Acute coronary syndrome in diclofenac sodium-induced type I hypersensitivity reaction : Kounis syndrome
Drug-induced type I hypersensitivity reactions are frequent. Sometimes, acute coronary syndrome (ACS) can be registered in such patients, which may have a serious impact on the course and management of the allergic reaction. Because of potentially atypical ACS clinical presentations, the ECG is an obligatory diagnostic tool in any allergic reaction. Coronary artery spasm is the pathophysiological basis of ACS, triggered by the action of potent vasoactive mediators (histamine, neutral proteases, arachidonic acid products) released from the cells involved in type I hypersensitivity. Allergic angina and allergic myocardial infarction are referred to as Kounis Syndrome. We describe herein a case of ACS in a patient with registered systemic immediate hypersensitivity reaction which developed following the muscular administration of diclofenac sodium.peer-reviewe
Impaired Insulin sensitivity and Insulin secretion in Haemodialysis patients with and without Secondary Hyperparathyroidism
The aim of our study was to investigate insulin sensitivity and beta cell function in hemodialysis (HD) patients without diabetes. We hypothesized that parathyroid gland function was a determinant of insulin sensitivity and/or beta cell function. The study was a randomized, cross-sectional one and patients were divided into two groups (total 27 patients), Gp.1 being those with relative hypoparathyroidism (iPTH<200 pg/ml) 9 (33.3%), Gp.2 those with hyperparathyroidism (iPTH200 pg/ml) 18 (66.6%) with Gp.3 (consisting of 43 healthy subjects acting as controls). Insulin resistance and insulin secretion were calculated from fasting serum insulin and glucose concentrations by the Homeostatic Model Assessment score (HOMA IR and HOMA BETA). The value of HOMA IR (3.28±1.3 for Gp.1, 4.80±2.4 for Gp.2, 1.70±0.8 for Gp.3) as well as the glucose level (5.0±1.0mmol/l in Gp.1, 5.2±0.8mmol/ l in Gp.2, 4.6±0.4mmol/l in Gp.3) was significantly higher in HD patients than in control subjects. Emulation of a Complex Instruction Set Computer with a Reduced Instruction Set Computer
This paper analyzes some of the difficulties of emulating a Complex Instruction Set Computer (CISC) with a Reduced Instruction Set Computer (RISC). It will be shown that although the speed advantage of a RISC is sacrificed, a CISC can be emulated with the exception of software constructs that support nonstandard hardware interfaces. Some concrete examples will be used to help illustrate the execution- time bottlenecks as well as to discuss possible solutions from an architectural point of view for both Silicon and Gallium Arsenide (GaAs). In addition, it will be shown that the most efficient method of emulation involves debugging compiled High-Level Language (HLL) source code on a CISC, and then recompiling the HLL code with a compiler that is familiar with the target RISC architectur

Luminosity measurement at ILC
In this paper we describe a method of luminosity measurement at the future
linear collider ILC that estimates and corrects for the impact of the dominant
sources of systematic uncertainty originating from the beam-induced effects and
the background from physics processes. Based on the relativistic kinematics of
the collision frame of the Bhabha process, the beam-beam related uncertainty is
reduced to a permille independently of the precision with which the beam
parameters are known. With the specific event selection, different from the
isolation cuts based on topology of the signal used at LEP, combined with the
corrective methods we introduce, the overall systematic uncertainty in the peak
region above 80% of the nominal center-of-mass energy meets the physics

Water demand management in Kuwait

Eng.)--Massachusetts Institute of Technology, Dept. of Civil and Environmental Engineering, 2006.Includes bibliographical references (leaves 87-91).Kuwait is an arid country located in the Middle East, with limited access to water resources. Yet water demand per capita is much higher than in other countries in the world, estimated to be around 450 L/capita/day. There are several reasons for such a high demand, but one is certainly the price. Water does have its pricing schedule in Kuwait, but in reality water bills are not collected. The main objective of this thesis is to investigate the impact of water pricing as a tool for managing water demand. The original idea, to construct a water demand model for Kuwait, was modified because of the lack of data about the effect of price increases and household water consumption characteristics in Kuwait. So, water demand models described in the literature for several arid regions were adapted and recalibrated for Kuwait. Simulations describing the influence of block tariffs, constant prices, free allowances followed by various pricing schemes were conducted. A pricing schedule has been proposed that consists of a free allowance followed by a constant price. The proposal has the following logic: if water is consumed wisely, only to satisfy vital needs, it should be free.(cont.) However, to limit over consumption, the quantity of water over the allowance should be priced. The results showed that this kind of pricing schedule would be efficient in significantly reducing demand. The models show that a price of water of $1/m3, after a 150L/capita/day allowance, would reduce the demand by about 35 percent (with a range of around 20-40 percent, depending on the demand model used).by Milan Milutinovic.M.Eng
Large excess of heavy nitrogen in both hydrogen cyanide and cyanogen from comet 17P/Holmes
From millimeter and optical observations of the Jupiter-family comet
17P/Holmes performed soon after its huge outburst of October 24, 2007, we
derive 14 N/15N = 139 +/- 26 in HCN, and 14N/15N = 165 +/- 40 in CN,
establishing that HCN has the same non-terrestrial isotopic composition as CN.
The same conclusion is obtained for the long-period comet C/1995 O1 (Hale-Bopp)
after a reanalysis of previously published measurements. These results are
compatible with HCN being the prime parent of CN in cometary atmospheres. The
15N excess relative to the Earth atmospheric value indicates that N-bearing
volatiles in the solar nebula underwent important N isotopic fractionation at
some stage of Solar System formation. HCN molecules never isotopically
equilibrated with the main nitrogen reservoir in the solar nebula before being
incorporated in Oort-cloud and Kuiper-belt comets. The 12C/13C ratios in HCN
