Should you treat a symptomatic patient by phone when his child has confirmed strep throat?

    Get PDF
Although no studies specifically evaluate this question, treatment for Group A beta- hemolytic streptococcal (GABHS) pharyngitis without laboratory confirmation in the general population is not advisable (strength of recommendation [SOR]: C, based on consensus guidelines) due to poor diagnostic accuracy. When you suspect GABHS pharyngitis either clinically or epidemio-logically, confirm the diagnosis of pharyngitis by a laboratory test. Patients with a positive throat culture or a rapid antigen detection test should receive appropriate treatment with antimicrobial therapy (SOR: A, based on clinical trials)

Bringing Scenario Planning Home to KU

    Get PDF
Energized by the immersive learning process at the ARL scenario-planning workshop in March 2011, the University of Kansas (KU) Libraries decided to introduce scenario planning to library staff later that spring, to support development of a new strategic plan. This article reviews the implementation of the ARL planning tool and the outcomes of the process at KU

Comparing Professional Development Experiences: Viewing the Constructivist Design Conference through the lens of a Professional Learning Community

    Get PDF
The first step in this comparison process is to gain a clear understanding of the term “professional learning community.” Ubiquitously used in current educational literature, there is an inherent need to define a professional learning community’s essential components (DuFour 2004). For the purposes of this piece, the concept of ‘professional learning community’ has four defining characteristics. These essential attributes are: (1) Supportive and collaborative conditions (2) Commitment to continuous improvement (3) Results orientation (4) Shared mission, vision, values and goals While definitions that are more elaborate exist, narrowing the essential characteristics that this piece will examine in light of the Constructivist Design Conference will allow subsequent reflections to be more applicable to one’s own professional development. Through the comparison of these two models, it is our intention to provide a framework for understanding other professional experiences

Thermal diffusion by Brownian motion induced fluid stress

The Ludwig-Soret effect, the migration of a species due to a temperature gradient, has been extensively studied without a complete picture of its cause emerging. Here we investigate the dynamics of DNA and spherical particles sub jected to a thermal gradient using a combination of Brownian dynamics and the lattice Boltzmann method. We observe that the DNA molecules will migrate to colder regions of the channel, an observation also made in the experiments of Duhr, et al[1]. In fact, the thermal diffusion coefficient found agrees quantitatively with the experimental value. We also observe that the thermal diffusion coefficient decreases as the radius of the studied spherical particles increases. Furthermore, we observe that the thermal fluctuations-fluid momentum flux coupling induces a gradient in the stress which leads to thermal migration in both systems.Comment: 6 pages, 5 figue

Assessing Burnout and Well-Being in Higher Education Health Science Faculty

    Get PDF
This study aimed to examine and compare the extent of burnout among health science faculty at a higher education institution and their self-reported perception of well-being during the COVID-19 pandemic. The design of the study was cross-sectional, descriptive survey research. An electronic questionnaire was developed to measure the constructs of burnout and well-being. Validated instruments used in the survey included the Oldenburg Burnout Inventory (OLBI) and the World Health Organization-5 Well-being Index. The Qualtrics ® platform was used to distribute the survey to all full-time faculty within the College of Health Sciences. 45 respondents from nursing, community and environmental health, kinesiology, social work, respiratory care, allied health sciences, and radiologic sciences completed the survey. Significant differences were observed in the extent of burnout and perception of well-being between faculty members who had clinical teaching responsibilities within their faculty role compared to those who did not, p = 0.005, Partial Eta Squared = 0.318. Faculty with a 9-month contract appointment had significantly lower OLBI-Disengagement scores (p = 0.024) and OLBI-Full Burnout scores (p = 0.047) compared with those with another contract length. There was a significantly negative relationship between the extent of burnout and perception of well-being. In this sample of health science faculty, burnout, as characterized by increased exhaustion and disengagement, was moderately prevalent and associated with poorer well-being

Blocking soluble Fas Ligand ameliorates pemphigus: PC111 efficacy in ex-vivo human pemphigus models

: Pemphigus is a life-threatening, chronic, autoimmune bullous disease affecting both the skin and the mucous membranes. Based on the mainstream concept that blister formation occurs upon binding of autoantibodies to their antigen proteins (desmoglein1, DSG1 and desmoglein3, DSG3), current therapies mostly aim to suppress the immune system. To avoid the severe side effects associated with the chronic use of immunosuppressive treatments, we have developed PC111, a fully human monoclonal antibody targeting human Fas ligand (FasL). We have provided a number of in vitro and in vivo evidences showing that soluble FasL induces keratinocyte apoptosis followed by acantholysis. An anti-murine FasL prevents blister formation in the pemphigus neonatal mouse model. To confirm the mechanism of action (MoA) and the efficacy of PC111 in a human pemphigus context, we used the keratinocyte dissociation assay and two independent Human Skin Organ Cultures (HSOC) pemphigus models. PC111 reduced acantholysis in vitro, as shown by the dose-dependent reduction of fragments in the monolayer cultures. In the first HSOC model, normal human skin was subcutaneously injected with a scFv antibody fragment directed against DSG1 and DSG3, resulting in a severe acantholysis (70-100%) after 24 hours. PC111 inhibited blister formation to around 50% of control. In the second model, normal human skin was injected with a mixture of pemphigus patients' autoantibodies resulting in a less severe acantholysis (20-30%). PC111 significantly suppressed blister formation to more than 75% up to 72 hours. These results confirm PC111 MoA and demonstrates the efficacy of the anti-FasL antibody also in a pemphigus setting

Micro-solid oxide fuel cells: status, challenges, and chances

    Get PDF
Abstract: Micro-solid oxide fuel cells (micro-SOFC) are predicted to be of high energy density and are potential power sources for portable electronic devices. A micro-SOFC system consists of a fuel cell comprising a positive electrode-electrolyte-negative electrode (i.e. PEN) element, a gas-processing unit, and a thermal system where processing is based on micro-electro-mechanical-systems fabrication techniques. A possible system approach is presented. The critical properties of the thin film materials used in the PEN membrane are discussed, and the unsolved subtasks related to micro-SOFC membrane development are pointed out. Such a micro-SOFC system approach seems feasible and offers a promising alternative to state-of-the-art batteries in portable electronics. Graphical abstract: Graphical Abstract tex
