    Identification of potentially zoonotic parasites in captive orangutans and semi-captive mandrills: phylogeny and morphological comparison

    Cysts and trophozoites of vestibuliferid ciliates and larvae of Strongyloides were found in fecal samples from captive orangutans Pongo pygmaeus and P. abelii from Czech and Slovak zoological gardens. As comparative material, ciliates from semi-captive mandrills Mandrillus sphinx from Gabon were included in the study. Phylogenetic analysis of the detected vestibuliferid ciliates using ITS1-5.8s-rRNA-ITS2 and partial 18S rDNA revealed that the ciliates from orangutans are conspecific with Balantioides coli lineage A, while the ciliates from mandrills clustered with Buxtonella-like ciliates from other primates. Morphological examination of the cysts and trophozoites using light microscopy did not reveal differences robust enough to identify the genera of the ciliates. Phylogenetic analysis of detected L1 larvae of Strongyloides using partial cox1 revealed Strongyloides stercoralis clustering within the cox1 lineage A infecting dogs, humas and other primates. The sequences of 18S rDNA support these results. As both B. coli and S. stercoralis are zoonotic parasites and the conditions in captive and semi-captive settings may facilitate transmission to humans, prophylactic measures should reflect the findings

    Self-reported use of tobacco products in nine rural INDEPTH Health and Demographic Surveillance Systems in Asia

    BACKGROUND: Tobacco use is the most preventable cause of premature death and disability. Even though tobacco use is common in many Asian countries, reliable and comparable data on the burden imposed by tobacco use in this region are sparse, and surveillance systems to track trends are in their infancy. OBJECTIVE: To assess and compare the prevalence of tobacco use and its associated factors in nine selected rural sites in five Asian countries. METHODS: Tobacco use among 9,208 men and 9,221 women aged 25-64 years in nine Health and Demographic Surveillance System (HDSS) sites in five Asian countries of the INDEPTH Network were examined in 2005 as part of a broader survey of the major chronic non-communicable disease risk factors. All sites used a standardised protocol based on the WHO STEPS approach to risk factor surveillance; expanded questions of local relevance, including chewing tobacco, were also included. Multivariable logistic regression was used to assess demographic factors associated with tobacco use. RESULTS: Tobacco use, whether smoked or chewed, was common across all sites with some notable variations. More than 50% of men smoked daily; this applied to almost all age groups. Few women smoked daily in any of the sites. However, women were more likely to chew tobacco than men in all sites except Vadu in India. Tobacco use in men began in late adolescence in most of the sites and the number of cigarettes smoked daily ranged from three to 15. Use of both forms of tobacco, smoked and chewed, was associated with age, gender and education. Men were more likely to smoke compared to women, smoking increased with age in the four sites in Bangladesh but not in other sites and with low level of education in all the sites. CONCLUSION: The prevalence of tobacco use, regardless of the type of tobacco, was high among men in all of these rural populations with tobacco use started during adolescence in all HDSS sites. Innovative communication strategies for behaviour change targeting adolescents in schools and adult men and women at work or at home, may create a mass awareness about adverse health consequences of tobacco smoking or chewing tobacco. Such efforts, to be effective, however, need to be supported by strong legislation and leadership. Only four of the five countries involved in this multi-site study have ratified the Framework Convention on Tobacco Control, and even where it has been ratified, implementation is uneven.Supplement: 1</p

    Capacity to control noncommunicable diseases in the countries of South-East Asia

    Objective To assess the status of national capacity for prevention the and control of noncommunicable diseases (NCDs) in the Member States of the South-East Asia (SEA) Region of the World Health Organization (WHO).Methods A survey was conducted in 2006-2007 in all 11 Member States of the SEA Region of WHO using a regional adaptation of the global WHO questionnaire. Information was collected on the existence of national legislation, policies, strategies, and programmes, implementation status of the WHO Framework Convention on Tobacco Control, and the Global Strategy on Diet, Physical Activity and Health, and the availability of services and financial resources for the prevention and control of NCDs. The results of this survey were compared with those of a similar survey conducted in the region in 2001.Results The comparison of two surveys showed progress made in the Region between 2001 and 2006 in strengthening commitment and capacity to prevent and control NCDs. Major gaps remain in the creation of appropriate legislative and regulatory environment, policy formulation for the promotion of healthy nutrition and physical activity, and generation of guidelines for the management of NCDs and their risk factors.Conclusion Member States of the SEA Region need to enhance the capacity to address critical gaps in their national polices and programmes for the prevention and control of NCDs.
