Cerebrovascular atherosclerosis in type III hyperlipidemia is modulated by variation in the Apolipoprotein A5 gene
<p>Abstract</p> <p>Objective</p> <p>Type III Hyperlipoproteinemia is a rare lipid disorder with a frequency of 1-5 in 5000. It is characterized by the accumulation of triglyceride rich lipoproteins and patients are at increased risk of developping atherosclerosis. Type III HLP is strongly associated with the homozygous presence of the Δ2 allele of the <it>APOE </it>gene.</p> <p>However only about 10% of subjects with APOE2/2 genotype develop hyperlipidemia and it is therefore assumed that further genetic and environmental factors are necessary for the expression of disease. It has recently been shown that variation in the <it>APOA5 </it>gene is one of these co-factors. The aim of this study is to investigate the development of cerebrovascular atherosclerosis in patients with Type III hyperlipoproteinemia (Type III HLP) and the role of variation in the APOA5 gene as a risk factor.</p> <p>Methods</p> <p>60 patients with type III hyperlipidemia and ApoE2/2 genotype were included in the study after informed consent. The presence of cerebrovascular atherosclerosis was investigated using B-mode ultra-sonography of the carotid artery. Serum lipid levels were measured by standard procedures. The APOE genotype and the 1131T > C and S19W SNPs in the APOA5 gene and the APOC3 sstI SNP were determined by restriction isotyping Allele frequencies were determined by gene counting and compared using Fisher's exact test. Continuous variables were compared using the Mann Whitney test. A p value of 0.05 or below was considered statistically significant. Analysis was performed using Statistica 7 software.</p> <p>Results</p> <p>The incidence of the APOA5 SNPs, -1131T > C and S19W and the APOC3 sstI SNP were determined as a potential risk modifier. After correction for conventional risk factors, the C allele of the 1131T > C SNP in the APOA5 gene was associated with an increased risk for the development of carotid plaque in patients with Type III HLP with an odds ratio of 3.69. Evaluation of the genotype distribution was compatible with an independent effect of APOA5.</p> <p>Conclusions</p> <p>The development of atherosclerosis in patients with Type III HLP is modulated by variation in the <it>APOA5 </it>gene.</p
Acupuncture for the treatment of phantom limb syndrome in lower limb amputees: a randomised controlled feasibility study
Background: Post amputation, the complication phantom limb pain (PLP) is prevalent and difficult to manage. This study aimed to determine whether it was feasible and acceptable to undertake a definitive multi-centred randomised controlled trial assessing the effectiveness of acupuncture for treating lower limb amputees with PLP. Methods: A mixed methods embedded design including a randomised controlled trial and semi-structured interviews was undertaken. A total of 15 participants with PLP were randomly assigned to receive either 8 pragmatic Traditional Chinese Medicine acupuncture treatments and usual care or usual care alone over four weeks. Outcomes measures were completed at baseline, weekly throughout the study and at one month post completion of the study and included; a numerical pain rating scale, Short-Form McGill Pain Questionnaire 2, EQ-5D-5L, Hospital Anxiety and Depression Scale, Perceived Stress Scale 10 item, Insomnia Severity Index, Patient Global Impression of Change. Post completion of the trial, participants in the acupuncture group were interviewed about their experience. Feasibility specific data were also collected. Results: Of 24 amputees meeting the study inclusion criteria 15 agreed to participate (recruitment rate 62.50%). Qualitatively acupuncture was perceived to be beneficial and effective. Quantitatively acupuncture demonstrated clinically meaningful change in average pain intensity (raw change=2.69) and worst pain intensity (raw change = 4.00). Feasibility specific data identified that before undertaking a definitive trial, recruitment, practitioner adherence to the acupuncture protocol, completion of outcome measures at one month follow up and blinding should be addressed. Appropriate outcome measures were identified for use in a definitive trial. Data were generated for future sample size calculations (effect size 0.64). Allowing for a 20% dropout rate, a sample size of 85 participants per group would be needed in a future definitive trial. Conclusions: A future definitive trial may be possible if the areas identified in this study are addressed. As acupuncture may be effective at treating PLP and as this feasibility study suggests a definitive trial may be possible, a multi-centred trial with adequate sample size and blinding is now needed. Calciphylaxis in chronic, non-dialysis-dependent renal disease
BACKGROUND: Calciphylaxis cutis is characterized by media calcification of arteries and, most prominently, of cutaneous and subcutaneous arterioles occurring in renal insufficiency patients. CASE REPORT: A 53-year-old woman with chronic cardiac and renal failure complained of painful crural, non-varicosis ulcers. She was hospitalized in an immobilized condition due to both the crural ulcerations and the existing heart-failure state (NYHA III-IV) having pleural and pericardial effusions, atrial fibrillation and weight loss of 30 kg over the past year. Despite normalization of calcium-phosphorus balance and improvement of renal function, the clinical course of crural ulcerations deteriorated during the following 3 months. After failure of surgical debridements, multiple courses of sterile-maggot therapy were introduced at a late stage to stabilize the wounds. The patient died of recurrent wound infections and sepsis paralleled by exacerbations of renal malfunction. CONCLUSIONS: The role of renal disease in vascular complications is discussed. Sterile-maggot debridement may constitute a therapy for the ulcerated calciphylaxis at an earlier stage, i.e. when first ulcerations appear
Effect of Cyclooxygenase(COX)-1 and COX-2 inhibition on furosemide-induced renal responses and isoform immunolocalization in the healthy cat kidney
BACKGROUND: The role of cyclooxygenase(COX)-1 and COX-2 in the saluretic and renin-angiotensin responses to loop diuretics in the cat is unknown. We propose in vivo characterisation of isoform roles in a furosemide model by administering non-steroidal anti-inflammatory drugs (NSAIDs) with differing selectivity profiles: robenacoxib (COX-2 selective) and ketoprofen (COX-1 selective). RESULTS: In this four period crossover study, we compared the effect of four treatments: placebo, robenacoxib once or twice daily and ketoprofen once daily concomitantly with furosemide in seven healthy cats. For each period, urine and blood samples were collected at baseline and within 48 h of treatment starting. Plasma renin activity (PRA), plasma and urinary aldosterone concentrations, glomerular filtration rate (GFR) and 24 h urinary volumes, electrolytes and eicosanoids (PGE(2), 6-keto-PGF1(α,) TxB(2)), renal injury biomarker excretions [N-acetyl-beta-D-glucosaminidase (NAG) and Gamma-Glutamyltransferase] were measured. Urine volume (24 h) and urinary sodium, chloride and calcium excretions increased from baseline with all treatments. Plasma creatinine increased with all treatments except placebo, whereas GFR was significantly decreased from baseline only with ketoprofen. PRA increased significantly with placebo and once daily robenacoxib and the increase was significantly higher with placebo compared to ketoprofen (10.5â±â4.4 vs 4.9â±â5.0 ng ml(â1) h(â1)). Urinary aldosterone excretion increased with all treatments but this increase was inhibited by 75 % with ketoprofen and 65 % with once daily robenacoxib compared to placebo. Urinary PGE(2) excretion decreased with all treatments and excretion was significantly lower with ketoprofen compared to placebo. Urinary TxB(2) excretion was significantly increased from baseline only with placebo. NAG increased from baseline with all treatments. Immunohistochemistry on post-mortem renal specimens, obtained from a different group of cats that died naturally of non-renal causes, suggested constitutive COX-1 and COX-2 co-localization in many renal structures including the macula densa (MD). CONCLUSIONS: These data suggest that both COX-1 and COX-2 could generate the signal from the MD to the renin secreting cells in cats exposed to furosemide. Co-localization of COX isoenzymes in MD cells supports the functional data reported here. Testing matter effects in propagation of atmospheric and long-baseline neutrinos
We quantify our current knowledge of the size and flavor structure of the
matter effects in the evolution of atmospheric and long-baseline neutrinos
based solely on the analysis of the corresponding neutrino data. To this aim we
generalize the matter potential of the Standard Model by rescaling its
strength, rotating it away from the e-e sector, and rephasing it with respect
to the vacuum term. This phenomenological parametrization can be easily
translated in terms of non-standard neutrino interactions in matter. We show
that in the most general case, the strength of the potential cannot be
determined solely by atmospheric and long-baseline data. However its flavor
composition is very much constrained and the present determination of the
neutrino masses and mixing is robust under its presence. We also present an
update of the constraints arising from this analysis in the particular case in
which no potential is present in the e-mu and e-tau sectors. Finally we
quantify to what degree in this scenario it is possible to alleviate the
tension between the oscillation results for neutrinos and antineutrinos in the
MINOS experiment and show the relevance of the high energy part of the spectrum

Early and Middle Holocene Hunter-Gatherer Occupations in Western Amazonia: The Hidden Shell Middens
We report on previously unknown early archaeological sites in the Bolivian lowlands, demonstrating for the first time early and middle Holocene human presence in western Amazonia. Multidisciplinary research in forest islands situated in seasonally-inundated savannahs has revealed stratified shell middens produced by human foragers as early as 10,000 years ago, making them the oldest archaeological sites in the region. The absence of stone resources and partial burial by recent alluvial sediments has meant that these kinds of deposits have, until now, remained unidentified. We conducted core sampling, archaeological excavations and an interdisciplinary study of the stratigraphy and recovered materials from three shell midden mounds. Based on multiple lines of evidence, including radiocarbon dating, sedimentary proxies (elements, steroids and black carbon), micromorphology and faunal analysis, we demonstrate the anthropogenic origin and antiquity of these sites. In a tropical and geomorphologically active landscape often considered challenging both for early human occupation and for the preservation of hunter-gatherer sites, the newly discovered shell middens provide evidence for early to middle Holocene occupation and illustrate the potential for identifying and interpreting early open-air archaeological sites in western Amazonia. The existence of early hunter-gatherer sites in the Bolivian lowlands sheds new light on the region's past and offers a new context within which the late Holocene "Earthmovers" of the Llanos de Moxos could have emerged. © 2013 Lombardo et al
Holographic Charged Fluid with Anomalous Current at Finite Cutoff Surface in Einstein-Maxwell Gravity
The holographic charged fluid with anomalous current in Einstein-Maxwell
gravity has been generalized from the infinite boundary to the finite cutoff
surface by using the gravity/fluid correspondence. After perturbing the boosted
Reissner-Nordstrom (RN)-AdS black brane solution of the Einstein-Maxwell
gravity with the Chern-Simons term, we obtain the first order perturbative
gravitational and Maxwell solutions, and calculate the stress tensor and
charged current of the dual fluid at finite cutoff surfaces which contains
undetermined parameters after demanding regularity condition at the future
horizon. We adopt the Dirichlet boundary condition and impose the Landau frame
to fix these parameters, finally obtain the dependence of transport
coefficients in the dual stress tensor and charged current on the arbitrary
radical cutoff . We find that the dual fluid is not conformal, but it has
vanishing bulk viscosity, and the shear viscosity to entropy density ratio is
universally . Other transport coefficients of the dual current turns
out to be cutoff-dependent. In particular, the chiral vortical conductivity
expressed in terms of thermodynamic quantities takes the same form as that of
the dual fluid at the asymptotic AdS boundary, and the chiral magnetic
conductivity receives a cutoff-dependent correction which vanishes at the

Every year, suicide is among the top 20 leading causes of death globally for all ages. Unfortunately, suicide is difficult to prevent, in large part because the prevalence of risk factors is high among the general population. In this review, clinical and psychological risk factors are examined and methods for suicide prevention are discussed. Prevention strategies found to be effective in suicide prevention
include means restriction, responsible media coverage, and general public education, as well identification methods such as screening, gatekeeper training, and primary care physician education. Although the treatment for preventing suicide is difficult, follow-up that includes pharmacotherapy, psychotherapy, or both may be useful. Complexity within an oil palm monoculture: The effects of habitat variability and rainfall on adult dragonfly (Odonata) communities
Recent expansion of oil palm agriculture has resulted in loss of forest habitat and forest-dependent species. However, large numbers of species â particularly insects â can persist within plantations. This study focuses on Odonata (dragonflies and damselflies): a charismatic indicator taxon, and a potentially valuable pest control agent. We surveyed adult Odonata populations biannually over three years within an industrial oil palm plantation in Sumatra, Indonesia. We assessed the effects of rainfall (including an El Niño Southern Oscillation-associated drought), the role of roadside ditches, and the importance of understory vegetation on Odonata populations. To assess the impacts of vegetation we took advantage of a long-term vegetation management experiment that is part of the Biodiversity and Ecosystem Function in Tropical Agriculture (BEFTA) Programme. We found 41 Odonata species, and communities varied between plantation core and roadside edge microhabitats, and between seasons. Abundance was significantly related to rainfall levels four months before surveys, probably indicating the importance of high water levels in roadside ditches for successful larval development. We found no significant effect of the BEFTA understory vegetation treatments on Odonata abundance, and only limited effects on community composition, suggesting that local understory vegetation structure plays a relatively unimportant role in determining communities. Our findings highlight that there are large numbers of Odonata species present within oil palm plantations, and suggest that their abundance could potentially be increased by maintaining or establishing waterbodies.
