Köhler disease: an infrequent or underdiagnosed cause of child's limping?

    Get PDF
Köhler disease (KD) is the osteochondrosis of the tarsal navicular bone of the young children, which is a self-limited condition. Typically the child reports pain with mechanical characteristics in the medial region of the foot, however, intermittent or continuous limping may be the only clinical manifestation, which delays the diagnosis and consequently exposes to unnecessary tests and treatments. We describe a bilateral KD whose clinical manifestation was unilateral limping with 4 months evolution. The radiologic evaluation showed flattening, sclerosis and irregular rarefaction of both navicular bones. Claudication persisted for four weeks, after which, the child remains asymptomatic.info:eu-repo/semantics/publishedVersio

A multistage stochastic approach for the optimal bidding of variable renewable energy in the day-ahead, intraday and balancing markets

ABSTRACT: Market agents with renewable resources face amplified uncertainty when forecasting energy production to securely place bids in electricity markets. To deal with uncertainties, stochastic modelling has been applied to optimize the bidding strategy of these market agents. However, studies found in the literature usually focus on day-ahead and balancing markets, leaving aside intraday markets that could be used to correct bidding positions as uncertainty gets resolved. This paper proposes a multistage stochastic decision-aid algorithm based on linear programming to optimize the bidding strategy of market agents in three different electricity markets -day-ahead, intraday, and balance markets. The market agent represents a Virtual Power Plant with wind, solar PV, and storage technologies, and its participation in three electricity markets was compared to the participation in DA and BM markets only. Results show that participating in all three markets increased the profit achieved by the VPP agent by 10.1% while also decreasing the incurred imbalances by 63.8%. The results demonstrate that having accurate tools to deal with the multi-settlement framework of electricity markets while considering the uncertainties of daily operations is key to a successful integration of renewable energy resources into electricity markets and power systems.info:eu-repo/semantics/publishedVersio

Integrated sizing and scheduling of wind/PV/diesel/battery isolated systems

    Get PDF
In this paper we address the optimal sizing and scheduling of isolated hybrid systems using an optimization framework. The hybrid system features wind and photovoltaic conversion systems, batteries and diesel backup generators to supply electricity demand. A Mixed-Integer Linear Programming formulation is used to model system behavior over a time horizon of one year, considering hourly changes in both the availability of renewable resources and energy demand. The optimal solution is achieved with respect to the minimization of the levelized cost of energy (LCOE) over a lifetime of 20 years. Results for a case study show that the most economical solution features all four postulated subsystems

A method to correct the flow distortion of offshore wind data using CFD simulation and experimental wind tunnel tests

The assessment of wind energy resource for the development of deep offshore wind plants requires the use of every possible source of data and, in many cases, includes data gathered at meteorological stations installed at islands, islets or even oil platforms—all structures that interfere with, and change, the flow characteristics. This work aims to contribute to the evaluation of such changes in the flow by developing a correction methodology and applying it to the case of Berlenga island, Portugal. The study is performed using computational fluid dynamic simulations (CFD) validated by wind tunnel tests. In order to simulate the incoming offshore flow with CFD models a wind profile, unknown a priori, was established using observations from two coastal wind stations and a power law wind profile was fitted to the existing data (a=0.165). The results show that the resulting horizontal wind speed at 80 m above sea level is 16% lower than the wind speed at 80 m above the island for the dominant wind direction sector

Bowel-associated dermatosis-arthritis syndrome in an adolescent with short bowel syndrome

Bowel-associated dermatosis-arthritis syndrome (BADAS) is a neutrophilic dermatosis, characterized by the occurrence of arthritis and skin lesions related to bowel disease with or without bowel bypass. We report an unusual case of BADAS in a 15-year-old white male with congenital aganglionosis of the colon and hypoganglionosis of the small intestine and multiple bowel surgeries in childhood complicated by short bowel syndrome. He presented with recurrent peripheral polyarthritis, tenosynovitis, and painful erythematous subcutaneous nodules located on the dorsolateral regions of the legs and on the dorsa of the feet. Histological examination disclosed a neutrophilic dermatosis confirming the diagnosis of BADAS.Although an uncommon disease, especially at pediatric age, it is important to evoke the diagnosis of BADAS in children and adolescents with bowel disease, because treatment options and prognosis are distinct from other rheumatologic conditions

This aggregator/end-consumer interface based on rational win-win choices helps the consumer evaluate his available flexibility and get due acknowledgement for it. He schedules his consumption and alternatives he deems acceptable; he may also define electricity bill offsets he requires for some of these alternative schedules. He should also prioritize his power needs by pointing the maximum price he will pay for each consumption segment. These data allow for net electricity bill optimization, i.e. after the abovementioned offsets. Demand is then re-scheduled according to local renewable generation and hourly market price forecasts and the consumer must define his preferred schedule as well as alternatives he may accept. Search space complexity and dimensionality require heuristic drive. MATLAB based simulations encourage methodology refinements and a more sophisticated options panel to be offered to the consumer as well as to the aggregator. Further, this tool may help the aggregator construe compromise between different interests emerging in smaller markets and energy communities.info:eu-repo/semantics/publishedVersio

Towards a high-resolution offshore wind Atlas : the Portuguese case

    Get PDF
ABSTRACT: An accurate offshore wind resource assessment is a key tool for planning marine wind renewable exploitation. To achieve this goal, without resort to an extensive and costly network of anemometric stations or buoys, it becomes necessary to use the so-called atmospheric mesoscale models. This work presents a high spatial resolution (1x1 km) offshore wind resource Atlas for Portugal and the model calibration steps. During the calibration steps, the most adequate: i) atmospheric parameterizations - physics options, ii) initial and boundary conditions (IBC) meteorological datasets, and iii) data assimilation scheme were achieved through sensitivity tests using the common statistical metrics and hourly simulated/observational data. Results show that the most significant improvements are associated with the IBC dataset and the data assimilation scheme used. Thus, the results show that the assimilation procedures coupled with the new ERA-5 reanalysis dataset reduce significantly the errors of the wind speed and direction, especially the normalized mean square error. This reduction, depending on the different calibration setup, can be above 50%. The new Atlas confirms the previous indicators, Portugal presents a high wind power potential, especially for deep offshore regions.info:eu-repo/semantics/publishedVersio

Pharmacopoeias are official books that follow the scientific and technological evolution of knowledge relating to medicines, which ensures their quality update and therefore safeguarding Public Health.Nowadays, in the world, there are 49 pharmacopoeias, 46 are national, 2 are regional and 1 is international. The European Pharmacopoeia (inter-governmental), the United States Pharmacopoeia (independent of the government) and the Japanese Pharmacopoeia (governmental) are the main ones for the delineation of a global policy for quality of medicines. In Portugal, throughout times there have been official and non-official pharmacopoeias. The first official pharmacopoeia dates from 1794 and the Portuguese Pharmacopoeia 9 is currently in vigor. The Pharmacopoeial Discussion Group, the International Conference on Harmonisation and the International Cooperation on Veterinary Harmonisation play an important role in the international harmonization. The Good Pharmacopoeial Practices aim is to facilitate and promote the cooperation, work sharing and harmonization among pharmacopoeias. This article makes a historical review of portuguese pharmacopoeias and articulates the issue with literature foreign counterpart, emphasizing the value for Public Health.
