    Semi-supervised segmentation of ultrasound images based on patch representation and continuous min cut.

    Ultrasound segmentation is a challenging problem due to the inherent speckle and some artifacts like shadows, attenuation and signal dropout. Existing methods need to include strong priors like shape priors or analytical intensity models to succeed in the segmentation. However, such priors tend to limit these methods to a specific target or imaging settings, and they are not always applicable to pathological cases. This work introduces a semi-supervised segmentation framework for ultrasound imaging that alleviates the limitation of fully automatic segmentation, that is, it is applicable to any kind of target and imaging settings. Our methodology uses a graph of image patches to represent the ultrasound image and user-assisted initialization with labels, which acts as soft priors. The segmentation problem is formulated as a continuous minimum cut problem and solved with an efficient optimization algorithm. We validate our segmentation framework on clinical ultrasound imaging (prostate, fetus, and tumors of the liver and eye). We obtain high similarity agreement with the ground truth provided by medical expert delineations in all applications (94% DICE values in average) and the proposed algorithm performs favorably with the literature

    An overview of treatment approaches for chronic pain management

    Pain which persists after healing is expected to have taken place, or which exists in the absence of tissue damage, is termed chronic pain. By definition chronic pain cannot be treated and cured in the conventional biomedical sense; rather, the patient who is suffering from the pain must be given the tools with which their long-term pain can be managed to an acceptable level. This article will provide an overview of treatment approaches available for the management of persistent non-malignant pain. As well as attempting to provide relief from the physical aspects of pain through the judicious use of analgesics, interventions, stimulations, and irritations, it is important to pay equal attention to the psychosocial complaints which almost always accompany long-term pain. The pain clinic offers a biopsychosocial approach to treatment with the multidisciplinary pain management programme; encouraging patients to take control of their pain problem and lead a fulfilling life in spite of the pain. © 2016 Springer-Verlag Berlin Heidelber

    A small number of heat-shock proteins have previously been shown to act protectively on aggregation of several proteins containing an extended polyglutamine (polyQ) stretch, which are linked to a variety of neurodegenerative diseases. A specific subfamily of heat-shock proteins is formed by the HSPB family of molecular chaperones, which comprises 10 members (HSPB1-10, also called small HSP). Several of them are known to act as anti-aggregation proteins in vitro. Whether they also act protectively in cells against polyQ aggregation has so far only been studied for few of them (e.g. HSPB1, HSPB5 and HSPB8). Here, we compared the 10 members of the human HSPB family for their ability to prevent aggregation of disease-associated proteins with an expanded polyQ stretch. HSPB7 was identified as the most active member within the HSPB family. It not only suppressed polyQ aggregation but also prevented polyQ-induced toxicity in cells and its expression reduces eye degeneration in a Drosophila polyQ model. Upon overexpression in cells, HSPB7 was not found in larger oligomeric species when expressed in cells and-unlike HSPB1-it did not improve the refolding of heat-denatured luciferase. The action of HSPB7 was also not dependent on the Hsp70 machine or on proteasomal activity, and HSPB7 overexpression alone did not increase autophagy. However, in ATG5-/- cells that are defective in macroautophagy, the anti-aggregation activity of HSPB7 was substantially reduced. Hence, HSPB7 prevents toxicity of polyQ proteins at an early stage of aggregate formation by a non-canonical mechanism that requires an active autophagy machinery

    Development of eye position dependency of slow phase velocity during caloric stimulation

    The nystagmus in patients with vestibular disorders often has an eye position dependency, called Alexander's law, where the slow phase velocity is higher with gaze in the fast phase direction compared with gaze in the slow phase direction. Alexander's law has been hypothesized to arise either due to adaptive changes in the velocity-to-position neural integrator, or as a consequence of processing of the vestibular-ocular reflex. We tested whether Alexander's law arises only as a consequence of non-physiologic vestibular stimulation. We measured the time course of the development of Alexander's law in healthy humans with nystagmus caused by three types of caloric vestibular stimulation: cold (unilateral inhibition), warm (unilateral excitation), and simultaneous bilateral bithermal (one side cold, the other warm) stimulation, mimicking the normal push-pull pattern of vestibular stimulation. Alexander's law, measured as a negative slope of the velocity versus position curve, was observed in all conditions. A reversed pattern of eye position dependency (positive slope) was found <10% of the time. The slope often changed with nystagmus velocity (cross-correlation of nystagmus speed and slope was significant in 50% of cases), and the average lag of the slope with the speed was not significantly different from zero. Our results do not support the hypothesis that Alexander's law can only be observed with non-physiologic vestibular stimulation. Further, the rapid development of Alexander's law, while possible for an adaptive mechanism, is nonetheless quite fast compared to most other ocular motor adaptations. These results suggest that Alexander's law may not be a consequence of a true adaptive mechanism

    How does an accreditation programme in residential aged care inform the way residents manage their healthcare and lifestyle?

    Our study examined how accreditation informs the way Australian aged care facility residents manage their healthcare and lifestyle in residential aged care. Residents did not draw direct links between accreditation programmes and their quality of life, yet accreditation standards implicitly underpinned aspects of quality of life that were important to them. There are opportunities for accreditation programmes to more fully utilise resident perspectives to improve their care and quality of life. More overt connections between accreditation standards and issues that are important to residents could promote partnerships with staff, and support enhanced quality of life and a resident-centred focus of care. Furthermore, residents’ perspectives and experiences may contribute to refining accreditation programmes, to ensure they align more with community expectations

    A Simplified Biorefinery Concept for the Valorization of Sugar Beet Pulp: Ecofriendly Isolation of Pectin as a Step Preceding Torrefaction

    The valorization of sugar beet pulp (SBP) from sugar industry as a source of valuable substances has been taken in consideration in this work. In particular, the eco-friendly extraction of pectins with citric acid has been adopted as a preliminary step in a simplified biorefinery concept where the pectin-free solid is subsequently subjected to a torrefaction treatment for its upgrading into a commodity solid biofuel. An extensive physicochemical characterization of the raw feedstock and the isolated pectins has also been performed, which may be useful to identify suitable application routes. Results show that the extraction conditions [1.5 pH, 90 °C, 4 h contact time and SBP-to-solvent ratio of 1:30 (g/mL)] selected in this work allow obtaining a relatively high yield (25% wt, db) of high methoxyl pectins (with some impurities), which exhibit the same colorimetric characteristics of commercial citrus pectins and are not conducive to microbial growth. A further purification step of isolated pectins is required to improve the emulsifying properties. Graphical Abstract: [Figure not available: see fulltext.]
