oxide fuel cell applications. Molecular dynamics simulations have been used to investigate the impact of uniaxial strain along the directions and rare-earth doping (Yb, Er, Ho, Dy, Gd, Sm, Nd, and La) on oxygen diffusion. We introduce a new potential model that is able to describe the thermal expansion and elastic properties of ceria to give excellent agreement with experimental data. We calculate the activation energy of oxygen migration in the temperature range 900-1900K for both unstrained and rare-earth doped ceria systems under tensile strain. Uniaxial strain has a considerable effect in lowering the activation energies of oxygen migration. A more pronounced increase in oxygen diffusivities is predicted at the lower end of the temperature range for all the dopants considered
