    Rural Children Experience Different Rates of Mental Health Diagnosis and Treatment

    Research indicates that privately insured, rural adults have lower use of office-based mental health services, but higher use of prescription medicines than their urban counterparts. Patterns for rural children may be different from urban children because of the limited supply of pediatric mental health providers in rural areas, which may lead to reduced access and lower use of mental health services in rural areas versus urban. Using data on children ages 5-17 from the 2002-2008 of the Medical Expenditure Panel Survey, researchers from the Maine Rural Health Research Center find that rural children are significantly less likely to be diagnosed and treated for non-ADHD mental health problems than urban children and are less likely to receive mental health counseling. The rural-urban difference is greatest among those children scoring in the “possible impairment” range on the Columbia Impairment Scale

    Introduction Research indicates that privately insured, rural adults have lower use of office-based mental health services, but higher use of prescription medicines than their urban counterparts. Similar studies for rural children have been limited to specific populations, diagnoses, or to single states. Patterns for rural children may be different than those of urban children and adults generally because of their high enrollment in Medicaid and the State Children\u27s Health Insurance Program, which tend to have more generous behavioral health benefits than private coverage and may equalize rural-urban treatment patterns. On the other hand, the more limited supply of specialty mental health providers in rural areas, particularly for children, could lead to lack of access and lower utilization of some types of mental health services in rural areas versus urban. Methods Using data on children ages 5-17 from the 2002-2008 Medical Expenditure Panel Survey, this study examines two research questions: 1) do patterns of children\u27s mental health diagnosis and service use (e.g., office visits and psychotropic medications) differ by rural-urban residence? and 2) what is the effect of income and insurance type on use of mental health services? Findings Controlling for demographic and risk factors, rural children are as likely as urban children to have an attention deficit or hyperactivity disorder (ADHD) diagnosis and less likely to have any other type of psychiatric diagnosis. Initially observed higher prevalence of mental health diagnoses among rural children is explained by underlying differences in demographic characteristics and risk factors, such as higher rates of poverty, public coverage, mental health impairment, and lower prevalence of minorities. Rural children with the highest mental health need are no more or less likely to be diagnosed or treated for mental health conditions. However, among those with a possible impairment, rural children are less likely to be diagnosed with a psychiatric illness other than ADHD and are less likely to receive counseling

    The British antibiotic and silver-impregnated catheters for ventriculoperitoneal shunts multi-centre randomised controlled trial (the BASICS trial): study protocol

    BACKGROUND: Insertion of a ventriculoperitoneal shunt (VPS) for the treatment of hydrocephalus is one of the most common neurosurgical procedures in the UK, but failures caused by infection occur in approximately 8% of primary cases. VPS infection is associated with considerable morbidity and mortality and its management results in substantial cost to the health service. Antibiotic-impregnated (rifampicin and clindamycin) and silver-impregnated VPS have been developed to reduce infection rates. Whilst there is some evidence showing that such devices may lead to a reduction in VPS infection, there are no randomised controlled trials (RCTs) to support their routine use. METHODS/DESIGN: Overall, 1,200 patients will be recruited from 17 regional neurosurgical units in the UK and Ireland. Patients of any age undergoing insertion of their first VPS are eligible. Patients with previous indwelling VPS, active and on-going cerebrospinal fluid (CSF) or peritoneal infection, multiloculated hydrocephalus requiring multiple VPS or neuroendoscopy, and ventriculoatrial or ventriculopleural shunt planned will be excluded. Patients will be randomised 1:1:1 to either standard silicone (comparator), antibiotic-impregnated, or silver-impregnated VPS. The primary outcome measure is time to VPS infection. Secondary outcome measures include time to VPS failure of any cause, reason for VPS failure (infection, mechanical failure, or patient failure), types of bacterial VPS infection (organism type and antibiotic resistance), and incremental cost per VPS failure averted. DISCUSSION: The British antibiotic and silver-impregnated catheters for ventriculoperitoneal shunts multi-centre randomised controlled trial (the BASICS trial) is the first multi-centre RCT designed to determine whether antibiotic or silver-impregnated VPS reduce early shunt infection compared to standard silicone VPS. The results of this study will be used to inform current neurosurgical practice and may potentially benefit patients undergoing shunt surgery in the future.     Light-induced reflectivity transients in black-Si nanoneedles

The change in reflectivity of black-Si (b-Si) upon optical excitation was measured by the pump-probe technique using picosecond laser pulses at 532 (pump) and 1064 nm (probe) wavelengths. The specular reflection from the random pattern of plasma-etched b-Si nano-needles was dominated by the photo-excited free-carrier contribution to the reflectivity. The kinetics of the reflectivity were found to be consistent with surface structural and chemical analysis, performed by scanning and transmission electron microscopy, and spectroscopic ellipsometry. The surface recombination velocity on the b-Si needles was estimated to be ~102cm/s. Metalization of b-Si led to much faster recombination and alteration of reflectivity. The reflectivity spectra of random b-Si surfaces with different needle lengths was modeled by a multi-step refractive index profile in the Drude formalism. The dip in the reflectivity spectra and the sign reversal in the differential reflectivity signal at certain b-Si needle sizes is explained by the model

    Sediment addition and legume cultivation result in sustainable, long-term increases in ecosystem functions of sandy grasslands

Sandy grasslands are fragile with low ecosystem productivity mainly because of the sandy soil structure with low water and nutrient holding capacities and especially low levels of nitrogen. Here, we evaluate the long-term impacts of sediment addition from a local reservoir, and grass and legume cultivation (artificial grasslands) on a sandy grassland in eastern Inner Mongolia, China. The results showed that even after 32 years, sediment addition had improved soil structure significantly, that is, increasing of silt and clay contents, soil bulk density, and water holding capacity. As the result of improved soil structure, ecosystem functions, including aboveground net primary productivity (ANPP) and soil carbon, nitrogen (N), and phosphorus storage, increased significantly. Net C, N, and P sequestration increased even after accounting for the sediment addition, due, at least partially, to the greater plant biomass trapping large quantities of wind-blown dust. Plant cultivation, especially the addition of a legume, further increased ANPP significantly, that is, the cultivation of Leymus chinensis and the legume Medicago sativa increased ANPP 6.99 and 44.62 times, respectively. Our study highlights that improvements in soil structure and cultivation with legume species can increased substantially the productivity of sandy grasslands and that the initial increases in grass biomass promoted the sequestration of wind-blown dust, which helped sustain the increases in productivity.National Key R&D Program of ChinaNational Natural Science Foundation of Chin

    Distortion Estimation Through Explicit Modeling of the Refractive Surface

    Precise calibration is a must for high reliance 3D computer vision algorithms. A challenging case is when the camera is behind a protective glass or transparent object: due to refraction, the image is heavily distorted; the pinhole camera model alone can not be used and a distortion correction step is required. By directly modeling the geometry of the refractive media, we build the image generation process by tracing individual light rays from the camera to a target. Comparing the generated images to their distorted - observed - counterparts, we estimate the geometry parameters of the refractive surface via model inversion by employing an RBF neural network. We present an image collection methodology that produces data suited for finding the distortion parameters and test our algorithm on synthetic and real-world data.     Development of a 3D workspace Shoulder Assessment Tool Incorporating Electromyography and an Inertial Measurement Unit - A preliminary study

    Traditional shoulder Range of Movement (ROM) measurement tools suffer from inaccuracy or from long experimental set-up times. Recently, it has been demonstrated that relatively low-cost wearable inertial measurement unit (IMU) sensors can overcome many of the limitations of traditional motion tracking systems. The aim of this study is to develop and evaluate a single IMU combined with an Electromyography (EMG) sensor to monitor the 3D reachable workspace with simultaneous measurement of deltoid muscle activity across the shoulder ROM. Six volunteer subjects with healthy shoulders and one participant with a ‘frozen’ shoulder were recruited to the study. Arm movement in 3D space was plotted in spherical coordinates while the relative EMG intensity of any arm position is presented graphically. The results showed that there was an average ROM surface area of 27291±538 deg2 among all six healthy individuals and a ROM surface area of 13571±308 deg2 for the subject with frozen shoulder. All three sections of the deltoid show greater EMG activity at higher elevation angles. Using such tools enables individuals, surgeons and physiotherapists to measure the maximum envelope of motion in conjunction with muscle activity in order to provide an objective assessment of shoulder performance in the voluntary 3D workspace

    A Gateway MultiSite Recombination Cloning Toolkit

    The generation of DNA constructs is often a rate-limiting step in conducting biological experiments. Recombination cloning of single DNA fragments using the Gateway system provided an advance over traditional restriction enzyme cloning due to increases in efficiency and reliability. Here we introduce a series of entry clones and a destination vector for use in two, three, and four fragment Gateway MultiSite recombination cloning whose advantages include increased flexibility and versatility. In contrast to Gateway single-fragment cloning approaches where variations are typically incorporated into model system-specific destination vectors, our Gateway MultiSite cloning strategy incorporates variations in easily generated entry clones that are model system-independent. In particular, we present entry clones containing insertions of GAL4, QF, UAS, QUAS, eGFP, and mCherry, among others, and demonstrate their in vivo functionality in Drosophila by using them to generate expression clones including GAL4 and QF drivers for various trp ion channel family members, UAS and QUAS excitatory and inhibitory light-gated ion channels, and QUAS red and green fluorescent synaptic vesicle markers. We thus establish a starter toolkit of modular Gateway MultiSite entry clones potentially adaptable to any model system. An inventory of entry clones and destination vectors for Gateway MultiSite cloning has also been established (www.gatewaymultisite.org)

    Investigating Agrobacterium-Mediated Transformation of Verticillium albo-atrum on Plant Surfaces

    Background: Agrobacterium tumefaciens has long been known to transform plant tissue in nature as part of its infection process. This natural mechanism has been utilised over the last few decades in laboratories world wide to genetically manipulate many species of plants. More recently this technology has been successfully applied to non-plant organisms in the laboratory, including fungi, where the plant wound hormone acetosyringone, an inducer of transformation, is supplied exogenously. In the natural environment it is possible that Agrobacterium and fungi may encounter each other at plant wound sites, where acetosyringone would be present, raising the possibility of natural gene transfer from bacterium to fungus. Methodology/Principal Findings: We investigate this hypothesis through the development of experiments designed to replicate such a situation at a plant wound site. A. tumefaciens harbouring the plasmid pCAMDsRed was co-cultivated with the common plant pathogenic fungus Verticillium albo-atrum on a range of wounded plant tissues. Fungal transformants were obtained from co-cultivation on a range of plant tissue types, demonstrating that plant tissue provides sufficient vir gene inducers to allow A. tumefaciens to transform fungi in planta. Conclusions/Significance: This work raises interesting questions about whether A. tumefaciens may be able to transform organisms other than plants in nature, or indeed should be considered during GM risk assessments, with furthe
