Non esiste, attualmente, un trattamento standard efficace per questa complicanza. Lo scopo del presente articolo è illustrare la nostra esperienza sull’uso degli antagosisti dei recettori per i leucotrieni nel trattamento della contrattura capsulare. I risultati mostrano che l’utilizzo di questi farmaci è in grado di ridurre il dolore e la distorsione della mammella in pazienti con un lungo decorso di contrattura capsulare

    AbstractThe Fondazione IRCCS Ca' Granda Ospedale Maggiore Policlinico is a five-century institution that, besides the unique clinical role in the center of Milan, may rely on benefactor donations such as fields and farming houses not far from the city, for a total of 8500 ha, all managed by the "Sviluppo Ca' Granda' Foundation". Presently, the main products of these fields are represented by rice and cow's milk. During the latest years, farmers and managers have developed a model of sustainable food production, with great attention to the product quality based on compositional analysis and functional nutritional characteristics.     Evidence of dynamical dipole excitation in the fusion-evaporation of the 40Ca +152Sm heavy system

    The excitation of the dynamical dipole mode along the fusion path was investigated for the first time in the formation of a heavy compound nucleus in the A ∼ 190 mass region. The compound nucleus was formed at identical conditions of excitation energy and spin from two entrance channels: the charge-asymmetric 40 Ca + 152 Sm and the nearly charge-symmetric 48 Ca + 144 Sm at E lab = 11 and 10.1 MeV / nucleon, respectively. High-energy γ rays and light charged particles were measured in coincidence with evaporation residues by means of the MEDEA multidetector array (Laboratori Nazionali del Sud, Italy) coupled to four parallel plate avalanche counters. The charged particle multiplicity spectra and angular distributions were used to pin down the average excitation energy, the average mass, and the average charge of the compound nucleus. The γ -ray multiplicity spectrum and angular distribution related to the nearly charge-symmetric channel were employed to obtain new data on the giant dipole resonance in the compound nucleus. The dynamical dipole mode excitation in the charge-asymmetric channel was evidenced, in a model-independent way, by comparing the γ -ray multiplicity spectra and angular distributions of the two entrance channels with each other. Calculations of the dynamical dipole mode in the 40 Ca + 152 Sm channel, based on a collective bremsstrahlung analysis of the reaction dynamics, are presented. Possible interesting implications in the superheavy-element quest are discussed

    Circulating pre-treatment Epstein-Barr virus DNA as prognostic factor in locally-advanced nasopharyngeal cancer in a nonendemic area

    The prognostic value of pre-treatment Epstein-Barr Virus (EBV) DNA viral load for non-endemic, locally-advanced, EBV-related nasopharyngeal cancer (NPC) patients is yet to be defined. All patients with EBV encoded RNA (EBER)-positive NPC treated at our Institution from 2005 to 2014 with chemotherapy (CT) concurrent with radiation (RT) +/- induction chemotherapy (ICT) were retrospectively reviewed. Pre-treatment baseline plasma EBV DNA (b-EBV DNA) viral load was detected and quantified by PCR. Median b-EBV DNA value was correlated to potential influencing factors by univariate analysis. Significant variables were then extrapolated and included in a multivariate linear regression model. The same variables, including b-EBV DNA, were correlated with Disease Free Survival (DFS) and Overall Survival (OS) by univariate and multivariate analysis. A total of 130 locally-advanced EBER positive NPC patients were evaluated. Overall, b-EBV DNA was detected in 103 patients (79.2%). Median viral load was 554 copies/mL (range 50-151075), and was positively correlated with T stage (p= 0.002), N3a-b vs N0-1-2 stage (p= 0.048), type of treatment (ICT followed by CTRT, p= 0.006) and locoregional and/or distant disease recurrence (p= 0.034). In the overall population, DFS and OS were significantly longer in patients with pre-treatment negative EBV DNA than in positive subjects at the multivariate analysis. Negative b-EBV DNA can be considered as prognostic biomarker of longer DFS and OS in NPC in non-endemic areas.     Trabecular carcinoma is a rare cutaneous neuroendocrine carcinoma that probably originates from the Merkel cells that are usually found in the basal layer of the epidermis. The treatment of Merkel cell carcinoma is controversial and there is no specific therapeutic protocol because of the small number of cases that have been published. The procedures used to treat Merkel cell carcinoma must be tailored to minimise morbidity while maximising survival. Because the condition is so rare and difficult to diagnose and treat, we report three cases of orbitopalpebral tumours that confirm the local and regional aggressiveness and the high metastatic potential of this tumour
