    Educational videos for practitioners attending Baby Friendly Hospital Initiative workshops supporting breastfeeding positioning, attachment and hand expression skills: Effects on knowledge and confidence

    UNICEF Baby Friendly Initiative (BFHI) is the global standard for maternity and community services requiring all practitioners to be trained to support mothers in the essential skills of supporting positioning and attachment, and hand expression. These studies aim to rigorously assess knowledge in nurses, midwives, and doctors in these skills, tested before and after watching short videos demonstrating these skills. Practitioners were attending BFHI education, and the video study was additional. In Phase 1 clinicians in England were randomised to one of two videos (practitioner role play or clinical demonstration). The results showed improvements in knowledge and confidence, and a preference for clinical demonstration by mothers and infants. The clinical demonstration video was evaluated in China in Phase 2 where expert trainers viewed the video after completing the BHFI workshop, and in Phase 3 practitioners viewed the video before the BHFI workshop. Phase 2 with expert trainers only showed improvement in knowledge of hand expression but not positioning and attachment. In Phase 3 clinicians showed improved knowledge for both skills. In all Phases there were statistically significant improvements in confidence in practice in both skills. Viewing short videos increased knowledge, particularly about teaching hand expression, and confidence in both skills

    Body Mass Index and Weight Change During Adulthood Are Associated With Increased Mortality From Liver Cancer: The JACC Study

    Background: We investigated the association of baseline body mass index (BMI) and weight change since age 20 years with liver cancer mortality among Japanese.Methods: The data were obtained from the Japan Collaborative Cohort Study for Evaluation of Cancer Risk (JACC Study). A total of 31 018 Japanese men and 41 455 Japanese women aged 40 to 79 years who had no history of cancer were followed from 1988 through 2009.Results: During a median 19-year follow-up, 527 deaths from liver cancer (338 men, 189 women) were documented. There was no association between baseline BMI and liver cancer mortality among men or men with history of liver disease. Men without history of liver disease had multivariable hazard ratios (HR) of 1.95 (95%CI, 1.07–3.54) for BMI less than 18.5 kg/m2 and 1.65 (1.05–2.60) for BMI of 25 kg/m2 or higher, as compared with a BMI of 21.0 to 22.9 kg/m2. BMI was positively associated with liver cancer mortality among women and women with history of liver disease. Weight change since age 20 years was positively associated with liver cancer mortality among women regardless of history of liver disease. Women with history of liver disease had a multivariable HRs of 1.96 (1.05–3.66) for weight gain of 5.0 to 9.9 kg and 2.31 (1.18–4.49) for weight gain of 10 kg or more, as compared with weight change of −4.9 to 4.9 kg.Conclusions: Both underweight (BMI <18.5 kg/m2) and overweight (BMI ≥25 kg/m2) among men without history of liver disease, and weight gain after age 20 (weight change ≥5 kg) among women with history of liver disease, were associated with increased mortality from liver cancer

    Exploration by Maximizing R\'enyi Entropy for Reward-Free RL Framework

    Exploration is essential for reinforcement learning (RL). To face the challenges of exploration, we consider a reward-free RL framework that completely separates exploration from exploitation and brings new challenges for exploration algorithms. In the exploration phase, the agent learns an exploratory policy by interacting with a reward-free environment and collects a dataset of transitions by executing the policy. In the planning phase, the agent computes a good policy for any reward function based on the dataset without further interacting with the environment. This framework is suitable for the meta RL setting where there are many reward functions of interest. In the exploration phase, we propose to maximize the Renyi entropy over the state-action space and justify this objective theoretically. The success of using Renyi entropy as the objective results from its encouragement to explore the hard-to-reach state-actions. We further deduce a policy gradient formulation for this objective and design a practical exploration algorithm that can deal with complex environments. In the planning phase, we solve for good policies given arbitrary reward functions using a batch RL algorithm. Empirically, we show that our exploration algorithm is effective and sample efficient, and results in superior policies for arbitrary reward functions in the planning phase.Comment: Accepted by AAAI-2

    The Fungal CYP51s: Their Functions, Structures, Related Drug Resistance, and Inhibitors

    CYP51 (Erg11) belongs to the cytochrome P450 monooxygenase (CYP) superfamily and mediates a crucial step of the synthesis of ergosterol, which is a fungal-specific sterol. It is also the target of azole drugs in clinical practice. In recent years, researches on fungal CYP51 have stepped into a new stage attributing to the discovery of crystal structures of the homologs in Candida albicans, Cryptococcus neoformans and Aspergillus fumigatus. This review summarizes the functions, structures of fungal CYP51 proteins, and the inhibitors targeting these homologs. In particular, several drug-resistant mechanisms associated with the fungal CYP51s are introduced. The sequences and crystal structures of CYP51 proteins in different fungal species are also compared. These will provide new insights for the advancement of research on antifungal agents

    Novel models for fatigue life prediction under wideband random loads based on machine learning

    Machine learning as a data-driven solution has been widely applied in the field of fatigue lifetime prediction. In this paper, three models for wideband fatigue life prediction are built based on three machine learning models, i.e. support vector machine (SVM), Gaussian process regression (GPR) and artificial neural network (ANN). The generalization ability of the models is enhanced by employing numerous power spectra samples with different bandwidth parameters and a variety of material properties related to fatigue life. Sufficient Monte Carlo numerical simulations demonstrate that the newly developed machine learning models are superior to the traditional frequency-domain models in terms of life prediction accuracy and the ANN model has the best overall performance among the three developed machine learning models

    Drought-Induced Alterations in Carbon and Water Dynamics of Chinese Fir Plantations at the Trunk Wood Stage

    Over the past three decades, China has implemented extensive reforestation programs, primarily utilizing Chinese fir (Cunninghamia lanceolata (Lamb.) Hook) in southern China, to mitigate greenhouse gas emissions and counter extreme climate events. However, the effects of drought on the carbon sequestration capacity of these forests, particularly during the trunk wood stage, remain unclear. This study, conducted in Huitong, Hunan, China, from 2008 to 2013, employed the eddy covariance method to measure carbon dioxide (CO2) and water fluxes in Chinese fir forests, covering a severe drought year in 2011. The purpose was to elucidate the dynamics of carbon and water fluxes during a drought year and across multi-normal year averages. The results showed that changes in soil water content (−8.00%), precipitation (−18.45%), and relative humidity (−5.10%), decreases in air temperature (−0.09 °C) and soil temperature (−0.79 °C), and increases in vapor pressure deficit (19.18%) and net radiation (8.39%) were found in the drought year compared to the normal years. These changes in environmental factors led to considerable decreases in net ecosystem exchange (−40.00%), ecosystem respiration (−13.09%), and gross ecosystem productivity (−18.52%), evapotranspiration (−12.50%), and water use efficiency (−5.83%) in the studied forests in the drought year. In this study, the occurrence of seasonal drought due to uneven precipitation distribution led to a decrease in gross ecosystem productivity (GEP) and evapotranspiration (ET). However, the impact of drought on GEP was greater than its effect on ET, resulting in a reduced water use efficiency (WUE). This study emphasized the crucial role of water availability in determining forest productivity and suggested the need for adjusting vegetation management strategies under severe drought conditions.     Comprehensive landscape and future perspectives of non-coding RNAs in esophageal squamous cell carcinoma, a bibliometric analysis from 2008 to 2023

    Objectives: Summarize the progress and hot topic evolution of non-coding RNAs (ncRNAs) research in esophageal squamous cell carcinoma (ESCC) in recent years and predict future research directions.Methods: Relevant articles from the Web of Science until 31 October 2023 were obtained. Bibliometric analysis of included articles was performed using software (VOSviewer, CiteSpace, and Bibliometrix). The volume and citation of publications, as well as the country, institution, author, journal, keywords of the articles were used as variables to analyze the research trends and hot spot evolution.Results: 1,118 literature from 2008 to 2023 were retrieved from database, with 25 countries/regions, 793 institutions, 5,426 authors, 261 journals involved. Global cooperation was centered on China, Japan, and the United States. Zhengzhou University, an institution from China, had the highest publication. The most prolific author was Guo Wei, and the most prolific journal was Oncology Letters. Analysis of keywords revealed that the research in this field revolved around the role of ncRNAs in the occurrence, development, diagnosis, treatment, and prognosis of ESCC, mainly including micro RNAs, long non-coding RNAs, and then circular RNAs.Conclusion: Overall, research on ncRNAs in ESCC remains strong. Previous research has mainly focused on the basic research, with a focus on the mechanism of ncRNAs in the occurrence, development, diagnosis, treatment, and prognosis of ESCC. Combining current research with emerging disciplines to further explore its mechanisms of action or shifting the focus of research from preclinical research to clinical research based on diagnosis, treatment, and prognosis, will be the main breakthrough in this field in the future

    We have defined Neutrosophic Over-/Under-/Off-Set and Logic for the first time in 1995 and published in 2007. During 1995-2016 we presented them to various national and international conferences and seminars. These new notions are totally different from other sets/logics/probabilities. We extended the neutrosophic set respectively to Neutrosophic Overset {when some neutrosophic component is > 1}, to Neutrosophic Underset {when some neutrosophic component is < 0}, and to Neutrosophic Offset {when some neutrosophic components are off the interval [0, 1], i.e. some neutrosophic component > 1 and other neutrosophic component < 0}. This is no surprise since our real-world has numerous examples and applications of over-/under-/off-neutrosophic components
