    A low-profile cutter assembly for use on a microscope and related cutting method are provided. The cutter assembly includes a support subassembly having a mounting ring for receiving and engaging the objective of a microscope. A cutter subassembly carried by the support subassembly is displaceable between a home and a forward position in response to actuation. Upon reaching the forward position, additional actuation causes the front end of the cutter subassembly to pivot toward the stage, thereby placing a cutter adjacent to or in contact with the specimen. By selectively actuating the cutter subassembly, multiple cuts may be performed, as necessary or desired for cutting a spore or isolating a particular specimen

    Monitoring sound levels in the intensive care unit: A mixed-methods system development project to optimize design features for a new electronic interface in the healthcare environment.

    BACKGROUND: Intensive care units (ICU) are busy round the clock and it is difficult to maintain low sound levels that support patient rest. To help ICU staff manage activities we developed a visual display that monitors and reports sound levels in real-time. This facilitates immediate feedback, encouraging proactive behavior change to limit disturbances. METHODS: Following the principles of user-centered design practices we created our 'user persona' to understand the needs and goals of potential users of the system. We then conducted iterative user testing with current members of the ICU team, primarily using the 'think aloud' method to refine the design and functionality of our novel system. Ethnography evaluated team use of the display. RESULTS: The final design was simple, clear, and efficient, and both functional and aesthetically pleasing for the key user demographic. We identified challenges in the implementation and adoption process that were separate from the 'usability' of the system itself. CONCLUSIONS: Embedding the design process within the core user demographic ensured the final product delivered relevant information for key users, and that this information was intuitive to interpret. Initiating sustainable change is not straightforward. It requires recognition of cultural practices within teams, departments, professions, organizations, and strategies to maximize engagement

    The Cool ISM in Elliptical Galaxies. II. Gas Content in the Volume - Limited Sample and Results from the Combined Elliptical and Lenticular Surveys

    We report new observations of atomic and molecular gas in a volume limited sample of elliptical galaxies. Combining the elliptical sample with an earlier and similar lenticular one, we show that cool gas detection rates are very similar among low luminosity E and SO galaxies but are much higher among luminous S0s. Using the combined sample we revisit the correlation between cool gas mass and blue luminosity which emerged from our lenticular survey, finding strong support for previous claims that the molecular gas in ellipticals and lenticulars has different origins. Unexpectedly, however, and contrary to earlier claims, the same is not true for atomic gas. We speculate that both the AGN feedback and merger paradigms might offer explanations for differences in detection rates, and might also point towards an understanding of why the two gas phases could follow different evolutionary paths in Es and S0s. Finally we present a new and puzzling discovery concerning the global mix of atomic and molecular gas in early type galaxies. Atomic gas comprises a greater fraction of the cool ISM in more gas rich galaxies, a trend which can be plausibly explained. The puzzle is that galaxies tend to cluster around molecular-to-atomic gas mass ratios near either 0.05 or 0.5.Comment: 37 pages, including 4 tables and 12 figures.     Increased risk for other cancers in individuals with Ewing sarcoma and their relatives.

    BackgroundThere are few reports of the association of other cancers with Ewing sarcoma in patients and their relatives. We use a resource combining statewide genealogy and cancer reporting to provide unbiased risks.MethodsUsing a combined genealogy of 2.3 million Utah individuals and the Utah Cancer Registry (UCR), relative risks (RRs) for cancers of other sites were estimated in 143 Ewing sarcoma patients using a Cox proportional hazards model with matched controls; however, risks in relatives were estimated using internal cohort-specific cancer rates in first-, second-, and third-degree relatives.ResultsCancers of three sites (breast, brain, complex genotype/karyotype sarcoma) were observed in excess in Ewing sarcoma patients. No Ewing sarcoma patients were identified among first-, second-, or third-degree relatives of Ewing sarcoma patients. Significantly increased risk for brain, lung/bronchus, female genital, and prostate cancer was observed in first-degree relatives. Significantly increased risks were observed in second-degree relatives for breast cancer, nonmelanoma eye cancer, malignant peripheral nerve sheath cancer, non-Hodgkin lymphoma, and translocation sarcomas. Significantly increased risks for stomach cancer, prostate cancer, and acute lymphocytic leukemia were observed in third-degree relatives.ConclusionsThis analysis of risk for cancer among Ewing sarcoma patients and their relatives indicates evidence for some increased cancer predisposition in this population which can be used to individualize consideration of potential treatment of patients and screening of patients and relatives

    Monitoring of Tumor Promotion and Progression in a Mouse Model of Inflammation-Induced Colon Cancer with Magnetic Resonance Colonography

    AbstractEarly detection of precancerous tissue has significantly improved survival of most cancers including colorectal cancer (CRC). Animal models designed to study the early stages of cancer are valuable for identifying molecular events and response indicators that correlate with the onset of disease. The goal of this work was to investigate magnetic resonance (MR) colonography in a mouse model of CRC on a clinical MR imager. Mice treated with azoxymethane and dextran sulfate sodium were imaged by serial MR colonography (MRC) from initiation to euthanasia. Magnetic resonance colonography was obtained with both T1- and T2-weighted images after administration of a Fluorinert enema to remove residual luminal signal and intravenous contrast to enhance the colon wall. Individual tumor volumes were calculated and validated ex vivo. The Fluorinert enema provided a clear differentiation of the lumen of the colon from the mucosal lining. Inflammation was detected 3 days after dextran sulfate sodium exposure and subsided during the next week. Tumors as small as 1.2 mm3 were detected and as early as 29 days after initiation. Individual tumor growths were followed over time, and tumor volumes were measured by MR imaging correlated with volumes measured ex vivo. The use of a Fluorinert enema during MRC in mice is critical for differentiating mural processes from intraluminal debris. Magnetic resonance colonography with Fluorinert enema and intravenous contrast enhancement will be useful in the study of the initial stages of colon cancer and will reduce the number of animals needed for preclinical trials of prevention or intervention

    Radio Continuum and Star Formation in CO-rich Early Type Galaxies

    In this paper we present new high resolution VLA 1.4 GHz radio continuum observations of five FIR bright CO-rich early-type galaxies and two dwarf early-type galaxies. The position on the radio-FIR correlation combined with striking agreements in morphology between high resolution CO and radio maps show that the radio continuum is associated with star formation in at least four of the eight galaxies. The average star formation rate for the sample galaxies detected in radio is approximately 2 solar masses per year. There is no evidence of a luminous AGN in any of our sample galaxies. We estimate Toomre Q values and find that the gas disks may well be gravitationally unstable, consistent with the above evidence for star formation activity. The radio continuum emission thus corroborates other recent suggestions that star formation in early type galaxies may not be uncommon.Comment: 21 pages, 7 figures, to be published in the Astronomical Journa

    Comparison of a Quick Drinking Screen with the Timeline Followback for Individuals with Alcohol Problems

    Objective: Two major strategies have typically been used to assess recent drinking: (1) Daily Estimation (DE) measures such as the Timeline Followback (TLFB) and (2) Quantity-Frequency (QF) summary measures. Although QF measures provide a quick and easy measure of consumption, they have been criticized as not being able to capture sporadic and unpatterned drinking (e.g., days that reflect important social and/or health risks). The TLFB, a psychometrically sound drinking assessment method, is able to capture all drinking, including sporadic heavy days and unpatterned drinking. In some situations, however, recall of daily drinking may not be possible or practical (e.g., limited time; no resources). This article compares results obtained by using a QF measure and a DE measure to assess problem drinkers’ pretreatment drinking. Method: The current study, part of a large community mail intervention with 825 alcohol abusers, compared results from two drinking measures covering the same time interval that were administered on two different occasions approximately 2.5 weeks apart. Both measures, the Quick Drinking Screen (QDS; a QF summary measure that collected data by telephone) and the TLFB (a self-administered daily estimation measure), collected drinking data for the year prior to the interview. Results: Although the QDS and the TLFB are very different drinking measures, remarkably similar aggregate drinking data were obtained for five drinking variables. Conclusions: When it is not necessary or possible to gather detailed drinking data, the QDS produces reliable brief summary measures of drinking, at least for not severely alcohol dependent individuals. Also, respondents do not appear to use a repetitive response pattern when completing the TLFB

    A reaction pairing strategy centered on utilization of a reaction triad (sulfonylation, SNAr addition and Mitsunobu alkylation) generating skeletally diverse benzofused tricyclic and bicyclic sultams is reported. Pairing sulfonylation and SNAr reactions yields bridged, tricyclic and bicyclic benzofused sultams. Application of the Mitsunobu reaction in a sulfonylation–Mitsunobu–SNAr pairing allows access to benzo-oxathiazocine-1,1-dioxides, while a simple change in the order of pairing to sulfonylation–SNAr–Mitsunobu affords structurally different, benzofused bridged tricyclic sultams
