    Background Height measurement was very important to toddlers and preschool children. Deformity or certain abnormality in toddler and preschool children caused height measurement was not possible. Arm span was known to be an alternative anthropometric parameter to height. Ration between height and arm span varied in different race, age, and sex.Aim The study was aim to know the ratio between height and arm span in toddlers and preschool children.MethodsThis was a cross-sectional study with observational analysis. This research was held in March-May 2015. The subject was toddlers and preschool children age 1-5 years. Height and arm span data was acquired by doing measurement to subjects. Statistical test used linear regression test.ResultsThe number of subjects were 197children aged 1-5 years. Linear regression test showed the ratio for male toddlers and preschool children aged 13-24 months was Height=27,793+0,685 arm span, aged 25-36 months was Height=21,364+0,771 arm span, aged 37-48 months was Height=32,157+0,686 arm span, aged 49-60 months was Height=54,681+0,461 arm span. The ratio for female toddlers and preschool children aged 13-24 months was Height=49,398+0,367 arm span, aged 25-36 months was Height=20,185+0,796 arm span, aged 37-48 months was Height=32+0,674 arm span, and aged 49-60 months was Height=13,861+0,884 arm span.Conclusion Mean height for male toddlers and preschool children was 91,4±10 cm, while mean height for female toddlers and preschool children was 89,2±10,4 cm. Mean arm span fot male toddlers and preschool children was 89,6±11,6 cm, while mean arm span for female toddlers and preschool children was 87,2±11,2 cm. Equation based on divided age predicted height better than equation based on the whole age 1-5 years

    Sequence of radiotherapy and chemotherapy in breast cancer after breast-conserving surgery

    Purpose The optimal sequence of radiotherapy and chemotherapy in breast-conserving therapy is unknown. Methods and Materials From 1983 through 2007, a total of 641 patients with 653 instances of breast-conserving therapy (BCT), received both chemotherapy and radiotherapy and are the basis of this analysis. Patients were divided into three groups. Groups A and B comprised patients treated before 2005, Group A radiotherapy first and Group B chemotherapy first. Group C consisted of patients treated from 2005 onward, when we had a fixed sequence of radiotherapy first, followed by chemotherapy. Results Local control did not show any differences among the three groups. For distant metastasis, no difference was shown between Groups A and B. Group C, when compared with Group A, showed, on univariate and multivariate analyses, a significantly better distant metastasis–free survival. The same was noted for disease-free survival. With respect to disease-specific survival, no differences were shown on multivariate analysis among the three groups. Conclusion Radiotherapy, as an integral part of the primary treatment of BCT, should be administered first, followed by adjuvant chemotherapy

    Serum Phthalate and Triclosan Levels Have Opposing Associations With Risk Factors for Gestational Diabetes Mellitus.

    Certain phthalates and bisphenol A (BPA) have been associated with insulin resistance and type 2 diabetes in non-pregnant adults, but studies of gestational diabetes mellitus (GDM) have reported conflicting results for phthalates and no associations with BPA. Our aim was to investigate the relationship between maternal serum levels of phthalate metabolites and phenols at 10-17 weeks of gestation and glucose homeostasis at 28 weeks of gestation. 232 women aged ≥16 years without type 1 or 2 diabetes with singleton male pregnancies were recruited from a single UK maternity centre between 2001 and 2009 as part of a prospective observational study (Cambridge Baby Growth Study). Serum levels of 16 phthalate metabolites and 9 phenols (including BPA) were measured using liquid chromatography/tandem mass spectrometry. Oral glucose tolerance tests were performed at 28 weeks. 47/232 (20.3%) women had GDM. First-trimester triclosan (TCS) was inversely associated with incident GDM (adjusted odds ratio per log increase in concentration 0.54, 95% confidence interval 0.34-0.86, p = 0.010). Amongst women without GDM, first-trimester mono-(2-ethylhexyl) phthalate and mono(carboxyisooctyl) phthalate levels were positively associated with 120-min plasma glucose (adjusted β 0.268 and 0.183, p = 0.0002 and 0.010, respectively) in mid-pregnancy. No other monotonic associations were detected between phthalate or phenol levels and fasting or stimulated plasma glucose, β-cell function, insulin resistance, or 60-min disposition index. Our results support a glycaemia-raising effect of phthalates during pregnancy, consistent with findings in non-pregnant populations and suggest a possible protective effect of exposure to TCS against GDM

    Polycystin-2 (PC2) is aTRP-type, Ca2+-permeable non-selective cation channel that plays an important role in Ca2+ signaling in renal and non-renal cells. The effect(s) of the cAMP pathway and kinase mediated phosphorylation of PC2 seem to be relevant to PC2 trafficking and its interaction with polycystin-1. However, the role of PC2 phosphorylation in channel function is still poorly defined. Here we reconstituted apical membranes of term human syncytiotrophoblast (hST), containing endogenous PC2 (PC2hst), and in vitro translated channel protein (PC2iv). Addition of the catalytic subunit of PKA increased by 566% the spontaneous PC2hst channel activity in the presence of ATP. Interestingly, 8-Br-cAMP also stimulated spontaneous PC2hst channel activity in the absence of the exogenous kinase. Either stimulation was inhibited by addition of alkaline phosphatase, which in turn, was reversed by the phosphatase inhibitor vanadate. Neither maneuver modified the single channel conductance but instead increased channel mean open time. PKA directly phosphorylated PC2, which increased the mean open time but not the single channel conductance of the channel. PKA phosphorylation did not modify either R742X truncated or S829A-mutant PC2iv channel function. The data indicate that the cAMP pathway regulates PC2-mediated cation transport in the hST.     A Global Survey of Physicians Knowledge About Nonalcoholic Fatty Liver Disease

    Background & Aims Despite rapidly increasing nonalcoholic fatty liver disease (NAFLD) prevalence, providers’ knowledge may be limited. We assessed NAFLD knowledge and associated factors among physicians of different specialties globally. Methods NAFLD knowledge surveys containing 54 and 59 questions covering 3 domains (epidemiology/pathogenesis, diagnostics, and treatment) were completed electronically by hepatologists, gastroenterologists (GEs), endocrinologists (ENDOs), and primary care physicians (PCPs) from 40 countries comprising 5 Global Burden of Disease super-regions. Over 24 months, 2202 surveys were completed (488 hepatologists, 758 GEs, 148 ENDOs, and 808 PCPs; 50% high-income Global Burden of Disease super-region, 27% from North Africa and Middle East, 12% Southeast Asia, and 5% South Asian and Latin America). Results Hepatologists saw the greatest number of NAFLD patients annually: median 150 (interquartile range, 60–300) vs 100 (interquartile range, 35–200) for GEs, 100 (interquartile range, 30–200) for ENDOs, and 10 (interquartile range, 4–50) for PCPs (all P < .0001). The primary sources of NAFLD knowledge acquisition for hepatologists were international conferences (33% vs 8%–26%) and practice guidelines for others (39%–44%). The Internet was the second most common source of NAFLD knowledge for PCPs (28%). NAFLD knowledge scores were higher for hepatologists than GEs: epidemiology, 62% vs 53%; diagnostics, 80% vs 73%; and treatment, 61% vs 58% (P < .0001), and ENDOs scores were higher than PCPs: epidemiology, 70% vs 60%; diagnostics, 71% vs 64%; and treatment, 79% vs 68% (P < .0001). Being a hepatologist or ENDO was associated with higher knowledge scores than a GE or PCP, respectively (P < .05). Higher NAFLD knowledge scores were associated independently with a greater number of NAFLD patients seen (P < .05). Conclusions Despite the growing burden of NAFLD, a significant knowledge gap remains for the identification, diagnosis, and management of NAFLD
