    Nursing at University College Hospital, London, 1862-1948 : from Christian vocation to secular profession

    This thesis examines the development of nursing at University College Hospital, London,\ud between 1862, the year in which the All Saints Sisters assumed responsibility for the\ud provision of nursing services at the hospital, and the introduction of the National Health\ud Service in July 1948. Although the care provided by the sisters marked a considerable\ud improvement on what had gone before, in 1899 they were replaced by nurses whose\ud motivation was professional rather than vocational. The profession of nursing was\ud confirmed by the Registration Act of 1919.\ud Following an introduction, the initial chapters of the thesis are concerned with defining\ud and developing the themes of Christian vocation and secular profession. Chapter four is\ud devoted to nursing management across the period, and the following chapter to patterns\ud of care. Prior to 1919 a system of primary care was in operation; this was superseded by\ud task allocation.\ud Chapters six and seven outline the introduction of nurse training in some of the London\ud teaching hospitals in comparison to the training of nurses at U.C.H. An analysis of the\ud probationer records from 1890 - 1948 demonstrates recruitment and retention through\ud these years. With the departure of the All Saints Sisters from U.C.H. in 1899 the school\ud of nursing was established. A preliminary training school was introduced in 1926; ten\ud years later University College Hospital school of nursing pioneered the block system of\ud training which became the norm for all schools of nursing after the Second World War.\ud The next two chapters concentrate upon nursing developments in wartime. The All\ud Saints Sisters were part of the British Red Cross team that served in the Franco-Prussian\ud War of 1870 and the hospital and its nurses were fully involved in the two World Wars.\ud Although this period saw the transformation of nursing at U.C.H. from a Christian\ud vocation to a secular profession, this thesis is as much concerned with continuity as with\ud change - for example in noting the similarities between rules for the probationers and for\ud the novitiates, which had continuing influence throughout the years of this study.\u

    The metformin in tuberous sclerosis (MiTS) study: A randomised double-blind placebo-controlled trial

    Background: Tuberous Sclerosis Complex (TSC) is a genetic disorder characterised by the development of benign tumours secondary to loss of inhibitory regulation of the mTOR (mechanistic Target of Rapamycin) intracellular growth pathway. Metformin inhibits the mTOR pathway. We investigated whether metformin would reduce growth of hamartomas associated with tuberous sclerosis complex. / Methods: In this multicentre randomized, double-blind, placebo-controlled trial, patients with a clinical diagnosis of tuberous sclerosis, aged over 10 years and with at least one renal angiomyolipoma of greater than 1 cm in diameter were enrolled. Participants were randomly allocated (1:1) by a secure website to receive metformin or placebo for 12 months. The primary outcome was percentage volume change of renal angiomyolipomas (AML) at 12 months compared to baseline. Secondary outcomes were percentage change at 12 months from baseline in volume of cerebral Subependymal Giant Cell Astrocytomas (SEGA); appearance of facial and ungual hamartomas; frequency of epileptic seizures; and adaptive behaviour. The trial is registered with The International Standard Randomised Controlled Trial Number (ISRCTN), number 92545532, and the European Union Drug Regulating Authorities Clinical Trials (EUDRACT), number 2011-001319-30. / Findings: Between 1 November 2012 and 30 September 2015 72 patients were screened and 55 were randomly assigned to metformin (28) or placebo (27). Four participants withdrew between randomisation and starting treatment. All 51 patients who started therapy completed the trial and were assessed for outcome at 12 months. The median percentage change in angiomyolipoma (AML) volume was +7.6% (IQR -1.8% to +42.6%) for the placebo group and +8.9% (IQR 1.3% to 19.5%) for the metformin group (p = 0.28). Twenty-seven patients had SEGAs: 13 received placebo and 14 metformin. The median percentage change in SEGA volume was +3.0% (IQR -22.8% to +27.7%) for the placebo group and – 20.8% (IQR – 47.1% to - 5.0%) for the metformin group (p = 0.03). Twenty-one patients were assessed for seizure frequency: 9 received placebo and 12 received metformin. In the metformin group, a mean reduction of 43.7% from baseline in seizures was observed and in the placebo group a 3.1% mean reduction was observed, with a difference in response of 40.6% (95% CI -3.1% to +84.2%, p = 0.03). There were no significant differences between metformin and placebo groups for the other secondary outcomes. There were no deaths. Three serious adverse events (SAEs) occurred during the trial (all patients on metformin). / Interpretation: Metformin did not reduce AML volume. Metformin did reduce SEGA volume and seizure frequency compared with placebo. There may be a role for metformin in slowing or reversing growth of some life-threatening hamartomas in TSC and for reducing seizure frequency.     Objective: To assess whether high levels of cerebrospinal fluid neurogranin are found in atypical as well as typical Alzheimer's disease. Methods: Immunoassays were used to measure cerebrospinal fluid neurogranin in 114 participants including healthy controls (n = 27), biomarker-proven amnestic Alzheimer's disease (n = 68), and the atypical visual variant of Alzheimer's (n = 19) according to international criteria. CSF total-tau, Aβ42, and neurofilament light concentrations were investigated using commercially available assays. All affected individuals had T1-weighted volumetric MR images available for analysis of whole and regional brain volumes. Associations between neurogranin, brain volumes, total-tau, Aβ42, and neurofilament light were assessed. Results: Median cerebrospinal fluid neurogranin concentrations were higher in typical and atypical Alzheimer's compared to controls (P < 0.001 and P = 0.005). Both neurogranin and total-tau concentrations, but not neurofilament light and Aβ42, were higher in typical Alzheimer's compared to atypical patients (P = 0.004 and P = 0.03). There were significant differences in the left hippocampus and right and left superior parietal lobules in atypical patients, which were larger (P = 0.03) and smaller (P = 0.001 and P < 0.001), respectively, compared to typical patients. We found no evidence of associations between neurogranin and brain volumes but a strong association with total-tau (P < 0.001) and a weaker association with neurofilament light (P = 0.005). Interpretation: These results show significant differences in neurogranin and total-tau between typical and atypical patients, which may relate to factors other than disease topography. The differential relationships between neurogranin, total-tau and neurofilament light in the Alzheimer's variants, provide evidence for mechanistically distinct and coupled markers of neurodegeneration
