    Prognostic Outcomes and Risk Factors for Patients with Renal Cell Carcinoma and Venous Tumor Thrombus after Radical Nephrectomy and Thrombectomy: The Prognostic Significance of Venous Tumor Thrombus Level.

    IntroductionTo evaluate the prognostic outcomes and risk factors for renal cell carcinoma (RCC) patients with venous tumor thrombus in China.Materials and methodsWe reviewed the clinical information of 169 patients who underwent radical nephrectomy and thrombectomy. Overall and cancer-specific survival rates were analyzed. Univariate and multivariate analyses were used to investigate the potential prognostic factors.ResultsThe median survival time was 63 months. The five-year overall survival and cancer-specific survival rate were 53.6% and 54.4% for all patients. For all patients, significant survival difference was only observed between early (below hepatic vein) and advanced (above hepatic vein) tumor thrombus. However, significant differences existed between both RV/IVC and early/advanced tumor thrombus groups in N0M0 patients. Multivariate analysis demonstrated that higher tumor thrombus level (p = 0.016, RR = 1.58), N (p = 0.013, RR = 2.60), and M (p < 0.001, RR = 4.14) stages and adrenal gland invasion (p = 0.001, RR = 4.91) were the most significant negative prognostic predictors.ConclusionsIn this study, we reported most cases of RCC patients with venous extension in China. We proved that patients with RCC and venous tumor thrombus may have relative promising long-term survival rate, especially those with early tumor thrombus

    Bilateral Unknown Key-Share Attacks in Key Agreement Protocols

    Unknown Key-Share (UKS) resilience is a basic security attribute in authenticated key agreement protocols, whereby two entities A and B should not be able to be coerced into sharing a key between them when in fact either A or B thinks that s/he is sharing the key with another entity C. In this paper we revisit some definitions of this attribute, the existing UKS attacks and the method of proving this attribute in the Bellare-Rogaway (BR) model in the literature. We propose a new UKS attack, which coerces two entities A and B into sharing a key with each other but in fact A thinks that she is sharing the key with another entity C and B thinks that he is sharing the key with another entity D, where C and D might or might not be the same entity. We call this attack a Bilateral Unknown Key-Share(BUKS) attack and refer to the existing UKS attacks, which are against one entity only, as a Unilateral UKS (UUKS) attack. We demonstrate that a few well-known authenticated key agreement protocols, some of which have been proved holding the UUKS resilience property, are vulnerable to the BUKS attack. We then explore a gap between the traditional BR-type proof of UUKS resilience and a BUKS adversary\u27s behaviour, and extend the BR model to cover the BUKS resilience attribute. Finally we provide a simple countermeasure to prevent a key agreement protocol from BUKS attacks

    Microscopic changing-mass model of PVA hydrogel under unidirectional compression

    PVA (Polyvinyl Alcohol) hydrogel is a kind of soft materials, with nontoxic and good biocompatibility. This kind of hydrogel shows strong nonlinearity in the static compression tests because of the high water content (~60‑90%), and the water is squeezed out when the PVA hydrogel is compressed. It means that the PVA hydrogel losts the mass during compression. Aiming at the changing mass mechanism and constitution in the compression of PVA hydrogel, we established a simplified microscopic model which is a single layer frame composite structure consisting of PVA fibers, water, and virtual membrane, in which the membrane wraps outside surfaces of the cubic cells and has no mass, but its membrane force has same properties with the surface tension of water. In the model, the deformation of PVA fibers and the compressive water sustains the external stress when the PVA hydrogel is compressed. In addition, by considering the limitation of the maximum membrane force induced by compressive water, the squeezed water will be calculated in each compressive step and the mechanism of the changing mass is determined quantitatively; in the meantime, the constitution of the PVA hydrogel may be deduced

    Numerical analysis of yield properties of closed-cell aluminum foam under multiaxial loads by 3D voronoi model

    Metallic foam is a typical porous material whose yield surface is related to not only von Mises equivalent stress but also the hydrostatic pressure. It is essential to study the yield properties of closed-cell aluminum foam under complex loading conditions. However, because the current experimental technique may support only a few proportions of multiaxial loading, it is hard to learn the yield surface well especially for the tensile hydrostatic pressure. In this article, we explored a numerical method to learn the yield properties of aluminum foam, in which the meso structures of aluminum foam were simulated by 3D Voronoi method. In addition, the yield surface of aluminum foam was drawn successfully with the numerical method. The main works included: (1) In our numerical simulation, we tested the calculating parameters such as mass scaling, elements number, and loading velocity on simulation results and verified the homogeneity of the 3D Voronoi model firstly. Furthermore, the optimized calculating parameters were determined by considering both reliability and feasibility of the calculation. Homogeneity of 3D Voronoi model was checked by the compression behaviors of aluminum in different directions. (2) In order to overcome the difficulty to determine critical yield state of metallic foams under complex loads, we recommended criterion by setting a dimensionless plastic dissipation value to determine the onset yield state of the foam under multiaxial loads. The critical value of dimensionless plastic dissipation was deduced from the common criterion—0.2% plastic strain in uniaxial loading, and the effect of relative densities on critical values was analyzed. (3) Three normal stresses were applied on cubic aluminum foam proportionally to implement the proportional loading. The different loading proportional factors of the three normal stresses were set widely to insure the yield surface including enough data, such as the hydrostatic loads cover from minimum negative to maximum positive values; each proportion has three loading proportional factors.     Deep Learning Overloaded Vehicle Identification for Long Span Bridges Based on Structural Health Monitoring Data

    Overloaded vehicles bring great harm to transportation infrastructures. BWIM (bridge weigh-in-motion) method for overloaded vehicle identification is getting more popular because it can be implemented without interruption to the traffic. However, its application is still limited because its effectiveness largely depends on professional knowledge and extra information, and is susceptible to occurrence of multiple vehicles. In this paper, a deep learning based overloaded vehicle identification approach (DOVI) is proposed, with the purpose of overloaded vehicle identification for long-span bridges by the use of structural health monitoring data. The proposed DOVI model uses temporal convolutional architectures to extract the spatial and temporal features of the input sequence data, thus provides an end-to-end overloaded vehicle identification solution which neither needs the influence line nor needs to obtain velocity and wheelbase information in advance and can be applied under the occurrence of multiple vehicles. Model evaluations are conducted on a simply supported beam and a long-span cable-stayed bridge under random traffic flow. Results demonstrate that the proposed deep-learning overloaded vehicle identification approach has better effectiveness and robustness, compared with other machine learning and deep learning approaches

    Vasculoprotective effects of rosiglitazone through modulating renin-angiotensin system in vivo and vitro

    <p>Abstract</p> <p>Background</p> <p>The peroxisome proliferator-activated receptor-γ (PPARγ) agonist rosiglitazone has been suggested to exert cardiovascular protection through the improvement of lipid metabolism, anti-inflammation, anti-proliferation etc. However, whether renin-angiotensin system (RAS) is involved in the vascular protective effects of PPARγ agonists is not fully understood. The present study aimed to investigate the effects of the renin-angiotensin system in vascular protection mediated by PPARγ agonists.</p> <p>Objective</p> <p>To investigate the actions of the renin-angiotensin system in vascular protection mediated by activation of PPARγ in vivo and in vitro.</p> <p>Methods</p> <p>Rats were fed a regular diet (n = 8), a cholesterol-rich diet plus methylthiouracil (80 mg/Kg/day, n = 10), a cholesterol-rich diet plus methylthiouracil and rosiglitazone (4 mg/kg/day, n = 10). The rosiglitazone treatment was started from one month after the start of cholesterol-rich diet plus methylthiouracil, and lasted five months. Cultured vascular smooth muscle cells (VSMCs) were pretreated with 1 μmol/L angiotensin II (ANG II) for 6 h and randomly divided into the control group; the ANG II group (1 μmol/L ANG II); the groups respectively treated with different concentration rosiglitazone (20, 30, 50) μmol/L for 12 h; the groups treated with 30 μmol/L rosiglitazone for (6, 12, 24) h. Morphology changes of the aortic tissues were observed by hematoxylin and eosin stain. The VSMC growth was detected by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) colorimetric assay. Angiotensin II and expression of angiotensin receptors were determined by radioimmunoassay, reverse transcription polymerase chain reaction (RT-PCR), western blot, and immunohistochemistry.</p> <p>Results</p> <p>After 6 months, lipid deposition, VSMC proliferation and migration toward intima were observed in aortic tissues in the rats on a cholesterol-rich diet plus methylthiouracil, while these pathological changes induced by the cholesterol-rich diet were significantly suppressed by rosiglitazone. In addition, VSMC proliferation induced by ANG II was markedly inhibited by rosiglitazone. Rosiglitazone markedly down-regulated expression of angiotensin type 1 receptor (AT<sub>1</sub>R) and up-regulated expression of angiotensin type 2 receptor (AT<sub>2</sub>R) in the aortic tissues and ANG II-treated VSMCs.</p> <p>Conclusions</p> <p>The present study demonstrated that PPARγ agonist rosiglitazone suppressed ANG II-induced VSMC proliferation in vitro and early atherosclerotic formation evoked by cholesterol-rich diet in vivo. These vasculoprotective effects of rosiglitazone were mediated at least partially by reduction in local tissue ANG II concentration, down-regulation of AT<sub>1</sub>R expression and up-regulation of AT<sub>2</sub>R expression both at the mRNA and protein levels.</p

    Xuebijing injection alleviates liver injury by inhibiting secretory function of Kupffer cells in heat stroke rats

    AbstractObjectiveTo evaluate the effects of Xuebijing (XBJ) injection in heat stroke (HS) rats and to investigate the mechanisms underlying these effects.MethodsSixty anesthetized rats were randomized into three groups and intravenously injected twice daily for 3 days with 4 mL XBJ (XBJ group) or phosphate buffered saline (HS and Sham groups) per kg body weight. HS was initiated in the HS and XBJ groups by placing rats in a simulated climate chamber (ambient temperature 40°C, humidity 60%). Rectal temperature, aterial pressure, and heart rate were monitored and recorded. Time to HS onset and survival were determined, and serum concentrations of tumor necrosis factor (TNF)-α, interleukin (IL)-1β, IL-6, alanine-aminotransferase (ALT), and aspartate-aminotransferase (AST) were measured. Hepatic tissue was harvested for pathological examination and electron microscopic examination. Kupffer cells (KCs) were separated from liver at HS initiation, and the concentrations of secreted TNF-α, IL-β and IL-6 were measured.ResultsTime to HS onset and survival were significantly longer in the XBJ than in the HS group. Moreover, the concentrations of TNF-α, IL-1β, IL-6, ALT and AST were lower and liver injury was milder in the XBJ than in the HS group. Heat-stress induced structural changes in KCs and hepatic cells were more severe in the HS than in the XBJ group and the concentrations of TNF-α, IL-β and IL-6 secreted by KCs were lower in the XBJ than in the HS group.ConclusionXBJ can alleviate HS-induced systemic inflammatory response syndrome and liver injury in rats, and improve outcomes. These protective effects may be due to the ability of XBJ to inhibit cytokine secretion by KCs

    Social Determinants of Community Health Services Utilization among the Users in China: A 4-Year Cross-Sectional Study

    Background To identify social factors determining the frequency of community health service (CHS) utilization among CHS users in China. Methods Nationwide cross-sectional surveys were conducted in 2008, 2009, 2010, and 2011. A total of 86,116 CHS visitors selected from 35 cities were interviewed. Descriptive analysis and multinomial logistic regression analysis were employed to analyze characteristics of CHS users, frequency of CHS utilization, and the socio-demographic and socio-economic factors influencing frequency of CHS utilization. Results Female and senior CHS clients were more likely to make 3–5 and ≥6 CHS visits (as opposed to 1–2 visits) than male and young clients, respectively. CHS clients with higher education were less frequent users than individuals with primary education or less in 2008 and 2009; in later surveys, CHS clients with higher education were the more frequent users. The association between frequent CHS visits and family income has changed significantly between 2008 and 2011. In 2011, income status did not have a discernible effect on the likelihood of making ≥6 CHS visits, and it only had a slight effect on making 3–5 CHS visits. Conclusion CHS may play an important role in providing primary health care to meet the demands of vulnerable populations in China. Over time, individuals with higher education are increasingly likely to make frequent CHS visits than individuals with primary school education or below. The gap in frequency of CHS utilization among different economic income groups decreased from 2008 to 2011
