    A single case report of granular cell tumor of the tongue successfully treated through 445 nm diode laser

    Oral granular cell tumor (GCT) is a relatively rare, benign lesion that can easily be misdiagnosed. Particularly, the presence of pseudoepitheliomatous hyperplasia might, in some cases, lead to the hypothesis of squamous cell carcinoma. Surgical excision is the treatment of choice. Recurrence has been reported in up to 15% of cases treated with conventional surgery. Here, we reported a case of GCT of the tongue in a young female patient, which was successfully treated through 445 nm diode laser excision. Laser surgery might reduce bleeding and postoperative pain and may be associated with more rapid healing. Particularly, the vaporization effect on remnant tissues could eliminate GCT cells on the surgical bed, thus hypothetically leading to a lower rate of recurrence. In the present case, complete healing occurred in 1 week, and no recurrence was observed after 6 months. Laser surgery also allows the possibility to obtain second intention healing. Possible laser-induced histopathological artifacts should be carefully considered

    Caracterização molecular de bovinos cruzados ½ Angus ½ Nelore e ½ Canchim ½ Nelore.

    Os cruzamentos entre raças de bovinos têm sido uma alternativa para a exploração de características de produção e qualidade da carne de raças de Bos taurus especializadas e da adaptação aos trópicos de raças de Bos indicus. A raça de bovinos Aberdeen Angus tem sido utilizada em cruzamentos com Nelore, particularmente por suas características favoráveis de qualidade da carne, além de boa fertilidade, longevidade, precocidade, rusticidade e facilidade de parto. A raça Canchim, constituída por 5/8 Charolês e 3/8 Zebu, apresenta-se como excelence altemativa para cruzamentos pois permite a utilização de couros em monta natural em regime de pastagens. No presente trabalho, produtos do cruzamento de touros Aberdeen Angus (TA) e Canchim (RC) com fêmeas da raça Nelore foram caracterizados quanto a cinco marcadores microssatelites. Os marcadores foram amplificados a partir de 50 ng de DNA genômico e analisados em um sequênciador ABI Prism 3100 .A:vant (Applied Biosystems). Os alemos foram identificados pelo programa Genotyper (Applied Biosyscems), a partir do que foram estimadas as frequências alélicas, heterozigosidade, diversidade gênica, conteúdo de informaçãopolim6rfica'pelo software Cervus e a distancia genética de Nei (1972), utilizando o software NTSYS PC. O numero de alelos variou de 2 a 17, com media de 8,2 nos animais TA e 9,8 nos animais RC. As medias de heterozigosidade foram de 0,7472 e 0,695 para os animais TA e RC, respectivamente. As medias de diversidade gênica foram de 0,756 e 0,6268, na mesma ordem, sugerindo. A distancia genética entre ''as duas populações foi de 0,2273. Essa distancia e comparável a distancia observada entre os grupos Bos taurus e Bas indicus e deve refletir a distancia entre os couros das raças Aberdeen Angus e Canchim, uma vez que nas duas populações, a raça materna foi a mesma

    Correlation between autofluorescence intensity and histopathological features in non-melanoma skin cancer: An ex vivo study

    Non-melanoma skin cancer (NMSC) is the most common malignant tumor affecting fair-skinned people. Increasing incidence rates of NMSC have been reported worldwide, which is an important challenge in terms of public health management. Surgical excision with pre-operatively identified margins is one of the most common and effective treatment strategies. Incomplete tumor removal is associated with a very high risk of recurrence and re-excision. Biological tissues can absorb and re-emit specific light wave-lengths, detectable through spectrophotometric devices. Such a phenomenon is known as autofluorescence (AF). AF spectroscopy has been widely explored for non-invasive, early detection of NMSC as well as for evaluation of surgical margins before excision. Fluorescence-aided diagnosis is based on differences in spectral characteristics between healthy and neoplastic skin. Understanding the biological basis of such differences and correlating AF intensity to histological features could improve the diagnostic accuracy of skin fluorescence spectroscopy. The primary objective of the present pre-clinical ex vivo study is to investigate the correlation between the intensity of cutaneous AF and the histopathological features of NMSC. Ninety-eight lesions suggestive for NMSCs were radically excised from 75 patients (46 M; 29 F; mean age: 79 years). After removal, 115 specific reference points on lesions (\u201ccases\u201d; 59 on BBC, 53 on SCC and 3 on other lesions) and on peri-lesional healthy skin (controls; 115 healthy skin) were identified and marked through suture stitches. Such reference points were irradiated at 400\u2013430 nm wavelength, and resulting emission AF spectra were acquired through spectrophotometry. For each case, AFIR (autofluorescence intensity ratio) was measured as the ratio between the number of photons emitted at a wavelength ranging between 450 and 700 nm (peak: 500 nm) in the healthy skin and that was captured in the pathological tissue. At the histological level, hyperkeratosis, neoangiogenesis, cellular atypia, epithelial thickening, fibrosis and elastosis were quantified by light microscopy and were assessed through a previously validated grading system. Statistical correlation between histologic variables and AFIR was calculated through linear regression. Spectrometric evaluation was performed on 230 (115 cases + 115 controls) reference points. The mean AFIR for BCC group was 4.5, while the mean AFIR for SCC group was 4.4 and the fluorescence peaks at 500 nm were approximately 4 times lower (hypo-fluorescent) in BCCs and in SCCs than in healthy skin. Histological variables significantly associated with alteration of AFIR were fibrosis and elastosis (p < 0.05), neoangiogenesis, hyperkeratosis and epithelial thickening. Cellular atypia was not significantly associated with alteration of AFIR. The intensity of fluorescence emission in neoplastic tissues was approximately 4 times lower than that in healthy tissues. Histopathological features such as hyperkeratosis, neoangiogenesis, fibrosis and elastosis are statistically associated with the decrease in AFIR. We hypothesize that such tissue alterations are among the possible biophysical and biochemical bases of difference in emission AF between neoplastic and healthy tissue. The results of the present evaluation highlighted the possible usefulness of autofluorescence as diagnostic, non-invasive and real-time tool for NMSCs

    Intercomparison of erythemal broadband radiometers calibrated by seven UV calibration facilities in Europe and the USA

    International audienceA bi-lateral intercomparison of erythemal broadband radiometers was performed between seven UV calibration facilities. The owners calibrations were compared relative to the characterisation and calibration performed at PMOD/WRC in Davos, Switzerland. The calibration consisted in the determination of the spectral and angular response of the radiometer, followed by an absolute calibration performed outdoors relative to a spectroradiometer which provided the absolute reference. The characterization of the detectors in the respective laboratories are in good agreement: The determination of the angular responses have deviations below ±4% and the spectral responses agree within ±20%. A "blind" intercomparison of the erythemally weighted irradiances derived by the respective institutes and PMOD/WRC showed consistent measurements to within ±2% for the majority of institutes. One institute showed slightly larger deviation of 10%. The differences found between the different instrument calibrations are all within the combined uncertainty of the calibration

    Anxiety and depression in keratotic oral lichen planus: a multicentric study from the SIPMO

    Objectives: Oral lichen planus with exclusive keratotic reticular, papular, and/or plaque-like lesions (K-OLP) is a clinical pattern of OLP that may be associated with a complex symptomatology and psychological alteration. The aim of the study was to evaluate the prevalence of anxiety (A) and depression (D) in patients with K-OLP, analyzing the potential predictors which can affect mental health status. Methods: Three hundred K-OLP patients versus 300 healthy controls (HC) were recruited in 15 Italian universities. The Numeric Rating Scale (NRS), Total Pain Rating Index (T-PRI), and Hamilton Rating Scales for Depression and for Anxiety (HAM-D and HAM-A) were administered. Results: The K-OLP patients showed statistically higher scores in the NRS, T-PRI, HAM-D, and HAM-A compared with the HC (p-value < 0.001**). A and D were found in 158 (52.7%) and 148 (49.3%) K-OLP patients. Strong linear correlations were identified between HAM-A, HAM-D, NRS, T-PRI, and employment status and between HAM-D, HAM-A, NRS, T-PRI, employment status, and female gender. Multivariate logistic regression revealed that HAM-D and HAM-A showed the greatest increase in the R2 value for A and D in the K-OLP patients, respectively (DR2 = 55.5% p-value < 0.001**; DR2 = 56.5% p-value < 0.001**). Conclusions: The prevalence of A and D is higher in the K-OLP patients compared with the HC, also found in K-OLP subjects without pain, suggesting that the processing of pain may be in a certain way independent of the processing of mood. Clinical relevance: Mood disorders and pain assessment should be carefully performed in relation to K-OLP to obtain a complete analysis of the patients

    Regulation of Cementoblast Gene Expression by Inorganic Phosphate In Vitro

    Examination of mutant and knockout phenotypes with altered phosphate/pyrophosphate distribution has demonstrated that cementum, the mineralized tissue that sheathes the tooth root, is very sensitive to local levels of phosphate and pyrophosphate. The aim of this study was to examine the potential regulation of cementoblast cell behavior by inorganic phosphate (P i ). Immortalized murine cementoblasts were treated with P i in vitro , and effects on gene expression (by quantitative real-time reverse-transcriptase polymerase chain reaction [RT-PCR]) and cell proliferation (by hemacytometer count) were observed. Dose-response (0.1–10 mM) and time-course (1–48 hours) assays were performed, as well as studies including the Na-P i uptake inhibitor phosphonoformic acid. Real-time RT-PCR indicated regulation by phosphate of several genes associated with differentiation/mineralization. A dose of 5 mM P i upregulated genes including the SIBLING family genes osteopontin ( Opn , >300% of control) and dentin matrix protein-1 ( Dmp-1 , >3,000% of control). Another SIBLING family member, bone sialoprotein ( Bsp ), was downregulated, as were osteocalcin ( Ocn ) and type I collagen ( Col1 ). Time-course experiments indicated that these genes responded within 6–24 hours. Time-course experiments also indicated rapid regulation (by 6 hours) of genes concerned with phosphate/pyrophosphate homeostasis, including the mouse progressive ankylosis gene ( Ank ), plasma cell membrane glycoprotein-1 ( Pc-1 ), tissue nonspecific alkaline phosphatase ( Tnap ), and the Pit1 Na-P i cotransporter. Phosphate effects on cementoblasts were further shown to be uptake-dependent and proliferation-independent. These data suggest regulation by phosphate of multiple genes in cementoblasts in vitro .     Objectives: To evaluate the thermal changes and histologic alterations produced in ex vivo and in vivo specimens of oral mucosa by the quantum molecular resonance (QMR) scalpel. Materials and methods: Using electronic probes positioned within the mandibles of pigs, we measured temperature changes induced in the mucosa during incisions made with (1) a conventional cold blade; (2) a QMR blade; (3) a Nd:YAG laser (1064 nm); (4) an Er:YAG laser (2940 nm). We also evaluated the epithelial, connective, and vascular tissue modifications in 49 biopsy specimens of benign fibrous oral lesions collected with these same surgical instruments. Results: The QMR blade was the instrument that produced the smallest increases in temperature and the mildest histologic artifacts (0.9°C; 22.8 ?m). Conclusions: The QMR blade is a useful tool in oral surgery. © 2011 Elsevier Srl. All rights reserved
