    Moduli Spaces of Curves with Homology Chains and c=1 Matrix Models

    We show that introducing a periodic time coordinate in the models of Penner-Kontsevich type generalizes the corresponding constructions to the case of the moduli space Sgnk{\cal S}_{gn}^k of curves CC with homology chains \gamma\in H_1(C,\zet_k). We make a minimal extension of the resulting models by adding a kinetic term, and we get a new matrix model which realizes a simple dynamics of \zet_k-chains on surfaces. This gives a representation of c=1c=1 matter coupled to two-dimensional quantum gravity with the target space being a circle of finite radius, as studied by Gross and Klebanov.Comment: IFUM 459/FT (LaTeX, 9 pages; a few misprints have been corrected and the introduction has been slightly modified

    We have studied the effects of hypocapnia on cerebrovascular changes in two MAC-equivalent anaesthetic regimens, using the transcranial Doppler technique as an index of cerebral blood flow (CBF) in 24healthy ASA I patients undergoing spinal surgery. Eight of the patients were subjected to carbon dioxide reactivity challenges in the awake state. Before surgery, the other 16 patients received, in random order, either 1.15% isoflurane in oxygen or 0.5% isoflurane with 70% nitrous oxide. Carbon dioxide reactivity was calculated for each group as the increase in flow velocity per kPa change in PÉCO2 (cm s−1 kPa−1). It was significantly greater for the isoflurane group (14.09 (SD 2.44) cm s−1 kPa−1) and significantly less for the isoflurane—nitrous oxide group (7.95 (1.32) cm s-−1 kPa−1) compared with the awake group (11.24 (0.95) cm s−1 kPa−1). We conclude that cerebrovascular responsiveness to changes in arterial carbon dioxide concentration is influenced markedly by the anaesthetic procedure.     Quantum rainbow scattering at tunable velocities

    Elastic scattering cross sections are measured for lithium atoms colliding with rare gas atoms and SF6 molecules at tunable relative velocities down to ~50 m/s. Our scattering apparatus combines a velocity-tunable molecular beam with a magneto-optic trap that provides an ultracold cloud of lithium atoms as a scattering target. Comparison with theory reveals the quantum nature of the collision dynamics in the studied regime, including both rainbows as well as orbiting resonances

    Effect of n-3 fatty acids on cerebral markers and the inflammatory response in sepsis

    Introduction ICM+ software encapsulates our 20 years' experience in brain monitoring. It collects data from a variety of bedside monitors and produces time trends of parameters defi ned using confi gurable mathematical formulae. To date it is being used in nearly 40 clinical research centres worldwide. We present its application for continuous monitoring of cerebral autoregulation using near-infrared spectroscopy (NIRS). Methods Data from multiple bedside monitors are processed by ICM+ in real time using a large selection of signal processing methods. These include various time and frequency domain analysis functions as well as fully customisable digital fi lters. The fi nal results are displayed in a variety of ways including simple time trends, as well as time window based histograms, cross histograms, correlations, and so forth. All this allows complex information from bedside monitors to be summarized in a concise fashion and presented to medical and nursing staff in a simple way that alerts them to the development of various pathological processes. Results One hundred and fi fty patients monitored continuously with NIRS, arterial blood pressure (ABP) and intracranial pressure (ICP), where available, were included in this study. There were 40 severely headinjured adult patients, 27 SAH patients (NCCU, Cambridge); 60 patients undergoing cardiopulmonary bypass (John Hopkins Hospital, Baltimore) and 23 patients with sepsis (University Hospital, Basel). In addition, MCA fl ow velocity (FV) was monitored intermittently using transcranial Doppler. FV-derived and ICP-derived pressure reactivity indices (PRx, Mx), as well as NIRS-derived reactivity indices (Cox, Tox, Thx) were calculated and showed signifi cant correlation with each other in all cohorts. Errorbar charts showing reactivity index PRx versus CPP (optimal CPP chart) as well as similar curves for NIRS indices versus CPP and ABP were also demonstrated. Conclusions ICM+ software is proving to be a very useful tool for enhancing the battery of available means for monitoring cerebral vasoreactivity and potentially facilitating autoregulation guided therapy. Complexity of data analysis is also hidden inside loadable profi les, thus allowing investigators to take full advantage of validated protocols including advanced processing formulas

    Cerebral pressure autoregulation and carbon dioxide reactivity during propofol-induced EEG suppression

    We studied cerebral pressure autoregulation and carbon dioxide reactivity during propofol-induced electrical silence of the electroencephalogram (EEG) in 10 patients. Anaesthesia was induced with propofol 2.5 mg kg−1, fentanyl 3 μg kg−1 and vecuronium 0.1 mg kg−1, and a propofol infusion of 250-300 μg kg−1 min−1 was used to induce EEG silence. Cerebral pressure autoregulation was tested by increasing mean arterial pressure (MAP) by 24 (SEM 5) mm Hg from baseline with an infusion of phenylephrine and simultaneously recording middle cerebral artery blood flow velocity (vmca) using transcranial Doppler. Carbon dioxide reactivity was tested by varying Paco2 between 4.0 and 7.0 kPa and recording vmca simultaneously. Although absolute carbon dioxide reactivity was reduced, relative carbon dioxide reactivity was within normal limits for all patients studied (mean 8.5 (SEM 0.8) cm s−1 kPa−1 and 22 (2)% kPa−1, respectively). No significant change in vmca (34 (2) and 35 (2) cm s−1) was observed with the increase in MAP (77 (4) to 101 (4) mm Hg) during autoregulation testing.     There are many causes for headaches after childbirth. Even though postdural puncture headache (PDPH) has to be considered in a woman with a history of difficult epidural anaesthesia, pre‐eclampsia should always be excluded as an important differential diagnosis. We report a case with signs of late‐onset pre‐eclampsia where administration of an epidural blood patch (EBP) was associated with eclampsia.     Improved setup for producing slow beams of cold molecules using a rotating nozzle

    Intense beams of cold and slow molecules are produced by supersonic expansion out of a rapidly rotating nozzle, as first demonstrated by Gupta and Herschbach. An improved setup is presented that allows to accelerate or decelerate cold atomic and molecular beams by up to 500 m/s. Technical improvements are discussed and beam parameters are characterized by detailed analysis of time of flight density distributions. The possibility of combining this beam source with electrostatic fields for guiding polar molecules is demonstrated

    Utilisation of outpatient services at Red Cross War Memorial Children's Hospital, Cape Town

    The demand for outpatient services continues to grow at Red Cross War Memorial Children's Hospital (RCCH). To determine current utilisation patterns, we conducted a 2-week survey in the outpatient department (OPD). In addition, we reviewed the RCCH Annual Reports for the period 1961 - 1988. Annual outpatient attendances have increased from around 42000 in 1957 to their highest level ever; nearly 350000 in 1988. This steady rise in outpatient attendance was stemmed during the 1970s by the expansion of health services in the greater Cape Town area, in particular the introduction ofday hospitals. In general, blacks are utilising the OPD as a primary community hospital for the treatment of infectious and environmentally induced diseases. In contrast, the white outpatient profile is more characteristic of a tertiary referral centre, with a higher proportion of specialist clinic attendances. The utilisation patterns for coloured children are intermediate. Analysis of the residential address of patients and their presenting diagnoses indicates an urgent demand for primary health care services in the most recently settled and poorest suburbs of Cape Town, many of which are remote from the hospital

    Haemophilus influenzae invasive disease in the United States, 1994-1995: near disappearance of a vaccine-preventable childhood disease.

    We analyzed national Haemophilus influenzae (Hi) surveillance data from 1994 and 1995 to describe the epidemiology of Hi invasive disease among persons of all ages. Serotype data were available for 376 (56%) of 669 reported Hi cases among children aged 4 years or younger; 184 (49%) were H. influenzae type b (Hib). Among children aged 4 or younger, incidence (per 100,000) of all Hi invasive disease was 1.8 in 1994 and 1.6 (p < 0.05) in 1995. Children aged 5 months or younger had the highest average annual incidence rate of Hib invasive disease (2.2 per 100,000); children aged 6 to 11 months had the next highest rate (1.2 per 100,000)(p < 0.05). Of 181 children with Hib invasive disease whose age in months was known, 85 (47%) were too young (aged 5 months or younger) to have completed a primary series with an Hib-containing vaccine. Of the 83 children with known vaccination status who were eligible to receive a primary series (aged 6 months or older), 52 (63%) were undervaccinated, and the remaining 31 (37%) had completed a primary series in which vaccine failed. Among persons aged 5 years or older with Hi invasive disease, the lowest average annual incidence was among those 20 to 39 years of age (0.15 per 100,000), and the highest was among those aged 80 years or older (2.26 per 100,000). Among persons aged 5 years or older, serotype data were available for 1,372 (71%) of the 1,940 Hi invasive disease cases; 159 (28%) of the 568 Hi cases with known serotype were due to Hib

    The modular geometry of Random Regge Triangulations

    We show that the introduction of triangulations with variable connectivity and fluctuating egde-lengths (Random Regge Triangulations) allows for a relatively simple and direct analyisis of the modular properties of 2 dimensional simplicial quantum gravity. In particular, we discuss in detail an explicit bijection between the space of possible random Regge triangulations (of given genus g and with N vertices) and a suitable decorated version of the (compactified) moduli space of genus g Riemann surfaces with N punctures. Such an analysis allows us to associate a Weil-Petersson metric with the set of random Regge triangulations and prove that the corresponding volume provides the dynamical triangulation partition function for pure gravity.Comment: 36 pages corrected typos, enhanced introductio
