    Drug-susceptibility Patterns of Mycobacterium tuberculosis in Mpumalanga Province, South Africa: Possible Guiding Design of Retreatment Regimen

    Multidrug-resistant tuberculosis (MDR-TB) has been a cause of concern in both developed and developing countries. The prevalence of drug resistance in Mycobacterium tuberculosis (MTB) isolates (n=692) from Mpumalanga province was assessed. In total, 692 (64%) MTB strains from cases with pulmonary TB were tested for susceptibility against rifampicin, isoniazid, ethambutol, and streptomycin using the MGIT 960 instrument. Two hundred and nine (30.2%) strains were resistant to one or more drugs. Resistance to one drug ranged from 1.4% for ethambutol to 17.7% for rifampicin. The prevalence of MDR-TB ranged from 6.7% for three drugs to 34% for four drugs, with significant predictors being patients’ age-groups of 25–54 years (p=0.0012) and >55 years (p=0.007). The result showed a high level (58.4%) of MDR-TB from cases in Mpumalanga province. To achieve a higher cure rate in this province, drug-susceptibility tests must be done for every case

    Major surgery in an osteosarcoma patient refusing blood transfusion: case report

    We describe an unusual case of osteosarcoma in a Jehovah's Witness patient who underwent chemotherapy and major surgery without the need for blood transfusion. This 16-year-old girl presented with osteosarcoma of the right proximal tibia requiring proximal tibia resection, followed by endoprosthesis replacement. She was successfully treated with neoadjuvant chemotherapy and surgery with the support of haematinics, granulocyte colony-stimulating factor, recombinant erythropoietin and intraoperative normovolaemic haemodilution. This case illustrates the importance of maintaining effective, open communication and exploring acceptable therapeutic alternative in the management of these patients, whilst still respecting their beliefs

    Intra-Africa agricultural trade: A South African perspective

    Purpose: The aim of this paper is to assess and provide an overview of the magnitude of current agricultural trade patterns between South Africa and the five leading regional economic communities (REC's) in Africa. This paper also seeks to examine some of the constraints limiting greater intra-African agricultural trade. This is done in order to better understand the role South Africa currently plays and could potentially play in promoting intra-Africa trade. Design/Methodology/Approach: Trade flows between South Africa and the leading REC's are outlined and explained. Trade data and tariff data is sourced from available databases. Non-tariff barriers and other impediments to greater intra-African trade are examined with reference to available literature and discussions the authors have had with trade experts and policy makers. Findings: South Africa is the most active country in intra-Africa agricultural trade. However, it is a relationship defined predominantly on exports to Africa with a low level of imports. South Africa exports a diverse range of value added products whilst imports remain concentrated in commodities. Significant imbalances in agricultural trade between South Africa and the respective REC's continue to persist. Regional trade arrangements have fostered greater trade but significant obstacles to greater trade remain. Implications: African countries that do not invest in infrastructure and create a trade-enabling environment and diversify their production, limit their potential to the supply of one or two commodities thereby perpetuating the trend of huge trade imbalances in favour of South Africa. Originality/Value: This work provides a platform for assessing trade relationships and examining impediments to greater trade. It is also relevant in guiding future research on priority markets in Africa as export destinations and import suppliers in light of increasing regional integration initiatives and governments commitment to African development

    Monitoring an isolated population of Cape Parrots Poicephalus robustus in the Limpopo province, South Africa

    Cape Parrots Poicephalus robustus inhabit forest patches in South Africa with <1 800 individuals remain in the wild. A small, genetically isolated population occurs in the northern part of its range in the Limpopo province. There, annual counts indicated ~100 remain, but count numbers fluctuate substantially between years creating uncertainty over population trends. To inform approaches to the monitoring of Cape Parrot populations in the Limpopo province, we investigated how local abundance counts varied within and across years at known parrot ‘hotspots’. Monthly counts were done at these sites during 2013–2015 and 2020–2021. We found large variability in counts both within and amongst years, although this differed between sites. Observations of behaviour suggest that a site close to overnight roosts was more reliable, whereas those close to food sources were more variable likely linked to fruit availability. The highest numbers were recorded at a pecan nut orchard away from the indigenous forest. To better understand population trends of this small, isolated population we recommend annual counts are complemented with a series of focused counts at pecan nut orchards, which are monitored with greater intensity during the fruiting season.
