    Early and late morbidity and mortality and life expectancy following thoracoscopic talc insufflation for control of malignant pleural effusions: a review of 400 cases

    <p>Abstract</p> <p>Background</p> <p>Malignant pleural effusion is a common sequelae in patients with certain malignancies. It represents a terminal condition with short median survival (in terms of months) and the goal is palliation. Aim of our study is to analyze morbidity, mortality and life expectancy following videothoracoscopic talc poudrage.</p> <p>Materials and methods</p> <p>From September 2004 to October 2009, 400 patients underwent video-assisted thoracic surgery (VATS) for malignant pleural effusion. The conditions of patients were assessed and graded before and after treatment concerning morbidity, mortality, success rate of pleurodesis and median survival.</p> <p>Results</p> <p>The median duration of follow up was 40 months (range 4-61 months). All patients demonstrated notable improvement in dyspnea. Intraoperative mortality was zero. The procedure was well tolerated and no significant adverse effects were observed. In hospital mortality was 2% and the pleurodesis success rate was 85%. A poor Karnofsky Performance Status and delay between diagnosis of pleural effusion and pleurodesis were statistically significant factors for in-hospital mortality. The best survival was seen in breast cancer, followed by ovarian cancer, lymphoma and pleural mesothelioma.</p> <p>Conclusions</p> <p>Video-assisted thoracoscopic talc poudrage is an effective and safe procedure that yields a high rate of successful pleurodesis and achieves long-term control with marked dyspnea decrease.</p

    Relatório de Estágio para obtenção do grau de Mestre em Engenharia Civil na Área de Especialização de EdificaçõesO presente relatório é o resultado de um estágio realizado num ambiente real de trabalho, promovido pelo Instituto Superior de Engenharia de Lisboa (ISEL), em consonânciacom a empresa Freyssinet – Terra Armada, Pré-esforço, Estudos e Construções, S.A., no contexto do Mestrado em Engenharia Civil, perfil de Edificações. O estudo em questão compreende a identificação de riscos profissionais e respectiva Análise dos Riscos associada à actividade de manutenção de Juntas de Dilatação em obras de arte rodoviárias, com o objectivo de realizar uma Avaliação de Riscos e posteriormente, apresentar Instruções Técnicas de Segurança que contribuam para melhorar as condições de Segurança na actividade acima referida. A metodologia utilizada para a identificação dos perigos foi o Método das Energias. Os acidentes de trabalho foram classificados com base no EEAT (EUROSTAT 2001), sendo o “contacto – modalidade da lesão” o que define a natureza do risco. A Avaliação dos Riscos e o Nível de Risco foi determinado com base na matriz e nos critérios da norma OHSAS –British Standard 8800:2004. Através desta Avaliação de Riscos foi possível concluir sobre quais os principais riscos inerentes à actividade da manutenção de Juntas de Dilatação, e também entender sobre quais existe maior exposição. Com base neste estudo foram propostas medidas de segurança específicas para a totalidade dos riscos identificados, e posteriormente, foram elaborados procedimentos e instruções de segurança, com o intuito de virem a ser implementados na empresa em trabalhos futuros da mesma natureza.Abstract: The present report is the result of a post-graduate training performed in a real work environment, promoted by the Instituto Superior de Engenharia de Lisboa (ISEL), in consonance with the company Freyssinet – Terra Armada, Pre-stress, Studies and Constructions, S.A., in the context of the Master's degree in Civil Engineering, profile of Edifications This study includes the identification of occupational risks and respective Risk Analysis associated to the maintenance activity of Expansion Joints on road bridges, in order to accomplish a risk assessment and thereafter, the presentation of Technical Safety Instructionswhich contributes to improve the Safety conditions on the activity referred to above. The methodology used for the dangers identification was the Energies Method. The occupational accidents were classified based in the EEAT (EUROSTAT 2001), being the "contact - modality of the injury" what defines the nature of the risk. The Risk Assessment and Risk Level was determined based on the matrix and the criteria of the norm OHSAS –British Standard 8800:2004. Through this Risk Assessment was possible to conclude about which the principal risks inherent to the maintenance activity of Expansion Joints, and also understand about which exists greater exposure. Based on this study were proposed specific security measures for all the risks identified, and thereafter, were developed safety procedures and instructions, with the intention that they will be implemented in the company in future works of the same nature

    A delineating procedure to retrieve relevant publication data in research areas: the case of nanocellulose

    Advances concerning publication-level classification system have been demonstrated striking results by dealing properly with emergent, complex and interdisciplinary research areas, such as nanotechnology and nanocellulose. However, less attention has been paid to propose a delineating method to retrieve relevant research areas on specific subjects. This study aims at proposing a procedure to delineate research areas addressed in case nanocellulose. We investigate how a bibliometric analysis could provide interesting insights into research about this sustainable nanomaterial. The research topics clustered by a Publication-level Classification System were used. The procedure involves an iterative process, which includes developing and cleaning a set of core publication regarding the subject and an analysis of clusters they are associated with. Nanocellulose was selected as the subject of study, but the methodology may be applied to any other research area or topic. A discussion about each step of the procedure is provided. The proposed delineation procedure enables us to retrieve relevant publications from research areas involving nanocellulose.     Validation of the test need for cognition: a study in behavioral accounting

    This study aimed to validate the Need for Cognition scale (NFC) in behavioral accounting. In addition, we sought to measure the possible correlations between the level of need for cognition and the existence of cognitive biases in decisions in accounting and financial information. Two validations were performed to carry out the process of full validation – criterion and construct. The analysis was done by the examination of a sample comprised by 128 graduation students. The statistical technique used for the validation of this test was a factorial analysis for it has the ability to determine the degree of influence of a particular variable in the explanation of a factor, and the processing logistic regression was used for the explanation of possible values as a function of known values or independent variables. The results of the construct of validity showed the legitimacy of the NFC as a unidimensional scale excluding three outputs of its original scale, since the criterion validity of the results confirmed the impact of the level of cognition in maximizing the occurrence of heuristics in managerial decisions
