    COMBINATION OF TETRALOGY OF FALLOT AND HEMOPHILIA IN A CHILD: CASE REPORT

    Congenital heart disease (CHD) is formed during intrauterine period but, as a rule, they are not hereditary. Hemophilia A belongs to the group of hereditary coagulopathies stipulated by deficiency of coagulation factor VIII and characterized by bleeding of hematoma like type. Combination of these two diseases is rather rare that requires cardiosurgeons’ apprehensive attitude.We present our experience in surgical treatment of a boy of 8 months old with Tetralogy of Fallot. Hemophilia A was found in preoperative examination. He was successfully operated with cardiac pulmonary bypass on the background of factor VII injection.Surgical treatment was successful without significant bleeding. Postoperative period was uneventful. For successful treatment of CHD with coagulation failure is very important timely diagnosis, that can be difficult due to the lack of specific symptoms in children.Surgical treatment is the method of choice in TOF.     Crystallization kinetics of Al86Ni8Gd6 amorphous alloy

    We study structure and crystallization kinetics of melt-spun Al 86Ni8Gd6 amorphous alloy. The X-ray diffraction and transmission electron microscopy detect pronounced nanoscale clusters in the structure of the amorphous state. Differential scanning calorimetry analysis shows complex three-stage crystallization of the amorphous alloy at different heating rates. The high temperature X-ray diffraction analysis reveals crystallization mechanism with competing reactions of intermetallic compounds formation. Summarizing the experimental data, we introduce multiversion non-linear kinetic model that describes the crystallization process completely and allows us to determine kinetic parameters with high accuracy. The activation energies obtained reveal relatively stable amorphous state. © 2013 Elsevier B.V

    The article is devoted to the analysis of the key reasons for the slowdown in the global economy. According to the analysis, the slowdown in the global economy is caused by the following seven key reasons: trade wars and the aggravation of trade relations in the international economic market; global instability and technological uncertainty; risks of increased financial uncertainty; the impossibility on the part of some countries to overcome the consequences of the global crisis and the sanctions policy of the West; reformatting of economic activity in a political manner; wave of debt accumulation in developing countries; slowdown in labor productivity growth. The authors conclude that the global economic slowdown trend has a long-term perspective

    Experimental study of intrinsic multiple Andreev reflections effect in GdO(F)FeAs superconductor array junctions

    We report the first observation of the intrinsic multiple Andreev reflections effect (IMARE) in S-n-S-...-S-arrays (S = superconductor, n = normal metal) formed by "break-junction" technique in GdO(F)FeAs superconductor (Tc = 48 - 53 K). We show that superconducting gap peculiarities at dI/dV-spectra sharpen dramatically in the arrays as compared with that in the single-contact spectra; this enables to improve significantly accuracy of the bulk superconducting parameters determination. Using IMARE, we determined the large and the small gap values \Delta_L = 11 +- 1.1 meV and \Delta_S = 2.6 +- 0.4 meV. The BCS-ratio 2\Delta_L/kTc^{local} = 5.0 - 5.9 > 3.52 (Tc^{local} is the contact area critical temperature) evidences for a strong electron-boson coupling. The results obtained agree well with our previous data by Andreev spectroscopy for single SnS-contacts.Comment: 6 pages, 6 figure

    The quality of medical care in a round-the-clock hospital during the COVID-19 pandemic

    Violations in the provision of medical care are detected by controlling the volume, timing, quality and conditions of medical care. The objective of the work is assessment of the activities of the round-the-clock hospital from the standpoint of the quality of medical care during the COVID-19 pandemic. Material and methods. A database of indicators reflecting the activity of round-the-clock hospital for four years, from 2017 to 2020 (Omsk) was formed. Analytical, statistical methods and the method of expert assessments were used to analyze the required indicators. Results. Evaluation of inpatient hospital activities during the COVID-19 pandemic compared to the “pre-pandemic” years showed a 1.9-fold and 2.9-fold decrease in the total number of quality examinations performed and the number of defects detected, respectively. The number of defects that were grounds for denial or reduction of payment for medical care found in the “before-COVID-19” years was 2.1 times higher than in 2020. In 2020, the weight of the share of those defects that directly reflect the quality of medical care increased signifcantly. The quantitative composition of the inpatient hospital staff changed insignifcantly during the analyzed period – the number of physicians increased  by only 5 % over four years, with no dynamics in the qualitative characteristics of the staff. The number of medical equipment increased by 27.6 %. Due to re-profling in 2020, there was a decrease in the number of hospitalized patients, with an increase in the proportion of patients admitted by emergency care (up to 91 %) and an increase in mortality by 2.7 times. Conclusions. One of the conditions for maintaining high quality of medical care is a balance between the main components of quality: accessibility, timeliness, sufciency, efciency and safety and the speed of its achievement, which in turn depends on the amount of resources and reserves

    Mеsozoic Tectonothermal Evolution of the Zagan Metamorphic Core Complex in Western Transbaikalia: 40Ar/39Ar and FTA Dating

    A model of tectonothermal evolution of the Zagan metamorphic core complex (MCC) based on the new data from 40Ar/39Ar dating of amphibole, mica, and apatite fission-track dating is discussed. A relationship with the long-range impact of processes from the collision of the North China (Amurian–North China) block with the Siberian continent in the Mesozoic era is proposed. The Zagan MСС was formed in the Cretaceous period on the southern flank of a high mountain uplift of Western Transbaikalia, composed of late Paleozoic granitoids of the Angara–Vitim batholith. According to 40Ar/39Ar dating of amphiboles and micas from the mylonite zone, the active development time of the Zagan MCC corresponds to the early Cretaceous epoch (131, 114 Ma). The tectonic exposure of the core from about 15 km to the depths of about 10 km occurred at a rate of tectonic erosion of 0.4–0.3 mm/year as a result of post-collisional extension of the Mongol–Okhotsk orogen. Apatite fission-track dating shows that further exhumation and cooling of the rocks to about 3 km occurred in the lower-upper Cretaceous epoch (112, 87 Ma).
