    The impact of maternal HIV infection on cord blood lymphocyte subsets and cytokine profile in exposed non-infected newborns

    <p>Abstract</p> <p>Background</p> <p>Children born to HIV+ mothers are exposed intra-utero to several drugs and cytokines that can modify the developing immune system, and influence the newborn's immune response to infections and vaccines. We analyzed the relation between the distribution of cord blood lymphocyte subsets and cytokine profile in term newborns of HIV+ mothers using HAART during pregnancy and compared them to normal newborns.</p> <p>Methods</p> <p>In a prospective, controlled study, 36 mother-child pairs from HIV+ mothers and 15 HIV-uninfected mothers were studied. Hematological features and cytokine profiles of mothers at 35 weeks of pregnancy were examined. Maternal and cord lymphocyte subsets as well as B-cell maturation in cord blood were analyzed by flow cytometry. The non-stimulated, as well as BCG- and PHA-stimulated production of IL2, IL4, IL7, IL10, IL12, IFN-γ and TNF-alpha in mononuclear cell cultures from mothers and infants were quantified using ELISA.</p> <p>Results</p> <p>After one year follow-up none of the exposed infants became seropositive for HIV. An increase in B lymphocytes, especially the CD19/CD5+ ones, was observed in cord blood of HIV-exposed newborns. Children of HIV+ hard drug using mothers had also an increase of immature B-cells. Cord blood mononuclear cells of HIV-exposed newborns produced less IL-4 and IL-7 and more IL-10 and IFN-γ in culture than those of uninfected mothers. Cytokine values in supernatants were similar in infants and their mothers except for IFN-γ and TNF-alpha that were higher in HIV+ mothers, especially in drug abusing ones. Cord blood CD19/CD5+ lymphocytes showed a positive correlation with cord IL-7 and IL-10. A higher maternal age and smoking was associated with a decrease of cord blood CD4+ cells.</p> <p>Conclusions</p> <p>in uninfected infants born to HIV+ women, several immunological abnormalities were found, related to the residual maternal immune changes induced by the HIV infection and those associated with antiretroviral treatment. Maternal smoking was associated to changes in cord CD3/CD4 lymphocytes and maternal hard drug abuse was associated with more pronounced changes in the cord B cell line.</p

    Reactions of pulmonary emphysema patients before and after VRS - Forcusing on the rehabilitation, activity, surgery -

    今まで内科的治療が主体であった肺気腫に対して,近年外科的治療(Volume Reduction Surgery 以後VRSと略す)が行われるようになった。VRSを受ける患者の看護では,手術前後において, リハビリテーション(以後リハビリと略す)や機能回復に向けてのケアが必要不可欠とされる。そこで,本研究は肺気腫患者の適切な看護援助を検討するために,手術前後に示す肺気腫患者のリハビリや活動に対する反応を明らかにすることを目的とした。対象者は当病棟に入院している肺気腫患者8名(VRS手術前8名,手術後はそのうちの6名である)で, リハビリ, 日常生活活動,呼吸,手術などについて,面接ならびに観察による調査を行った。分析の結果,手術前においてリハビリでは≪義務感≫,頑張って行えば呼吸が楽になるという≪期待感≫,≪サポート≫,≪不安≫が,活動では≪活動の制限≫,手術では呼吸が楽になるのではという≪期待感≫,≪おまかせ≫,≪いちかばちかの賭け≫,≪不安≫,≪回復に向けての欲求の高まり≫が明らかになった。手術後においてリハビリでは≪呼吸が楽になるための手段≫が, 日常生活活動では≪今の状態よりは良くなると いう期待感≫が,手術では≪達成感≫ と≪身体的苦痛≫が,将来については≪ささやかな欲求≫が明らかになった。Pulmonary emphysema has been mainly treated with medicine, but recently VRS(volume reduction surgery) has been receiving much attention. In nursing care for VRS patients, it is important to help the patient improve his/her daily activities and recover physical function smoothly after surgery. The purpose of this study is to clarify the reactions of pulmonary emphysema patients before and after VRS for appropriate nursing. The subjects were eight pulmonary emphysema patients at a ward in Okayama University Hospital, but six of them completed both before and after VRS data collection. The data were collected by interview and observation, and from medical and nursing records. Contents of the interview included rehabilitation, daily activities, respiratory symptoms, operative stress, and so on.     Tabaco na mídia: análise de matérias jornalísticas no ano de 2006 Tobacco in the media: analysis of journalistic texts in the year of 2006

    Visando compreender a relação entre saúde, imprensa e políticas públicas sobre tabaco no Brasil, foram analisadas matérias sobre tabaco divulgadas na imprensa brasileira no ano de 2006. Através de clipping jornalístico de oito principais jornais e revistas do país, as matérias sobre tabaco foram identificadas e posteriormente submetidas à análise de conteúdo, que permitiu categorização e classificação dos textos. A frequência de matérias de 2006 foi comparada aos anos de 2000 e 2003. Foi observado predomínio de matérias com abordagem factual (46,7%). A maioria das matérias fez menção a consequências negativas, como problemas de saúde física (44,2%), morte (20%) e dependência (14,2%). Na análise das manchetes e lides, as principais categorias observadas foram políticas de controle e o movimentos antitabagistas e divulgação de pesquisas. A frequência de matérias de 2006 (N=120) foi semelhante ao ano de 2003 (N=124) e inferior a 2000 (N=174). A cobertura jornalística sobre tabaco em 2006 foi predominantemente restrita aos danos a saúde e ações antitabagistas. A elevada proporção de abordagem factual e a estabilização da frequência de matérias (2003-2006) podem indicar um empobrecimento na discussão sobre o tema no país.<br>Aiming at understanding the relation among health, press and public policies on Tobacco in Brazil, this article analyses the texts about Tobacco published in the Brazilian press in 2006. In the clipping process of eight newspapers and magazines, the information about Tobacco were identified and then submitted to content analysis allowing categorization and classification of the texts. The frequency of the texts in 2006 was compared to that of in 2000 and 2003. We observed a higher prevalence of factual approach among the texts (46.7%). Most of the texts mentioned the negative consequences, such as physical health problems (44.2%), death (20%) and dependence (14.2%). The analysis of the headlines and lead-ins showed control policies, anti-smoking movements and spreading of results as the main categories observed. The frequency of the articles in 2006 was similar to that of in 2003 and lower to that of in 2000. The journalistic coverage on Tobacco in 2006 was restricted predominantly to harm to health and anti-smoking movements. The high proportion of the factual approach and the stabilization in the frequency of texts (2003-2006) might suggest an impoverishment of the discussion on this issue in the country

    Fitness, Diet and Coronary Risk Factors in a Sample of Southeastern U.S. Children.

    The purpose of this study was to evaluate the relationship between physical fitness variables and nutrient intake to coronary risk factors (CRF) in a sample of children living in the Southeastern U.S. A total of 22 sixth-grade children of whom 10 were boys (mean age = 11.83 ± 0.3) and 12 were girls (mean age 11.7 ± 0.3) volunteered for this study. Results indicated that boys in comparison to girls weighed more (54.0 ± 10.8 kg versus 42.1 ± 8.0 kg; p<0.05), had a higher body mass index (BMI) (23.6 ± 2.7versus 20.2 ± 3.3; p<0.05), a higher lean body mass (37.8 ± 6.0 kg versus 30.7 ± 3.8 kg; p<0.01), and a higher systolic blood pressure (115.7 ± 11.1 versus 106.4 ± 8.1; p<.0001). There were, however, no significant gender differences in serum lipoproteins or nutrient intake. Stepwise multiple regression analyses indicated that physical fitness variables which included VO2max, one-mile run for time, grip strength, and leg strength could significantly predict resting diastolic blood pressure (DBP) (F=3.06; p<0.05) and percent body fat (F=4.98; p<0.01) in children. Analysis of food intake revealed that total and saturated fat, and carbohydrate intake could predict serum triglycerides (TG) (F=5.18; p=0.01) while total kilocalorie, fat, and carbohydrate intake could significantly predict percent body fat (F=3.42; p<0.03). These findings may be clinically relevant since both serum triglyceride levels and percent body fat were well above the 50th percentile according to U.S. norms. In summary, the present study showed that measurements of muscular strength in addition to aerobic fitness are associated with DBP and percent body fat in children. Furthermore, it is recommended that nutrient intake be used when evaluating CRF in children due to its ability to predict TG and percent body fat
