Transcatheter Mitral Valve-in-Valve Dislocation: A Rescue Strategy

In the transcatheter aortic valve replacement era, transapical valve-in-valve mitral valve implantation is considered an alternative treatment for high-risk patients with degenerated mitral bioprosthesis. We report an unusual strategy to remedy the potentially fatal complication of the mitral prosthesis migration into the aortic arch. The dislocated prosthesis was successfully stabilized in the aortic arch with a bare aortic stent, ensuring adequate perfusion of epiaortic vessels. \ua9 2018 The Society of Thoracic Surgeon

Ocorrência e caracterização de bactérias isoladas de nódulos de amendoinzeiro (Arachis hypogaea L.) em solos paranaenses, Brasil.

RESUMO: O objetivo deste trabalho foi avaliar a ocorrência e caracterizar morfofisiologicamente e geneticamente bactérias isoladas de nódulos de amendoinzeiro (Arachis hypogaea L.), em solos paranaenses, Brasil. Em amostras de 36 municípios representativos do estado Paraná foi observada nodulação do amendoim em 84,4%, independente se em áreas cultivadas, ou em florestas. Nos testes de caracterização morfofisiológica de 44 estirpes autenticadas, 75% das estirpes apresentaram taxa de crescimento rápido em meio de cultura com manitol; 25 acidificaram o meio, cinco alcalinizaram e 14 não resultaram em modificação do pH. A capacidade de produzir sideróforos foi observada em 43% das estirpes e 16% foram capazes de solubilizar fosfato de cálcio em meio de cultura. O sequenciamento do gene 16S rRNA mostrou a formação de três principais grupos, correspondentes a três filos: Alfaproteobacteria, Betaproteobacteria e Firmicutes. The Debrisoft ® monofilament debridement pad for use in acute or chronic wounds: A NICE medical technology guidance

As part of its Medical Technology Evaluation Programme, the National Institute for Health and Care Excellence (NICE) invited a manufacturer to provide clinical and economic evidence for the evaluation of the Debrisoft ® monofilament debridement pad for use in acute or chronic wounds. The University of Birmingham and Brunel University, acting as a consortium, was commissioned to act as an External Assessment Centre (EAC) for NICE, independently appraising the submission. This article is an overview of the original evidence submitted, the EAC’s findings and the final NICE guidance issued. The sponsor submitted a simple cost analysis to estimate the costs of using Debrisoft® to debride wounds compared with saline and gauze, hydrogel and larvae. Separate analyses were conducted for applications in home and applications in a clinic setting. The analysis took an UK National Health Service (NHS) perspective. It incorporated the costs of the technologies and supplementary technologies (such as dressings) and the costs of their application by a district nurse. The sponsor concluded that Debrisoft® was cost saving relative to the comparators. The EAC made amendments to the sponsor analysis to correct for errors and to reflect alternative assumptions. Debrisoft® remained cost saving in most analyses and savings ranged from £77 to £222 per patient compared with hydrogel, from £97 to £347 compared with saline and gauze, and from £180 to £484 compared with larvae depending on the assumptions included in the analysis and whether debridement took place in a home or clinic setting. All analyses were severely limited by the available data on effectiveness, in particular a lack of comparative studies and that the effectiveness data for the comparators came from studies reporting different clinical endpoints compared with Debrisoft®. The Medical Technologies Advisory Committee made a positive recommendation for adoption of Debrisoft® and this has been published as a NICE medical technology guidance (MTG17).The Birmingham and Brunel Consortium is funded by NICE to act as an External Assessment Centre for the Medical Technologies Evaluation Programme

Bayesian inference on compact binary inspiral gravitational radiation signals in interferometric data

Presented is a description of a Markov chain Monte Carlo (MCMC) parameter estimation routine for use with interferometric gravitational radiational data in searches for binary neutron star inspiral signals. Five parameters associated with the inspiral can be estimated, and summary statistics are produced. Advanced MCMC methods were implemented, including importance resampling and prior distributions based on detection probability, in order to increase the efficiency of the code. Do cover crops compete with young grapevines for fertilizer nitrogen?

Vineyard soils of the Campanha Gaúcha region of Rio Grande do Sul are sandy and have low to medium organic matter content, displaying low natural ability to supply nitrogen (N). Therefore, maintenance of cover crops is essential or the protection of the soil surface from the impact of raindrops and water erosion. The application of nitrogen fertilizers is also necessary. However, cover crops can absorb part of the nitrogen applied in the soil, decreasing the availability to young vines, which may slow the growth of root and shoot, and thus, the beginning of grape production

O milho, var. Piranao, foi cultivado em solucao nutritiva com niveis crescentes de N, P e K. Houve resposta linear a adicao de N e assintotica as doses de P e de K. A determinacao a atividade da reductase de nitrato se correlacionou melhor com a producao total nas folhas, por sua vez refletiu melhor o estado nutricional que a determinacao de putrescina nas folhas

URBAN CENTER. Una casa di vetro per le politiche urbane.

Nella cultura di governo della città, il termine "Urban Center" (o "Casa della città") designa una serie di strutture il cui denominatore comune risiede nello svolgimento di attività di servizio per le comunità urbane ai fini di soddisfare la crescente domenda di democrazia partecipativa e deliberativa nei processi di trasformazione degli insediamenti. Traendo spunto dalla storicizzazione del fenomeno e dal confronto tra i consolidati modelli statunitensi e le recenti esperienze in Italia, il volume si interroga sulla maturazione delle missioni dell' "Urban Center" nel passaggio da asettico spazio di informazione a luogo provilegiato per la costruzione trasparente di politiche urbane condivise. Il percorso logico del volume si sviluppa seguendo un fil rouge articolato in quattro parti. Il primo blocco si apre con due tematiche che costituiscono dialetticamente la cornice di riferimento entro cui può essere correttamente collocata la questione degli UC: l’urbanistica partecipata e il marketing urbano. Nella seconda parte attraverso lo studio di casi si ricostruisce il quadro delle articolate declinazioni statunitensi di Urban Center, consolidatesi in diversi decenni di storia. Sono strutture fortemente caratterizzate e autonome per stile, missioni, obiettivi, priorità, modalità operative, ma allo stesso tempo accomunate da un equilibrato mix di passione civile e pragmatismo professionale. Il terzo gruppo di saggi è dedicato alla condizione attuale e di prospettiva degli UC in Italia, delineando criticamente una sorta di “mappa dinamica” delle diverse strutture attivate e in divenire, caratterizzate per soggetti ispiratori, missioni “stili” e protagonismo degli attori coinvolti. Il cerchio delle riflessioni si chiude nella quarta parte discutendo la questione dell’innovazione di metodo per la costruzione di un UC sia attraverso la dimensione teoretica che le potenzialità operative.
