    Routine piloting in systematic reviews--a modified approach?

Please cite the published version which is available via the DOI link in this record.BACKGROUND: A continuous growth in the publication of research papers means that there is an expanding volume of data available to the systematic reviewer. Sometimes, researchers can become overwhelmed by the sheer volume of data being processed, leading to inefficient data extraction. This paper seeks to address this problem by proposing a modification to the current systematic review methodology. PROPOSED METHOD: This paper details the routine piloting of a systematic review all the way through to evidence-synthesis stage using data from a sample of included papers. RESULTS AND DISCUSSION: The result of piloting a sample of papers through to evidence-synthesis stage is to produce a 'mini systematic review'. Insights from such a pilot review may be used to modify the criteria in the data extraction form. It is proposed that this approach will ensure that in the full review the most useful and relevant information is extracted from all the papers in one phase without needing to re-visit the individual papers at a later stage. CONCLUSIONS: Routine piloting in systematic reviews has been developed in response to advances in information technology and the subsequent increase in rapid access to clinical papers and data. It is proposed that the routine piloting of large systematic reviews will enable themes and meaning in the data to become apparent early in the review process. This, in turn, will facilitate the efficient extraction of data from all the papers in the full review. It is proposed that this approach will result in increased validity of the review, with potential benefits for increasing efficiency

    A Comparison of Sex-Role Attitudes of Hearing and Deaf Young Men and Women

    Legislation in recent years has provided unparalleled opportunities for the vocational development of deaf persons. Previous studies conducted in the early 1980\u27s indicated that deaf men and women hold much more traditional views of their sex roles than their hearing peers. The purpose of this study was to examine whether deaf adolescents still hold significantly more traditional sex-role attitudes than their hearing peers. A 25-item survey scale was administered to hearing and deaf high school students replicating studies conducted by Anderson and Krueger (1982) and Kolvitz and Ouellette (1980). Results indicated that deaf young men and women are still significantly more conservative than their hearing peers, with deaf males tending towards more liberal attitudes than in previous studies. Discussion is also offered regarding the implications of these findings for career awareness and vocational opportunities of young deaf adults

    A systematic review of speech recognition technology in health care

    BACKGROUND To undertake a systematic review of existing literature relating to speech recognition technology and its application within health care. METHODS A systematic review of existing literature from 2000 was undertaken. Inclusion criteria were: all papers that referred to speech recognition (SR) in health care settings, used by health professionals (allied health, medicine, nursing, technical or support staff), with an evaluation or patient or staff outcomes. Experimental and non-experimental designs were considered. Six databases (Ebscohost including CINAHL, EMBASE, MEDLINE including the Cochrane Database of Systematic Reviews, OVID Technologies, PreMED-LINE, PsycINFO) were searched by a qualified health librarian trained in systematic review searches initially capturing 1,730 references. Fourteen studies met the inclusion criteria and were retained. RESULTS The heterogeneity of the studies made comparative analysis and synthesis of the data challenging resulting in a narrative presentation of the results. SR, although not as accurate as human transcription, does deliver reduced turnaround times for reporting and cost-effective reporting, although equivocal evidence of improved workflow processes. CONCLUSIONS SR systems have substantial benefits and should be considered in light of the cost and selection of the SR system, training requirements, length of the transcription task, potential use of macros and templates, the presence of accented voices or experienced and in-experienced typists, and workflow patterns.Funding for this study was provided by the University of Western Sydney.     Community Literacy: A Rhetorical Model for Personal and Public Inquiry

    This paper develops a rhetorically centered model of community literacy in the theoretical and practical context of local publics—those spaces where ordinary people develop public voices to engage in intercultural inquiry and deliberation. Drawing on fifteen years of action research in the Community Literacy Center and beyond, the authors characterize the distinctive features of local publics, the deliberative, intercultural discourses they circulate, and the literate practices that sustain them. They identify four critical practices at the heart of community literacy: assessing the rhetorical situation, creating local publics, developing citizens’ rhetorical capacities, and supporting change through the circulation of alternative texts and practices

    Exercise-based cardiac rehabilitation for patients following open surgical aortic valve replacement and transcatheter aortic valve implant: a systematic review and meta-analysis

    Objectives: Exercise-based cardiac rehabilitation (CR) may be beneficial to patients following transcatheter aortic valve implantation (TAVI) and open surgical aortic valve replacement (SAVR). We aimed to undertake a systematic review and meta-analysis to evaluate the efficacy, safety and costs of exercise-based CR post-TAVI and post-SAVR. Methods: We searched numerous databases, including Embase, CENTRAL and MEDLINE, up to October 2017. We included randomised controlled trials (RCTs) and non-randomised controlled trials (non-RCTs) of exercise-based CR compared with no exercise control in TAVI or SAVR patients ≥18 years. Data extraction and risk of bias assessments were performed independently by two reviewers. Narrative synthesis and meta-analysis (where appropriate) were carried out for all relevant outcomes, and a Grading of Recommendations Assessment, Development and Evaluation (GRADE) analysis was also performed. Results: Six studies, all at low risk of bias, were included: three RCTs and three non-RCTs (total of 27 TAVI, 99 SAVR and 129 mixed patients), with follow-up of 2–12 months. There was an increase in pooled exercise capacity (standardised mean difference: 0.41, 95% CI 0.11 to 0.70; moderate certainty evidence as assessed by GRADE), with exercise-based rehabilitation compared with control. Data on other outcomes including quality of life and clinical events were limited. Conclusions: Exercise-based CR probably improves exercise capacity of post-TAVI and post-SAVR patients in the short term. Well conducted multicentre fully powered RCTs of ≥12 months follow-up are needed to fully assess the clinical and cost-effectiveness of exercise-based CR in this patient population.     Experimental Line Parameters of the b^(1)Σ^(+)_g ← X^(3)Σ^(-)_g Band of Oxygen Isotopologues at 760 nm Using Frequency-Stabilized Cavity Ring-Down Spectroscopy

    Positions, intensities, self-broadened widths, and collisional narrowing coefficients of the oxygen isotopologues ^(16)O^(18)O, ^(16)O^(17)O, ^(17)O^(18)O, and ^(18)O^(18)O have been measured for the b^(1)Σg + ← X^(3)Σg − (0,0) band using frequency-stabilized cavity ring-down spectroscopy. Line positions of 156 P-branch transitions were referenced against the hyperfine components of the ^(39)K D_1 (4s ^(2)S_(1/2) → 4p ^(2)P_(1/2)) and D_2 (4s ^(2)S_(1/2) → 4p ^(2)P_(3/2)) transitions, yielding precisions of ~0.00005 cm^(−1) and absolute accuracies of 0.00030 cm^(−1) or better. New excited b^(1)Σg + state molecular constants are reported for all four isotopologues. The measured line intensities of the ^(16)O^(18)O isotopologue are within 2% of the values currently assumed in molecular databases. However, the line intensities of the ^(16)O^(17)O isotopologue show a systematic, J-dependent offset between our results and the databases. Self-broadening half-widths for the various isotopologues are internally consistent to within 2%. This is the first comprehensive study of the line intensities and shapes for the ^(17)O^(18)O or ^(18)O_2 isotopologues of the b^(1)Σg + ← X^(3)Σg − (0,0) band of O_2. The ^(16)O_2, ^(16)O^(18)O, and ^(16)O^(17)O line parameters for the oxygen A-band have been extensively revised in the HITRAN 2008 database using results from the present study

    Modulation of Telomeres in Alternative Lengthening of Telomeres Type I Like Human Cells by the Expression of Werner Protein and Telomerase

    The alternative lengthening of telomeres (ALT) is a recombination-based mechanism of telomere maintenance activated in 5–20% of human cancers. In Saccharomyces cerevisiae, survivors that arise after inactivation of telomerase can be classified as type I or type II ALT. In type I, telomeres have a tandem array structure, with each subunit consisting of a subtelomeric Y′ element and short telomere sequence. Telomeres in type II have only long telomere repeats and require Sgs1, the S. cerevisiae RecQ family helicase. We previously described the first human ALT cell line, AG11395, that has a telomere structure similar to type I ALT yeast cells. This cell line lacks the activity of the Werner syndrome protein, a human RecQ helicase. The telomeres in this cell line consist of tandem repeats containing SV40 DNA, including the origin of replication, and telomere sequence. We investigated the role of the SV40 origin of replication and the effects of Werner protein and telomerase on telomere structure and maintenance in AG11395 cells. We report that the expression of Werner protein facilitates the transition in human cells of ALT type I like telomeres to type II like telomeres in some aspects. These findings have implications for the diagnosis and treatment of cancer
