    Actinic cheilitis:proposal of a clinical index

    Actinic cheilitis is a potentially malignant lesion most commonly found in the lower lip of individuals with chronic exposure to ultraviolet radiation. The aim of this study was to develop and to test a clinical index that can be used to assess the severity of actinic cheilitis. The clinical index of actinic cheilitis was applied to 36 patients. An incisional biopsy was obtained to grade oral epithelial dysplasias following the World Health Organization (WHO) and binary systems, and to evaluate their association with clinical characteristics by Fisher?s exact test (P<0.05). The accuracy of the index was evaluated based on sensitivity, specificity, positive and negative predictive values, and receiver operating curve. The blurring between the border of the lip and the skin was significantly associated with cases without dysplasia/mild epithelial dysplasia (P=0.041) and with low risk of malignancy (P=0.005). Ulcers and crusts were significantly associated with moderate/severe epithelial dysplasia (P=0.002 and P=0.012, respectively) and high risk of malignancy (P=0.005 and P=0.045, respectively). Erosion showed a significant association only with high-risk cases of malignancy (P=0.024). The cut-off values of the diagnostic test showing the best performance were 10 for the WHO grading system and 11 for the binary system. The index cut-offs with the highest accuracy were considered indicators for a biopsy. Erosion, ulceration and crusts were associated with more severe oral epithelial dysplasias

    Clinicopathological study of oral focal mucinosis : a retrospective case series

    Oral focal mucinosis (OFM) is a rare soft tissue lesion of unknown etiology that exhibits tumor-like growth. It is considered the oral counterpart of cutaneous focal mucinosis or cutaneous myxoid cyst. This is a retrospective study of oral OFM diagnosed over a period of 42 years at an oral pathology service. Clinical, histopathological and immunohistochemical data were analyzed. Alcian blue staining and S-100 immunohistochemistry were performed. Eleven cases were retrieved (4:1 female-to-male ratio). The mean age was 44 years. The gingiva was the most affected site. The main clinical presentation was sessile or pedunculated lesions of fibrous or hyperplasic appearance, most of them asymptomatic. Positive Alcian blue staining and absence of S-100 protein were observed in all specimens, which supported the histological diagnosis of OFM. Surgical excision was the treatment of choice. Although rare, this study supports the inclusion of OFM in the differential diagnosis of intraoral myxoid lesions

    Leucemia Linfoblástica Aguda (LLA) na população pediátrica: marcadores moleculares e implicações terapêuticas

    A Leucemia Linfoblástica Aguda (LLA) é uma forma comum de câncer pediátrico, representando cerca de 80% dos casos de leucemia em crianças. A patologia é caracterizada pela proliferação descontrolada de células-tronco hematopoéticas na medula óssea, e avanços recentes na pesquisa genômica têm proporcionado uma compreensão mais profunda da complexidade molecular subjacente à doença. O presente estudo tem como objetivo oferecer uma visão abrangente dos principais marcadores moleculares e implicações terapêuticas associadas à LLA na população pediátrica. Este estudo, baseado em uma revisão sistemática da literatura científica, abrange o período de 2013 a 2023, utilizando as bases de dados PubMed (Medline), Cochrane Library e Scientific Electronic Library Online (SciELO). Marcadores moleculares preponderantes, como rearranjos cromossômicos específicos (t(12;21), t(1;19), t(9;22)), mutações genéticas distintivas (ETV6-RUNX1, E2A-PBX1, TP53) e amplificação do gene BCR-ABL1, têm sido objeto de estudo aprofundado. Esses marcadores desempenham um papel crucial na estratificação de risco e prognóstico, permitindo uma abordagem mais personalizada no tratamento da LLA em crianças. As implicações terapêuticas derivadas desses marcadores são vastas, destacando a promissora era das terapias direcionadas. Terapias específicas para mutações, como aquelas direcionadas à mutação BCR-ABL1, e inovações em imunoterapia estão moldando o cenário do tratamento da LLA, proporcionando resultados mais eficazes e menos tóxicos. Os resultados destacam a eficácia das terapias direcionadas e a necessidade contínua de pesquisa para otimizar a intervenção terapêutica, melhorar a qualidade de vida dos pacientes pediátricos afetados pela LLA e explorar novas facetas do tratamento. Em conclusão, este artigo fornece uma análise aprofundada dos marcadores moleculares e terapias associadas à LLA na população pediátrica, destacando avanços significativos e delineando áreas para investigação futura

    Multiple Second Primary Oral Squamous Cell Carcinomas in a Nonsmoker and Nondrinker Woman: Case Report and Review of the Literature

    AIM: This manuscript aims to describe an unusual case of multiple second primary squamous cell carcinomas (SCCs) in several sites of the oral mucosa in a nonsmoker and nondrinker woman and to discuss the diagnostic criteria, clinicopathological aspects and outcome of second primary tumor (SPT). BACKGROUND: Patients treated for SCC of the head and neck are at high risk for developing SPT arising from the same dysplastic mucosal feld. Currently, there is no reliable method to predict which of the patients will develop SPT. CASE DESCRIPTION: A 64-year-old nonsmoker and nondrinker woman developed several second primary oral SCCs in 7 years of follow-up, most of them being synchronic, treated by surgery without and with chemotherapy and radiotherapy. CONCLUSION: Patients treated for SCC require a long-term and careful follow-up as the development of SPT contributes with significantly negative impact on the prognosis. CLINICAL SIGNIFICANCE: This report describes the diagnosis and management of a very unusual case of several SPTs affecting different sites of the oral mucosa in the same patient. Moreover, the patient had no apparent risk factors associated with the development of the oral cancer. Therefore, a brief update concerning SPT and its diagnosis and management is also provided
