    The ALE-method with triangular elements: direct convection of integration point values

    The arbitrary Lagrangian-Eulerian (ALE) finite element method is applied to the simulation of forming processes where material is highly deformed. Here, the split formulation is used: a Lagrangian step is done with an implicit finite element formulation, followed by an explicit (purely convective) Eulerian step. The purpose of this study is to investigate the Eulerian step for quadratic triangular elements. To solve the convection equation for integration point values, a new method inspired by Van Leer is constructed. The new method is based on direct convection of integration point values without intervention of nodal point values.\ud The Molenkamp test and a so-called block test were executed to check the performance and stability of the convection scheme. From these tests it is concluded that the new convection scheme shows accurate results. The scheme is extended to an ALE-algorithm. An extrusion process was simulated to test the applicability of the scheme to engineering problems. It is concluded that direct convection of integration point values with the presented algorithm leads to accurate results and that it can be applied to ALE-simulation

    The influence of preferred place of birth on the course of pregnancy and labor among healthy nulliparous women: a prospective cohort study

    __Abstract__ BACKGROUND: Most studies on birth settings investigate the association between planned place of birth at the start of labor and birth outcomes and intervention rates. To optimize maternity care it also is important to pay attention to the entire process of pregnancy and childbirth. This study explores the association between the initial preferred place of birth and model of care, and the course of pregnancy and labor in low-risk nulliparous women in the Netherlands. METHODS: As part of a Dutch prospective cohort study (2007-2011), we compared medical indications during pregnancy and birth outcomes of 576 women who initially preferred a home birth (n = 226), a midwife-led hospital birth (n = 168) or an obstetrician-led hospital birth (n = 182). Data were obtained by a questionnaire before 20 weeks of gestation and by medical records. Analyses were performed according to the initial preferred place of birth. RESULTS: Low-risk nulliparous women who preferred a home birth with midwife-led care were less likely to be diagnosed with a medical indication during pregnancy compared to women who preferred a birth with obstetrician-led care (OR 0.41 95% CI 0.25-0.66). Preferring a birth with midwife-led care - both at home and in hospital - was associated with lower odds of induced labor (OR 0.51 95% CI 0.28-0.95 respectively OR 0.42 95% CI 0.21-0.85) and epidural analgesia (OR 0.32 95% CI 0.18-0.56 respectively OR 0.34 95% CI 0.19-0.62) compared to preferring a birth with obstetrician-led care. In addition, women who preferred a home birth were less likely to experience augmentation of labor (OR 0.54 95% CI 0.32-0.93) and narcotic analgesia (OR 0.41 95% CI 0.21-0.79) compared to women who preferred a birth with obstetrician-led care. We observed no significant association between preferred place of birth and mode of birth. CONCLUSIONS: Nulliparous women who initially preferred a home birth were less likely to be diagnosed with a medical indication during pregnancy. Women who initially preferred a birth with midwife-led care - both at home and in hospital - experienced lower rates of interventions during labor. Although some differences can be attributed to the model of care, we suggest that characteristics and attitudes of women themselves also play an important role

    Size does matter: Cre-mediated somatic deletion efficiency depends on the distance between the target lox-sites

    Cre/lox recombination in vivo has become an important tool to induce chromosomal rearrangements like deletions. Using a combination of Ds transposition and Cre/lox recombination in two independent experiments on chromosomes 6 and 7 of tomato, two sets of somatic deletions up to a size of 200 kb were obtained. The efficiency of somatic deletion decreased with increasing deletion size. The largest germinally transmitted deletion had a size of only 55 kb. The results show that Cre-mediated deletion in somatic cells is less efficient when the lox sites are separated over larger distances. A further drop of the deletion efficiency after germinal transmission of the larger deletions can be explained by the probable loss of genes that are of vital importance to gametophyte function. Plasmid rescue of an 8.4 kb circularised deleted DNA showed that the Cre-mediated deletion takes place in tomato as expected. Since the circular Cre-deleted DNA could only be PCR amplified in plant cells where the deletion was not complete, the double-stranded DNA circle is assumed to be instable

    Cre recombinase expression can result in phenotypic aberrations in plants

    The cre recombinase gene was stably introduced and expressed in tomato, petunia and Nicotiana tabacum. Some plants expressing the cre gene driven by a CaMV 35S promoter displayed growth retardation and a distinct pattern of chlorosis in their leaves. Although no direct relation can be proven between the phenotype and cre expression, aberrant phenotypes always co-segregate with the transgene, which strongly suggests a correlation. The severity of the phenotype does not correlate with the level of steady-state mRNA in mature leaves, but with the timing of cre expression during organogenesis. The early onset of cre expression in tomato is correlated with a more severe phenotype and with higher germinal transmission frequencies of site-specific deletions. No aberrant phenotype was observed when a tissue-specific phaseolin promoter was used to drive the cre gene.     Structure-performance relations in homogeneous Pd catalysis by in situ EXAFS spectroscopy

    X-ray absorption fine structure (XAFS) spectroscopy is used to unravel the structure of homogeneous catalysts in their catalytically active phase (solution), for which other characterization techniques failed in providing detailed structural information. Application of in situ EXAFS shows that the conformation of the allylic fragment of (P-P)Pd(1,1-dimethylallyl) catalytic intermediate complexes in solution (i.e., reaction medium) differs from that in the solid state, dependent on the bidentate ligand. The change in orientation directly explains differences in regioselectivity in the allylic alkylation reaction, displayed by the distinct complexes

    Background: An economic evaluation was performed alongside an RCT investigating flap fixation in reducing seroma formation after mastectomy. The evaluation focused on the first year following mastectomy and assessed cost-effectiveness from a health care and societal perspective. Methods: The economic evaluation was conducted between 2014 and 2018 in four Dutch breast clinics. Patients with an indication for mastectomy or modified radical mastectomy were randomly assigned to: conventional closure (CON), flap fixation with sutures (FFS) or flap fixation with tissue glue (FFG). Health care costs, patient and family costs and costs due to productivity losses were assessed. Outcomes were expressed in incremental cost-effectiveness ratios (ICERs): the incremental cost per quality-adjusted life year (QALY). Bootstrapping techniques, sensitivity and secondary analyses were employed to address uncertainty. Results: The FFS-group yielded most QALYs (0.810; 95%-CI 0.755–0.856), but also incurred the highest mean costs at twelve months (€10.416; 95%-CI 8.231–12.930). CON was the next best alternative with 0.794 QALYs (95%-CI 0.733–0.841) and mean annual costs of €10.051 (95%-CI 8.255–12.044). FFG incurred fewer QALYs and higher costs, when compared to the CON group. The ICER of FFS compared to CON was €22.813/QALY. Applying a willingness to pay threshold in the Netherlands of €20.000/QALY, the probability that FFS was cost-effective was 42%, compared to 37% and 21% for CON and FFG, respectively. Conclusion: The cost-effectiveness of FFS following mastectomy, versus CON and FFG, is uncertain from a societal perspective. Yet, from a health care and hospital perspective FFS is likely to be the most cost-effective intervention
