    The authority of next-of-kin in explicit and presumed consent systems for deceased organ donation: an analysis of 54 nations

    Background. The degree of involvement by the next-of-kin in deceased organ procurement worldwide is unclear. We investigated the next-of-kin’s authority in the procure-ment process in nations with either explicit or presumed consent. Methods. We collected data from 54 nations, 25 with presumed consent and 29 with explicit consent. We char-acterized the authority of the next-of-kin in the decision to donate deceased organs. Specifically, we examined whether the next-of-kin’s consent to procure organs was always required and whether the next-of-kin were able to veto procurement when the deceased had expressed a wish to donate. Results. The next-of-kin are involved in the organ procure-ment process in most nations regardless of the consent principle and whether the wishes of the deceased to be a donor were expressed or unknown. Nineteen of the 25 nations with presumed consent provide a method for individuals to express a wish to be a donor. However, health professionals in only four of these nations responded that they do not override a deceased’s expressed wish because of a family’s objection. Similarly, health profes-sionals in only four of the 29 nations with explicit consent proceed with a deceased’s pre-existing wish to be a donor and do not require next-of-kin’s consent, but caveats still remain for when this is done. Conclusions. The next-of-kin have a considerable influ-ence on the organ procurement process in both presumed and explicit consent nations

    Making health information accessible for all: The impact of universal design in public libraries

    On a world basis, 15% of the population has a disability. Having a disability can result in a higher frequency of health-related information needs than other users might experience. The Web represents a widely used source for health information. People with disabilities, however, often encounter barriers during online searching, such as inaccessible information, poorly designed search user interfaces and lack of compatibility with assistive technology. Consequently, many users are potentially excluded from a range of information sources. Measures are therefore needed to remove these barriers to avoid health disparities that can result from unequal access to information. Public libraries have a social responsibility to include all user groups, and should aspire to make fully accessible services. A good tool in this context is the implementation of the universal design mind-set, where the purpose is to develop services that are available to all people. This chapter discusses how universal design can be a premise for equal access to health information and potentially reduce health disparities in the context of users with disabilities. Both library services and education of librarians will be addressed

More recently, e-scooters (or electric kickbikes), the focus of this chapter, have flooded cities in unprecedented speed and volume – and have caught virtually every city and competent authority off guard. The failure of current regulatory frameworks to address new challenges posed by e-scooters is explored. This chapter first briefly describes major developments of the shared e-scooter market. It then presents rationales for, and to some extent against, e-scooter regulation as well as policy tools available for e-scooter regulation. E-scooters open the door for new and innovative – and potentially efficient – ways to regulate, including geofencing, zoning, mandatory data sharing and mandatory cooperation.     Management of transboundary and straddling fish stocks in the Northeast Atlantic in view of climate-induced shifts in spatial distribution

    The introduction of 200 n.m. exclusive economic zones (EEZs) in the late 1970s required increased collaboration among neighbouring coastal states to manage transboundary and straddling fish stocks. The established agreements ranged from bilateral to multilateral, including high‐seas components, as appropriate. However, the 1982 United Nations Convention on the Law of the Sea does not specify how quotas of stocks crossing EEZs should be allocated, nor was it written for topical scenarios, such as climate change with poleward distribution shifts that differ across species. The productive Northeast Atlantic is a hot spot for such shifts, implying that scientific knowledge about zonal distribution is crucial in quota negotiations. This diverges from earlier, although still valid, agreements that were predominately based on political decisions or historical distribution of catches. The bilateral allocations for Barents Sea and North Sea cod remain robust after 40 years, but the management situation for widely distributed stocks, as Northeast Atlantic mackerel and Norwegian spring‐spawning herring, appears challenging, with no recent overall agreements. Contrarily, quotas of Northern hake are, so far, unilaterally set by the EU despite the stock's expansion beyond EU waters into the northern North Sea. Negotiations following the introduction of EEZs were undertaken at the end of the last cooler Atlantic Multidecadal Oscillation (AMO) period, that is, with stock distributions generally in a southerly mode. Hence, today's lack of management consensus for several widely distributed fish stocks typically relates to more northerly distributions attributed to the global anthropogenic signal accelerating the spatial effect of the current warmer AMO
