    Infecções bacterianas multirresistentes emergem como um pesadelo para a saúde pública mundial. Na medicina veterinária sua ocorrência não está bem estabelecida, mas fatores que contri­buem para sua incidência em humanos também estão se tornando comuns nos animais, como o aumento de cuidados intensivos, cirurgias complexas bem como o uso indiscriminado de antimicrobianos e fármacos imunossupressores. O acenitobacter sp. ganhou visibilidade pelo número de surtos em humanos e sua alta letalidade, recebendo como protocolo de tratamento uma terapia tipicamente empírica, com diversos fármacos para infecções graves. Neste trabalho, objetivou-se descrever a utilização da oxigenoterapia hiperbárica e a ozonioterapia como terapias integrativas ao protocolo antimicrobiano específico a esta estirpe bacteriana e a avaliação de seus efeitos durante as 20 sessões realizadas em gato vítima de trauma por projétil de arma de fogo, com fratura consolidada, mas com colonização do ferimento pelo acinetobacter sp. caracterizada por fístulas indolentes e inflamação do membro torácico, em processo de cronicidade de 3 anos, com diversas abordagens terapeuticas. Concluiu-se que a utilização da oxigenoterapia hiperbárica associada a ozonioterapia ajudaram na efetiva atuação no combate à inflamação e ao agente infeccioso, promovendo de forma diferenciada e precoce a cicatrização do membro, restabelecendo a funcionalidade ao doente, deste modo casos seriados devem ser continuados

    Phosphene-guided transcranial magnetic stimulation of occipital but not parietal cortex suppresses stimulus visibility.

    Transcranial magnetic stimulation (TMS) applied over the occipital lobe approximately 100 ms after the onset of a stimulus decreases its visibility if it appears in the location of the phosphene. Because phosphenes can also be elicited by stimulation of the parietal regions, we asked if the same procedure that is used to reduce visibility of stimuli with occipital TMS will lead to decreased stimulus visibility when TMS is applied to parietal regions. TMS was randomly applied at 0-130 ms after the onset of the stimulus in steps of 10 ms in occipital and parietal regions. Participants responded to the orientation of the line stimulus and rated its visibility. We replicate previous reports of phosphenes from both occipital and parietal TMS. As previously reported, we also observed visual suppression around the classical 100 ms window both in the objective line orientation and subjective visibility responses with occipital TMS. Parietal stimulation, on the other hand, did not consistently reduce stimulus visibility in any time window. http://dx.doi.org/10.1007/s00221-014-3923-z

    How do national cultures influence lay people's preferences toward doctors' style of communication?: a comparison of 35 focus groups from an European cross national research

    Background: The evidence that inspires and fosters communication skills, teaching programmes and clinical recommendations are often based on national studies which assume, implicitly, that patients' preferences towards doctors' communication style are not significantly affected by their cultural background. The cross-cultural validity of national results has been recognized as a potential limitation on how generally applicable they are in a wider context. Using 35 country-specific focus group discussions from four European countries, the aim of the present study is to test whether or not national cultures influence lay people's preferences towards doctors' style of communication. Methods: Lay people preferences on doctor's communication style have been collected in Belgium, the Netherlands, the United Kingdom and Italy. Each centre organized between eight and nine focus groups, where participants (n = 259) were asked to comment on a video of a simulated medical interview. The discussions were audiotaped, transcribed and coded using a common framework (Guliver Coding System) that allowed for the identification of different themes. Results: The frequency distribution of the topics discussed highlights lay people's generally positive views towards most part of doctors interventions. The regression model applied to the Guliver categories highlighted slight national differences and the existence of a cross-cultural appreciation, in particular, of five types of intervention: Doctors attitudes (both Task-Oriented and Affective/Emotional), Summarizing, Structuring and Providing solution. Conclusion: Lay panels valued doctors' communication style in a similar manner in the countries selected. This highlights the existence of a common background, which in the process of internationalization of heath care, might foster the implementation of cross-national teaching programmes and clinical guidelines

    Experimental characterization of leak detection systems in HLM pool using LIFUS5/Mod3 facility

    In the framework of the European Union MAXSIMA project, the safety of the steam generator (SG) adopted in the primary loop of the Heavy Liquid Metal Fast Reactor has been studied investigating the consequences and damage propagation of a SG tube rupture event and characterizing leak rates from typical cracks. Instrumentation able to promptly detect the presence of a crack in the SG tubes may be used to prevent its further propagation, which would lead to a full rupture of the tube. Application of the leak-before-break concept is relevant for improving the safety of a reactor system and decreasing the probability of a pipe break event. In this framework, a new experimental campaign (Test Series C) has been carried out in the LIFUS5/Mod3 facility, installed at ENEA Centro Ricerche Brasimone, in order to characterize and to correlate the leak rate through typical cracks occurring in the pressurized tubes with signals detected by proper transducers. Test C1.3_60 was executed injecting water at about 20 bars and 200°C into lead-bismuth eutectic alloy. The injection was performed through a laser microholed plate 60 μm in diameter. Analysis of the thermohydraulic data permitted characterization of the leakage through typical cracks that can occur in the pressurized tubes of the SG. Analysis of the data acquired by microphones and accelerometers highlighted that it is possible to correlate the signals to the leakage and the rate of release

    Somatotropic gene response to recombinant growth hormone treatment in buffalo leucocytes

    The use of recombinant bovine growth hormone (rbGH) to increase milk yield in cows is banned in some countries. In others, where it is authorised, it has triggered harsh debates on labelling of dairy products. If many studies have been performed on bovines, there is a lack of information on buffaloes, which are sometimes treated with rbGH and re­present an important economical resource for dairy products in some countries. Analytical methods with legal value for surveillance of rbGH treatments do not yet exist. Research on gene expression biomarkers is one of the most promising approaches to this purpose. For this reason, we treated five buffaloes for 10 weeks with a sustained-release formulation of rbGH and analysed the response of 20 somatotropic axis genes in leucocytes by real-time polymerase chain reaction. Overall changes in gene expression levels were of low magnitude and sometimes affected by the ‘time’ factor. Only the IGFBP-1 gene showed a significant under-expression (about two-fold; p <0.001) in treated animals. Taken together, these results give evidence that expression analysis of the somatotropic axis genes in leuco­cytes is little helpful for discrimination of rbGH-treated buffaloes, but do not exclude that another array of genes could provide useful patterns of variation

    NUV-HD SiPMs with metal-filled trenches

    In this paper we present the performance of a new SiPM that is sensitive to blue light and features narrow metal-filled trenches placed in the area around the single-photon avalanche diodes (SPADs) that allow an almost complete suppression the internal optical crosstalk. In particular, we show the benefits of this technological upgrade in terms of electro-optical SiPM performance when compared to the previous technology which had only a partial optical screening between the SPADs. The most relevant effect is the much higher bias voltage that can be applied to the new device before the noise diverges. This allows to optimize and improve both the photon detection efficiency and the single-photon time resolution. We also coupled the SiPMs to LYSO scintillators to verify the performance for possible application in Positron-Emission Tomography. Thanks to the better electro-optical features we were able to measure an improved coincidence time resolution. Furthermore, the optimal voltage operation region is substantially larger, making this SiPM more suitable for real system application where thousands of channels have to provide stable and reproducible performance

    Attitudes of nursing home staff towards influenza vaccination: opinions and factors influencing hesitancy

    Seasonal influenza is recognized to be a significant public health problem and a cause of death, especially in fragile persons. In nursing homes (NHs), vaccination for both residents and staff is the best preventive strategy. However, professionals\u2019 immunization rates are far from reaching the international recommended values. This study aims to describe the adherence and attitudes of NH staff towards flu vaccination and to explore staff hesitancy. A questionnaire was developed based on a literature review and on the 3Cs (confidence, complacency, convenience) of the WHO framework and administered among the staff of four NHs of a province in the northeast of Italy. Results demonstrated a low adherence towards annual vaccination (i.e., only 3% declared getting the flu vaccination each year). Complacency, confidence and convenience all showed a significant impact on the attitude towards vaccination both in univariate and multivariable analysis, with complacency being the most strongly associated area. The area of confidence resulted in strongly challenging factors. Only 24.8% of interviewees appeared trustful towards the efficacy of receiving immunization and 34% declared safety issues. Insights from the study can support the implementation of effective interventions to improve vaccination adherence in NHs. Specifically, increasing complacency by raising awareness related to the risks of influenza appears to be an essential strategy to effectively promote vaccination uptake

    The influence of posterior parietal cortex on extrastriate visual activity: A concurrent TMS and fast optical imaging study

    The posterior parietal cortex (PPC) is a critical node in attentional and saccadic eye movement networks of the cerebral cortex, exerting top-down control over activity in visual cortex. Here, we sought to further elucidate the properties of PPC feedback by providing a time-resolved map of functional connectivity between parietal and occipital cortex using single-pulse TMS to stimulate the left PPC while concurrently recording fast optical imaging data from bilateral occipital cortex. Magnetic stimulation of the PPC induced transient ipsilateral occipital activations (BA 18) 24 to 48ms post-TMS. Concurrent TMS and fast optical imaging results demonstrate a clear influence of PPC stimulation on activity within human extrastriate visual cortex and further extend this time- and space-resolved method for examining functional connectivity
